1.EFFECTS OF OPEN SURGERY AND ENDOVASCULAR GRAFT EXLUSION FOR ABDOMINAL AORTIC ANEURYSMS ON LIVER AND RENAL FUNCTIONS

To compare the effects of open surgery (OS) and endovascular graft exclusion (EVGE) for abdominal aortic aneurysms on the liver and renal functions, the serum total protein, albumin, globulin and the ratio of A/G of 57 patients with alodominal aortis aneurysm were analyzed before and after the two operations.The results showed that the serum total protein,albumin,globalin and the ratio of A/G were signifcantly decreased in the open surgery group, while remarkabe decrease was found in globulin but no significant change in the ratio of A/G in the EVGE group.Creatinine and urea introgen were significantly decreased in the open surgery graup and remained normal in the EVGE group.It is suggested that the effect of EVGE on the liver and renal function is much less than traditional(open) surgery.
2.ENDOLEAKS AFTER ENDOVASCULAR GRAFT EXCLUSION FOR THORACIC AORTIC DISSECTION

Thirty two patients with Stanford B thoracic aortic dissections accepted endovascular graft exclusions. Four endoleaks occurred after the stent grafts were deployed. Three were cured with additional endovascular stent graft placement, while one sealed spontaneously. It is concluded that many factors are related to the occurrence of endoleaks. The detecting methods of intraoperative DSA and postoperative spiral CT of the thoracic aorta are effective on revealing endoleaks. It is advocated that additional endovascular procedures should be done immediately after the endoleaks are detected. Low volume of the leakage may seal spontaneously. But the topic of the mamagement and prognosis of delayed endoleaks is to be studied.
3.PREOPERATIVE EVALUATION OF THE TEARS OF THORACIC AORTIC DISSECTIONS BEFORE ENDOVASCULAR GRAFT EXCLUSION

To evaluate the significance and methods of preoperative revealing of the tears of thoracic aortic dissections before endovascular graft exclusion, 32 cases were studied. Among them, 26 cases were evaluated with spiral CT only, 5 with CT and MR together, while one with MR only. Compared with intra operative Digital Subtraction Angiography, the detectable rate of CT,MR and DSA for revealing the tears was 90 32%, 66 7%, and 100% respectively.The most frequently appeared locations of the tears were at the section between 3 and 7 o'clock. In the case of multi entrances, the results evaluated with CT were different from those with DSA. It is concluded that accurate evaluation of the tears would be profitable to stent grafts selection and operative decision, and spiral CT images are most effective in preoperative evaluation of the tears.
4.THE VALUE OF DSA IN ENDOVASCULAR GRAFT EXCLUSION FOR DISSECTING THORACIC AORTIC ANEURYSMS

In order to evaluate digital subtraction angiography(DSA) in the management of Endovascular Graft Exclusion (EVGE) for dissecting thoracic aortic aneurysms,the operation procedures and results in 32 cases of EVGE were retrospectively analysed. The stent grafts were sent to the segment where the tears were located through the incision on the common femoral artery. After the deployment, the tears were sealed. Intraoperatively, 100% of the tears were revealed by DSA, and 4 endoleaks were also detected and managed effectively. The value of DSA in preoperative evaluation, choice of operative procedure, intraoperative monitoring, success ratio evaluation, complication prevention and management was determined. It is concluded that DSA can monitor EVGE on real time and throughout the operation. It can meet the needs of measuring, locating, evaluating thoracic aortic aneurym and deciding its operation,as well as detecting and controlling its complications. Moreover, it provides a large visual field and operating space, and is a high valuable monitoring method for EVGE.
5.Role of vascular remolding induced by nitric oxide and matrix metalloproteinase in aneurysm

6.CONSTRUCTION OF A NOVEL CANINE ABDOMINAL AORTIC ANEURYSM MODEL

To construct a steady canine abdominal aortic aneurysm model to study endovascular graft exclusion, fifteen hybrid canines(averaged 13 5kg) accepted bovine jugular vein segments replacement at infra kidney abdominal aortic to construct aneurysms, while lumbar arteries were reserved. All the fifteen aneurysms had satisfactory figures as clinical patterns. The aneurysm parameters were proximal neck diameter(6 0?0 8)mm, distal neck diameter(5 7?0 6)mm, aneurysm sac diameter(22 7?13 3)mm, length(36 1?17 9)mm. It is concluded that the designation of this model is suitable for endovascular graft exclusion,and can be applicated in many procedures.
8.Construction of a canine endoleak model in the course of endovascular exclusion for abdominal aortic aneurysm

Objective To design an experimental model of endoleak after endovascular exclusion(EVE). Methods Infrarenal aortic aneurysms were created with bovine jugular vein segements or patches. Then they were undergone endovascular stent graft exclusion of the aneurysm. Using modification of prosthetic vessel and changing the attachment site, endoleaks were formed druing the course of aneurysm exclusion. Results All the 6 aneurysms possessed satisfactory configuration just as clinical patterns. Intraoperative arteriography revealed endoleaks in 5 dogs after the exclusion, two of which were proximal and three were distal.Conclusions This experiment shows the hemodynamics and treatment of endoleak for EVE.
9.Choice of intraocular lens power calculation formula after refractive corneal surgery

To the cataract patient after corneal refractive surgery, if the intraocular lens power was determined by conventional formulas, different refractive errors may be produced after intraocular lens implantation.This inaccuracy of the lens power mainly came from 2 aspects-errors of corneal refractive power measurement and errors of formulas.Besides, imprecise of ocular axial length measurement and invalid lens position calculation also contributed to the inaccuracy of lens power.Therefore, in order to reduce refractive errors of cataractous surgery in patient underwent refractive surgery, appropriate method should be used to estimate the refractive power of corneal, and proper formula should be selected to calculate intraocular lens power.
10.Microsurgical treatment of hypertensive basal ganglia hemorrhage via lateral fissure approach

Objective To study the clinical effect of microsurgical treatment via lateral fissure approach on hypertensive basal ganglia hemorrhage.Methods Small bone flap via lateral fissure approach were applied to remove basal ganglia hematoma.Thirty two cases were recruited in the retrospective analysis.Results By head CT scan follow-up 24 h post operation,over 80% percent of hematoma was successfully removed in 28 cases and over 40% of hematoma was resided in 4 case.Patients were followed up for 3 - 6 months,according to the ADL (ADL)classification,the outcomes were:Grade Ⅰ in 4 cases( 12.5% ),grade Ⅱ in 13 cases(40.6% ),grade Ⅲ in 12 cases (37.5%)and grade Ⅳ in 3 cases (9.4%).No death occurred.Conclusion Small window approach on lateral fissure for microsurgical operation of hypertensive basal ganglia hemorrhage is a less invasive procedure with good surgical exposure and homeostasis.Neurological function was well recovered after surgery.
