1.Impact of Hyperbaric Oxygen Therapy on Blood Pressure and Heart Rate in Adults with Spinal Cord Injury

Objective To observe the impact of hyperbaric oxygen (HBO) therapy on blood pressure and heart rate in adults with spinal cord injury (SCI). Methods 48 adult SCI patients who accepted HBO therapy from March to November, 2014, were observed. Other 48 non-SCI patients matched sexes and ages were as the controls. Their blood pressure and heart rate were measured before chambering, before compression, HBO for 30 minutes, before decompression and at the end of decompression. The patients were divided into high SCI group and low SCI group based on the injury levels above or below T4. Results The blood pressure was lower in the high SCI patients than their controls (t>6.337, P<0.001), and increased as taking in the oxygen. There was no significant difference between the high SCI patients than their controls as they took in for 30 minutes and before the decompression (P>0.05). No difference was found between both groups in heart rate (P>0.05). For the low SCI patients, neither the blood pressure nor the heart rate was different from their controls (P>0.05). Conclusion The blood pressure may increase during HBO intaking in the high SCI patients, which need be paid attention to.
2.Clinical analysis of 24 patients with POEMS syndrome

ObjectiveTo analyze the characters of POEMS syndrome, raise physician awareness of diagnosis and therapy,and explore the optimal treatment strategies.MethodsThe clinical features,laboratory examination and therapy of 24 patients with POEMS syndrome were analyzed,and relative literatures were reviewed. ResultsA strong predominance of male over female was found, 18 vs 6. All patients were over 40 years old,with a mean age 56.5 years old,indicating a common adult involvement.All presented with polyneuropathy,which was also the most common complaints when admitted,which reminded neurologists of underlying possibility of a rare plasma cell disorder.Organomegaly was found,including 19 cases with hepatomegaly,17 patients with splenomegaly.Endocrinal abnormalities were also found in 15 cases.18 patients were M protein positive. Skin pigmentation was recognized in 21 cases. Melphalan in combination with prednisone was applied and 100 % response was observed. One of the patients received peripheral blood stem cell transplantation(PBSCT)2 years ago and a durable response was observed with a continuous complete remission of polyneuropathy and the absence of M protein following 2 years post PBSCT.ConclusionPOEMS syndrome is a rare multisystem disorder, the combination of symptoms and signs is highly complex,which is easy to misdiagnose or missed diagnose.A polyneuropathy with either organomegaly,endocrinal abnormalities, skin disorder or serositis is required a further investigation of clonal M protein and bone marrow.Melphalan and prednisone probable are the optimal regimens.PBSCT provides a new choice for therapy and research of POEMS syndrome.
3.The clinical evaluation of the effect of two different apical surgery timing

Objective:To observe the effect of two different apical surgery timing. Methods:68 patients with periapical lesion were divided into 2 groups. 30 patients( control group) were operated by apical surgery at least 1-2 months after root canal therapy( RCT) , while 38 patients(experimental group) were operated immediately after RCT. The patients were followed up 3, 6 and 12 months after apical surgery. Results:The curative effect analysed with the age, sex and tooth position showed no statistical difference between 2 groups(P>0. 05). Conclusion:Immediate and delayed apical surgery apical surger after RCT are similarly effective.
5.Expression and significance of long non-coding RNA MALAT1 and interleukin 6 in premature infants with bronchopulmonary dysplasia

Objective:To investigate the expression of long non-coding RNA MALAT1, interleukin 6(IL-6) and apoptosis induced factor(AIF) in peripheral venous blood of premature infants with bronchopulmonary dysplasia (BPD) and its clinical significance.Methods:Preterm infants admitted to the Department of Neonatology, Shanghai Children′s Hospital from January 2015 to December 2016 were enrolled.The selection criteria included gestational age (GA) ≥28 weeks and ≤32 weeks, and birth weight (BW) < 1 500 g. According to the diagnosis, they were divided into BPD group (20 cases) and control group (20 cases). The clinical data of the two groups of premature infants were collected and analyzed, and the levels of MALAT1, IL-6 and AIF in the blood of 40 premature infants were detected by real-time polymerase chain reaction (RT-PCR). T test was used to compare gestational age, birth weight, MALAT1, IL-6 and AIF between the two groups. Results:(1)There was no significant differences in sex ( χ2=1.76), gestational age ( t= 0.17) and birth weight ( t=1.25) of premature infants in BPD group, compared with the control group (all P >0.05). (2)Compared with the control group, the expression of MALAT1 in the peripheral blood of premature infants in BPD group were significantly increased (0.273 4±0.067 3 vs. 0.375 5±0.081 9, P<0.05). (3)Compared with the control group, the expression of IL-6 in the peripheral blood of premature infants in BPD group were obviously decreased (1.448 8±0.191 8 vs.4.444 6±0.165 7, P<0.05). (4)Compared with the control group, the expression of AIF in the peripheral blood of premature infants in BPD group were remarkably decreased(0.006 8±0.002 0 vs.0.004 5±0.001 9, P<0.05). Conclusions:MALAT1 and IL-6 levels of long non-coding RNA in BPD and non-BPD preterm infants are different, which may be related to the incidence of BPD.IL-6 may be a predictor of BPD, and MALAT1 may protect premature infants with BPD.
6.Hyaluronidase with α - lipoic acid and Chinese medicine in the treatment of diabetic vitreous hemorrhage

AIM: To study the effectiveness of hyaluronidase periocular injection combined with α - lipoic acid and Chinese medicine in the treatment of diabetic vitreous hemorrhage.
METHODS: Retrospective analysis of 67 patientswith proliferative diabetic retinophathy ( PDR ) and vitreous hemorrhage, were divided into two groups: treatment group and control group. There were 32 cases in every group, the treatment group were giving hyaluronidase periocular injection combined with α - lipoic acid intravenously and with traditional Chinese medicine in promoting blood circulation to remove blood stasis, control group were only given Chinese medicine treatment in promoting blood circulation to remove blood stasis.
RESULTS: Followed up 2-3mo, the total efficiency of treatment group ( 78%) was better than control group (57%), there was statistically significant difference ( P<0-05 ) . The efficiency of the two groups' patients with vitreous hemorrhage more than 2wk time was 22% and 25% respectively, there was no statistically significant difference in the two groups (P>0. 05).
CONCLUSION: Hyaluronidase periocular injection combined with α-lipoic acid and Chinese medicine in the treatment of PDR combined with vitreous hemorrhageis more effective than only use Chinese medicine. But it only has effect for the early, over 2wk, it has no significant effect.
8.Correlation research between single nucleotide polymorphism of methylenetetrahydrofolate reductase and Alzheimer′s disease in South China Han people

Objective To investigate the relationship between single nucleotide polymorphism (677C→T) of methylenetetrahydrofolate reductase and Alzheimer′s disease(AD) in south China Han people. Methods By applying polymerase chain reaction and restriction fragment length polymorphism (PCR-RFLP), MTHFR 677C→T mutation was detected in 45 AD patients and 48 healthy controls. Results The frequency of MTHFR 677C→T mutation of patients showed no significant difference to that of healthy controls (P > 0.05). There is no statistic significance between AD group and controls in C, T gene frequency(P>0.05). But T gene frequency is higher in AD group than in control group. Conclusion MTHFR C 677T is not the pathogenic factor for AD, but could have some effects on AD.
9.Impact of mode of delivery on gut microflora in breast-fed infants

Objective The gastrointestinal tract of neonate becomes colonized with microorganisms from environment and mother immediately after birth. Strong evidences suggested that early composition of the microflora in neonates play an important role for postnatal development of the immune system. This study was designed to investigate the effect of mode of delivery on the development of gut microflora and characteristics of the stool in breast-fed infants by molecular biology methods. Methods Sixty healthy breast-fed term infants were enrolled in the study(including vaginal delivery group and cesarean section delivery group,each group included 30 infants). Anthropometric measurements and stool study were done at 6-week,8-week,10-week,and 12-week. Fecal samples were subjected to quantitative fluorescence real-time polymerse chain reaction assays for the enumeration of lactobacili,bifidobacteria,and subtype of bifidobacteria(bifidobacterium longum and bifidobacterium breve). Results The numbers of lactobacilli and bifidobacterium longum species were less in cesarean delivery group than those in vaginal delivery group(P < 0.05). Stool mean pH was lower in vaginal delivery group than that in cesarean section delivery group(P < 0.05). There were no differences in stool characteristics and growth between two groups. Conclusions Cesarean section is associated with the decrease of bifidobacteria and lactobacilli in breast-fed term infants.
10.SWOT Analysis of Centralized Purchasing Management Model for Medicinal Material Under Separation of the Four Powers in Tertiary General Hospital

OBJECTIVE:To standardize hospital purchasing management for medicinal material. METHODS:The separation of the four powers,including plan,purchase,supply,and payment,was practiced on the basis of centralized purchasing medicinal material in our hospital. SWOT method was used to analyze the strengths,weaknesses,opportunities and threats of the model;im-provement measures were formulated;finally the effects of the model were evaluated. RESULTS:This model played an active role in the professional advantage of purchasing management,made up for the lack of management system and manpower shortage dis-advantage;adapted to policies to seize the opportunities for improvement,reduced the threat of standard deletion and purchasing cost,and promoted reasonable consumption of medicinal materials. And then the purchasing cost of our hospital was reduced after system procedure improvement,fine management of medicinal material and professional level improvement,medicine ratio fell from 42% in 2010 to 37% in 2014;logistics support became stable and standard,and the level of medicinal material supply was improved. CONCLUSIONS:The purchasing model for medicinal material under separation of the four powers provides the refer-ence for the correct course of the standardized,normalized and modernized hospital purchasing management.
