1.Influence of enhanced patient education on the patients' knowledge and awareness of peripherally inserted central catheter

Objective To explore the usefulness of enhanced patient education in increasing the patients' knowledge and awareness of peripherally inserted central catheter (PICC) protection.Methods Totally 200 patients with PICC in Neijiang First People's Hospital from June 2009 to June 2012 were divided into control group (odd numbers,conventional patient education,n =100) and observation group (even numbers,enhanced patient education,n =100) according to their admission numbers.The patients' knowledge and awareness of PICC protection were analyzed after treatment,and the incidences of adverse events and catheter indwelling time were also compared between these two groups.Results Compared with the control group,the observation group had significantly higher PICC protection knowledge rate (91% vs.78%,P =0.001),awarehess rate (95% vs.80%,P=0.011),lower incidence of adverse events (1% vs.5%,P =0.034),and longer PICC indwelling time [(125.8 ± 10.6) d vs.(106.7 ±8.2) d,P =0.026].Conclusion Enhanced patient education can effectively enhance the patients' knowledge and awareness of PICC protection.

Objective:To study the effect of the three kinds of EE on the cognitive function and the expression of SYN and MAP-2,and the dentritic growth of pyramidal cells of the cerebral cortex in rats.Method:Forty-eight male SD rats were divided into preserving the physique EE group (EE1),preserving the spirit EE group (EE2),preservation for both physique and spirit EE group(EE3) and standard group (SE),12 rats in each group.Rats were put into the three kinds of EE or standard cages after pre-training.The environmental enrollment lasted for 14 days and followed by a 6-days Morris water maze test.After the Morris water maze test,all the rats were sacrificed for histological study,including using the immunohistochemistry method to detect the expression of SYN and MAP-2,and using the Golgi-Cox staining to study the dentritic growth of pyramidal cells in cerebral cortex.Result:In the Morris water maze,the EE3 group exhibited shorter escape latency than the MCAO+SE group (P<0.05) on day 5,while no significant differences were seen for the time spent in the target quadrant among the 4 groups (P> 0.05).The results of immunohistochemical staining of SYN showed that the expression of SYN was the lowest in the SE group,and there was no significant difference between EE1 and EE3 groups but significantly higher than the EE2 group.The expression of MAP-2 was the lowest in the SE group,and there was no significant difference of expression between EE1 and EE2 groups but significantly lower than the EE3 group.The results of Golgi-Cox staining showed that there was no difference of the dentritic branch among groups in mPFC (P> 0.05) while the spine density of the EE1 group was significantly greater than that of the SE and EE2 groups (P < 0.05),but significantly lower than that of the EE3 group (P < 0.05).Conclusion:Two weeks of preservation from both physique and spirit EE can improve the cognitive function,and promote the expression of MAP-2 and SYN and the dentritic growth of pyramidal cells in normal rats.

Earthquake may result in various urologic problems by direct injury of urologic organs or by spinal cord injury.This article introduces the main urologic problems after earthquake,classified into early and long-term.Such as urinary retention,oliguria or anuria,renal injury,bladder and urethra injury,neurogenic bladder caused by spinal cord injury,et al.We analyze the causes of the problems and discuss the correct management principle,all kinds of rehabilitation measures to reduce the complication and sequela,also the problems should be cautioned during the long term follow-up.
8.Application of different doses of dexmedetomidine and midazolam in dental implant surgery.

OBJECTIVETo compare the sedative effects of different doses of dexmedetomidine (DEX) and midazolam (MDZ) in dental implant surgery:
METHODSSixty patients undergoing dental implantation were selected and divided randomly into six groups (10 in each group). In group 1 (G1), a highdose of MDZ alone was administered intravenously. In group 2 (G2), a relatively low dose of MDZ and DEX was administered intravenously. In group 3 (G3), a highdose of MDZ and DEX was administered intravenously. In group 4 (G4), a lowdose of MDZ and a highdose of DEX were administered. In group 5(G5), a highdose of MDZ and a low dose of DEX were administered. In group 6 (G6), a highdose of DEX alone was administered intravenously. Blood pressure (BP), heart rate (HR), and arterial oxygen saturation values were measured. Sedation was assessed by determining the Ramsay sedation scores(RSS) during infiltration anesthesia (T1), incision (T2), cutting (T3), and suturing (T4). Before discharging from the hospital, patients were asked if they remembered the T1 to T4 procedures. The visual analogue scale (VAS) and the degree of patient satisfaction were assessed at the same time.
RESULTSIn T1 to T4, the systolic blood pressure (SBP) in G3 was the lowest and was significantly different from that in G1 and G2 (P < 0.05). In G1, the SBP was higher than that in G6 at the T3 and T4 points (P < 0.05). HR was below the preoperative (P < 0.05) in the G3 only. RSS was the lowest in G1 during the whole procedure. In G2, most patients obtained scores of 2-3. In G3, sedation was deeper, had more cases ofup to 5. In G4, most patients obtained scores of 3-4. In G5, some patients had a score of up to 5. The RSS in G6 was lower than that in G3 and G4 or G5 in T1 to T2, closer to G4 in T2. An evaluation of the VAS and the degree of patient satisfaction did not show any differences among the groups.
CONCLUSIONThe combination of DEX and MDZ is superior to a single intravenous injection. Low-dose MDZ combined with high-dose DEX achieved the highest quality of sedation in this study.

9.Preparation and anti-bacterial tests of silver-modified titanium surface.

OBJECTIVETo deposit silver nanoparticles on a titanium surface to obtain antibacterial properties. To reduce the incidence of peri-implantitis, and improve the success rate of implantation.
METHODSA silver nanoparticle-modified titanium (Ti-nAg) surface was prepared using silanization method, and its surface was characterized by using X-ray photoelectron spectroscopy (XPS) and energy dispersive X-ray spectrometer (EDX). Two species of bacteria, namely, Staphylococcus aureus and Escherichia coli, were used to test the antibacterial effect of Ti-nAg surface.
RESULTSScanning electron microscope (SEM) revealed that a small quantity of silver nanoparticles were deposited on the titanium surface. XPS analyses revealed that 6.8% of silver was present on the titanium surface. After 24 h of incubation, 94.23% of Staphylococcus aureus and 95.34% of Escherichia coli were killed on the Ti-nAg surface.
CONCLUSIONResults suggest that silver nanoparticle-modified titanium is a promising material with an antibacterial property that may be used as an implantable biomaterial.

10.Value of contrast-enhanced ultrasound in ultrasound-guided percutaneous biopsy of peripheral lung lesions

Objective To investigate the role of contrast-enhanced ultrasound (CEUS) on the diagnostic efficacy of ultrasound-guided percutaneous biopsy of peripheral lung lesions. Methods Ninty-six patients with pleural-based lesions underwent percutaneous ultrasound-guided biopsy. Of the 96 patients,54patients underwent CEUS before biopsy and enhancement information was referred for the selecting of tissue sampling site. The rest of 42 patients didn' t undergo CEUS before biopsy. The difference in the diagnostic accuracy between the two groups was analyzed to evaluate the value of CEUS before biopsy.Results Of the 52 patients in the CEUS group,48 were malignant,and 6 were benign. Of the 42 patients in the un-CEUS group,36 were malignant, and 6 were benign. Necrosis (areas lacking of enhancement) was demonstrated in 41.7% (20/48) of the patients with malignant lesions in CEUS group, and necrosis incidence was increased with the increase of the dimension of the malignant lesions. No necrosis area was demonstrated in 6 benign lesions. The rate of successful initial biopsy in CEUS group (98.1 % ,53/54) was statistically higher than that in un-CEUS group ( 85.7%, 36/42 ) ( P < 0.05 ). Tissue necrosis and denaturalization attributed to most of the inconclusive pathologic diagnosis. Conclusions CEUS before transthoracic peripheral lung lesion biopsy improved the diagnostic accuracy of the procedure by providing information for differentiating viable or necrotic tissue and consequently provide more accurate information about the site of biopsy.
