1.Prenatal ultrasound diagnosis of clubfoot

Objective To investigate prenatal ultrasound diagnosis of clubfoot and its clinical value.Methods Three thousands pregants ranging gestational age from 12 through 40 weeks were checked using Ascuson Sequoia 512 equipment.The diagnosis of clubfoot was made prospectively at the time of the scan by the following criteria: the ball of foot had to be visible in the same plane as the longitudinal section of bones of the lower leg,unchanging with fetal activity and reproducible for several images.All prenatal ultrasonographic dignoses of clubfoot were proved by autopsy or newborn physical examination.Results A total of 17 cases clubfoot fetuses were prenatal diagnosed by ultrasonography.Fourteen of 17 clubfoot fetuses had associated with complex abnormalities,three of them were isolated clubfoot.Fourteen of 17 cases with clubfoot prenatal diagnosed by ultrasound were proved by autopsy.Another three cases with clubfoot were proved by newborn physical examination,two of them were false positive diagnosis.Conclusions Prenatal diagnosis of clubfoot is reliable by ultrasound.It is helpful in diagnosis with other structural abnormalities associated with clubfoot.It can also provide direction for clinical obstetrical management.
2.Prenatal ultrasound diagnosis of fetal radial ray abnormalities

Objective To investigate prenatal ultrasound diagnosis of radial ray abnormalities and its clinical value. Methods All the fetus that entered usual prenatal ultrasound diagnosis were scanned each limb using a systematic continuous sequence approach (SCSA) with Acuson Sequoia 512 and GE Logiq 400 equipments. All the artificial aborted fetal radius malformation also had ultrasound examination in a simulating intrauterine estate and /or X-ray examination. Results Correctly diagnosis was made in 13 cases fetal radial ray abnormalities by prenatal ultrasonography following SCSA through the scaning course. All the fetal radial ray abnormalities had radius absence(only one skeleton in the forearm) or radius dysplasia and characteristic radial club hand. Ray abnormalities can be classifyied into three types on ultrasonography: type Ⅰ, radius absence; type Ⅱ,radius part absence; type Ⅲ,radius dysplasia.Conclusions Mastering characteristic radial club hand and SCSA are the keys to diagnose fetal radial ray abnormalities by prenatal ultrasonography.
3.STUDY ON IMPROVED McAb-ELISA IN DETECTING BLOOD STAGE P.VIVAX ANTIGEN

This paper reports an improved McAb-ELISA test in detecting blood stage P.vivax antigen in which the plates were coated with rabbit anti-P.cynomolgi poly-antibody and two monoclonal antibodies were used together for reaction. The coincidence rate with microscopically conformed P.vivax blood samples was 94.3% and the coincidence rate with microscopically negative blood samples was 96.1%. The sensitivity was over 1 parasite/106 erythrocytes.
4.Targeted monitoring on healthcare-associated infection in intensive care unit of a tumor hospital

Objective To understand the results and characteristics of targeted monitoring of healthcare-associated infection (HAI)in an intensive care unit (ICU)of a tumor hospital,so as to provide scientific basis for formulating measures of prevention and control of HAI.Methods Patients who admitted to an ICU from January to December 2013 were performed targeted monitoring,incidence of HAI was adjusted with an average severity of illness score (ASIS),three types of invasive procedure-related infection,distribution of pathogens,and multidrug-resistant or-ganisms were monitored.Results A total of 455 patients were monitored,the total patient-days were 2 483 days, 52 cases of HAI occurred,incidence of HAI was 11 .43%,incidence of HAI per 1 000 patient-days was 20.94‰,af-ter the adjustment with ASIS,incidence of HAI per 1 000 patient-days was 6.29‰.The main infection site was re-spiratory system(63.46%),followed by urinary system(19.23%);Incidence of ventilator-associated pneumonia per 1 000 ventilation-days,catheter-related bloodstream infection per 1 000 catheter-days,and catheter-associated urinary tract infection per 1 000 urinary catheter days were 12.08‰,0.95‰,and 4.07‰ respectively.A total of 181 path-ogenic isolates were detected,74.59% (n=135)of which were gram-negative bacteria,18.23%(n=33)were gram-positive bacteria,and 7.18%(n=13)were fungi;18.23%(n=33)were multidrug-resistant strains.Conclusion ICU is the high risk department of the occurrence of HAI,the main infection is respiratory tract infection,the major pathogen is gram-negative bacteria.Targeted monitoring can timely reflect the deficiency in infection control practice,is helpful for formula-ting effective prevention and intervention measures,and reducing the occurrence of HAI.
5.Nasopharyngeal Carcinoma:Correlation Between Peripheral Deeper Infiltration and Prognosis and Expression of P53 Protein

Objective To investigate the correlation between degree of deeper infiltration of NPC on CT scan,prognosis and expression of P53 protein,in order to further recognize bilogical features of NPC.Methods Collect 70 NPC biopsy specimens,which had performed CT scan before radiotherapy and had detailed clinical records.The expression of P53 protein was examined by means of S-P immunohistochemical technique.The correlation between expression of P53 protein and peripheral deeper infiltration and prognosis was analysed in detailed.Results The results of these cases showed that the positive rate of over expression P53 were 68.57%,which had no relation with the histological types and pathological grades.The positive rate of P53 in Ⅱ、Ⅲ types had a higher positive rate than that of the Ⅰ type of NPC.Expression of P53 protein had no relation with neck lymph node metastasis,but had significant correlation with the size of metastatic lymph node,which is the positive rate of P53 in diameter of metastatic lymph node <4 cm was higher than that of diameter ≥4 cm.Expression of P53 had no relation with local recurrence and distant metastasis after radiotherapy.Conclusion With the tumor infiltrating the deeper layer of nasopharynx,the positive rate of P53 expression is more and more high,and degree of malignant is increasing.The three types on CT scan represents the biological features of NPC.
6.Comparative diagnosis of malformations of forearm and crus by prenatal and fetus in vitro ultrasonography

Objective To assess the value of prenatal ultrasonography on fetal forearm and crus malformations by studying the ultrasonographic characteristic in relation to methods of examination. Methods All fetus were evaluated by using a systematic continuous sequence approach (SCSA) with ultrasonography. A close attention was paid on shapes, structures and movement of fetal forearm and crus, and fetus specimens after induced labor were rescanned by ultrasonography under the condition of mimic intra-uterus. Results Thirty of 33 cases (90.9%) with fetal forearm and crus malformations (totally 48 of 54 limb anormalies, 88.9%) were correctly diagnosed by prenatal ultrasonography. Six limb malformations of 3 cases were missed. Conclusion Our study demonstrates that the malformations of forearm and crus detected with prenatal ultrasonography are highly accordant with the malformations revealed in fetus in vitro. It is important to abide by the SCSA in ultrasonographic diagnosis of fetal forearm and crus malformation.
7.SURVEILLANCE ON RESISTANCE OF ANOPHELES VECTORS TO PYRETHROID IN JIANGSU PROVINCE

Objective To understand the resistance of Anopheles vectors to pyrethroid after several years mosquito control with the impregnated bed net method. Methods The resistant level was measured by the tentative diagnostic dose and degree method of China combined with the standard methods of WHO. Results In the area after more than 5 years mosquito control with the impregnated bed net method, An.sinensis had initial resistance to deltamethrin and permethrin. However, An.anthropophagus did not be captured. In the area after less than 5 years mosquito control with the impregnated bed net method, both An. sinensis and An.anthropophagus were susceptive to deltamethrin and permethrin. Conclusion In this research, no resistance of An.anthropophagus to deltamethrin and permethrin has been found. An.sinensis has initial resistance to them. So we must pay more attention to monitor the resistance in future malaria control programs.
8.Prenatal sonographic diagnosis on agenesis of fetal corpus callosum

Objective To assess the value of prenatal sonographic diagnosis on fetal agenesis of corpus callosum(ACC). Methods A retrospective study was on prenatal sonographic findings of 10 cases with ACC malformation and their abnormalities in central nervous system (CNS) or extra-CNS.Results The special sonographic findings established the diagnosis of ACC malformation in all 10 cases,with 7cases diagnosed absence agenesis. Among all patients with ACC,6 cases were accompanied with abnormalities in extra-CNS,8 in CNS and 5 in both. Conclusions Prenatal ultrasonography plays a vital role in accurate diagnosis on fetal ACC. Attentions should be paid to the indirect encephalic features and complicated abnormalities so as to make accurate and prompt diagnosis.
9.Predictive value of endometrial receptivity and pregnancy outcome by hysteroscopy examination at the phase of implantation window in unexplained infertile women

Objective To explore predictive value of endometrial receptivity and pregnancy outcome by hysteroscopy examination at the phase of implantation window in unexplained infertile women.Methods From Oct.2007 to Mar.2009,93 unexplained infertile women underwent hysteroscopy examination at 7-9 days after a spontaneous ovulation in Family Planning Research Institute of Guangdong Province.According to the endometrial glandular openings and vascular shape,79 cases without pathological endometrial changes were divided into 60 cases in good endometrium group and 19 cases in poor endometrium group.The following clinical parameters were analyzed and compared between two groups,including endometrial configuration,thickness,secretion,the development and number of pinopodes,vascular distribution,and the level of sex hormone,leukemia inhibitory factor (LIF) and glycodelin in the uterine flushing,and pregnancy outcome.Results (1)There was no statistical difference in the level of serum estrogen and progesterone at the phase of implantation window,which were (518 ± 176)pmol/L,(40 ±20)nmol/L in good group and (513 ±244) ptnol/L,(37 ± 19) nmol/L in poor group (P<0.05).The endometrium thickness at periovulatroy and implantation window days (1.06 ±0.10)cm/(1.16 ± 0.08)cm in good group did not show significant difference with (0.93 ±0.12) cm /(1.02 ±0.10) cm in poor group (P>0.05).The proportion of type A,B and C endometrium at periovulatory days were 63% (12/19),37% (7/19) and 0 (0/19) in good group and 23% (14/60),77% (46/60) and 0 (0/60) in poor group.When compared with those of type A or B between two groups respectively,it all showed statistical difference (P<0.05).However,at phase of implantation window,endometrium configurations were all type B at both groups.(2)90% (17/19)of women in good group and 7% (4/60)of women in poor group showed normal endometrial secretion function,which showed significant differences (P< 0.01).(3)The percentage of fully developed pinopodes and abundant pinopodes [84% (16/19) and 90% (17/19)] in good group were significantly higher than 42% (25/60)and 57% (34/60) in poor group (P<0.05).(4) The level of CD_(34) expression and microvessel density[MVD; (40.1 ± 1.2) positive unit(PU) and(21.7 ±4.0)/high power field (HP)] in good group were significantly higher than(18.1 ± 1.3) PU and (8.5 ± 1.3)/HP in poor group (P< 0.01).(5)The level of LIF and glycodelin in uterine flushing [(72 ± 54)ng/L and (196 ±20)μg/L] in good group were significantly higher than (15±16) ng/L and (116 ±26) μg/L in poor group (P<0.05).(6) The rate of clinical pregnancy,spontaneous abortion and term delivery were 74% (14/19),0 (0/14) and 100% (14/14) in good group and 23% (14/60),14% (2/14) and 86% (12/14) in poor group,the rate of clinical pregnancy and term delivery in good group were significantly increased when compared with those in poor group (P<0.01).Conclusions Hysteroscopy examination at the phase of implantation window could reflect the development of glandular openings and vasculature.It is a preferable method to evaluate the endometrial receptivity and predict pregnancy outcome.
10.Review of relationship between knee degeneration and gender-age change

Objective To analyze the relationship between knee degenerative changes and gender-age changes and review the literature. Methods 2357 cases of knee joint X-ray films from January 2015 to October 2017 in all patients and health examination population in our hospital were retrospectively analyzed. All observers were grouped according to gender and age and divided into 5 groups according to age: 40 to 49 years old, 50 to 59 years old, 60 to 69 years old, 70 to 79 years old, and 80 years old or older. For all knee joints, the X-ray film was graded by the Kellgren & Lawrence classification method and was specifically divided into grade 0, grade I, grade Ⅱ, grade Ⅲ, and grade Ⅳ total 5 grades. Analyzed the ratio of all levels in each age group. Results Regardless of men or women, the degree of degeneration of knee joints increased with age, and in the same age group at all ages, the degree of female knee degeneration was higher than that of men. Conclusion The incidence of degenerative osteoarthritis of knee joint increases with age. Prophylaxis, early detection, early diagnosis and early treatment are all significant to improve the quality of life of patients, delay the progress of the disease as far as possible.
