2.Effect of electronic colonoscopy on indicators of cardiovascular events in elderly patients

Objective To observe the effect of electronic colonoscopy on heart rate,blood pressure,arterial hemoglobin oxygen saturation and electrocardiography in elderly patients.Methods 60 patients were divided into two groups,elderly group (over 60 years old,n=30) and non-elderly group (< 60 years old,n =30).The changes in heart rate,blood pressure,arterial hemoglobin oxygen saturation and electrocardiography before and during colonoscopy were compared between the two groups.The increased heart rate was observed in both groups(the non-elderly group:t=10.746,P<0.001; the elderly group:t=4.892,P<0.001),and the increment of heart rate was higher in non-elderly group than in elderly group [(6.9±3.7) /min vs.(4.0±3.8) /min,t=2.88,P< 0.01].Results Systolic and diastolic pressures were increased in both groups during colonoscopy as compared with pre-colonoscopy(t=3.88,5.88,6.55 and 7.43,respectively,P<0.01 or 0.001),but there were no significant differences in the increment between the two groups(t=0.31 and 0.32,bothP>0.05).Arterial hemoglobin oxygen saturation was decreased in both groups during colonoscopy as compared with pre-colonoscopy(t=3.05 and 2.98,P<0.05 or 0.01),and there was no significant difference in the decrease between the two groups(t=-0.13,P>0.05).Abnormal ECG can be detected in both groups,but had no statistical significance between the two groups (20.0% vs.16.7%,x2 =0.11,P> 0.05).Conclusions Electronic colonoscopy can be performed relatively safely in the elderly.
3.Correlation analysis of macular thickness with optical coherence tomography and visual function in diabetic macular edema

Objective To explore the clinical features of diabetic macular edema (DME) with optical coherence tomography (OCT) and correlation with visual function. Methods Forty-nine eyes from 40 patients with DME (DME group) and 31 eyes from 31 patients without DME (control group) were examined with OCT,pattern reversal visual evoked potentials (P-VEP),macular perimetry. According to proliferative diabetic retinopathy (PDR), 49 eyes with DME were divided into group A (without PDR, 30eyes) and group B (with PDR, 19 eyes). Results The retinal macular thickness of central fovea in DME group [(299.25±63.87)μm] was more than that in contol group [(204.35 ± 37.94)μm], visual acuity and macular visual field in DME group were significantly different than those in control group, respectively (P < 0.05). The retinal macular thickness of central fovea,visual acuity and visual field were no significant differences between group A and group B (P>0.05). OCT macular thickness and visual correlation coefficient was -0.437(P< 0.05 ); OCT macular thickness and mean defect correlation coefficient was 0.441(P < 0.05). Conclusions OCT can provide a useful tool for monitoring the occurrence and development of DME, can assess the response to treatment. With increasing of the macular retinal thickness, the visual acuity and macular visual field of visual function are more damaged.
5.Clinical analysis of autoimmune pancreatitis: a report of 11 cases

Objective To summarize the laboratory, pathologic and imaging characteristics of patients with autoimmune pancreatitis (AIP) retrospectively for early diagnosis and treatment. Methods Eleven AIP patients were selected in our hospital from 2007 to 2009. Laboratory parameters including complete blood count, tumor markers, hemodiastase and autoantibodies were tested. Abdominal ultrasonography and computerized tomography (CT) scanning were also performed. Results Five patients had elevated hemodiastase and liver enzymes, four patients had hyperbilirubinemia and three patients had hyperglobulinemia. Increased serum CA199 levels (mean 78.4 U/ml) were found in six patients. All of the 11 patients had enlarged pancreas on imaging. Four cases were treated with three months of drug therapy. After the treatment, the pancreas volumereturned to normal size, pancreatic duct and bile duct became normal in size. Seven cases received surgical operation. Pathology examination showed fibrous tissue and folliculus lymphaticus formation in the pancreas,with a large number of lymphocytes and plasma cells infiltration. Two of seven cases were diagnosed with Sj(o)gren's syndrome and rheumatoid arthritis after being followed-up for six to twelve months post-operation.Their symptoms relieved after corticosteroid and immunosuppressive therapy. Conclusion AIP is a special type of chronic pancreatitiswith characteristic laboratory, imaging and histological features. Glucocorticoid and immunosuppressive agents therapy are effective for AIP.
6.Clinical research on the effect of abdominal Tuina and acupuncture in treating Swiss generalized anxiety disorder

Objective To observe the clinical effect of abdominal Tuina and acupuncture in treating Swiss generalized anxiety disorder (GAD). Methods 80 patients of GAD caused by liver depression-qi stagnation were randomly recruited into a treatment group and a control group, 40 patients in each. The treatment group was treated with abdominal tuina & acupuncture; while the control group was treated with acupuncture exclusively. Clinical effects and hamilton anxiety scale (HAMA) score were observed in both groups before and after the treatment. Results ①There was significant difference in total efficacy between the two groups CP<0.05), and the treatment group was better than the control group. ②The HAMA score of the two groups after the treatment was lower than that before the treatment, and the difference was significant (P< 0.05). There was also significant difference of HAMA score between the two groups after the treatment (/><0.05), with the treatment group showing better results than the control group. Conclusion Abdominal tuina combined with acupuncture has sound effects in treating Swiss GAD due to liver depression-qi stagnation.
7.Study on extracting process and formulation of Mashi Granules

Objective: To ascertain the process condition of extraction Mashi Granules(Herba Ephedrae, Gypsum Fibrosum, Radix Puerariae, Herba Methal, Semen Armeniacae Amerum, etc.) Methods: In combination with area under the curve of hypothermy in rabbits, process water, macerating time and boiling times were used as parameters in uniformidy design. And then appropriate excipients were selected on the basis of hygroscopicity and granulation. Results: 20 times amount of water as much as herbs was added, macerating time reached as far as 40 min, and extracted 3 times, 2h a time. Excipient formula consisted of lactose and mannitol(4∶1, w/w). Conclusion: The experimental results provide the basis for the ascertainment of extracting process and formulation of Mashi Granules.
8.The effect of different times of He-Ne laser irradiation on healing of rabbit achilles tendons

Objective To evaluate the influence of duration of laser irradiation on histomorphometric meas- urements in experimentally tenotomized and repaired rabbit Achilles tendons and to explore the best irradiation time. Methods A total of 20 male New Zealand rabbits aged 10-12 weeks were used and randomly divided into 4 groups:a control group and three experimental groups.All the animals underwent surgical excised and then repair of their Achillis tendon.The animals in the control group were then treated with sham laser irradiation,while those in the three experimental groups were treated with 10,20 and 30 minutes of He-Ne laser irradiation(632.8 nm, 18.9 mW)daily,respectively,for 14 days.On the 28th day after surgical operation,the animals were sacrificed and their Achilles tendons were sampled.HE stain and Van Geison stain were used to observe morphometric changes of tendons.The SDS-PAGE electrophoresis method and CS-930 photodensity scan instrument were employed to measure the content of typesⅠandⅢcollagen.Results it was shown that laser irradiation enhanced cell proliferation,cel- lular content,granulation tissue formation and collagen deposition in laser-treated tendons,especially in those irradia- ted for 20 minute daily,as compared to the control group.TypeⅠand typeⅢcollagen levels were significantly in- creased at the 28th day in the healing tendons and the ratio of collagenⅢtoⅠincreased in all the 3 experimental groups,and the increase of both collagen content and ratio of collagen typeⅢtoⅠwas significantly greater in those ir- radiated 20 minutes daily(P
9.Value of histogram analysis of susceptible signal intensity in differentiating papillary from chromophobe renal cell carcinoma

Objective To evaluate the feasibility of histogram analysis of susceptible signal intensity in differentiating papillary (pRCC) from chromophobe renal cell carcinoma(ChRCC). Methods Thirteen cases with pRCC and 9 cases with ChRCC, who underwent susceptible-weighted imaging(SWI), MR common scanning and enhancement, were assessed retrospectively. The histogram parameters of susceptible signal intensity were measured, including maximum, minimum, mean, median, skewness and kurtosis. The independent samples t test(normal distribution) and Mann-Whitney rank sum test(skewed distribution) were used to compare the differences in SWI parameters between pRCCs and ChRCCs. Receiver operating characteristic curve was used to evaluate the efficiency of the whole-tumor SWI parameters in differentiating pRCCs from ChRCCs. Results The significant differences of the minimum, mean, median and skewness between pRCCs and ChRCCs were present(P<0.05), and there was no significant differences in maximum and kurtosis between between pRCCs and ChRCCs(P>0.05). In all the histogram parameters, the area of ROC curve of mean value was largest(0.80). The sensitivity of minimum value (84.62%) was the highest and and the specificity of skewness(100.00%) was the highest. Conclusion Histogram analysis of susceptible signal intensity can help differentiate pRCCs from ChRCCs.
10.Efficacy and safety of caspofunjin on patients with refractory invasive fungal infections

Objective To evaluate the efficacy and safety of Caspofunjin on invasive fungal infections in patients refractory to Fluconazole in ICU.Methods The open clinical trial was conducted in 44 invasive fungal infections patients who was refractory to Fluconazole,including 7 cases of confirmed,28 cases of suspected and 9 cases for empire treatment.They were treated with Caspofunjin in a dose of 70 mg in the first day,and then in a dose of 50 mg/d for 10~40 d.Results The cure rate of patients was 42.86%,the effective rate was 76.19%,and the eradication rate was 65.22%.All patients were well tolerated.Conclusion Caspofunjin is effective and safe in the treatment of severe invasive fungal infections.
