3.Progress in low anterior resection syndrome of rectal cancer

With the development of surgical instruments and improved surgical techniques, more and more patients with low rectal cancer can get the effect of the radical cure while preserving the anus. Still, nearly 60%-90% of patients will undergo low anterior resection after surgery Syndrome (down anterior resection syndrome, LARS) LARS can seriously affect patients′ quality of life after surgery. This article will explain the concept, symptoms, possible pathogenesis, prevention, and treatment measures of LARS and provide help for the diagnosis, treatment and prevention of LARS.
4.Difference of human epididymis protein 4 and carbohydrate antigen 125 in patients with ovarian cancer before and after menopause

Objective:To compare the difference of human epididymis protein 4(HE4) and carbohydrate antigen 125(CA125) indexes before and after menopause in ovarian cancer patients, and to evaluate the efficacy of risk of ovarian malignancy algorithm(ROMA) index in early diagnosis.Methods:The clinical data of 236 patients with ovarian cancer (malignant ovarian tumor group) who were hospitalized in Cancer Hospital of China Medical University from April 2018 to September 2019 were retrospectively analyzed, and 312 benign patients treated at the same period were selected as the control group (benign ovarian tumor group). On the basis of clinical pathological section confirmation, the serum samples were collected to detect HE4 and CA125, and the ROMA index was calculated according to the formula.The differences of HE4, CA125 and ROMA indexes before and after menopause, as well as the diagnostic efficacy of ROC survival curve of the three were compared, and the relationship between the different clinical characteristics of ovocarcinoma patients was evaluated to comprehensively analyze the early diagnostic effect of ROMA index.Results:In the serum of patients with malignant ovarian cancer, HE4, CA125 and ROMA index were abnormally high expression, and the difference between HE4 before(263.2(128.4, 385.6) pmol/L) and after menopause(354.8(179.5, 554.2) pmol/L) was statistically significant( Z=2.09, P=0.037). Through fitting the ROC curve: the AUC of premenopausal ROMA index was 0.939, and the AUC of postmenopausal ROMA index was 0.933, which were higher than HE4 (0.937, 0.906) and CA125 (0.913, 0.924), and their corresponding Youden correct diagnostic indexes were 0.800 and 0.742 respectively, which were higher than HE4 (0.788, 0.653) and CA125 (0.498, 0.565). The two categories of HE4 were associated with the occurrence of peritoneal effusion (the percentage of postmenopausal and premenopausal HE4<140 nmol/L and premenopausal HE4<70 nmol/L were 25.77%, 74.23% and 74.23% respectively), and the difference was statistically significant (χ 2=10.155, P=0.006). CA125 was associated with the classification of the largest diameter of parenchymal tumor and the occurrence of peritoneal effusion (all P<0.05). ROMA index was associated with menopause or not, classification of maximum diameter of mass, classification of maximum diameter of parenchymal tumor and occurrence of ascites, and the differences were statistically significant (all P<0.05). Conclusion:ROMA index has a good diagnostic efficiency in the diagnosis of malignant ovarian cancer, especially premenopausal patients with good diagnostic efficiency, for early diagnosis of ovarian cancer has a certain guiding value, the follow-up will further carry out confirmatory research.
5.Prevention of ventilator-associated pneumonia in neonatal intensive care unit

Ventilator-associated pneumonia (VAP) is a common hospital acquired infection in neonatal intensive care unit with high incidence in China. There is no diagnostic gold standard of VAP. The risk factors include premature birth, low birth weight, dura-tion of mechanical ventilation, episodes of intubation, blood stream infections. The recommended precautions include rising head of bed, closed tracheal suction, sucralfate, selective decontamination in digestive tract, improvement of mechanical ventilation, oral and skin care, hand hygiene. However, it is controversial that sucralfate, selective decontamination in digestive tract, passive humidifiers, oral and skin care can reduce the incidence of VAP.

Multidetector CT scanners contribute to reduced scanner time, decreased section collimation and increased scan length.

The hyperamplification in centrosomes give rise to a series of biochemical events including formation of multipolar spindle, abnormal segregation of chromosome and aneuploid, promoting the performance of chromosome instability, which is the main pathogenesis of multiple human malignancies.STIL participates in the formation of centrioles, activates CDK1/CyclinB1 complex and promotes mitotic entry.Moreover, it regulated expression of related downstream genes through Sonic Hedgehog signal pathway.Thus, STIL correlates with gastric and pancreatic carcinogenesis and progression.We aim to review the structure and function of STIL, association with malignancies and its potential mechanism.

Objective To study the protective effect of Mailuoning on ischemia/reperfusion injury in extremities. Methods The experimental models of ischemia/reperfusion injury in extremities were produced in 20 healthy adult rabbits, which were randomly divided into control group and Mailuoning group (10 rabbits each). Normal saline and Mailuoning injection were infused intravenously into ear margin of rabbits in the control group and Mailuoning group respectively just before blood reperfusion. At preischemia, two hours post ischemia, one hour post reperfusion and four hours post reperfusion, blood sample was collected from the femoral veins respectively for measurements of aspartate aminotransferase (AST), lactic dehydrogenase (LDH), creatine kinase (CK), malondialdehyde (MDA) and superoxide dismutase (SOD). Meanwhile, the tibialis anterior muscles were harvested for examination of the microstructure and the ultrastructure by using optical microscope and electron microscope. 10.Expression of EBF3 in hepatocellular carcinoma and the effect of EBF3 overexpression on HepG2 cell proliferation

Objective To investigate the expression and clinical significance of early B-cell factor 3 (EBF3) mRNA and protein in hepatocellular carcinoma(HCC) and the effect of EBF3 overexpression on HepG2 cell proliferation. Methods The expression levels of EBF3 mRNA in 20 pairs surgical specimens of HCC and their distant noncancerous tissues were detected by fluorescence quantitative real-time polymerase chain reaction(FQ-RT-PCR). Western blot was used to detect the expression levels of EBF3 protein in 5 pairs surgical specimens of HCC and distant noncancerous tissues. The fusion protein EBF3-EGFP was ex-pressed in HepG2 cells by transfection of pEGFP/EBF3 into the synchronized HepG2 cells using lipo-fectAMINE 2000 regent. Expression of EBF3-EGFP fusion protein was observed under the inverted fluores-cence microscope. S-phase fraction(SPF) and proliferating index(PI) were analyzed with flow cytometry (FCM). Results The ratio of EBF3 mRNA to β_2 mRNA in HCC tissues was significantly higher than that in distant liver non-cancerous tissues(0.55 ±0.12 versus 0. 22 ± 0.23, t = 5.69, P < 0.001 ). EBF3 protein in nuclear extracts of HCC tissues was about 4 fold that in distant non-cancerous tissues (26.35 ±14.06 versus 7.86 ± 8.47, t = 2.52, P = 0.036). Fluorescence microscopy revealed that 24 h after trans-fection of pEGFP/EBF3 into hepatoma HepG2, the fluorescence of EBF3-EGFP fusion protein was mainly observed in the nucleus. After transfection for 24 h and 48 h, SPF and PI were markedly increased in HepG2 cells transfected by pEGFP/EBF3 as compared with that in pEGFP-N1 transfeeted cells. Conclusion The expression of EBF3 at both mRNA and protein levels was up-regulated in HCC tissues. EBF3 promotes HepG2 cells proliferation through DNA replication, effect of EBF3 in ttCC needs to be further investigated.
