OBJECTIVE To investigate the clinical feature of nosocomial infection and the type of pathogens in general hospital, and provide the scientific theory basis in order to prevent and control the nosocomial infection. METHODS An analysis was made on the basis of the survey, the data came from hospitalized cases from 2000 to 2006. RESULTS Their prevalence rate was 3.8%. The highest infected rate was in ICU (16.9%). The main infection site was respiratory tract. Among pathogens isolated from nosocomial infection cases, 45.65% were Gram-negatives, 31.9% were fungi, and 20.58% were Gram-positives. The formation rate of the Enterococcus gradually increasesd from year to year. CONCLUSIONS There are many effective actions to reduce the nosocomial infection, such as strengthening the medical training, carrying out the aseptic technology strictly, reducing the invasive operation, raising the executable rate of hand hygiene, and using the antibacterial drug reasonably.

OBJECTIVE To evaluate the clinical application of the two surgical hand antisepsis methods on hand sterilizing effect. METHODS Totally 123 medical staff of operating theater were divided into Avagard surgical washing liquid (60) and orthodox surgical hand antisepsis (63) groups at random. The samples were derived from fingers and cultured before,just after and 2h after sterilization. RESULTS The colony counts were 4.64?4.87,0.05?0.16 and 0.02?0.10 CFU/cm2,respectively in avagard surgical washing liquid group;vs 4.10?4.03,0.08?0.26 and 0.49?1.31 CFU/cm2,respectively,in orthodox surgical hand antisepsis group. Objective To explore the clinical significance of preserving the intercostobrachial nerve (ICBN) in the radical resection of breast cancer. Method All of 140 cases of breast cancer with Ⅰ - Ⅲ a stage operable eligible,were randomly divided into the experimental group with 79 cases (7 cases were rejected, 1 case was lost) that patients received radical resection of breast cancer with preservation of ICBN,and the control group with 61 cases (2 cases were lost) that patients received routine radical resection of breast cancer based on the time of admission. Analyzed postoperative local recurrence and compared to sensory dysfunction of the two groups. Results There was no significant difference between the two groups of patients in the rate of postoperative local recurrence [0-4.2% ( 3/71 ) vs. 0-3.4% (2/59)] ( P ＞ 0.05 ). The local sensory disturbance of the experimental group [2.8% (2/71 )-11.3% (8/71)] was significantly lower than the control group [54.2%(32/59)-71.2%(42/59)], there was significant difference between the two groups (P ＜ 0.01 ). Conclusion Radical resection of breast cancer with preservation of ICBN is an ideal operation in breast cancer.

Objective To analyze the CT appearances and the classification of gray matter heterotopia.Methods The clinical and CT data of 26 cases with gray matter heterotopia were retrospectively reviewed,11 were males and 15 were females,ranged in age from 2 days to 9 years with a mean of 2.6 years.Results The classification of heterotopia included:(1)Subependymal heterotopia in 14,5 cases with encephaloceles,1 case with Dandy-Walker malformation and 1 case with arachnoid cyst of cisterna megna.(2)Subcortical heterotopia in 7,4 cases with callosal agenesis were accompanied.(3)Band heterotopia in 5.Conclusion CT scan can not only reveal the appearances of the subependymal,subcortical and band types heterotopia,but also show other associated abnormalities.

Objective To investigate the value of high frequency 2-Dimensional and color Doppler flow imaging combined with mammography on the diagnosis of breast cancer. Methods 90 patients with breast cancer and benign breast lesions in 22 cases, respectively, separate by color Doppler ultrasound and mammography and their combination were compared. Results The blood flow signals were Ⅱ～Ⅲ grade(76.32% ) in breast cancer and benign breast lesions less blood flow to 0 ～Ⅰ grade ( 87.10% ); The detection rate of blood flow 89.48% in breast cancer were significantly higher than 38.71% in benign breast lesions ( x2 = 4.57, P ＜ 0. 05 ); 68 cases (75.5 % ) were diagnosed by mammography 38 cases(55.8% ) in diameter≤2. 0em were significantly higher than 30 cases(44.4% ) in ≤ 1.0 cm( x2 = 3.97, P ＜ 0. 05 ); The corresponding rate of Joint Inspection 85.7 % (96/112) was higher than the r 72.3% (81/112) in color Doppler ultrasound and 75.8% (85/112) in mammography ( x2 = 3.87, x2 = 3.85, all P ＜0. 05 ). Conclusion Color ultrasonography combined with mammography could raise diagnostic rate of breast cancer.Their combination is the best one of breast imaging examinations.

Objective To explore an evaluation method of quality of care using the 2012 medical record front page.Methods The admission pathway and discharge method information of inpatients discharge recorded in the medical record front pages of the hospital were chosen and evaluated with contingency tables.Results The outcomes indicated that the differences between discharge methods of those admitted through different pathways are significant statistically (x2 =856.55,P ＜ 0.05).The correlation analysis showed that the coefficient of Spearman correlation between the inpatients admitted through different pathways and their discharge methods was -0.11 (P＜0.05).Calculated as such,the score of quality of care was 4.95 for the hospital as a whole.Conclusion The establishment of this evaluation system showed that the indicators set for quality of care using the 2012 medical record front page are scientific,and the results are more objective.

Objective To study the appearances of split cord malformation(SCM)and evaluate the diagnostic value of CT for SCM.Methods Clinical and CT data of 48 cases with SCM were analyzed retrospectively ,21 were males and 27 were females,ranged from 1 day to 8 years with a mean of 11.6 months. All cases evaluated by plain CT with coronal and sagittal reconstructions.Results Type I accounted 75%, consisted of two hemicords, each contained in its dural tube and separated by a rigid median septum .TypeⅡaccounted 25%, consisted two hemicords contained in a single dural sac separated by a non-rigid, fibrous median septum. Associated abnormalities: tethered cord syndrome(n=38), syringomyelia(n=9), intradural lipomas(n=10), meningocele(n=18).Conclusion CT can clearly demostrate the position, the septum and the shape of the SCM, as well as associated abnormalities.

Objective To investigate ATM deletion [del (ATM)] in chronic lymphocytic leukemia (CLL) and study its correlation with the clinical stage. Methods Spectrum Orange~(TM) labeled sequence specific DNA probe for ATM locus on 11q22.3 and fluorescence in Situ hybridization (FISH) was used to examine del (ATM) in 28 newly diagnose patients with CLL. FISH analysis were performed on bone marrow smears. Clinical staging was done according to Binet Method.Fisher exact propability was used to study the relations between del (ATM) and clinical feature. Results 4 out of the 28 cases were found with deletion of ATM. The incidence of del (ATM) in BinetA, BinetB and BinetC was 1/9 (11.1 %), 1/8 (12.5 %), 2/11 (18.2 %), respectively. Fisher exact propability showed that deletion of ATM was not associated with its clinical feature. Conclusion Application of FISH on archival bone marrow smears is a simple, liable method, and can be readly used to retrospective study of clonal blood system diseases. Deletion of ATM was common cytogenetic changes in CLL patients.And the significance of del (ATM) in the prognosis of CLL in China needs to be further investigated.

Objective To explore the extensively drug resistant mechanism and clinical treatment strategy of Klebsiella pneu-moniae .Methods The isolate was identified by Vitek2 Compact System.Antimicrobial susceptibility testing was conducted by Kirby-Bauer method.KPC-2 carbapenemase was detected by modified Hodge test.The gene encoding KPC-2 carbapenemase was amplified by polymerase chain reaction (PCR)and then sequenced.Results The strain was resistant to all antibiotics used in routine antimicrobial susceptibility testing except amikacin.Modified Hodge test showed positive result.KPC-2 gene was detected by PCR.The sequence was consistent with that of 11844849 in GenBank.After treatment for one month,no exten-sively drug resistant K.pneumoniae strain was detected from the patient.Conclusions It is necessary to strengthen the monito-ring and improve the awareness of extensively drug resistant K.pneumoniae for better control of such infections.
