Objective:To investigate the follow-up colonoscopy of patients after colorectal adenoma (CRA) resection, so as to improve colonoscopy quality and reduce missed CRA diagnosis. Methods:A total of 189 patients after CRA resection who underwent follow-up colonoscopy from October 2020 to October 2021 were consecutively recruited. The effect of bowel preparation was evaluated by the Boston bowel preparation scale (BBPS). The location, the number, and gross morphology of adenomas during CRA resection and two colonoscopies during 1 year follow-up after the resection were recorded, and their correlation with major clinicopathologic features were analyzed, and the missed CRA in follow-up colonoscopy were also analyzed. Results:The BBPS scores of the right colon, transverse colon, left colon and total colon in 189 patients who underwent CRA resection were 1.93±0.42, 2.53±0.52, 2.77±0.45, and 7.22±1.03 points at the 1-year follow-up colonoscopy and 2.08±0.35, 2.70±0.46, 2.81±0.40, and 7.57±0.84 points at the time of CRA resection. Except for the left colon segment ( χ 2=0.98, P>0.05), the differences in other colon segments were significantly different ( P<0.05). At the time of CRA resection, 62, 66, 210 and 338 CRAs were detected in the right colon, transverse colon, left colon and total colon segments, respectively. At the 1-year follow-up colonoscopy, 21, 38, 49 and 108 CRAs were missed in the corresponding colon segments and total colon, respectively. CRA missed diagnosis was related to patient age (≥60 years accounting for 54.2%), gender (males accounting for 81.9%), adenoma location (predominantly in the left colon comprising 36.2%) and gross morphology (flat types representing 72.3%) ( P<0.05), but not related to being vegetarian, history of cholecystectomy or appendectomy, smoking or alcohol consumption, number of adenomas or pathological type ( P>0.05). Conclusion:The missed diagnosis of CRA in patients after CRA resection primarily involves the flat type adenomas, which is associated with the age, gender, adenoma location within the colon and the gross morphology.

Objective:To explore the factors influencing the recovery of swallowing function after a brainstem hemorrhage and to construct a prediction model. Methods:Clinical data on 134 persons with dysphagia after a brainstem hemorrhage were collected retrospectively. According to their swallowing ability at discharge, the patients were divided into a swallowing recovery group and a non-recovery group. Univariate correlation analysis and multivariate logistic regression analysis were used to explore the independent factors influencing the recovery of swallowing function and to construct a prediction nomogram. The receiver operating characteristics (ROC) curves were evaluated to analyze the nomogram′s predictive value and those of the relevant influencing factors.Results:Sixty-two of the patients (46%) had recovered their swallowing function at discharge, while 72 (54%) had not. Univariate correlation analysis showed that there had been significant differences in tracheal intubation, NIHSS score, FOIS score, Barthel index and Glasgow coma scale (GCS )score between the two groups, on average. The multivariate logistic regressions showed that a low NIHSS score, a high FOIS score and a high GCS score were independent predictors of swallowing function recovery, so they were used in the prediction model. ROC curve analysis showed that the area under the curve (AUC) of the prediction model was 0.953 (95% CI: 0.902~0.982) with a sensitivity of 87% and a specificity of 93%. The model′s predictions were thus better than using an NIHSS score, GCS score or FOIS score alone. Conclusions:NIHSS score, GCS score and FOIS score can independently predict the recovery of swallowing function after a brainstem hemorrhage. A prediction model constructed using all three has good predictive power.

Chinese Journal of Modern Nursing 2025;31(3):293-298
Objective:To explore the current status and influencing factors of the core competencies of nursing interns based on a mixed-methods approach, with the goal of providing a reference for the reform of high-quality teaching models and the enhancement of nursing students' core competencies. Methods:An explanatory sequential research design of mixed-methods research was adopted. Using convenience sampling, 105 nursing interns who were on clinical practice at Beijing Tiantan Hospital, Capital Medical University, from July to August 2022, were selected as participants. A general information questionnaire and a core competency scale for nursing students were used for data collection, and multiple linear regression analysis was conducted to identify the factors influencing the core competencies. From September to October 2022, purposive sampling was used to select six nursing interns for semi-structured interviews. Thematic framework analysis was used to extract opinions, categorize inferences, and summarize themes. Results:The overall average score of the core competencies of the 105 nursing interns was (6.11±0.74). The highest mean score was for the "moral and responsibility" dimension, which was (6.36±0.93), while the lowest was for the "clinical biomedical science" dimension, with a mean score of (5.21±1.04). Parental recognition of the nursing profession was identified as a factor influencing the core competencies ( P<0.05). The qualitative interviews yielded three themes: the teaching ability of instructors directly impacts the enhancement of interns' core competencies; professional identity influences nursing interns' motivation to improve core competencies; clinical practice is an effective way to enhance the core competencies of nursing interns. Conclusions:In this study, the core competencies of nursing interns were found to be at a moderately high level, influenced by various factors. Insufficient teaching resources and clinical practice opportunities are objective factors affecting the development of core competencies. The ability of interns to gain recognition of the nursing profession through external or self-practice and gradually build professional confidence is an intrinsic motivation for the development of core competencies.
