1.Diagnostic significance of blink reflex in cranial Guillain-Barré Syndrome

Objective To study diagnostic significance of blink reflex(BR) in cranial Guillain-Barré Syndrome (cGBS). Methods A comparative study was carried out on BR in 7 cGBS cases and 39 normal subjects.Results As compared with controls,the cGBS group showed significant differences on latency and amplitude of R1,R2i, R2c component of BR,M wave and R1 conduction time (R1CT) inside the cranial part(P<0.001). Conclusion The result suggested that BR and facial electromyogram were of great significance in the diagnosis of cGBS and might provide an objective index for clinical differential diagnosis.
2.Operative treatment of the phalangeal neck fracture in hand

It is generaly accepted that displaced phalangeal neck fractures in hands accelerate the onset of degenerative changes, with increasing pain and stiffness of the affected joint. However, reports on the results of surgical treatment in phalangeal neck fractures are relatively few. We have followed sixteen patients, who had operations for phalangeal neck fractures. The average length of follow up was 13 months. The interval between injury and operation ranged from three days to six years, whith a mean of 18 months. The second finger was the most frcquently injured. The phalangeal neck had been displaced with rotation in 9 cases and slightly displaced without rotation in 7 cases. The most common surgical approach was a mid-dorsal incision. Of 16 cases, thirteen required open reduction and internal fixation with a K-wire, either for dorsal displacement and volar angulation of the head or for a 180 degree proximal rotation of the head. The pull-out wire technique was the second most common fixation method. Excellent and good results were noted in 10 cases(62%). Complications were found in seven cases and loss of motion was the most frequent. Late malrotation with angulation occurred in one case.

4.Characteristics of Gastric Cancer in Korea - with an Emphasis on the Increase of the Early Gastric Cancer (EGC).

Gastric cancer is the most common malignancy and the second leading cause of cancer-related death in Korea. Age-adjusted annual incidence of gastric cancer per 100,000 persons is 62.8 for male and 25.7 for female. The proportion of early gastric cancer among surgically treated gastric cancer patients was 28.6% in 1995, 32.8% in 1999, and 47.4% in 2004. Screening upper endoscopy is quite commonly performed in Korea, and the proportion of early gastric cancer in the screened population is almost 75%. Gastric adenoma is considered to be a very important precancerous lesion. Gastric adenomas are actively treated in Korea, and at least 1/3 of the gastric adenomas with high grade dysplasia show cancerous focus when endoscopically resected. The prevalence of Helicobacter pylori is decreasing in Korean population, which may lead to the decreased incidence of gastric cancer in the near future. Paradigms of the diagnosis and treatment of gastric cancer are rapidly changing in Korea. Efforts to establish robust scientific data for new approaches are strongly required. The proportion of early gastric cancer among all gastric cancers has been and will be more increased in Korea.

5.A Clinical Study of Mycoplasma Pneumoniae Pneumonia.

A clinical study was made on 71cases of Mycoplasma Pneumoniae Pneumonia from March, 86 to February, 89. The results were as follows; 1. Among the 315 cases of pneumonia, the incidence of mycoplasma infection was 22.5% 2. The peak incidence of age was between 5 to 9years of age (53.5%) 3. The sex ratio of male to female was 1.3:1 4. Monthly distribution showed relatively high frequency from October to January (59.2%) 5. Most common clinical symptoms were cough (98.6%) and then followed by fever (49.3%), coryza (19.7%). Rales were the most common finding (95.7%) and followed by pharyngeal injection (49.3%) and wheezing (18.3%) 6. The leukocyte counts in peripheral blood were most common in the range of 5000-10000/mm³ (47.9%) and the ESR was increased in 57.7%, and positive CRP cases were 87.3% 7. The most common radiologic finding of pulmonary infiltration was interstitial infiltration (45.1%) and then followed by disseminated lobular (39.4%) and lobar pneumonia (15.5%) 8. There are a few cases associated disease or complication: otitis media (5.6%), hepatitis (4.2%) acute glomerulonephritis, bronchial asthma and sinusitis (2.8%), thrombocytopenia (1.4%)

8.The changes of serum homocysteine, C-reactive protein levels in acute cerebral infarction patients and its clinical significance

Objective To investigate the changes of serum homocysteine ( Hcy),C-reactive protein ( hsCRP) levels in acute cerebral infarction patients and its significance.Methods 64 patients with acute cerebral infarction were selected as research group,and 60 healthy volunteers were selected as control group.Detected the acute cerebral infarction patients after 1d,the first 7d,14d Hcy,hs-CRP,total cholesterol,triglycerides,high density lipoprotein,low density lipoprotein levels and the control group the level of these indicators.Results Of total cholesterol,triglycerides,low density lipoprotein of the study group was higher than those of the control group of high-density lipoprotein(t =3.073,2.874,2.015,all P <0.05);after admission ld,treatment of the first 7d,14d showed no significant difference between the two groups ( t =2.223,2.268,all P < 0.05 ).Conclusion Hcy,hs-CRP and the occurrence of acute cerebralinfarction was closely related to the development,and testing to determine the severity of the disease for the guide had important clinical significance.
9.Preservation of isolated rabbit hearts after normothermia storage with stroma-free bovine hemoglobin solution

Objective To evaluate the protective effects of stroma-free bovine hemoglobin(SFBHB)on preservation of the donor heart.Methods 24 isolated perfused rabbit hearts were equally divided into two groups.In control group,the cardiac arrest were induced with St Thomas Ⅱ solution and repeated every 30 min.the myocardial temperature remained at 40℃.In experimental group,the isolated rabbit hearts were perfused continuously with St Thomas Ⅱ solution containing stroma-free bovine hemoglobin and the myocardial temperature remained at 37℃.The preservation lasted 6 h.Results There were significant differences between the two groups in the follows:(1)The mean values of the left ventricular developed pressure(LVDP)and coronary flow rate(CFR)were higher in experimental group(P<0.001);(2)The ATP level and myocardial oxygen uptake were significantly decreased in control group(P<0.001);(3)The mean values of released creatine phosphate kinase(CPK),lactec dehydrogenase(LDH)and myocardial contents of malondialdehyde(MDA)were higher in control group(P<0.01);(4)More water was present in the myocardial in control group(P<0.01);(5)The myocardial ultrastructural injuries were greater in control group.Conclusion The stroma-free bovine hemoglobin could offer great protective effects on preservation of the isolated rabbit hearts.
