1.Sequential Therapy of Tacalcitol Combined with Halometasone Treat Plaque Psoriasis

[Objective] To investigate the clinical efficacy of tacalcitol combined Halometasone sequential therapy in the patients of plaque psoriasis. [Methods] 117 cases of plaque psoriasis col ected between 2011.01 and 2013.02 were randomly divided into treatment group and control group. The treatment group were treated with tacalcitol and Halometasone therapy, and required topical use Halometasone(1 time/day) in the morning ,tacalcitol use in the evening(1 time/day) before the first 2 weeks. Continuous two weeks treatment from Monday to Friday with tacalcitol(2 times/day), Saturday and Sunday with Halometasone(2 times/day), final y the transition to be used alone tacalcitol(2 times/day) in the next two weeks of continuous treatment. Control group of pa-tients given alone Halometasone(2 times/day) for 6 consecutive weeks. Observe the adverse reactions PASI scores during and after completion of treatment within the two groups. Al patients were fol owed for three months, observed recurrence. [Results] The PASI score between two groups of patients declined during treatment, especial y at the 4th and 6th week within the treatment group. Both groups had no serious adverse reactions. In treatment group, 54 patients had effective responses, the total effective rate was 90%, significantly better than the control group(41 cases,71.9%). In the fol ow-up of 3 months, the recur-rence rate was 11.1%in the treatment group, significantly lower than the control group(31.6%). The difference of total efficiency and the recurrence rate with-in the two groups was statistical y significant. [Conclusion] Tacalcitol combined Halometasone sequential therapy of plaque psoriasis has significant clinical effi-cacy and low recurrence rate.
2.Elevated levels of substance P and prostaglandins in sheep during endotoxin induced lung injury

A high dose E. Coli endotoxin was infused into unanesthetized sheep withchronic lung lymph fistula. The contents of substance P in plasma, lung lymph, lungparenchyma and also TXB_2 and 6-keto-PGF_1? were measured. The results showed that 120min after infusion substance P increased in plasma and lymph, but its content in paren-chyma decreased at 360 min after infusion. At 30 min after infusion, TXB_2 and 6-keto-PGF_1? levels in plasma showed a marked increase, but their peak values in lung lymphwere shown at 60 min after infusion, and the levels of TXB_2 were much higher than thatof 6--keto--PGF_1?

4.Vitamin D status among neonates and its impact on lung maturation

Recent studies show that vitamin D plays a role in lung development and regulation of lung maturation in the fetus. The AAP and the Institute of Medicine( IOM) both define vitamin D insufficiency as ser-um 25-OH-D concentrations <50 nmol/L in the pediatric population. The surveys in our country show the prev-alence of vitamin D insufficiency is higher according to the criteria defined by AAP. Evidence from animal study suggests that vitamin D may increase alveolar count,alveolar septal thickness,lung volume and decrease airway resistance,immune cell aggregation leading to the maturation of lung. The cohort studies suggest low 25-OH-D level in infants at birth is associated with greater need for assisted ventilation,increased rate of ARDS,increased duration of oxygen requirement.

6.A case of scleredema.

Scleredema is a rare skin disease of unknown cause involving the face, neck, upper portion of the trunk, and proximal upper extremities. The clinical features of it are nonpitting, solid emema or stiffness of the neck, which may be sudden or insidious onset. Preceeding upper respiratory infection or diabetes mellitus have been frequently discribed to be associated with the disease. A monoclonal gammopathy has been frequently observed in patients with scleredema. We present a case of scleredema developed the face, upper portion of the back and shoulder in 16-year-old girl who had a preceeding upper respiratory infection.

9.Laparoscopic operations in gynecologic emergency

Objective To investigate the clinical significance of laparoscopic operations in gynecologic emergency. Methods We retrospectively analyzed 145 cases of gynecologic emergency dealt with laparoscopic operations in May 1997~August 2001. Out of the 145 cases, tubal pregnancy accounted for 100 cases (Laparoscopy Group), which were further compared with 89 cases of tubal pregnancy in January 1994~April 1997 treated by open operations (Open Group). Results Out of the 145 operations, 139 (95.9%) were successfully carried out under laparoscope, with the conversion to open operations in 6 cases. There were 61 cases of salpingectomy, 28 cases of oviduct fenestration, 11 of fenestration converting into salpingectomy, 16 of partial ovariectomy, 8 of adnexectomy for ovarian cyst pediculotorsion, 4 of cyst resection, 3 for rupture of endometrial cyst of ovary, 6 for pelvic abscess and 2 for pelvic encapsulated fluid. Compared with the Open Group, the Laparoscopy Group had longer length of surgical intervention (t=3.9, P=0.000) and more intraoperative blood loss (t=5.8, P=0.000). Conclusions Laparoscopic operations play double actions both in diagnosis and in treatment for most gynecologic emergencies, being a safe and reliable approach. Laparoscopic salpingectomy (non-oviduct-sparing operations), however, should be chosen for urgent patients.
10.Expression of PR and Bcl-2,Bax in Ectopic and Eutopic Endometrium in Patients with Endometriosis

Objective:The pathogenesis of endometriosis(EMT)was explored by detecting the relationship of progesterone receptors(PR)and apoptosis-associated factors with EMT.Methods:Immunohistochemical imaging analysis was used to quantify the expression of PR,Bcl-2 and Bax in ectopic and eutopic endometrium in patients with EMT.Results:Expressions of PR and Bcl-2 in eutopic endometrium are higher than those in ectopic endometrium(P<0.05).Expression of Bax in ectopic endometrium is higher than that in eutopic endometrium(P<0.05).There is no statistical difference of PR,Bcl-2 and Bax expression between proliferative and secretary endometrium.Conclusions:Either in proliferative or secretary phase,expressions of PR in eutopic endometrium are higher than those in ectopic endometrium,thus eutopic endometrium is more liable to proliferate in low level continuously,migrate and implant.Expressions of Bcl-2 and Bax in eutopic and ectopic endometrium have no relationship with endometrium cycling.The apoptosis of ectopic endometrium is weakened and its proliferative power is continuously enhanced.
