1.In vitro evaluation of indirubin self-emulsifying drug delivery system

AIM: To prepare indirubin self-emulsifying drug delivery system(SEDDS) and evaluate it in vitro. METHODS: The criteria of SEDDS formation was studied and indirubin SEDDS preparation optimized with triangle phase diagram.In order to investigate self-emulsifying ability,the time of self-emulsifying,particle size of emulsion and the dissolution of indirubin in vitro were determined. RESULTS: The best formulation of SEDDS was made up of labrasol-peceol-transcutol P(85∶10∶5),which served as oil phase,emulsifier and assistant emulsifier,respectively.The selected formulation could completely emulsify in 5 min and the particle size was about 100 nm.As compared with indirubin suspension,SEDDS could improve drug dissolution significantly. CONCLUSION: SEDDS can give rise to the dissolution increase of slightly soluble drug in water in vitro.
3.Construction of the quality-evaluation index system of clinical teaching of operating room

ObjectiveTo constructe the quality-evaluation index system of clinical teaching of operating room. MethodsThe quality-evaluation indicator system was constructed using literature study and Delphi. Then the Precedence Chart and Analytic Hierarchy Process was adopted to ensure the index's weight. ResultsThe scale was formed by 5 items of level 1 and 21 items of level 2 of the index system. ConclusionThe index system has high reliability,which can be confirmed from experts' authority degree, their attentive involvement and the concentrating degree of the views. The Quality-Evaluation Index System ( QEIS ) is constructed which can be used to evaluate clinical teaching of operating room.
4.Sodium hyaluronate injection immediately versus 2 weeks after arthroscopic debridement for knee osteoarthritis

BACKGROUND:Sodium hyaluronate injection after arthroscopic debridement of knee osteoarthritis can reduce postoperative pain and improve joint function, but there is a controversy on the time for the hyaluronate injection after arthroscopic debridement of knee osteoarthritis.
OBJECTIVE:To observe the time of hyaluronate injection after arthroscopic debridement of knee osteoarthritis, and to compare recent rehabilitation effect of knee sodium hyaluronate injection for knee function after
arthroscopic debridement of knee osteoarthritis between injection just after knee arthroscopy and injection 2 weeks after knee arthroscopy.
METHODS:The clinical data of 100 knee osteoarthritis patients undergoing sodium hyaluronate injection
immediately and 2 weeks after arthroscopic debridement were analyzed with prospective randomized control ed trial method, and the preoperative visual analog scale score, Lysholm score, 6 weeks postoperative visual analog scale score, 3 months postoperative visual analog scale score, and 3 months postoperative Lysholm score were recorded. The effects of recently rehabilitation of knee joint after surgery in two groups were compared.
RESULTS AND CONCLUSION:There was no significant difference in postoperative visual analog scale score between immediate injection group (6.52±2.38) and 2 weeks postoperative injection group (6.54±2.37). The preoperative Lysholm score in the immediate injection group (43.44±16.18) was lower than that in the 2 weeks postoperative injection group (51.12±16.3). The 6 weeks postoperative visual analog scale score in the immediate injection group (3.2±2.46) was significantly higher than that in the 2 weeks postoperative injection group (5.1±2.68). The 3 months postoperative visual analog scale score/Lysholm score in the immediate injection group (2.72±2.70)/(80.58±15.63) were significantly higher than those in the 2 weeks postoperative injection group (4.72±3.07)/(64.96±21.68). The results indicate that sodium hyaluronate injection immediately after arthroscopic debridement of knee osteoarthritis is more favorable for recent rehabilitation.
5.Recent researches on effects of absorbable internal fixators on epiphyseal plates

Nowadays,more and more basic and clinical researches on absorbable internal fixators are car- fled out in China.This paper reviews the recent researches on biodegradation,absorption,and mechanical prop- erties of absorbable internal fixators,their effects on bone mineral density and structure and injury of epiphyseal plate,and their clinical applications.Some problems that have not been solved and future research key points are highlighted and discussed.
6.Research on the application of Net Present Value method in medical equipment procurement

Objective:To strengthen the management and improve the efficiency of medical equipment purchasing, promote the growth of hospital equipment resource value, find scientific equipment procurement ways and methods, establish standardized procurement mode, strive to the code of conduct for the extensive experience into the scientific method of procurement. Methods: Using Net Present Value(NPV) method in financial management on medical equipment purchasing economic benefit analysis, and combined with specific cases and data to analyze the procurement of medical equipment suppliers to provide the equipment procurement scheme.Results: On the assumption that the cost of capital rate is 10%, the three schemes are according to the straight line depreciation method, scheme a net present value for 6.96625 million yuan; scheme 2 net present value for 1.061918 million yuan; project net present value of 5.74608 million yuan; scheme 2 net present value, the second scheme, the net present value of the maximum. Net present value method is an important guarantee for hospital equipment purchase and scientific investment and improved management. Conclusion: Application of net present value method in the purchase of medical equipment show that in equipment procurement program diversification, multiple choice, with net present value method were quantified in order to provide the basis for decision-making, the use of net present value method and the traditional procurement scheme option compared to standards and the scientific nature of medical equipment procurement management, accuracy and objectivity of data analysis.
7.Clinical analysis on arthroscopic treatment of osteoarthritis of the knee

[Objective]To evaluate the therapeutic effect of arthroscopic treatment for osteoarthritis of the knee,and explore the factors related to clinical outcome. [Methods]Three hundred and sixty-two patients with knee osteoarthritis were treated by arthroscopic surgery in the 88th hospital from May 2000 to November 2007. Of them,124 patients were followed-up completely. Quantified clinical data were input into computer and analyzed statistically by SPSS 16.0 software. Lysholm score was used to evaluate the therapeutic effect. Lysholm scores before surgery and at follow up were compared with paired t test. Patients were divided into different subgroups according to sex,locking,X -ray grade,arthroscopic grade and surgical methods. Lysholm scores at the final follow up of different subgroups were compared with variance analysis. The factors related to operation outcome were explored by using multiple stepwise linear regression analysis.[Results]One hundred and twenty-four patients were followed up from 7 to 96 months with an average of 39.6 months. Preoperative Lysholm score was (46.10?2.80),the score at follow up was (70.69+2.90),there was significant difference between preoperative score and at follow up (t=9.362,P0.05). 8.Research on application of cost benefit analysis in medical equipment management

Objective:Using scientific methods, data analysis, improve the ability and level of medical equipment management.Methods: Using the investment return method, the payback period method and the guaranteed business method, the cost benefit analysis of medical equipment is carried out with the actual case(PET/CT).Results: Worked out according to the formula, the equipment in the operation for six years, the investment return rate reaches the highest 58.03%, 3 years basically to recover investment costs, annual check touchdowns 581 cases can achieve guaranteed business income 7558699.61 yuan(RMB).Conclusion: Through the cost-benefit analysis, it can track the benefits of the existing medical equipment, and can also estimate the benefits of the proposed medical equipment, and provide the decision-making basis for the hospital management.
9.Bennett fracture repair:implant option and treatment progress

BACKGROUND:Bennett fracture repair is one of the problems for orthopedic surgeons, although there are a variety of clinical treatment for Bennett fracture, no treatment strategy is widely accepted. <br> OBJECTIVE:This study aims to provide evidence for developing an effective treatment strategy for Bennett fracture. <br> METHODS:A computer-based retrieval of PubMeb database (http://www.ncbi.nlm.nih.gov/PubMed), CNKI China Journal Ful-Text database (http://www.cnki.net/) and Wanfang database (http://g.wanfangdata.com.cn/) between January 2004 and July 2014 was performed by the first author. Meantime, Campbel ’s Operative Orthopaedics, Shiyong Guke Xue and other orthopedic monographs were manual y retrieved, to analyze research progress about Bennett fracture. <br> RESULTS AND CONCLUSION:A total of 37 literatures associated with Bennett fracture were involved in this study. Conventional closed reduction plaster or Kirschner wire fixation is characterized by low cost and easy operation, but it fails to achieve anatomical reduction. AO principle for fracture treatment has been widely accepted in recent years, the application of AO screw and AO plate is increasing in treatment of Bennett fracture, it can not only repair bone fracture but also repair and reconstruct the damaged ligament, but the cost is very high and secondary surgery is needed to remove the implant. Traditional Chinese orthopedics and integrated traditional and western medicine are experienced methods for treatments of Bennett fracture, and Chinese herbs have obvious symptomatic treatment effect and effectively improve the symptoms of patients. As the thumbs play a crucial role in the hand movement, the optimal treatment strategy for Bennett fractures has important clinical significance.
10.The study on duodenogastric reflux after cholecystectomy

Objective To observe the alteration of duodenogastric reflux after cholecystectomy so as to explore the basis for diagnosis and therapy. Methods Intragastric bile reflux during 24 hours was assessed using ambulatory bilirubin monitoring and 24-hour pH monitoring techniques in 20 cholecystectomy patients, 10 cholelithiasis patients and 15 healthy volunteers. Results Bile reflux and alkaline reflux of cholecystectomy patients did not increase compared with those of cholelithiasis and the control. Conclusion Duodenogastric reflux of patients receiving postcholecystectomy patients does not increase.
