1.Therapy effect of 3.0 MR for gamma knife radios surgery on trigeminal neuralgia

Objective To investigate the application of 3.0 MR for Gamma knife radios surgery(GKS) for treatment trigeminal neuralgia(TN) and the effect of 3.0 MR on therapy.Methods Sixty-two patients with TN underwent GKS,between May 2005 to June 2011 at Wanjie Hospital were included as our subjects.3.0MR 3-D FIESTA sequence scan were performed before treatment,during treatment 3.0MR orientation guided to select the trigeminal neuralgia (TN) brain cistern segment target spot.The 4 mm collimator was used and the maximum dose was to 70-90 Gy.Results The followed up periods was 6-36 months,total effective rate was 92 %,the total alleviate cases was 51 part for 8 cases,none for 3,ipsilateral facial numbness,tinnitus,masticatory muscle weakness was found.Conclusion 3.0MR can clearly display trigeminal neuralgia and duty blood vessel,nicety orientation TN root,and then enhance GKS treatment TN rate,as well as avoid complication.
2.Rat skeletal muscle alterations after coagulation with novel microwave antenna

AIM: To evaluate the feasibility and safety of microwave coagulation with novel microwave antenna through observing physiological and pathological changes after the coagulation on rat skeletal muscle. METHODS: The experiment was performed at the Central Laboratory of Zhujiang Hospital of Southern Medical University from December 2006 and September 2007. ①Using MTC-3C microwave tissue coagulation device, monopole microwave antenna after anti-adhesion was vertically inserted into SD rat skeletal muscle for heat coagulation at 20 W?8 s. ②The instant enzyme activity changes after microwave coagulation therapy (MCT) was detected by 2, 3, 5-Triphenyl Tetrazolium Chloride (TTC) staining. The cell apoptosis rate in outer fields of coagulation was evaluated by flow cytometry 0, 12 and 24 hours after coagulation, and the treated muscle was examined pathologically on days 0, 2, 7, 30, and 60. RESULTS: ①The coagulation areas were stained in white immediately after MCT by TTC staining, while the outer normal areas were in red. There were apoptotic cells around coagulation areas, and the highest percentage appeared 12 hours later, reduced thereafter. ②In 24 hours after coagulation, only mild alteration of cellular morphology was found. Two days after MCT, necrosis cells were prominent in transition zone, and the inflammation cells were predominantly composed of macrophage and lymphocytes in the zone, while the cells in the coagulation zone remained structure; 7 days after MCT, the fibroblast and granulation tissue occurred in the transition zone, and the myocyte in the coagulation zone were of necrosis; 30 days after MCT, there were still many inflammation cells in the coagulation zone with some collagenous fibrils in the transition zone; 60 days after MCT, the necrosis cells reduced and the outer margin of the transition zone was encapsulated by marked collagenous fibrils. Tissue repair started from the outer margin of the transition zone to the central coagulation zone. CONCLUSION: Novel microwave tissue coagulation is safe and feasible to be used in the treatment of some minor-carcinoma and some tumors on body face.
3.Experimental study of rat sciatic coagulation with a novel microwave antenna

Objective To observe the sequential ultrastroctural and electrophysiological changes in the sciatic nerve coagulated by a newly-designed microwave antenna. Methods A total of 75 Sprague-Dawley rats were randomly divided into groups A,B and C and irradiated with microwaves at 10,20 or 30 Watts,for 6 seconds to coagulate the left sciatic nerve.Electrophysiological effects and sequential uhrastructural changes were observed on the 0th,2nd,7th,30th and 60th days after coagulation.A static sciatic index was calculated based on measurements of the footprint on the 7th,30thand 60th days after coagulation.Results On the Oth,2nd,7th and 30th days after cpagulation,the static sciatic index,the nerve conduction velocity and the amplitude of the action potentials in groups B and C had decreased significantly compared with those before coagulation.On the 60th day after coagulation.significant recovery was observed in groups A and B,but not in group C.Only mild alteration in uhrastructure was found,and only in group A.The prominent changes in uhrastructure in group B included broken Schwann cell membranes and myelin disintegration.There were severe injuries in group C,including myelin disintegration,cell deformity,coagulative necrosis,axon necrosis,basement membrane necrosis and demyelination.The structure of the sciatic nerve in group B had partially recovered after 60 days,but group C showed no recovery at all. Conclusion Microwave coagulation of a nerve can block its conduction.and even destroy the nerve.Percutaneous microwave coagulation is clinically feasible and call be an alternative treatment for pain.
4.Dynamic characteristics of the lumbosacral vertebrae based on three-dimensional finite element models

BACKGROUND: Inherent modal analysis and harmonic response analysis on the human normal lumbosacral vertebraehave been reported, but there is a lack of comparative research on their modal analysis results before and after pediclescrew fixation.OBJECTIVE: To explore the dynamic characteristics of human lumbosacral vertebrae using three-dimensional finiteelement method.METHODS: Finite element model of lumbosacral vertebrae (L1-S1) before and after pedicle screw fixation was developedand validated based on CT images, and the modal analysis and harmonic response analysis were then conducted.RESULTS AND CONCLUSION: (1) Representative nodes were selected at the spinous process segments of L1, L3 andL5, and numbered as A, B, and C, respectively. (2) The maximum displacement of each node in Y and Z directions oflumbosacral vertebral model after internal fixation was significantly decreased compared with those of the normallumbosacral vertebral model, suggesting that screw fixation system plays a protective role in lumbosacral vertebrae, andreduces its amplitude under external load, thus diminishing its sensitivity to external load. (3) The lumbosacral vertebralmodal analysis can provide basis for further study on dynamic analysis, and the parameters such as natural frequency,modal shape and vibration amplitude of the lumbar spine have been determined.
5.Effect of volume therapy with 6 % hydroxyethyl starch 130/0.4 on mesenteric microcirculation in a rabbit model of hypovolemia

Objective To investigate the effect of volume therapy with 6% hydroxyethyl starch (HES) 130/0.4 on mesenteric microcirculation in a rabbit model of hypovolemia.Methods Sixty-four adult male rabbits,weighing 2.0-2.3 kg,were randomly divided into 4 groups( n =16 each):control group (group C),hypovolemia group (group HM),Ringer's solution group (group RS) and 6% HES 130/0.4 group (group HES).The animals were anesthetized with sodiun pentobarbital 3.0-3.5 mg/kg.Femoral artery,femoral vein and right internal jugular vein were cannulated for MAP monitoring,blood withdrawing and fluid adminstration.Hypovolemia was induced by withdrawing blood from femoral vein of 30% blood volume in 30 min in groups HM,RS and HES.In group C,no blood was withdrawed.In group RS,Ringer's solution 3 times of the volume of blood withdrawn was infused,while 6% HES 130/0.4 equal volume of blood withdrawn was infused via right internal jugular vein immediately at the end of blood withdrawing.MAP and HR were recorded and femoral artery and femoral vein blood samples were taken before blood withdrawing (T0),immediately at the end of blood withdrawing(T1 ),immediately at the end of volume therapy(T2 ) and 30 min after volume therapy(T3 ).DO2,VO2 and ERO2 were calculated.Diameter and blood flow speed of microvessel were determined.Results Compared with group C,HR was significantly increased,MAP decreased,diameter and blood flow speed of microvessel were decreased at T1~3,DO2 was increased at T1 in group HM.Compared with group HM,MAP was significantly increased at T2,HR decreased at T2,3,DO2and VO2 were increased at T1~3,diameter of arteriole was increased at T2 whlie decreased at T3,diameter of veinule and blood flow speed of microvessel were increased at T2,3 in group RS,MAP was significantly increased at T2,HR was decreased whlie DO2 and VO2 were decreased,VO2,ERO2,diameter and blood flow speed of microvessel were increased at T2,3 in group HES.Compared with group RS,DO2,VO2 and ERO2 were significantly decreased at T3 in group HES.Conclusion Volume therapy with 6% HES 130/0.4 can improve mesenteric microcirculation,organ perfusion and oxygen metabolic in a rabbit model of hypovolemia.
6.The prognostic value of CD14+HLA-DRlow/-for evaluating the severity of acute pancreatitis

Objective To investigate the prognostic value of CD 4+CD25+/high CD127 low/-and CD14+HLA-DRlow/-for evaluating the severity of acute pancreatitis .Methods The percentages of CD4+CD25+/highCD127low/-and CD14+HLA-DRlow/-and the CD64 index were measured by flow cytometry in pa-tients with acute pancreatitis ( including 43 cases of mild acute pancreatitis and 24 cases of severe acute pan-creatitis).Moreover, the levels of C-reactive protein (CRP), acute physiology and chronic health evaluationⅡ( APACHEⅡ) score and CT severity index ( CTSI ) were detected for a correlation analysis .Results The percentages of CD4+CD25+/highCD127low/-and CD14+HLA-DRlow/-and the CD64 index in patients with severe and mild acute pancreatitis were significantly higher than those in healthy subjects .Patients with se-vere acute pancreatitis showed higher percentages of CD 14+HLA-DRlow/-than patients with mild acute pan-creatitis.With the disease progression, the CD64 index and the levels of CD4+CD25+/highCD127low/-, CD14+HLA-DRlow/-and CRP were significantly dropped after an initial increase in patients with mild acute pancrea -titis, while these indexes were continuously elevated in patients with severe acute pancreatitis .The percent-age of CD14+HLA-DRlow/-was positively correlated with CD64 index, CRP level, APACHEⅡ score and CTSI.Conclusion CD14+HLA-DRlow/-level was closely related to the severity of acute pancreatitis , which could be used as immune parameter for the estimation of the clinical severity of acute pancreatitis .
7.The suggestions on the entry of Chinese medicines into European Community monographs for traditional herbal medicinal products

Directive 2004/24/EC of the European Parliament and the Council entered into force on April 30th, 2004. After 7 years, there is no Chinese medicine to be registered successfully in European market as traditional herbal medicinal products. The thesis gives some ideas to tackle this problem. Procedure for the Preparation of Community Monographs for Traditional Herbal Medicinal Products (EMEA/HMPC/182320/2005) published by European Medicines Agency is an important guidance for traditional herbal products to enter European Community monographs. The thesis introduces and details the procedure as well as gives feasible suggestions about the procedure. It suggests that Chinese medicines enter European Community monographs first, and then apply the registration according to the directive 2004/24/EC. This is an easier access to European market.
8.Clinical Application of Intra-aortic Balloon Pump in Septic Shock

Objective:To analyze clinical application of intra?aortic balloon pump in the treatment of septic shock.Methods:A total of 62 patients with septic shock in our hospital were assigned into the control group and the observation group according to digital table method, with 31 cases in each group. Patients in the control group were treated with conventional antishock method and patients in the observation group were given intra?aortic balloon pump therapy based on the treatment.Blood pressure, heart rate, urine volume and adverse event in the two groups were compared after treatment.Results:After treatment,blood pressure, heart rate and urine volume of the observation group were improved significantly compared with that in the control group (P<0.05).The overall incidence of adverse events in the observation group (16.13%) was significantly lower than that in the control group (45.16%) after treatment (P<0.05). Conclusion:Intra-aortic balloon pump therapy exerts obvious effect in the treatment of septic shock, significantly improves the patient??s blood pressure and heart rate, increases urine volume, causes less adverse events,and is safe and effective.
9.Shapes, properties and microstructures of gadfly.

OBJECTIVETo search and identify the microstructure of medicinal properties of six kinds of gadfly.
METHODThe medicinal properties, different body parts and bristles microscopic characteristics were compared.
RESULTThe two-spotted yellow gadfly had a smaller size. Its length no more than 14 mm, body wall debris was yellow brown or pale yellow, the seta was smaller than that of the other gadflies; while the other five varieties' body lengths all exceeded 15 mm, and their body wall debris mostly were brown black or gray and black.
CONCLUSIONThe authentication between the two-spotted yellow gadfly and the other five varieties can be made by the microscopic characteristics of their medicinal properties, body wall debris colors, bristle lengths, and basal diameters.
