Background The reduced glutathione(GSH) is a water soluble antioxidant in retina and it is an important component of thiol.Diabetes induce the oxidative response of retina.It is very important to detect the content of thiol in retina for the evaluation of its antioxidant role.Objective This study was to investigate the effect of carnosine,aspirin and a combination of carnosine and aspirin eye drops on the change of the thiol contents in the retina of the diabetic rats. Methods Diabetic models were induced in 76 clean male Sprague-Dawley(SD) rats by intraperitoneal injection of streptozotocin(65 mg/kg).The 0.02 mi sodium citrate buffer was injected in 12 control rats.1%camosine eye drops or 0.05%aspirin eye drops were topically administered respectively in carnosine group (19 rats)or aspirin group(19 rats)two times per day for a period of 8 weeks,and the combination of 1%carnosine eye drops and 0.05%aspirin eye drops was used in the combination group at the same way(19 rats).At the end of 8 weeks.The animMs were sacrificed and the eyeballs were enucleated for the detect of thiols in the retina by colorimetry.Results The body weight of SD rats was significantly reduced and the blood glucose level was evidently arose in model group,camosine group and aspirin group in comparison with control group(P＜0.05),but those of combination group compared with control group was insignificantly changed(P＞0.05).The level of thiol in retina was(1 90.35+46.62)ms/gprot in model group,showing a considerable decrease in comparison with control (202.77+53.49 ms/gprot)(P＜0.05).However,the thiol level rose to(378.40±165.06)mg/gprot in camosine group and(351.88±211.62)mg/gprot in aspirin group,showing a statistically significant difference in comparison with control group(P＜0.05).The content of thiol in combination group compared with control group Was insignificant changed(166.972±50.287 mg/gprot us 202.77±53.49 mg/gprot).Curiously the levels in the combination group were not different from the normal control group. Conclusion The carnosine and aspirin eye drops may protect the retina against the oxidative damage caused by diabetes.

Objective To investigate the peripheral blood Th17 and CD4 + CD25 + regulatory T cell levels and their correlations in patients with primary hepatocellular carcinoma (PHC).Methods Peripheral blood samples were collected from 30 PHC patients and 25 healthy controls in the First Affiliated Hospital of Zhejiang University from June 2008 to May 2009.Mononuclear cells were isolated and the Th17 and CD4 + CD25 + regulatory T cells were detected by flow cytometry and compared between patients and controls by t test.Spearman test was performed to analyze the correlation of Th17 with CD4 + CD25 +regulatory T cell concentrations.Results The levels of Th17 and CD4 + CD25 + regulatory T cells in peripheral blood in healthy controls were (2.10 ± 0.87) ％ and (7.10 ± 2.32) ％ ; while those in PHC patients were (3.38±1.68)％ and (11.78±5.62)％ (t=3.640 and 4.162,P＜0.01).The level of Th17 cells was positively associated with that of CD4 + CD25 + regulatory T cells in PHC patients (r =0.821,P ＜0.01).Conclusion The levels of Th17 and CD4 + CD25 + regulatory T cells in peripheral blood are high in PHC patients and positively correlated with each other,which indicates that CD4 + CD25 + regulatory T cells may contribute to the disease progression and pathogenesis of carcinoma through inducing Th17 cells differentiation.

Objective To investigate new cases of endemic cretinism in high-risk areas of Inner Mongolia, the prevalence of endemic goiter, the implementation of comprehensive measures, and to provide scientific basis for developing control strategies against iodine deficiency disorders (IDD). Methods In 2007 and 2008, a search for new cretin cases was conducted among children under the age of 10 in 11 historical serious epidemic cretinism prevalent areas of Songshan district, Keshiketeng qi, Kalaqin qi, Zhalaite qi, Zhuozi county,Liangcheng county, Qingshuihe county, Helin county, Dongsheng district, Zhungeer qi and Yinjinhuoluo qi. Three to 5 towns were selected in each qi(county, district) and 3 to 5 village primary schools were selected in each town.One hundred and twenty to 200 children aged 8 to 10 were selected in each school to inspect thyroid by B ultrasound and palpation and to test intelligence and urinary iodine. Two villages were selected in each town and 30 households were selected to determine urinary iodine of housewives and salt iodine. Results A total of 56 cases of suspected cases of endemic cretinism were found from the 11 counties but no case was confirmed. The goiter rate of children aged 8 to 10 was 5.2% (309/5922) by palpation and 4.3% (252/5922) by B-ultrasound. The median urinary iodine of children aged 8 to 10 was 241.5 μg/L, and urinary iodine was 100 to 300 μg/L, accounted 65.9%(3901/5920). Median urinary iodine of the 9 Qis(counties, districts) was in the range of 200 and 300μg/L.Higher than 5% of the 11 Qis(counties, districts) with urinary iodine lower than 50 μg/L was zero. Higher than 10% of the 11 Qis(counties, districts) with urinary iodine lower than 100 μg/L was 1. The median urinary iodine of housewives was 225.6 μg/L, and lower than 50 μg/L accounted for 2.2%(35/1597). Higher than 10% of the 11 Qis(counties, districts) with urinary iodine lower than 100 μg/L was 4. A total of 2109 households were investigated and 97.45%(2055/2109) of them ate iodized salt and 2.55%(54/2109) of them ate non-iodized salt. Conclusions In the 11 investigated counties, goiter rate of children has decreased to less than 10%, and no new cretin was found. It could be concluded that the fulfillment of prevention and control of IDD is effective. The iodine nutrition of children and women of childbearing age is in an adequate level. The coverage rate of iodized salt has maintained at a higher level, the sales network is sound, the salt price is reasonable and salt is easy to get and the children's intelligence is protected effectively. But, the health education about IDD is still weak, need to be reinforced.

Objective To genotype Yersinia pestis and explore intrinsic relationship among different ecotypes of Yersinia pestis in Yunnan foci.Methods A total of 171 strains from three types of Yersinia pestis,house mouse,wild-type mouse and Yulong Yersinia pestis,were tested.Twenty-three different regions (DFR) were used to genotype and cluster analysis was performed using BioNumerics 5.0.Results A total of 171 Yersinia pestis were divided into 7 genotypes by 23 DFRs,which were Genomovar5,Genomovar7,Genomovar9 and 4 newly discovered genotypes.The genotypes of all Yulong plague were Genomovar5.The genotypes of the 16 strains of wild-type mouse plague (the highland of northwestern Yunnan Province type) were divided to 3 genotypes,13 of them were Genomovar 7,2 of them were Genomovar9,and 1 of them was newly discovered genotype Genomovaryn1.The genotypes of the 148 strains of house mouse plague(the residential area of Yunnan and Fujian Provinces type) were divided into 4 genotypes,145 of them were Genomovar9,and 3 of them were newly discovered including Genomovar-yn2,-yn3 and-yn4.The ecological typing results of clustering showed genotype of Yulong plague was similar to the highland of northwestern Yunnan Province type(wild-type mouse plague),and the percentage of similarity was up to 87.20％,but only up to 73.75％ to the residential area of Yunnan and Fujian Provinces type (house mouse plague).The genotypes of 2 wild-type strains of the highland of northwestern Yunnan Province type(wild-type mouse) and main genotypes of the residential area of Yunnan and Fujian Provinces type(house mouse)were Genomovar 9.The genotype of Genomovar-yn 1 of the highland of northwestern Yunnan Province type was similar to Genomovar 7,but lack of DFR 11.The genotypes of Genomovar-yn2,-yn3 and-yn4 of the residential area of Yunnan and Fujian Provinces type were similar to Genomovar 9,but lack of DFR 10,DFR 9 and DFR 11,respectively.Conclusions One newly genotype strain is found in wild-type mouse plague and 3 newly genotype strains are founded in house mouse plague.Wild-type mouse strains are founded in the house mouse strains.The similarity of genotype between Yulong plague and the highland of northwestern Yunnan Province type (wild-type mouse plague) is high while the similarity between Yulong plague and the residential area of Yunnan and Fujian Provinces type(house mouse plague) is low.

Objective To explore the diagnosis and treatment of tumor associated gastrocolic fistula (GCF).Methods The records of the 4 patients with GCF between August 2008 to February 2014 were retrospectively analyzed.Three female and one male patients,those whose average age were 61 years,have been pathologically diagnosed postoperatively as gastrocolic fistula caused by malignant diseases.The main clinical symptoms were diarrhea (3 cases),fecal vomitus (3 cases),weight loss (4 cases),and abdominal pain (4 cases).Positive diagnostic tests for GCF included gastroscope (3 cases),colonoscope (1 case),barium enema (1 case),upper gastrointestinal contrast (2 cases).Results En-bloc resection of the involved gastrocolic region have been performed for all,2 patients underwent radical gastrectomy and colon resection and 2 patients were taken on palliative procedure.Pathology indicated adenocarcinoma all,Immunohistochemical detection for CK20,CDX-2 were applied for identifying the originations of tumors.Delayed gastric emptying and DIC occurred in one patient who died in 3 months after the operation,anastomotic leakages were found in 2 cases.The survival patients were all discharged and taken capecitabine combined with Oxaliplatin for chemotherapy.Conclusions Endaoscopy and gastrointestinal imaging are main evidences for diagnosis of GCF.En-bloc resection of the involved gastrocolic region were recommended,enterostomy was safer than entero-anastomosis in one stage procedure.The originations of tumors may be identifying according to the pathological characteristic and CDX-2,CK20 staining.Adjuvant chemotherapy should be applied.

Objective To study the effects of salt sensitivity on evolution of blood pressure and develope- ment to hypertension from adolescents to youth.Methods A baseline survey was carried out in 4623 adolescents aged 6-15 years old in Hanzhong rural area in 1987,310 of them were recruited for determination of salt sensitiv- ity using the tests of oral saline load and furosemide sodium-volume depletion.Salt sensitivity (SS) were diag- nosed in 101 while 209 subjects as no-sah sensitivity (NSS).This cohort of adolescents were followed up for av- erage 18 years.Results The response rate for this cohort of adolescents was 71.9%.At the end of follow up period,BP in subjects with baseline SS was higher in youth than that in NSS (SBP:122.9?13.1 vs 117.3?12.4, P

Objective To analyze the related rules and prognosis analysis for lymphatic metastasis in patients for Siewert Ⅱ and Ⅲ adenocarcinoma of the gastroesophageal junction.Methods This study retrospectively reviewed 65 patients who underwent radical surgery for Siewert Ⅱ and Ⅲ adenocarcinoma of the gastroesophageal junction in Beijing Friendship Hospital between July 2013 and March 2017.The primary indicators were sex,age,tumor site,tumor size,macroscopic type,histological type,depth of invasion,operation,tumor emboli and carcinoma nodes.The clinical characteristics and pathological features were analyzed by using Logistic regression and Kaplan-Meier survival analysis.Follow-tup using outpatient examination and telephone interview was performed to detect patients' diseases and postoperative survival up to April 2017.Results Lymph node metastasis was observed in 41 of 65 patients (63.1％).Lymph node metastasis frequency was found in No.1,No.2,No.3,No.7,No.11 and No.110 lymph nodes.The incidences of those lymph node metastasis were 45.3％,32.5％,28.8％,22.5％,19.4％ and 8.2％.The metastasis rate of the tumors with diameter over 2 cm was higher than the tumors which were less than 2 cm (71.9％ vs 0;P =0.000);The observed rate of lymph node metastasis was 0,12.5％,69％,95.2％ in early cancer (T1) and advanced cancer (T2-T4) (P=0.000) respectively.The metastasis rate of highly differentiated Siewert Ⅱ and Ⅲ adenocarcinoma of the gastroesophageal junction was 45.2％ lower than poorly differentiated tumors 79.4％ (P =0.009).Thirty-three cases were found with lymphatic embolus,28 of them (84.8％) were found to be combined with lymph node metastasis among them (P =0.001).All the differences were statistically significant (P ＜ 0.05).The results of multivariable analyzed by binary logistic regression analysis revealed that neoplasms histologic type and lymphatic embolus were independent risk factors for lymph node metastasis of Siewert Ⅱ and Ⅲ adenocarcinoma of the gastroesophageal junction.Sixty-five patients were followed up for 1-45 months,with a median time of 18.81 months.Although there was no statistically significant difference,3 years-overall survival rate showed tumor without lymph node metastasis has better prognosis than with lymph node metastasis and early stage tumor also has better prognosis than advanced tumor.Conclusions Lymph node metastasis of Siewert Ⅱ and Ⅲ adenocarcinoma of the gastroesophageal junction is related with histological type of tumor and lymphatic embolus.For Siewert Ⅱ and Ⅲ adenocarcinoma of the gastroesophageal junction,the incidence of lymphatic metastasis were higher appear in abdominal cavity,which included No.1,No.2,No.3,No.7 and No.11;Meanwhile,the metastasis of mediastinal lymph node is given priority to No.110 lymph node.Therefore,radical total gastrectomy combined with standard D2 lymphadenectomy and No.110 lymphadenectomy are performed in order to improving the prognosis.For patients with lymph node metastasis and tumor in advanced stage,long-term prognosis remains to be further study.
