1.Clinical observation ofFuzheng-Shugan decoction on treating thyroid nodule

Objective To investigate the clinical effect ofFuzheng-Shugan decoction on the treatment of patients with the treatment nodule.Methods A total of 80 patients with thyroid nodule from Jan.2014 to Dec.2015, divided into two groups. The efficacy, thyroid stimulating hormone, high risk factors, the maximum diameter and volume of thyroid nodules were observed.Results The effective rate of observation group was significantly higher than of the control group (90.00%vs.27.50%,χ2=32.237,P=0.000). The shortness of breath (1.62 ± 0.86vs. 4.35 ± 1.13,t=12.159), foreign body sensation (2.53 ± 0.62vs. 4.23 ± 0.56,t=12.289), irritability (1.56 ± 0.53vs. 3.52 ± 1.25,t=9.130), sweating (1.85 ± 0.62vs.4.23 ± 0.56,t=18.017), tongue (1.58 ± 0.93vs. 3.71 ± 1.25,t=8.646) and neck pain (0.82 ± 0.35vs. 3.92 ± 0.65,t=26.558) in the obversation group were significantly lower than those in the control group (P<0.05). The thyroid stimulating hormone levels in the observation group (1.05 ± 0.21 mIU/Lvs. 2.16 ± 0.71 mIU/L,t=9.482), were significantly lower (P<0.05). The maximum diameter (8.52 ± 4.72 mmvs. 12.27 ± 5.69 mm,t=3.208) nodule volume (0.26 ± 0.71 cm3vs. 0.96 ± 1.02 cm3,t=3.562) and high risk factors (2.73 ± 1.45vs. 4.05 ± 1.36,t=4.199) in the observation group were significantly lower than those in the control group (P<0.05).Conclusion TheFuzheng-Shugan decoction can improve TCM syndrome, reduce the risk factors, and effectively reduce the nodule maximum diameter and volume.
2.Imaging findings of adult sphenoid spontaneous cerebrospinal fluid rhinorrhea and accompanying intracranial hypertension

Objective To study the CT and MR imaging feature of adult sphenoid spontaneous cerebrospinal fluid (CSF) rhinorrhea and accompanying intracranial hypertension. Methods Thirty consecutive patients including 11 males and 19 females with mean age of (50 ± 8) years (range, 31 to 64 years) were retrospectively reviewed. Imaging findings in 30 patients with adult sphenoid spontaneous CSF rhinorrhea (CT in 26 patients, MR in 29 patients, and both CT and MR in 25 patients) were analyzed. The MR imaging and CT features were evaluated by two experienced head and neck radiologists. The CT and MR imaging parameters of 30 fistulas were evaluated, including side, location, size, amount, bony change, and the adjacent structures change. Results Of the 30 patients of adult sphenoid spontaneous CSF rhinorrhea lesions, the site of the CSF fistula confirmed by endoscopy surgery was at the junction of the roof of the inferolateral recess and the floor of the middle cranial in 25 (83%, 25/30) patients, the roof of the inferolateral recess in 3 (10%,3/30) patients, and the lateral wall of the sphenoid sinus in 2 (7%,2/30). CT images revealed the osseous defects of the sphenoid sinus walls in 21 patients (80.7%, 21/26) patients, excessive pneumatization of the inferolateral recess of the sphenoid sinuses in 25 cases (96.1%, 25/26). MRI demonstrated the linear hyperintensity communicating subarachnoid space and sphenoid cavity, accompanying meningoencephalocele in 26 (89.6%, 26/29) patients, sphenoid sinus filled with CSF in 24 (82.7%,24/29) patients and air-fluid level in the sphenoid sinus in 8 cases (27.6%, 8/29), excessive expansion of adjoining lateral fissure cistern in 22 cases (75.9%,22/29), adjoining sulcus in one case (3.4%, 1/29), adjoining lateral ventricle in one case (3.4%,1/29). The imaging feature of accompanying intracranial hypertension included empty sella in 29 cases (100%, 29/29), augmentation of the complex of the optic sheath in 27 cases (93.1%, 27/29), the arachnoid pits in the middle cranial fossa in 30 cases (100%). Conclusions The spontaneous CSF fistula coexists with intracranial hypertension. The combined application of CT and MRI can accurately identify the fistula with accompanying symptom and the intracranial hypertension.
3.Role of P2X3 receptors in dorsal root ganglion in development of incisional pain in rats

Objective To evaluate the role of P2X3 receptors in dorsal root ganglion in development of incisional pain in rats.Methods Twenty-four healthy male SD rats weighing 200-220 g were randomly divided into 3 groups(n = 8 each): control group(group C),incisional pain(group IP)and P2X3 receptor antagonist + IP group(group A).In group IP and A,a 1 cm longitudinal incision was made in the plantar surface of left hindpaw according to the method described by Brennan et al.in isoflurane-anesthetized rats.P2X3 receptor antagonist TNP-ATP 200 nmol was injected into the plantar surface of left hindpaw 30 min after plantar incision was made in group A,while equal volume of normal saline was given instead of TNP-ATP in group C and IP.The behavior of the hindpaw of the rats were assessed using cumulative pain score within 1 h after injection.The animals were sacri ficed 2 h after injection and the dorsal root ganglion was removed for determination of P2X3 receptor expression and intracellular Ca2+ concentrations.ResultsThe cumulative pain scores,P2X3 receptor expression and Ca2 + concentrations were significantly higher in group IP and A than in group C(P < 0.05).The cumulative pain scores,P2X3 receptor expression and Ca2+ concentrations were significantly lower in group A than in group IP(P <0.05).Conclusion P2X3 receptors in dorsal root ganglion is involved in the development of incisional pain through increasing intracellular Ca2+ concentrations in rats.
4.Substance P inhibits GABA-activated currents in bullfrog primary sensory neurons by protein kinase C

Objective To research the mechanism or pathway through which Substance P(SP) inhibits r-aminobutyric acid(GABA) activated currents in bullfrog dorsal root ganglion(DRG) neurons.Method The experiment was conducted on freshly isolated bullfrog dorsal root ganglion neurons using a whole-cell patch-clamp technique.Results SP could caused a slow inward current when SP was applied to DRG neurons;SP could inhibits GABA-activated currents;The inhibition could be reduced largely when protein kinase C(PKC) inhibiter,1-(5-isoquinolinesulphorry)-2-methylpiperazine(H-7), was dialyzed in cell body.Conclusion The SP ton inhibit GABA-activated currents through protein kinase C.
5.MRI diagnosis of solitary fibrous tumor in the orbit

Objective To explore the MRI features of solitary fibrous tumor(SFT)in the orbit.Methods The MRI findings of 7 patients with SFT in the orbit confirmed by histopathology were analyzed retrospectively.Re sults Of the 7 lesions,5 occurred in the right orbit and 2 in the left orbit.Six lesions were located in the extraconal space near the lacrimal gland fossa,including 5 in the superomedial region and 1 in the inferolateral region.The other one was located in the retrobulbar intraconal space.The lesions with well-defined margin showed elliptic shape in 6 cases and lobulated configuration in 1.The maximum diameter of the lesions ranged from 18 to 40 mm(mean,31 mm).The lesions showed homogeneous isointense relative to gray matter on T1-weighted images in 6 patients.On T2-weighted images,the lesions showed heterogeneous hypointense in 5 patients,isointense and hyperintense in one patient respectively.SFT demonstrated markedly homogeneous enhancement in 6 patients and inhomogeneous enhancement in one patient The time-intensity curves(TIC)of 7 patients exhibited a rapidly enhancing and rapid washout pattern on dynamic contrast-enhanced(DCE)MRI.Conclusion Hypointense signal on T2WI,marked enhancement on contrast-enhanced T1 WI,and a rapidly enhancing and rapid washout pattern TIC on DCE MRI are the typical MRI features of orbital SFT.
6.Study on the Pharmacokinetic-pharmacodynamic Model of Nisoldipine Controlled-release Patches in Spon-taneously Hypertensive Rats

OBJECTIVE:To establish the pharmacokinetic-pharmacodynamic(PK-PD) model of Nisoldipine controlled-release patches(NCRP)in spontaneously hypertensive rats(SHR). METHODS:SHR were randomized into a patch(NCRP)group and a tablet(Nisoldipine tablets)group,with 6 rats in each group. The microdialysis probes were implanted in SHR. Each rat was given 5 mg nisoldipine. Plasma microdialysate was collected within 36 h after administration. HPLC was adopted to determine the plasma concentration of nisoldipine,and WinNonlin 5.3 was employed to calculate Pharmacokinetic parameters. With heart rate and blood pressure as pharmacodynamic indexes,PK-PD model study was conducted. RESULTS:Vs. nisoldipine tablets,NCRP has con-trolled release effect. The relationship between NCRP drug effect and effect-site concentration met the Sigmoid-Emax model. The main parameters of the PK-PD model for heart rate and systolic blood pressure were as follows as Emax of (2.65 ± 0.06) and (10.71 ± 0.87),EC50 of (83.65 ± 35.25) and (1.29 ± 0.26) ng/ml,γ of (0.83 ± 0.91) and (1.2 ± 0.35),Keo of (0.37 ± 0.53) and (0.91±0.24)h-1. CONCLUSIONS:PK-PD model of NCRP in SHR has been established successfully.
7.The value of the apparent diffusion coefficient in MR diffusion weighted imaging for the differential diagnosis of sinonasal masses

Objective To investigate the diagnostic value of apparent diffusion coefficient in the evaluation of sinonasal masses.Methods Sixty-seven sinonasal solid masses over 1 cm in diameter confirmed by pathology were retrospectively analyzed,all patients underwent preoperative routine MRI with DWI,the ADC values were measured in ROI within the solid mass.The patients were divided into benign and malignant groups by the histopathology,according to pathological findings,the patients were further divided into the hematolymphoid tumors,the malignancy of epithelium and mesenchymal tissue,the benign tumors of epithelial and mesenchymal tissue,and vasogenic masses.ANOVA test and t test were used to compare the ADC values of different groups.The receiver operating characteristic curve (ROC) was constructed using various cut points of ADC for different parameters to confirm the diagnostic threshold value and evaluate the diagnostic efficacy.Results All lesions were solitary.There were 22 malignant tumors,of which 6 lesions were hematolymphoid tumors and 16 lesions malignancy from epithelium and mesenchymal tissue.There were 45 benign tumors,of which 22 lesions were benign tumors from epithelium and mesenchymal tissue and 23 lesions vasogenic masses.The mean ADC value of malignant and benign masses was(0.88 ± 0.26) × 10-3 mm2/s and (1.54 ± 0.41) × 10-3 mm2/s respectively.There was statistically significant differences between them (t =6.897,P < 0.01).The mean ADC value was(0.63 ± 0.10) × 10-3 mm2/s in hematolymphoid tumors,(0.97 ±0.24) × 10 3 mm2/s in malignancy from epithelium and mesenchymal tissue,(1.38 ± 0.23) × 10-3 mm2/s in benign tumors from epithelium and mesenchymal tissue,(1.68 ± 0.49) × 10-3 mm2/s in vasogenic masses respectively.There was statistically significant difference among all 4 groups(F =22.788,P < 0.01),and the differences between any 2 groups were still statistically significant(P < 0.05).The area under the ROC calculated was 0.945.Using an ADC value of 1.08 × 10-3 mm2/s as the threshold value for differentiating malignant from benign lesions,the best result obtained had a sensitivity of 81.8% (18/22),specificity of 97.8% (44/45),accuracy of 92.5 % (62/67).Conclusion The ADC value is a valuable tool in differentiating benign from malignant masses and different kinds of masses in sinus and nasal cavity.
8.Construction of recombinant plasmid pCMV-Myc-PIASx? and its protein expression

Objective To construct the recombinant plasmid pCMV-Myc-PIASX? and express the fusion protein in mammalian cells.Methods PIASx? fragment was digested from the original vector pGADT7 with SalⅠand NotⅠ,and then was inserted into the targeted pCMV-Myc vector by the recombinant DNA technique.After identification,the recombinant plasmid was transfected into CHO cells.The expression of recombinant Myc-PIASx? fusion protein was detected by Western blotting.Results By the restriction enzyme digestion,fragment purification,ligation and transformation,the recombinant plasmid was obtained.The right recombinant plasmid pCMV-Myc-PIAS3 was identified with enzyme digestion and sequencing.By EcoRⅠ digestion analysis,pCMV-Myc-PIASx? showed a 5641 bp band.By XbaⅠdigestion analysis,pCMV-Myc-PIASx? showed two expected band of 3291 bp and 2349 bp.A specific protein expression band at 68 000(PIASx? fusion protein) was showed in Western blotting,which matched recombinant plasmid.Conclusion The recombinant plasmid of pCMV-Myc-PIASx? is sucssesefully constructed,which provids a good tool for further function study on PIAS family.
9.Cause analysis and preventive measures for blood sample errors

Objective To study the reason and preventive measures of inconformity of the extraction appeared in the process of transfusion blood specimens with the patient's blood type.Methods The reasons of transfusion errors why extracting required blood type was not consistent with the patient's blood type examplesa in Zhengzhou transfusion of hospital from 2008 to 2012 were retrospectively analyzed.Results The experimental results showed that blood specimen inconformity was 49.60%,the error rate of blood extraction and blood infusion was 31.20%,infusion error only was 15.20%,blood type change after stem cell transfusion was 4.00%.Reasons of blood type change after stem cell transplantation to extract blood samples mainly included the blood center or blood did not accord with logo type blood stations provided (16.13%),blood use application form to fill in error(20.97%),check the wrong blood type (6.45%),blood samples taken(29.03%),the blood sample tag(27.42%).Conclusion In order to ensure the safety for clinical use must adopt measures to prevent resolutely put an end to a blood transfusion errors.
10.Influence of drinking pattern and alcoholic fatty liver disease in coal miners

Objective To investigate the drinking pattern and the condition of alcoholic fatty liver disease in a certain coal mine workers in Shanxi Province.Methods A total of 1501 workers in a coal mine in Shanxi Province were surveyed by field investigation method.Contents include questionnaire, physical measurement, abdominal ultrasound liver and fasting blood glucose, blood lipid, liver function, cholesterol, blood biochemical indicator detection.ALD diagnostic criteria for fatty liver and alcoholic liver disease group were .recommended by the Chinese Medical Association in 2010.The t test,X2 test and multiariable logistic regression analysis were conducted by SPSS17.0 software.Results This study involved ALD patients with 265 people, accounting for 17.65% of the total survey.The drinking pattern, such as drinking patterns in the initial drinking age,long duration of drinking, drinking frequency, drunkenness, fasting drinking, average daily alcohol intake as the risk factors of alcoholic liver disease.The Logistic regression analysis of alcoholic liver disease related factors showed that, drinking age, drinking way and daily average alcohol intake were closely related to the occurrence of ALD(OR=0.942,P=0.769;OR=2.811,P=0.000;OR=1.756,P=0.000;OR=542.844,P=0.001) .Conclusion In the coal mine workers, drinking pattern in the initial drinking age, drinking age, daily average alcohol intake are closely related to ALD illness.
