In recent years, there is increasing evidence that the pathogenesis of Parkinson's disease (PD) may involve the more generation of reactive oxygen species, and investigations on patients have shown that PD is under a status of oxidative stress. The defense against the toxic effects of reactive oxygen species is an essential task within the brain. An important component of the cellular detoxification of reactive oxygen species is the antioxidant glutathione. Consequently, it seems reasonable to propose that increase of brain concentrations of glutathione including glutathione analogs or precursors could be very effective in diminishing the cumulative effects of oxidative damage, and have been investigated as potential therapeutic targets in the treatment and prevention of PD or other neurodegenerative disease.

Objective:To investigate the operation technique and the effect of the posterior microendoscopic discectomy (MED) system on lumbar intervertebral disc hermiation (LIDH). Methods: We retrospectively reviewed and analyzed 25 cases of LVDH treated with the MED system from March 2004 to August 2005. Results: All the patients were ambulant 20 hours to 7 days (mean 2.4 days) after the operation, with a mean hospital stay of 5.2 days. And all were healed within the first stage except 1 case of infection. The patients were followed for 2-15 months (mean 6 months). Excellent results were achieved in 18 cases, good results in 5 and fairly good results in 2, the first two groups making up 92%. Conclusion: The key to successful MED with no complications is familiarity with the equipment, proper choice of indication, exact surgical location and skilful operation.

Objective To investigate the effects of somatostatin combined with early enteral nutrition on inflammatory factors and intestinal permeability in patients with severe acute pancreatitis (SAP).Methods 70 patients with severe acute pancreatitis were randomly divided into the control group (35 cases) and the observation group (35 cases).The control group were given double somatostatin, the observation group were received double somatostatin combined with early enteral nutrition support.Before and after treatment in the two gorups, TNF-a and IL-1 levels were measured by ELISA;intestinal permeability index was indirect calculated by high performance liquid chromatography.Clinical curative effect and inflammatory factors and intestinal permeability were compared in the two groups.Results Seven days after treatment, the secondary infection rate, operation rate and mortality in the observation were lower than those in the control group (P<0.05);before treatment, TNF-a and IL-1 level in the two groups had no significant difference;Seven days after treatment, TNF-a and IL-1 in the observation group were lower than those in the control group (P<0.05);seven days after treatment,blood lactate levels in the observation group was higher than that in the control group and diamineoxidase was lower than that in the control group (P<0.05).Conclusion Double somatostatin combined with early enteral nutrition support can improve inflammatory factors and intestinal permeability in patients with severe acute pancreatitis, and is worthy of popularization and application.

Objective To analysis the reason why patients with shoulder and back pain caused by pulmonary cancer osseous metastasis were misdiagnosed.Methods A patient of pulmonary cancer with osseous metastasis who was misdiagnosed as herniation of cervical disc,shoulder and back chronic lesion were analysed and reviewed with the literature.Results The involved vertebral bodies were osteolysis or wedging,the tumor encroach in cortical bone or soft tissue could result in shoulder and back pain.Conclusion A complete history and physical examine,systemic differential diagnosis,proper radiographic examination can decrease the incidence of misdiagnosis.

The incidence of gastric cancer has declined worldwide in the past decades. However, due to the large number of population in mainland of China, the absolute number of newly-diagnosed patients in this developing country is still very high. Most of the major clinical centers have established multi-disciplinary team, but the strategy in treating this life-threatening disease still needs to be optimized, including the precise preoperative staging by various techniques, perioperative chemotherapy for locally advanced diseases, role of minimally invasive surgical approaches in treating patients with different stages, translational research to fill the gap between clinical trials and basic research, as well as the individualized treatment for gastric cancer. We believe that the biological behaviors and molecular events of gastric cancer in eastern countries are different from that of Western countries, so the clinical evidences derived from Eastern countries need to be strengthened to enrich the unmet demands in the clinical practice in China.

OBJECTIVETo analyze rationality of internal fixation with original fixation in the second operation for femoral nonunion.

METHODSA retrospective study was conducted to analyze the clinical data of 19 femoral nonunion patients treated by internal fixation with original fixation in the second operation since January 2009 to July 2014, including 14 males and 5 females with an average age of (43.63 ± 3.95) years old ranging from 18 and 75 years old. The original fixations involved plate fixation in 11 cases and interlocking intramedullary nail in 8 cases (including 3 cases of retrograde interlocking intramedullary nails). There were 13 cases of atrophic nonunion and 6 cases of hypertrophic nonunion in pathological classification. After second operation, the fracture healing time, Harris hip score and range of motion of the knee joint was recorded and compared with preoperative.

RESULTSAfter the second operatioin, all patients were followed up for 6 to 30 months with an average of (18.67 ± 7.59) months. All patients got fracture healing, and the fracture radiological healing time was from 7 to 29 months with an average of (15.78 ± 4.97) months. Harris hip scores were improved from preoperative 62.37 ± 19.48 to postoperative 87.42 ± 8.86. The range of motion of the knee joint was improved from preoperative (52.16 ± 20.10)° to postoperative (96.32 ± 22.10)°.

CONCLUSIONIt has a better clinical healing rate for using original fixation to treat femoral nonunions, and theoretically speaking it was rationality.

Objective: To prepare valved mitral stent and to discuss the feasibility of transcatheter implantation of the valved mitral stent in vitro. Methods: We designed a self-expandable stent made of super-elastic Ni-Ti shape memory alloy. The stent was composed of double disks with tubular linkage between them. A valvular ring made of nitinol wire was sutured on the tubular part. Fresh sheep pericardium was cross-linked with a 0.6% glutaraldehyde solution for 36 hours then sutured into a valvular ring; the latter was sutured onto the nitinol self-expandable stent. Through a sheath across the interatrial septum positioned in the left ventricle of isolated sheep heart via vena cava, the device was delivered into the native mitral valve position. When the left ventricle disk was deployed, it was applied to the mitral annulus by pulling back the external sheath. The waist and the left atrial disk were deployed by pulling the sheath in the annulus and left atrium, respectively. Thus the whole device was deployed in the mitral native. Water was injected into the pulmonary veins and left ventricle to test competence of the prosthetic heart valves. Results: The prepared valved mitral stent could be stably positioned at the native valves. There was no stent migration when it was repeatedly pulled. The two disks sandwiched the native mitral valve with one disk lying in the left ventricle and the other one in the left atrium. The prosthetic heart valves showed satisfactory function without structure damage. Conclusion: This self-expandable stent is well designed and can be safely implanted in the mitral valve position via transcatheter approach, and the function of the stent is satisfactory.
