1.Expression level of Annexin Ⅱ in lung cancer tissue, bronchoalveolar lavage fluid and pleural effusion and its value in the diagnosis of lung cancer

Objective To investigate the expression level of Annexin Ⅱ in bronchial lung cancer tissue,alveolar lavage fluid(BALF) and pleural effusion and its value in the diagnosis of lung cancer.Methods From January 2014 to March 2016 in Jingzhou Infectious Disease Hospital,80 cases of patients with pleural effusion of lung cancer (Lung Cancer Group),40 patients with benign pleural effusion (benign group) were chose.The expression level of Annexin of two groups of patients with pleural effusion,BALF were detected and compared.Sixty specimens of department of pathology past (after lung cancer surgery cancer) and 30 cases of previously collected normal lung tissues (normal lung tissue) were collected,differential expression by immunohistochemical staining were compared between the two groups of Annexin Ⅱ protein.Results The positive expression of Annexin Ⅱ in lung cancer was 61.67% (37/60),higher than that in normal lung tissue of 23.33% (7/30),the difference was statistically significant(P<0.001).The positive expression of Annexin Ⅱ in lung cancer patients was significantly correlated with rate of lymph node metastasis and clinical stage of lung cancer(P =0.039,0.029).Group of patients with pleural effusion,BALF expression of Annexin Ⅱ the level was higher than that in benign group(P<0.001).When the critical value of Annexin in pleural effusion of lung cancer diagnosis was 10.94 μg/L,the sensitivity of diagnosis of lung cancer was 83.47%,the specificity was 80.66%,AUC value was 0.837.The critical value when BALF Annexin Ⅱ in the diagnosis of lung cancer was 52.08 μg/L,the sensitivity of diagnosis of lung cancer was 86.19%,the specificity was 89.22%,the AUC value was 0.898.Conclusion The expression level of Annexin Ⅱ in the BALF and pleural effusion of patients with bronchial lung cancer and lung cancer is increased,and it has a certain clinical value for the differential diagnosis of lung cancer.

4.Ambulatory urodynamics monitoring system in diagnosis of female stress urinary incontinence

Objective To compare the diagnostic accuracy of conventional urodynamic (CUD) and ambulatory urodynamic monitoring (AUM) in diagnosis of female stress urinary incontinence (SUI).Methods Forty women with SUI were prospectively enrolled and performed urodynamic studies after the ICI-Q-SF questionnaire.According to clinical symptoms,patients were divided into three groups:mild,moderate and severe group.Half patients in each groups performed CUD exam,and the other half of patients performed AUM exam.And two micturition cycles were recorded during AUM.Results There were no significant differrences in age,pregnant production times and ICI-Q-SF score between two groups.Three patients with SUI symptoms had negative findings in AUM group and 15 patients in CUD group (P<0.05).Among women with SUI,1 1 patients had positive findings in AUM group and 2 patients in CUD group (P< 0.05).Conclusion AUM can provide objective evidence for the majority of patients with SUI than CUD.
5.Clinical evaluation of IVP,CTU and MRU in the diagnosis of urinary system stones

Objective:To explore the clinical application in urinary system stones of the inspection technologies intravenous pyelography (IVP),computer tomography urography (CTU) and magnetic resonance urography (MRU). Methods:We collected 100 cases of urinary system stones on which 80 were carried out to IVP,60 to CTU and 35 to MRU,25 patients were implemented all 3 examinations. CTU and MRU are reconstructed with multi-level reorganization (MPR),curved projection reproduction (CPR),volume reproduction (VR) and the maximum intensity projection (MIP). Results:Stones were found in 73 patients of the 80 people by IVP,the diagnosis rate is 91.3 %. Stones were found in all the 60 patients except 1 by CTU with images direct and accurate,the diagnosis rate was 98.1 %. By the MRU,stones were found in 20 of the 35 patients at a display rate of 57.1 %,with images displayed in a low stone location signal,filling defect and the hydrocele circs of renal pelvis and ureter. Conclusion:IVP is a simple method,easy-to-patient acceptance and has a high detection rate of stones; CTU has a good image quality and the detection rate is higher than the stone IVP. By combining with the original cross-sectional image,the rate of diagnosis is up to 98%; as a non-invasive and no contrast agent approach,MRU is also very important in the diagnosis of stones.
6.Effect of isoflurane on muscle relaxation produced by rocuronium

Objective To evaluate the potentiation of neuromuscular block produced by rocuronium during isoflurane anesthesia. Methods Sixty ASA Ⅰ -Ⅱ patients (39 male, 21 female), aged 18-56 yr, weighing 42-88 kg undergoing ENT, oral or facial plastic surgery were enrolled in this study. The patients were premedicated with atropine 0.01 mg?kg-1 . Anesthesia was induced with diazepam 0.1 mg?kg-1 , pethidine 1mg?kg-1 and droperidol 50?g?kg-1. Oro- or naso-tracheal intubation was accomplished under topical anesthesia. After intubation the patients received hydroxydione 2.5 g and fentanyl 5 ?g?kg-1 . The patients were mechanically ventilated. Anesthesia was maintained with inhalation of 50% N2O in O2 alone (groupⅠ) or combined with 0.6% isoflurane (group Ⅱ ) or 1.2% isoflurane (group Ⅲ ) . A bolus of rocuronium 0.6 mg?kg-1 was given during maintenance of anesthesia. Neuromuscular function was monitored using TOF response measured by accelerography (Biometer, Denmark). (1) Onset time (time from the end of injection to maximum depression of T1 ) , (2) the duration of T1 = 0, (3) time from the end of injection to recovery of T1 to 25% , 50% and 90% control, (4) recovery index (time from T, 25%-75%) were recorded. End-tidal isoflurane concentration was monitored.Results The onset time was significantly longer in group Ⅰ[ (1.7?0.2) min] than that in group Ⅱ[ (0.9?0.1) min] and groupⅢ[(0.8?0.1) min] ( P
7.Effect of addition of various doses of fentanyl intrathecally on perioperative analgesia and subarachnoid block characteristics in inguinal hernia repair surgery

ObjectiveTo study the effect of adding various doses fentanyl to 0.75% bupivacaine for spinal anesthetic in the population undergoing inguinal hernia repair surgery. MethodsThe population of 100 patients belonging to ASA class Ⅰ and Ⅱ ,scheduled for elective inguinal hernia repair, were randomized to receive a spinal anesthetic with 2 ml 0.75% bupivacaine and 1.0 ml saline (group B),or 50 μg/ml fentanyl 0.1,0.2,0.4,0.6 ml(group BF5, group BF10, group BF20,group BF30),each group with 20 cases.Subarachnoid block was established in the left lateral position in L2-3 space by use of a midline approach. The volume of injected drug was kept constant at 3 ml. The time to the highest level of sensory block, the time for regression until T12 level, the time for complete motor recovery, the maintenance duration of the analgesia,the anesthesia effect of the groups, duration of surgery, drug-related side effects were assessed and recorded.ResultsThe time of the highest level of sensory block in group BF30 was shorter than that in group B,group BF5, group BF 1 0, group BF20 (P < 0.05 ), but group B and group BF5 was longer than group BF10,group BF20, group BF30(P < 0.05 ). The time of regression to T12 level in group BF20 was longer than that in group B, group BF5, group BF10, group BF30 (P<0.05),but group B and group BF5 was shorter than group BF10, group BF20, group BF30(P< 0.05 ). The time for complete motor recovery and the maintenance duration of the analgesia in group B and group BF5 was shorter than that in group BFI0,group BF20, group BF30 (P < 0.05 ). The occurrence of pruritus in group B (0) was lower than that in group B F5 (8 cases),group BF10 (3 cases), group BF20 (6 cases), group BF30 (6 cases)(P<0.05). There were no cases of respiratory depression in five groups. ConclusionsIt suggests that in the population receiving spinal anesthetic for hernia repair surgery, addition of 10 μ g fentanyl to 0.75% bupivacaine significantly improves the quality and duration of analgesia. No further advantage occurs if the dose of fentanyl is increased among 30 μg.

Objective To evaluate if the enteral nutrition have effect on the immune function and in- flammatory reaction after operating about gastric carcinoma.Methods 58 postoperative patients suffering from stomach cancer and colon cancer were randomly divided into the EN group and the TPN group.On the first postoperative day,nutrition fibre were given via nasal intestinal tube,increasing the capacity and drop- ping speed day by day until patients can eat liquid diet.While patients in the TPN group didn't eat anything until enterokinesia completely recovered.Observing on preoperative day 1 and on postoperative day 3 and day 8 respectively to check IgA,IgG,IgM,C3,C4,CRP,LYM,LYM%,TP,ALB,PA.Results The results showed that on the postoperative day 8,the target ascension extent was higher than that in the PN group.The statistical significance was very obvious(P
9.Expression of hypoxia-inducible factor 1α, vascular endothelial growth factor and protein kinase B in lichen planus lesions

Objective To explore relationships of expression of hypoxia?inducible factor?1α(HIF?1α), vascular endothelial growth factor(VEGF)and protein kinase B(P?Akt)with angiopoiesis and cell apoptosis. Methods Biopsy specimens were collected from skin lesions of 32 patients with lichen planus and normal skin of 20 patients with lipomyoma, and subjected to paraffin embedding. Immunohistochemical staining was performed to measure expression of HIF?1α, VEGF and P?Akt, and TUNEL technique was used to detect apoptosis of keratinocytes in these paraffin?embedded tissue sections. Microvessel density (MVD)was assessed by counting CD34?labeled vascular endothelial cells. Results HIF?1α, VEGF and P?Akt were moderately or strongly expressed in lichen planus lesions, but absent or weakly expressed in normal skin of controls, and the expression of HIF?1α, VEGF and P?Akt was significantly higher in the lichen planus group than in the control group (all P < 0.01). HIF?1α was mainly expressed in nuclei of keratinocytes, while VEGF and P?Akt were expressed in the cytoplasm of keratinocytes. In addition, the lichen planus group showed significantly increased MVD(21.27 ± 6.54 vs. 10.26 ± 1.10 microvessels/high?power(200 ×)field, t = 5.607, P < 0.01)and apoptosis rate of keratinocytes(72.81% ± 9.234% vs. 28.16% ± 3.464%, t = 8.431, P < 0.01) compared with the control group. Pearson correlation analysis showed that there were positive correlations between HIF?1αand VEGF expression, between VEGF and P?Akt expression, and between P?Akt and HIF?1αexpression in the lichen planus group(r=0.625, 0.453, 0.455, respectively, all P<0.01), and expression of HIF?1α, VEGF and P?Akt was all positively correlated with MVD(r=0.721, 0.646, 0.671, respectively, all P<0.01). Conclusion HIF?1αand its downstream target genes VEGF and P?Akt may play a certain role in the occurrence of lichen planus.
10.A Case-control Study on Relationship between Helicobacter pylori Infection and Duodenal Brunner ’s Gland Hyperplasia

Background:The incidence of duodenal Brunner’s gland hyperplasia is low and the etiology and pathogenesis has not yet been clarified. Aims:To investigate the relationship between duodenal Brunner’s gland hyperplasia and Helicobacter pylori( Hp ) infection. Methods:A total of 96 patients with duodenal Brunner ’s gland hyperplasia diagnosed by pathological examination from October 2005 to December 2013 at People’s Hospital of Xinjiang Uygur Autonomous Region were enrolled in this study,and 270 cases without duodenal Brunner’s gland hyperplasia were served as controls. Hp infection was detected by 14 C-urea breath test( 14 C-UBT). The relationship between duodenal Brunner’s gland hyperplasia and Hp infection was analyzed. Results:In the duodenal Brunner ’s gland hyperplasia group,75 patients were Hp positive,the proportion of Hp infection was 3. 57( 75/21 ). In the control group,110 cases were Hp positive,the proportion of Hp infection was 1. 45(160/110). The difference between the two groups was statistically significant(χ2 =10. 97,P=0. 001). Hp infection was a risk factor of the genesis of duodenal Brunner’s gland hyperplasia( OR=2. 46, 95% CI:1. 44-4. 19). Conclusions:Hp infection may be related with the genesis of duodenal Brunner’s gland hyperplasia.
