1.Effect of intracoronary verapamil on coronary flow,myocardial perfusion and clinical outcome during percutaneous coronary intervention for acute myocardial infarction

Objective To compare the effects of intracoronary verapamil on coronary flow,myocardial perfusion and clinical outcome during primary percutaneous coronary intervention(PCI) for acute ST elevation myocardial infarction(STEMI).Methods A total of 99 consecutive STEMI patients undergoing primary PCI were randomly assigned to receive intracoronary verapamil or intracoronary heparinised saline immediately after stent deployment.Coronary flow was assessed by Thrombolysis in Myocardial Infarction(TIMI) flow grade(TFG) and corrected TIMI frame count(CTFC),and myocardial perfusion was assessed by TIMI myocardial perfusion grade(TMPG) and TIMI myocardial blush grades(MBG).Coronary and myocardial perfusion were assessed before and after PCI,as well as after drug administration by two doctors independently.Echocardiography were performed one week after PCI.Incidence of major adverse cardiac events in hospital and 3 months follow-up were compared between the two groups.Results Eight patients were excluded from the final analysis.Among the remaining 91 patients,47 patients were in the verapamil group and 44 patients were in control group.Baseline characteristics and angiographic characteristics were not significantly different between the two groups.CTFC,TFG,TMPG,and MBG before and after PCI were of no significant differences between the two groups.However,after drug administration,the verapamil group showed better outcome compared with the control group in CTFC(27.1?14.2 vs 39.0?23.8,P=0.011),TFG≥2(100% vs 90.9%,P=0.035),MBG≥2(91.5%% vs 75.5%,P=0.034),and TMPG≥2(89.4% vs 72.7%,P=0.042).LVEF measured by echocardiography was not significantly different between the two groups one week after PCI.The combined incidence of MACE in hospital(4.3% vs 9.1%,P=0.613) and at 3-month follow-up(23.9% vs 22.7%,P=0.894) was similar between the verapamil group and the control group.Conclusion Intracoronary administration of verapamil can improve coronary flow and myocardial perfusion in STEMI patients underwent primary PCI but show no obvious improvement in short-time clinical prognosis.
2.Regulation of B-lymphocyte stimulator expression levels in response to IFN-γ and IL-6

Objective To investigate the regulation of B-lymphocyte stimulator(BLyS) levels in response to IFN-γand IL-6. Methods Flow cytometry, quantitative polymerase chain reaction, ELISA and Western blot were applied to examine the expression level of BLyS in response to IFN-γ and IL-6 . Results IFN-γand IL-6 induced BLyS expression in KM3 cells. After treated with BAY11-7082, an IkB-α phospho- rylation inhibitor, the up regulation of BI,yS induced by IFN-γ was completely inhibited. Inhibiting the nu-clear faetor-kB (NF-kB) and mitogen activated protein kinase(MAPK) activation in KM3 cells reduced BLyS protein and gene expression. Conclusion MAPK and NF-kB pathways are involved in the regulation of BLyS expression, which suggests that MAPK and NF-kB might be used for the treatment of multiple mye- loma.
3.Clinical features and outcomes of seronegative rheumatoid arthritis in western China after one-year treatment with traditional disease modifying anti-rheumatoid drugs

ABSTRACT:Objective To investigate the clinical features of seronegative rheumatoid arthritis (RA)in western China and its outcomes after one-year treatment with disease modifying anti-rheumatoid drugs (DMARDs) so as to provide evidence for effective therapy.Methods We made a retrospective analysis of 240 RA patients treated in our department from May 2013 to June 2014.We compared the 47 seropositive and 25 seronegative RA patients in clinical features,laboratory parameters and outcomes after one-year DMARDs medication.Results The percentage of seronegative RA was 10.4% (25/240).The number of swollen small joints was significantly smaller in seronegative RA group (P<0 .0 1 ).Compared with those in seropositive RA,the level of hemoglobin was lower,the level of platelets was higher,and the level of alkaline phosphatase was lower in seronegative RA (P<0 .0 5 ).The remission rate was higher in seronegative RA group than in seropositive RA group after one-year DMARDs administration (P<0.05).Conclusion Seronegative RA is not rare in clinic.Even though seronegative RA patients often present fewer swollen small joints, it is difficult to distinguish between seronegative and seropositve RA just based on the clinical features.Besides,hematological damage is more severe in some patients with seronegative RA.Only after one-year treatment with DMARDs,the remission rate is higher in seronegative RA patients than in seropositve RA ones.
4.Study on Quality Standard for Tangzhixiao Capsules

Objective:To establish the quality standard for Tangzhixiao capsules. Methods: The qualitative identification of Salvia miltiorrhiza,Atractylodes and Hawthorn was detected by TLC, and the quantitative determination of salvianolic acid B was determined by HPLC. The HPLC determination was performed on a YMC-Triart C18 (250 mm × 4. 6 mm, 5μm ) column with the mobile phase consisted of acetonitrile-0. 1% phosphoric acid (24∶ 76) at the flow rate of 1. 0 ml·min-1 , the column temperature was 30℃ and the detection wavelength was set at 286 nm. Results: The TLC spots were fairly clear, and the negative samples showed no interference. The concentration of salvianolic acid B within the range of 0. 012-0. 120 mg·ml -1 was linear with peak area(r = 0. 999 5), the average recovery was 100. 06% and RSD was 0. 83%(n = 6). Conclusion: The method is accurate, reliable and reproducible, which can be used in the quality control of Tangzhixiao capsules.
5.Clinical features of combined pulmonary fibrosis and emphysema syndrome in elderly patients

Objective To compare clinical features,pulmonary functions,chest imaging and prognosis between combined pulmonary fibrosis and emphysema syndrome (CPFE) and idiopathic pulmonary fibrosis(IPF) without emphysematous changes in elderly patients.Methods 88 elderly IPF patients in Beijing hospital from January 2000 to October 2012 were divided into CPFE (n=30) and IPF (n =58) groups according to the CT imaging.Clinical features,blood gas analysis,pulmonary function,chest CT and survival time were compared between the two groups.Results 30 CPFE patients with the mean age of(75.5 ±7.6) years and 58 IPF patients without emphysema with the mean age of(73.7±6.8) years were enrolled.The proportions of male patients,smoking history and mortality were higher in CPFE patients than in IPF group(86.7% vs.63.8%,28 vs.36,76.7% vs.43.1%,x2 =5.09,9.74,8.98,P<0.05 or 0.01).CPFE patients had a higher force vital capacity(FVC) and total lung capacity(TLC) as compared with IPF group [(2.6±0.9) L vs.(2.1± 0.5) L,(5.4±1.9) L vs.(4.4±1.1) L,t=2.69,2.35,P<0.01 or 0.05].There were no significant differences in forced expiratory volume in one second(FEV1) and the diffusion capacity for carbon monoxide(DLCO) between the two groups.The main type of emphysema by HRCT scan were centrilobular emphysema in CPFE patients.There were lower median survival time in CPFE group than in IPF group [(3.0±0.2) years vs.(4.0±1.0) years,x2=4.50,P<0.05].Conclusions The majority of elderly CPFE patients are males and smokers.The lung volume is increased in elderly CPFE patients as compared with IPF patients.Centrilobular emphysema is the primary type in CPFE patients.The prognosis is worse in elderly CPFE patients than in IPF patients.
6.STUDY ON THE ANTITUMOR EFFECT AND IMMUNE MODULATION OF EXTRACT FROM CHLORELLA PYRENOIDOSA CHICK

Objective: To study the antitumor activity and immune modulation of extract from Chlorella pyrenoidosa Chick. (CE). Methods:Cyclophosphamide is positive control and saline is negative control. S180-bearing and HCA-bearing mice were used as animal model. The changes of the growth of S180 and HCA tumor cells in vitro, the changes of tumor weight in vivo and the effect on immune system such as phagocytosis of macrophages,proliferation of lymphocytes were investigated when CE were used. Results: The tumor cells were killed by CE in vitro. CE inhibited tumor growth markedly in S180 and HCA bearing mice and the effect was enhaced if CE and cyclophosphamide were used simultaneously. CE also enhanced the immune function. Conclusion: CE has the antitumor effect and the effect of immune modulation.
7.An experimental study of 3-dimension zygomatic suture extension for suture osteogenesis.

OBJECTIVETo explore a methods of 3-Dimension expansion of zygomatic suture in a goat model.
METHODSSeven goats were used in this study. The 3-D extensive applicator was designed and used to extend the zygomatic suture of the goats by placing it in the zygomatic bone through an infraorbital incision. Ten days after the first operation, it was gradually extended on a speed of 0.09 cm/d for 7 days. The zygomatic movement and the osteogenesis of the suture was evaluated in two weeks.
RESULTSThe zygomatic bone was extended for 0.6 cm long in average, and the osteogenesis was also shown significantly in the suture.
CONCLUSIONThe above mentioned technique could be a safe and effect method to be applied for the zygomatic extension.

8.Correlation between the Handedness and Clinical Findings in Children with Epilepsy.

PURPOSE: Some reports left handedness is more common in epileptic patients compared with normal populations, but the study on this topics are rare. This can be due to epilepsy itself or brain lesion in epileptic patients. The study about the handedness in epileptic patients may help identifying the relationship of handedness and epilepsy, symptomatic epilepsy, sites of brain lesion, age at first seizure attack, family history of epilepsy. METHODS: Subjects were 158 children with epilepsy who were admitted and able to describe the handedness at the Department of Pediatrics, Pusan national university hospital between January 1996 and August 1999. We reviewed their sex, age, handedness. At patient group, age at first seizure, age distribution, family history of epilepsy, seizure type, etiology of epilepsy, causes of symptomatic epilepsy, site of brain lesion, localization by EEG were analyzed. Control group is 158 children without neurologic problem. We analysed handedness according to its variables. RESULTS: There was no statistical significance in handedness between epileptic group and control group. There was no significant difference in handedness according to age at first seizure attack, seizure type, family history of epilepsy and age distribution. The left handedness is more common significantly in the group of symptomatic epilepsy, left side brain lesion and focal abnormality by EEG recording. CONCLUSION: We can find significant difference between symptomatic epileptic patient group and control group, especially in the case of the left brain lesion and focal abnormality by EEG recording. In the case of bilateral or diffuse brain lesion, most of patients showed right handedness(87.5%). Accordingly, we concluded that high prevalence of left-handedness in children with epilepsy is more related with left side brain lesion than epilepsy itself. We assumed the social factors strongly affect the handedness of epileptic patients when we see that most of patients show the right handedenss in the bilateral or diffuse brain lesions.

ObjectiveTo investigate the activation of MAPK signal pathway in multiple myeloma and the regulation of BLyS expression levels through MAPK signal pathway; preliminarily study the role of MAPK signal pathway in the up-regulation of BLyS expression levels induced by IFN-γ.MethodsActivated MAPK pathway were detected by Western blot,while the expression of BLyS were detected with RT-PCR and Western blot,and Western blot investigated the effect of MAPK pathway on BLyS expression levels induced by IFN-γ.ResultsIn addition to the expression of ERK,JNK,p38,p-JNK was also expressed in MM cell lines,the MAPK pathway inhibitor targeting JNK SP600125 can down-regulate the expression of BLyS,and its activator anisomycin can up-regulate the expression of BLyS.SP600125 restrained the proliferation and survival of MM cells.ConclusionJNK/SAPK pathway was activated in MM cells; The activated degree of JNK/SAPK pathway and the expression level of BLyS was positively correlated.JNK/SAPK pathway play an important role in the up-regulation of BLyS expression levels induced by IFN-γ.
10.Disturbance of peripheral blood B cells homeostasis in rheumatoid arthritis and the influence of therapy on B cells homeostasis

Objective To investigate the characteristics and the frequencies of B cell subsets in peripheral blood of rheumatoid arthritis (RA) patients,and to study the correlation between B cell subsets and clinical indices and influence of different therapies on B cell subsets to deeply understand the pathogenesis of RA.Methods Peripheral blood witched memory B cells,non-switched memory B cells,naive B cells,and double negative B cells of 141 patients and 33 healthy controls were measured by flow cytometry.Patients were divided into three groups based on their therapeutic regimen,including tumor necrosis factor-or (TNF-α) inhibitors combined with disease modifying antirheumatic drugs (DMARDs),DMARDs only and patients without any therapy.The relevance between B cells subsets and clinical manifestations,lab test results exemption were assessed as well as the influence of different therapies.All data were were analyzed by Statistical product and service solutions (SPSS) 23.0 statistical analysis for unpaired t test,analysis of variance and Spearman's correlations analysis.Results ① New-onset RA patients with less than 12 weeks disease duration and never accepted any drugs had a significantly lower frequency of peripheral blood memory B cells,including non-switched memory B cells [(8 ±4)% vs (13 ±4)%,P<0.05,t =3.3)] and switched memory B cells [(18±10)% vs (23±7)%,P<0.05,t=2.2)],than healthy individuals.② There was a negative association between non-switched memory B cells and disease activity score in 28 joints (r=-0.23,P<0.05).③ Negative association between non-switched memory B cells and erythrocyte sedimentation rate (ESR),lgG was found,while therewas no association between pre-switched B cells and other laboratory test results.④ Non-switched memory B cells and switched memory B cells increased after TNF-α arntagonist or DMARDs therapy.Conclusion The results of this study suggest that B cell abnormalities in new-onset RA patients with short disease duration are reduced non-switched memory B cells and switched memory B cells.A negative correlation has been found between non-switched memory B cells and ESR and lgG.B cells subsets frequency are changed by TNF-α antagonist and DMARDs,which suggests that changes of B cell subsets may contribute to the occurrence and development of RA.
