[Objective] To investigate the clinical efficacy of tacalcitol combined Halometasone sequential therapy in the patients of plaque psoriasis. [Methods] 117 cases of plaque psoriasis col ected between 2011.01 and 2013.02 were randomly divided into treatment group and control group. The treatment group were treated with tacalcitol and Halometasone therapy, and required topical use Halometasone(1 time/day) in the morning ,tacalcitol use in the evening(1 time/day) before the first 2 weeks. Continuous two weeks treatment from Monday to Friday with tacalcitol(2 times/day), Saturday and Sunday with Halometasone(2 times/day), final y the transition to be used alone tacalcitol(2 times/day) in the next two weeks of continuous treatment. Control group of pa-tients given alone Halometasone(2 times/day) for 6 consecutive weeks. Observe the adverse reactions PASI scores during and after completion of treatment within the two groups. Al patients were fol owed for three months, observed recurrence. [Results] The PASI score between two groups of patients declined during treatment, especial y at the 4th and 6th week within the treatment group. Both groups had no serious adverse reactions. In treatment group, 54 patients had effective responses, the total effective rate was 90%, significantly better than the control group(41 cases,71.9%). In the fol ow-up of 3 months, the recur-rence rate was 11.1%in the treatment group, significantly lower than the control group(31.6%). The difference of total efficiency and the recurrence rate with-in the two groups was statistical y significant. [Conclusion] Tacalcitol combined Halometasone sequential therapy of plaque psoriasis has significant clinical effi-cacy and low recurrence rate.

Recent studies show that vitamin D plays a role in lung development and regulation of lung maturation in the fetus. The AAP and the Institute of Medicine( IOM) both define vitamin D insufficiency as ser-um 25-OH-D concentrations <50 nmol/L in the pediatric population. The surveys in our country show the prev-alence of vitamin D insufficiency is higher according to the criteria defined by AAP. Evidence from animal study suggests that vitamin D may increase alveolar count,alveolar septal thickness,lung volume and decrease airway resistance,immune cell aggregation leading to the maturation of lung. The cohort studies suggest low 25-OH-D level in infants at birth is associated with greater need for assisted ventilation,increased rate of ARDS,increased duration of oxygen requirement.

A high dose E. Coli endotoxin was infused into unanesthetized sheep withchronic lung lymph fistula. The contents of substance P in plasma, lung lymph, lungparenchyma and also TXB_2 and 6-keto-PGF_1? were measured. The results showed that 120min after infusion substance P increased in plasma and lymph, but its content in paren-chyma decreased at 360 min after infusion. At 30 min after infusion, TXB_2 and 6-keto-PGF_1? levels in plasma showed a marked increase, but their peak values in lung lymphwere shown at 60 min after infusion, and the levels of TXB_2 were much higher than thatof 6--keto--PGF_1?

Scleredema is a rare skin disease of unknown cause involving the face, neck, upper portion of the trunk, and proximal upper extremities. The clinical features of it are nonpitting, solid emema or stiffness of the neck, which may be sudden or insidious onset. Preceeding upper respiratory infection or diabetes mellitus have been frequently discribed to be associated with the disease. A monoclonal gammopathy has been frequently observed in patients with scleredema. We present a case of scleredema developed the face, upper portion of the back and shoulder in 16-year-old girl who had a preceeding upper respiratory infection.

Objective To investigate the spectrum of spiral CT imaging findings of blunt liver trauma.Methods Clinical data of 17 patients with blunt liver trauma were retrospectively collected. All patients underwent standardized spiral CT examination of the upper abdomen, which include plain scan, arterial phase and portal venous phase acquisition. The morphology, density and integrity of liver parenchyma and intrahepatic venous structures were carefully observed, as well as regions of porta hepatis, peritoneal cavity and retroperitoneal space.Results Twelve cases (70.6%) developed hepatic parenchymal laceration. There were 9 cases (52.9%) of traumatic hematoma, among which 5 were intraparenchymal and 4 were subcapsular. One case (5.9%) showed active bleeding within an intrahepatic hematoma, while two cases (11.8%) had injury (laceration) of hepatic veins. There were 7 patients (41.2%) who demonstrated the so-called “halo sign” around the intrahepatic portal branches. Thirteen patients were associated with peritoneal fluid (blood) collection, 3 with hematoma or hemorrhage of the right adrenal gland, 8 with plural effusion and 3 cases with rib fractures of right lower chest. Conclusion CT imaging findings of blunt liver trauma include parenchymal laceration, intraparenchymal and /or subcapsular hematomas, active hemorrhage, and tear of hepatic veins. Plain CT scan and contrast-enhanced dual-phase acquisition is very important for the comprehensive evaluation of patients with blunt liver trauma.

Because of its unique optical properties, the gold nanorod (GNR), that is a rod-shaped nano materials, has shown great potential applications in the biomedical field, especially, in GRNs thermotherapy, radio-sensitizing effect therapy and targeting therapy of the tumor. Being a newly-developed therapy, gold nanorods combined with internal radiotherapy has already obtained excellent durative effect in targeting therapy of tumor. This paper aims to make a comprehensive review about its mechanism and its clinical application and research situation in recent years.

Objective To investigate the feasibility,safety and effect of gasless laparoscopic surgery used in patients at the second trimester.MethodsFrom Aug.2006 to Aug.2008,the data of 10 cases at gestational 15 - 24 weeks complicated by ovary benign tumors ( the diameters more than 5 cm) undergoing gasless laparoscopic surgery were studied retrospectively.Operation time,bleeding loss,period of hospitalization,the status of fetus and newborn babies,and peri-operative or postoperative complications were observed.ResultsAll 10 cases were treated by gasless laparoscopic surgery,including 4 cases with mature teratoma,5 cases with epithelial tumors and 1 case with ovarian endometrioma.The mean time of surgery was (30 ± 5) minutes,the mean bleeding in the surgery was (30 ± 8) ml,the mean period of in hospitalization was ( 6 ± 2 ) days.The blood gas ( pH:7.42 ± 0.11,7.41 ± 0.18,7.42 ± 0.12 ),blood pressure [ mean arterial pressure (MAP):(90.4 ± 3.1 ),(90.6 ± 0.7 ),( 89.7 ± 0.8) mm Hg( 1 mm Hg =0.133 kPa) ] and heart rates [ ( 84.0 ± 1.6),( 84.3 ± 1.7 ),( 82.7 ± 1.1 ) bpm ] in all cases at pre,peri and postoperative time did not show statistical difference ( P all ＞ 0.05 ).The fetal hearts were all in the normal range preoperative,intraoperative and postoperative time (P ＞ 0.05 ).No perioperative and postoperative complications were observed.All the cases underwent term delivery.Fetal weight and Apgar score did not show abnormality.ConclusionGasless laparoscopic surgery at the second trimester is feasible and safe to mother and fetus.

Objective:The pathogenesis of endometriosis(EMT)was explored by detecting the relationship of progesterone receptors(PR)and apoptosis-associated factors with EMT.Methods:Immunohistochemical imaging analysis was used to quantify the expression of PR,Bcl-2 and Bax in ectopic and eutopic endometrium in patients with EMT.Results:Expressions of PR and Bcl-2 in eutopic endometrium are higher than those in ectopic endometrium(P<0.05).Expression of Bax in ectopic endometrium is higher than that in eutopic endometrium(P<0.05).There is no statistical difference of PR,Bcl-2 and Bax expression between proliferative and secretary endometrium.Conclusions:Either in proliferative or secretary phase,expressions of PR in eutopic endometrium are higher than those in ectopic endometrium,thus eutopic endometrium is more liable to proliferate in low level continuously,migrate and implant.Expressions of Bcl-2 and Bax in eutopic and ectopic endometrium have no relationship with endometrium cycling.The apoptosis of ectopic endometrium is weakened and its proliferative power is continuously enhanced.
