1.Translational research progress in malignant lymphoma

Rituximab in the combination of CHOP regimen has been widely used as the standard treatment of several kinds of B cell non-Hodgkin lymphoma (B-NHL),but there are still about 1/3 of the late B-NHL patients become primary and secondary resistant to the drug.Recently,many translational research progress in malignant lymphoma promoted the development of promising candidate drugs for the treatment of lymphoma.The advances in translational research field were summarized in this manuscript.

Advances in clinical transformation research has facilitated discovery of a number of novel somatic mutations and aberrant intracellular signaling pathways involved in malignant proliferative B-cell lymphoma.Meanwhile,small molecular inhibitors targeting those mutation genes or signaling pathways have also been shown to be effective in treating relapsed and refractory B-cell non-Hodgkin lymphoma.This paper reviewed important discoveries in clinical transformation research in lymphoma field in 2013.
5.Application of multi-slice CT in measurement of morphological parameters of waist muscle and its clinical significance

Objective To investigate the changes of waist muscle density and area in the subjects with spiral CT scan examination,and through the quantitative analysis of different age and gender to get the degree of lumbar muscle degeneration.Methods 240 cases of lumbar CT scan reconstruction images (including the patients suffering from low back pain)were collected.According to the age,20 - 29 years old group,30 - 39 years old group,40 -49 years old group,50-59 years old group,60-69 years old group,and 70-79 years old group were set up.The lumbar transverse psoas muscle,lumbar side muscle,erector spinae cross-sectional areas (CSA)and mean CT values were measured at different levels.Scanning equipment:Toshiba Activion 16-slice spiral CT scanners, acquisition range from the waist 1 to S1.Results ①Compared between the measurement results of the same age groups (except 70-79 years old group),the average CSA of the male at the same level waist was higher than the female (P <0.05).②Compared between the same level groups of 20-29 years,30-39 years and 40-49 years, there were no significant differences (P >0.05).Compared between the same level groups of 50-59 years,60-69 years and 70-79 years,there was no significant difference (P >0.05).③Compared the CSA of man’s erector spinae muscle between 40-49 years group and 50-59 years group,there were significant differences (P <0.05).④Compared the same level CSA of female between 20-29 years,30-39 years,40-49 years,50-59 years,60-69 years,70-79 years groups,and the adjacent groups,there were no significant differences (P >0.05).⑤The same level average density of erector spinae muscle was lowest compared with the psoas major muscle and quadratus lumborum muscle,and falling faster,the psoas major muscle was relatively higher.The same level average density of female’s erector spinae muscle was lower than man’s,specially in 70-79 years group.Conclusion Multi-slice spiral CT can be used to diagnose the lumbar degenerative conditions.
6.Effects of lornoxicam combining with fentanyl on postoperative arrhythmia and platelet function in patients with coronary artery disease after abdominal surgery

Objective To investigate the effects of patient-controlled intravenous analgesia (PCIA) with lornoxicam and fentanyl on arrhythmia and the expression of platelet membrane glycoproteins in patients with coronary artery disease (CAD) after abdominal surgery.Methods Eighty ASA II or III patients with CAD aged 51-66 yrs weighing 59-68 kg presenting for bdominal surgery participated in this study. CAD was diagnosed by clinical symptoms and ischemic changes on ECG. The patients were premedicated with intramuscular henobarbital 0.1 g and scopolamine 0.3 mg. Anesthesia was induced with fentanyl, droperidol, propofol and vecuronium and maintained with propofol, fentanyl and vecuronium. The patients received PCIA after operation. The PCIA solution contained fentanyl 0.9 mg and droperidol 5 mg in 100 ml of normal saline (N.S. ) in group A ( n - 40) or lomoxicam 56 mg, fentanyl 0.2 mg and dropendol 5 mg in 100 ml N.S. in group B ( n = 40). In group A the loading dose was fentanyl 0.05 mg and in group B lornoxicam 4 mg. PCIA included a background infusion at 2 ml ? h-1 and a bolus of 0.5 ml with a 15 min lock-out. VAS (0 = no pain, 10 = worst pain) was used to measure pain intensity In addition to BP, HR and SpO2 monitoring ECG was continuously monitored with a Holler monitor after operation Blood samples were taken from peripheral vein before and 6h after operation and on the 1st, 2nd, 7th and 8th postoperative days for determination of the expression of CD62p, CD63 and CD41/CD61 on the platelet membrane, platelet count, prothrombin time ( PT) thrombin time (TT) and partial thromboplastin time (PTT) Results The two groups were comparable with respect to sex, age, body weight, severity of CAD, duration of operation and intraoperative blood loss. The patients received no blood transfusion during operation. There was no significant difference in VAS score, platelet count, PT, TT and PIT between the two groups. The incidence of atrial and ventricular premature beat on ECG and the expression of CD41 /CD61 , CD62P and CD63 on the platelet membrane were significandy lower in group B than in group A on the 7th and 8th postoperative days (P
7.GASTRIC CANCER CELL GC9811 TRANSFECTED BY MURINE ?1,3-GALACTOSYLTRANSFERASE GENE

The full length cDNA of ?1,3 galactosyltransferase gene was amplified by RT PCR from the peritoneal macrophages of BALB/c mice, which was subcloned into an eukaryotic expression vector pCDNA3 1/V5 HisA, and it was confirmed by DNA sequencing. The gastric cancer cell GC9811 was transfected with pCDNA3 1/V5 HisA?1,3GT by Lipofectamine TM 2000. The positive clone was screened by G418 for two months and confirmed by RT PCR. The gastric cells expressing Gal?(1,3)Gal epitope were detected by immuohistochemisty, and they were killed by naive antidoby specific to Gal?(1,3)Gal epitope. The results showed that the pCDNA3.1/V5 HisA?1,3GT was successfully constructed, and the transfected gastric cancer cells could highly express Gal?(1,3)Gal, which promoted lysis of the cancer cells by normal human serum.
8.Clinical Study on the Treatment of Adriamycin-induced Cardiotoxicity with the Method of Nourishing Yin

Objective To investigate the preventive effect of treating cardiotoxicity induced by adriamycin (ADM) with the method of nourishing yin. Methods 90 cancer patients underwent adriamycin therapy were randomly recruited intoa treatment group (45 cases) and a control group (45 cases). The treatment group was treated with Wushen decoction, while the control group was treated with antioxidant drugs. The changes of ECG, blood SOD, GSH-Px, MDA content etc were observed after one course of four weeks' treatment. Results The incidence of cardiactoxicity was 15.56% in the treatment group and 40.00% in the control group, showing significant difference ( U=2.02, P<0.05 ) ; the incidence of ECG abnormalities was 13.33% in the treatment group and 37.78%, in the control group, showing significant difference (U= 2.18, P< 0.05 ) : the effective rate was 82.22% in the treatment group and 62.22% in the control group, showing significant difference ( U=2.18, P <0.05) ; and SOD, GSH-Px, MDA content at the 1, 7, 14, and 28days between the two groups also demonstrated significant differences (P<0.05) . Conclusion The method of nourishing yin has excellent attenuation to adriamycin-induced cardiotoxicity, besides it enhances therapeutic effects of chemotherapy.
9.Effects of children's mentality on mental symptoms during recovery period of ketamine anesthesia

Objective To investigate the effects of children's mentality on mental symptoms during recovery period of ketamine anesthesia. Methods According to the mentality, forty-five children undergoing smalminor surgery were assigned into active group, stable group and fretful group with 15 cases each. Child was anaesthetized by ketamine alone. RP, BP, HR and SpO_2 were recorded at the end of operation, 5,10 and 30 min after operation. Meanwhile the expression, feeling, behavior, talking and activity were observed. Results RR, BP, HR in active group and fretful group were significantly higher than those in stable group. There was a significant difference in the expression, feeling, behavior, talking and activity among three groups. Conclusion Children's mentality has a significant effect on mental symptoms and hemodynamics during recovery period of ketamine anesthesia.
10.Research status and progress in monitoring of anesthesia depth

Anesthesia is the most crucial part of clinical surgery in modern medicine,and it is highly risked due to complications of general anesthesia.Researches have proved that intraoperation monitoring depth of anesthesia efficiently reduced the dosage of anesthetics and led to earlier recovery,so that the risk of anesthesia has been minimized.Currently,the research on depth of anesthesia monitoring methods has become an international hotspot.This paper reviews the methods for monitoring depth of anesthesia,including bispectral index,auditory evoked potential,Entropy and heart rate variability,and highlights advantages,disadvantages of these methods and the relationship between these methods.This paper also introduces the relationship between heart rate variability and the depth of anesthesia monitoring,the method for analyzing heart rate variability in both time domain and frequency domain,which laid a theoretical foundation for the research on monitoring depth of anesthesia.
