1.Research on Prevention and Treatment of Cardiovascular Disease by Translational Medicine Based Chinese Medicine.

Translational medicine is inevitable in the development of modern medicine, and the uprising concept of translational medicine provides an opportunity for the development of Chinese medicine (CM). Their ideas are well communicated. There are two patterns of researching on CM based on translational medicine: 'literature to bench to bedside' and 'bench to bedside to bench'. CM has her advantages in preventing and treating cardiovascular disease. Effective methods for preventing and treating cardiovascular disease by CM should be further studied based on translational medicine concepts.

Objective The infection of hepatitis B virus(HBV),which is a hepatotropic DNA virus,can cause acute or chronic viral hepatitis and further may develop liver fibrosis,cirrhosis and even hepatocellular carcinoma. In recent years,with the establishment of HBV transfection cell and animal models,the anti -hepatitis B virus effect of ingredients extracted from many traditional Chinese medicine has been proved.Therefore,chemical composition extracted from the resourceful library of traditional Chinese medicine,with high efficiency and low toxicity,might be the direction for developing anti -hepatitis B virus medicine.

5.Clinicopathological analysis of a case of adult Kasabach-Merritt syndrome

A 52-year-old female presented with unexplained thrombocytopenia for 29 years and a prunosus plaque on the right cheek for 9 years.She had ever been treated with glucocorticoids,which resulted in a slight improvement of the condition,but the platelet count remained below the normal range.During glucocorticoid tapering,she had administrated traditional Chinese medicines for a long peroid.Recently,the plaque on the cheek became swollen and painful with a progressive decrease in platelet count.Routine blood test showed that the count of white blood cells and platelets was 3.0 × 109/L and 2 × 109/L respectively,and the concentration of hemoglobin was 85 g/L.Computed tomography (CT) scan revealed multiple liver hemangiomas,one of which in the left liver lobe measured 14 cm × 5.7 cm in size.After medical treatment,the giant hemangioma in the left lobe was resected.Thereafter,the platelet count increased to 120 × 109/L with the subsidence of swelling of the right cheek.Postoperative pathological examination confirmed a diagnosis of giant cavernous hemangioma of the liver with extensive intravascular thrombosis and thrombus organization.Based on these findings,the case was diagnosed as adult Kasabach-Merritt syndrome.

7.Study on the mechanism of reversal effect of Curcuma Wenyujin on MDR of SGC7901/VCR.

OBJECTIVETo study the mechanism of reversal effect of Curcuma Wenyujin n-Butyl alcohol extract (CWNAE) on multiple drugs resistance (MDR) of SGC7901/VCR cells.
METHODSSGC7901/VCR cells were co-culured with different concentrations CWNAE (80, 40, and 20 μg/mL) and Verapamil (VP, 10 μg/mL) for 24 h, and then acted with Adriamycin (ADM) for 1, 2, and 4 h, respec- tively. SGC7901/VCR cells with no intervention were taken as the vehicle control group. SGC7901/VCR cells treated with ADM alone were taken as the control group. The effect of CWNAE on intracellular ADM concentration was detected by flow cytometry (FCM). Cells were treated as mentioned before without any intervention of ADM. SGC7901/VCR with no ADM intervention were taken as the control group. The effect of CWNAE on the expression of P-glycoprotein (P-gp), lung resistance protein (LRP), and glu- cosylceramide synthase (GCS) was studied by Western blot. The effect of CWNAE on the location and expression quantity of P-gp was further illustrated by immunohistochemistry (IHC).
RESULTSCompared with the ADM group, the expression ratio obviously increased in the W80, W40, W20, and VP10 groups with statistical difference (all P < 0.05). The comparative expression quantity of P-gp, GCS, and LRP in SGC7901/VCR cells was obviously higher than that of non-MDR with statistical difference (all P < 0.05). The expression quantity of P-gp and GCS could be obviously down-regulated by 80 and 40 μg/mL CWN- AE, and 10 μg/mL VP, with no effect on the expression of LRP. Results of IHC proved that P-gp was mainly expressed on the cytomembrane or in the plasma, and it was also expressed on the nuclear membrane. P-gp in different locations could all be down-regulated by CWNAE.
CONCLUSIONSCWNAE could reverse the MDR of SGC7901/VCR cell line probably by inhibiting the expression of P-gp and GCS. CWNAE had no effect on LRP that also highly expressed on SGC7901/VCR. So we supposed that CWNAE could become a potential drug to reverse MDR of highly expressed P-gp and GCS.

8.Analysis of one-stage surgical treatment of rotator cuff tear combined with shoulder stiffness

Objective:To compare the outcomes of arthroscopic rotator cuff repair in patients with or without stiffness.Methods:Retrospective analysis was performed on the data of 20 patients (stiffness group) who underwent arthroscopic rotator cuff repair combined with capsular release from January 2017 to November 2019. There are 9 males and 11 females, age 62.2±8.7 years old (range 45 to 80 years old), preoperative duration 5.2±4.3 months (range 1 to 12 months). 54 patients who underwent arthroscopic rotator cuff repair without stiffness were used as the control group. There were 16 males and 38 females, aged 60.9±9.1 years old (range 46 to 81 years old), preoperative duration 8.2±13.0 months (range 1 to 60 months). Pain, function and range of motion (ROM) were recorded at 1 month, 3 months, 6 months post-operatively and last follow-up, and compared with that of pre-operatively. Satisfaction and complication were recorded at the last follow-up. The main outcome measurements included the visual analogue scale (VAS), the American Shoulder and Elbow Surgeons (ASES) score, the University of California at Los Angeles shoulder rating scale (UCLA), Constant-Murley score, and range of motion.Results:The mean follow-up period was 19.7±4.6 months (12-30 months) in the stiffness group, and 18.6±4.4 months (12-29 months) in non-stiffness group. VAS, ASES score, UCLA score, Constant-Murley score and ROM were significantly improved post-operatively. One month post-operatively, the VAS score was 4.2±1.5 in stiffness group, and 3.4±1.1 in the non-stiff group, and the difference was statistically significant between two groups ( t=2.381, P=0.020). There was no significant difference at 3 months post-operatively. The ASES score, UCLA score and Constant-Murley score were 52.3±10.2, 17.8±4.2 and 51.7±9.7 in stiffness group at 3 months post-operatively, and 57.4±7.4, 21.6±3.8, 63.2±13.5 in non-stiffness group, respectively. The difference was statistically significant ( t=2.363, P=0.021; t=3.713, P<0.001; t=3.484, P<0.001). There was no significant difference at 6 months post-operatively. The postoperative satisfaction of stiffness group and the non-stiffness group were 95.0% and 96.3%, respectively. The difference was not statistically significant ( χ2=0.2511, P=0.802). Conclusion:The results of stiffness group are statistically the same as those in non-stiffness group at last follow-up, although the recovery period is longer in patients who combined with stiffness at the first 6 months.
9.Laparoscopic D3 lymphadenectomy combined with pelvic autonomic nerve preservation for rectal cancer

Objective To investigate the feasibility of laparoscopic D3 lymphadenectomy combined with pelvic autonomic nerve preservation for rectal cancer.Methods The clinical data of 34 patients with rectal cancer who had been admitted to OUr hospital from January 2007 to December 2007 were retrospectively analyzed.All patients underwent laparoscopic D3 lymphadenectomy combined with pelvic autonomic nerve preservation.The operation time,lymph node dissection,postoperative complications were assessed.The erectile function of male patients and the short-term outcome of the operation were evaluated.Results All patients were successfully operated on,and no conversion Was required.The mean operation time,blood loss,number of lymph node dissected,time of catheterization,mearl volume of residual urine and CEA were(265+46)minutes,(123±27)ml and 19±3,(5.5±1.6)days,(22.5±7.8)ml and(8.0±4.6)U/L,respectively.The incidence of postoperative complications and male sexual dysfunction were 9%(3/34)and 14%(3/21),respectively.No local recurrence or distal metastasis Was observed at the end of August 2008.Conclusions Laparoscopic D3 lymphadenectomy combined with pelvic autonomic nerve preservation for rectal cancer is relatively safe and feasible.
10.Effects of different cryopreservation methods on the ultrastructure and viability of amniotic membrane

BACKGROUND: There are currently many cryopreservation methods for the aminotic membrane, which have varying effects on the ultrastructure and biological activity of amniotic membrane, but on no one is effective.
OBJECTIVE: To compare the effects of different cryopreservation methods on the ultrastructure and viability of aminotic membrane and to seek the ideal cryopreservation method.
METHODS: Aminotic membrane separated from the fresh placenta was preserved respectively with deep-frozen cryopreservation and vitrification, and everyway was run for 3 and 6 months. Fresh aminotic membrane was used as control. The ultrastructure of aminotic membrane was observed by transmission electron microscopy, and the viability of aminotic membrane was assessed by microcomputer analysis system for biological oxygen consumption, and immunohistochemical staining combined with image analysis system was used for lactate dehydrogenase activity.
RESULTS AND CONCLUSION:After 3 and 6 months of crypreservation, the damage to the ultrastructure of aminotic membrane by vitreous cryopreservation was slighter than that of amniotic membrane cryopreserved at-80℃. Compared with the fresh aminotic membrane, the gray value of lactate dehydrogenase and partial pressure of oxygen were significantly decreased in the cryopreserved aminotic membrane by deep-frozen cryopreservation at 3 and 6 months (P < 0.05) and by vitreous cryopreservation at 6 months (P < 0.05), but there was no statisticaly significant difference in the change rate of oxygen partial pressure and the gray value of lactate dehydrogenase between the fresh aminotic membrane and the cryopreserved aminotic membrane by vitreous cryopreservation at 3 months. The present study led to the conclusion that vitreous cryopreservation protocol alows to not only maintain the integrity of AM, but also to preserve the viability of the cels. So the vitreous cryopreservation is superior to the deep-frozen cryopreservation for cryopreservation of aminotic membrane.
