1.Prenatal ultrasound diagnosis of clubfoot

Objective To investigate prenatal ultrasound diagnosis of clubfoot and its clinical value.Methods Three thousands pregants ranging gestational age from 12 through 40 weeks were checked using Ascuson Sequoia 512 equipment.The diagnosis of clubfoot was made prospectively at the time of the scan by the following criteria: the ball of foot had to be visible in the same plane as the longitudinal section of bones of the lower leg,unchanging with fetal activity and reproducible for several images.All prenatal ultrasonographic dignoses of clubfoot were proved by autopsy or newborn physical examination.Results A total of 17 cases clubfoot fetuses were prenatal diagnosed by ultrasonography.Fourteen of 17 clubfoot fetuses had associated with complex abnormalities,three of them were isolated clubfoot.Fourteen of 17 cases with clubfoot prenatal diagnosed by ultrasound were proved by autopsy.Another three cases with clubfoot were proved by newborn physical examination,two of them were false positive diagnosis.Conclusions Prenatal diagnosis of clubfoot is reliable by ultrasound.It is helpful in diagnosis with other structural abnormalities associated with clubfoot.It can also provide direction for clinical obstetrical management.
2.Prenatal ultrasound diagnosis of fetal radial ray abnormalities

Objective To investigate prenatal ultrasound diagnosis of radial ray abnormalities and its clinical value. Methods All the fetus that entered usual prenatal ultrasound diagnosis were scanned each limb using a systematic continuous sequence approach (SCSA) with Acuson Sequoia 512 and GE Logiq 400 equipments. All the artificial aborted fetal radius malformation also had ultrasound examination in a simulating intrauterine estate and /or X-ray examination. Results Correctly diagnosis was made in 13 cases fetal radial ray abnormalities by prenatal ultrasonography following SCSA through the scaning course. All the fetal radial ray abnormalities had radius absence(only one skeleton in the forearm) or radius dysplasia and characteristic radial club hand. Ray abnormalities can be classifyied into three types on ultrasonography: type Ⅰ, radius absence; type Ⅱ,radius part absence; type Ⅲ,radius dysplasia.Conclusions Mastering characteristic radial club hand and SCSA are the keys to diagnose fetal radial ray abnormalities by prenatal ultrasonography.
3.Changes of portal hemodynamics in the development of experimental liver cirrhosis

AIM: To understand the formation of portal hypertension through the change of portal hemodynamics on experiment cirrhosis. METHODS: Carbon tetrachloride was subcutaneously injected in the rat. The changes of the portal hemodynamics in the pathological process of liver tissue were observed. RESULTS: The liver underwent degeneration, necrosis of hepatocytes, and the normal architecture of the liver lobules was replaced by pseudolobule, which consist of regenerative hepatocytes and fibrous septa. The diameter, the blood flow velocity and the blood flow quantity of portal were significantly higher than that in former group (P0 05). CONCLUSION: The changes in hemodynamics of the portal are in accordance with the changes in pathology of liver in the formation of liver cirrhosis.
4.Occupational Exposure of Medical Personnel and Their Protection

OBJECTIVE To evaluate the reason and protection measures of occupational exposure.METHODS From Jan 2005-Mar 2008,all medical personnel in our hospital were investigated by face to face investigation and checking recording data.The causes,modalities,management and prophylaxis of occupational exposure were analyzed.RESULTS Seventy-six person suffered occupational exposure,including 51 nurses(67.11%),18 doctors(23.68%) and the other 7 workers.Occupational exposure mainly occurred in 20-30 years old medical workers.61 persons(80.26%) suffered stab injury and sharp instrument injury.All 76 front medical personnel could make appropriate management after occupational exposure and without further infection.CONCLUSIONS Some first front medical personnel do not recognize the hazard of the occupational exposure.It′s necessary to enhance the management and training and to establish a corresponding standard to protect medical personnel from being harmed by occupational exposure.
5.Effect of EGF on expression of EGFR and EGF mRNA in the injuried lung tissue of neonatal premature rats exposed to hyperoxia

AIM:To investigate the effect of exogenous epidermal growth factor(EGF) on expression of EGFR and EGF mRNA of the injuried lung tissue of neonatal premature rats exposed to hyperoxia.METHODS:The neonatal Sprague-Dawley rats of 21-day gestational age model of hyperoxia induced lung injury were made by continually inhaling 95% oxygen.The model rats were divided into two groups randomly,the EGF trail group and the NS control group.The other rats were taken into the air control group.Each group was divided into three subsets:a(1-3 days),b(4-6 days) and c(1-6 days) according to different application times of EGF or NS.Rats in sub groups were executed and the lung tissues were removed at postnatal 3th,7th,14th day respectively.Using immunohistochemistry method,the expression of EGF-R of lung tissues in different groups was observed,and the expression of EGF mRNA was determined by reverse transcriptase polymerase chain reaction(RT-PCR).RESULTS:The expression of EGF mRNA increased by degrees following the increasing postnatal days.Compared with the air control group,the expression of EGF-R and EGF mRNA increased in the hyperxia group at 7th day and 14th day.The expression of EGF-R increased in corresponding hyperxia NS control group at 14th day(P
6.Comparative diagnosis of malformations of forearm and crus by prenatal and fetus in vitro ultrasonography

Objective To assess the value of prenatal ultrasonography on fetal forearm and crus malformations by studying the ultrasonographic characteristic in relation to methods of examination. Methods All fetus were evaluated by using a systematic continuous sequence approach (SCSA) with ultrasonography. A close attention was paid on shapes, structures and movement of fetal forearm and crus, and fetus specimens after induced labor were rescanned by ultrasonography under the condition of mimic intra-uterus. Results Thirty of 33 cases (90.9%) with fetal forearm and crus malformations (totally 48 of 54 limb anormalies, 88.9%) were correctly diagnosed by prenatal ultrasonography. Six limb malformations of 3 cases were missed. Conclusion Our study demonstrates that the malformations of forearm and crus detected with prenatal ultrasonography are highly accordant with the malformations revealed in fetus in vitro. It is important to abide by the SCSA in ultrasonographic diagnosis of fetal forearm and crus malformation.
7.Prenatal sonographic diagnosis on agenesis of fetal corpus callosum

Objective To assess the value of prenatal sonographic diagnosis on fetal agenesis of corpus callosum(ACC). Methods A retrospective study was on prenatal sonographic findings of 10 cases with ACC malformation and their abnormalities in central nervous system (CNS) or extra-CNS.Results The special sonographic findings established the diagnosis of ACC malformation in all 10 cases,with 7cases diagnosed absence agenesis. Among all patients with ACC,6 cases were accompanied with abnormalities in extra-CNS,8 in CNS and 5 in both. Conclusions Prenatal ultrasonography plays a vital role in accurate diagnosis on fetal ACC. Attentions should be paid to the indirect encephalic features and complicated abnormalities so as to make accurate and prompt diagnosis.

AIM: To investigate the effect of glycine on endotoxin and hypoxia-induced necrotizing exterocolitos (NEC) in rats. METHODS: In glycine+NEC group, twenty anesthetized and artificially ventilated rats received 1g/kg glycine (20%, iv). Five minutes later, the rats were treated with 2 mg/kg lipopolysaccharide (LPS). In control group (NS+NEC), twenty rats were treated with normal saline as a substitute for glycine. In all animals, FiO 2 was reduced after 90 min from 21% to 5% and ventilation continued until 180 min or death. At the end of the experiment, the samples of blood and intestine were obtained immediately. Serum TNF? was measured with ELISA, serum NO was determined by nitrate reductase. The histopathology of the necrotic lesions were categoried: grade Ⅰ, focal mild injury confined to villous tips; grade Ⅱ, partial loss of villi; grade Ⅲ, necrosis extending to submucosa; grade Ⅳ, transmural necrosis. Objective To study the influence of aerobic exercise and relaxation therapy on quality of life and symptoms of breast cancer patients undergoing chemotherapy.Methods A total of 116 female breast cancer patients in our hospital were divided into study group and control group by random number table.The control group took routine nursing intervention,and the study group applied aerobic exercise and relaxation therapy on the basis of the control group.And the symptoms,quality of life and emotions before and after intervention were compared.Results After intervention,the psychological symptoms,hormone related symptoms,the digestive system symptoms and self-image disorder symptoms scores in the study group were lower than that in the control group (P < 0.05),the physiological status,functional status,emotion,social/family conditions and additional attention scores were higher than that in the control group (P < 0.05).Conclusion Aerobic exercise and relaxation therapy for patients with breast cancer chemotherapy has a significant effect,and it can effectively improve the quality of life and symptoms,so it is suitable for clinical promotion.
