3.Adjuvant epirubicin and gemcitabine sequential perfusion therapy for non-muscle invasive bladder tumor after transurethral resection

Objective To evaluate the safety and efficacy of epirubicin (EPI) and gemcitabine (GEM) alternating sequential intravesical chemotherapy after transurethral resection in the treatment of non-muscle invasive bladder cancer.Methods 240 patients with primary non-muscle invasive bladder urothelial carcinoma were randomly divided into 2 groups.There were 120 cases for each group,EPI group was given EPI 50 mg (once a week),bladder perfusion,while the EPI+GEM group was given EPI 50 mg (once every other week),GEM 1000 mg (once every other week),alternating sequential perfusion.The follow-up time ranged from 6 to 24 months when the time of tumor recurrence and adverse reactions of chemotherapy were observed and recorded.Results The 2-year tumor free survival rate for EPI group recurrence was 60.0 % (72/120),and 75.0 % (90/120) for EPI+GEM group.There were statistical significance between the differences of the 2 groups (x2 =5.489,P < 0.05).3 cases in EPI group and 2 cases in EPI+GEM group progressed to muscle invasive bladder cancer,and there was no statistical significance between the differences of bladder cancer progression rates for the 2 groups (2.5 % and 1.7 %) (x2 =0.000,P < 0.05).The main adverse reaction during the treatment was gastrointestinal discomfort (15 cases and 6 cases respectively),no serious haematological toxicity and other adverse reactions were frequent urination,urgency,dysuria and hematuria.The differences between the occurrence rates of adverse reactions for the 2 groups were 25.0 % and 11.7 %,which were statistically significant (x2 =6.252,P < 0.05).Conclusion For non-muscle invasive urothelial bladder carcinoma,the curative effect is better to use EPI and GEM sequential intravesical chemotherapy than to use EPI alone,which can not only reduce the 2 years recurrence rate after the operation,but also reduce the incidence rate of adverse reactions.Yet,this method cannot change the progress of bladder cancer.
4.Construction and application in renal cell carcinoma 769-P cells of miR-218 expression plasmids

Objective To construct a stable expression plasmids miR-218,to lay the experimental foundation for the expression and mechanism of miR-218 in human renal cell carcinoma.Methods The primers were designed by Has-miR-218 and the miR-218 fragment were amplified by PCR,and then which was connected with the carrier pcDNA3.1 (+) to build a stable expression plasmids.The recombinant plasmids were trasfected into renal cell carcinoma cell line 769-P,and the expression of miR-218 in these cells were detected.Results The plasmids were constructat successfully,which was determind by enzyme digestion and sequencing results.The constructed expression plasmids pcDNA3.1-miR-218 transfection of human renal cell carcinoma cell line 769-P,miR-218 cxpression level of cells was significantly increased after trasfected by recombinant plasmids carrying miR-218 gene (relative expression quantity was 1.64).Conclusion The miR-218 expression plasmids pcDNA3.1-miR-218 were successfully constructed,the plasmids can be applied to the study of miR-218 function and mechanism in renal cell carcinoma.
5.Clinical research of the ankle-brachial index in the elderly patients with coronary heart disease with metabolism syndrome

Objective To investigate the relationship between the ankle-brachial index (ABI)and the extent of coronary artery stenosis and the left ventricular function in elderly patients with coronary heart disease (CHD)with metabolism syndrome (MS). Methods A total of 140 patients with coronary artery disease was study subjects. In which, 66 cases complicated MS (MS group), 74 cases without MS (non MS group). ABI and the left ventricular function was examined in an patients. Results ABI was 0.90±0.32 in MS group, and 1.03±0.26 in non MS group, there was significant difference between two groups(P=0.015). Compared with non MS group, significant lower ratio of ABI in MS group [43.9%(29/66) vs 27.0%(20/74)] (P=0.038); whereas much more 3-vessel disease in ABI≤0.9 patients compared with those of ABI > 0.9 patients [86.3%(25/29) vs 37.8%(14/37) ], significant lower of the left ventricular function in ABI≤0.9 patients compared with those of ABI >0.9 patients (P<0.05). Conclusion There is more severe of coronary artery stenosis in ABI≤0.9 patients for CHD with MS, more attention should be payed to CHD patients combined with MS.
6.Retrospective analysis on 1 124 case-times of renal transplantation

Objective To sum up the experience of renal transplantation in order to improve transplant effect and enhance the long-term survival and recovery rate.Methods The data of renal transplantation of 1 124 cases who received renal transplantation from Jul. 1978 to Oct. 2001 were summarized. All the possible factors that could influence the transplant effect such as pre-operative preparation, HLA match, donors' quality, surgical procedure, immunosuppressive therapy and so on were analyzed. Results The 1-, 3-, 5-year survival rate (%) of patient/kidney was 83.1/ 70.3, 74.6/ 68.6 and 62.6/ 56.1 respectively. The rate of AR was decreased significantly from 3.09?% only with LXM to 0.89?% with LXM test in combination with HLA serological test. The rate of HAR and AR in 130 cases with additional PRA test were lower than those only with LXM test.Conclusions The well pre-operative preparation, the ideal HLA match, the high quality of donor's kidney and transplant operation were keys for successful renal transplantation. Reduction of the complications, individual and combined medication in reason and long-term follow-up were very important in the long-term survival time.
7.Protective role and immunoregulatory effect of CXCR3 in hepatic ischemia-reperfusion injury

Objective To investigate the role and action mechanism of chemokine (C-X-C motif)receptor 3 (CXCR3)in hepatic ische-mia-reperfusion injury (IRI).Methods Forty-eight mice were divided into operation group and sham-operation group.The operation group was treated to establish a mouse model of IRI.Liver tissues were obtained at 3,6,12,and 24 h after IRI,with 6 mice at each time point.The expression of chemokine (C-X-C motif)ligand 9-1 1 (CXCL9-1 1 )and their receptor CXCR3 were measured by real-time PCR and Western blot.The effect of CXCR3 was blocked by its specific antagonist C6.Hepatic injury was estimated based on the activity of hepatic transaminase and morphological indices.The distribution of subsets of infiltrating T cells was analyzed by flow cytometry.All data were expressed as mean ±standard deviation.Comparison between groups was made by one-way analysis of variance.Results Compared with the sham-operation group,the operation group had significantly upregulated expression of CXCL9-1 1 and CXCR3 at all time points after IRI (P<0.05).Blocking CXCR3 significantly protected liver function and morphology (P<0.05).Antagonist C6 significantly re-duced Th1 cell infiltration (P<0.01),but significantly increased Treg infiltration (P<0.01).Conclusion CXCR3 is an ideal therapeu-tic target in IRI treatment due to its relationship with immunoregulation.
8.Analysis of the death causes of 187 cases of kidney transplant recipients

Objective To analyze the death causes of cadaveric kidney transplantation recipients. Methods The clinical data of 1400 cases of cadaveric kidney tansplantation between 1978 and 2003 were analyzed retrospectivvely. The death rate, causes and period were analyzed among the died recipients after kidney transplantation. Results 187 patients were died after renal transplantation in this stage with the ratio being 13.36% (187/1400). The death causes contained infection, cardiocerebra l vascular diseases and hepatic failure by turns, and the ratio was 37.97% , 31.56% and 14.97% , respectively. There were 87 ( 46.5% ) death cases w ith normal transplanted-kidney function. Conclusions The leading causes of patients' death were infection, cardiocerebral vascular d iseases, and hepatic failure. About half of these patients were died with normal transplanted-kidney function.
9.Study on the Standards of Residents′Competency Cultivation in Zhuhai

From the view of medical professionals, this paper explored the standards of residents′ competency cultivation in Zhuhai using Delphi method. The research outcome that consisted of 53 factors and 6 dimensions was obtained after two rounds of expert consultation. There are 6 first-level indexes including professionnalism and body-mind quality, clinical practice skill, the competency of lifelong learning and self-improvement, humanistic medicine practice skill, medical knowledge, and the competency of promoting the medical system to develop. As well, there are 18 second-level indexes and 29 third-level indexes. The standards stated that the clinical prac-tice skill would not be the main part of residents′competency cultivation. Qualified residents should reach the na-tional standard of the clinical practice skill but also the other five indexes.
10.A Study on Relationship between iNOS and p53 Expression in Human Prostatic Carcinoma

Objective To investigate the association of inducible nitric oxide synthetase (iNOS) expression with the expression of mutation type p53 (mt p53) in prostatic carcinoma (PCa). Methods The expression levels of iNOS and mt p53 were in 28 PCa specimens and 10 cases of benign prostatic hyperplasia (BPH) were detected by immunohistochemical method. Results The expression levels of both iNOS and mt p53 in PCa were higher than those in BPH. The expression of iNOS was related to the expression of p53 protein. Conclusion iNOS and mt p53 may play a role in the pathogenesis of PCa.
