Objective To initially observe the antitumor immune of PVAX1-HPV58mE6E7FcGB composite DNA vaccine.Methods Before detecting immune effect of the vaccine,the B16-HPV58E6E7 tumor cell line was built which could steadily express HPV58E6E7 fusion gene.Then,HPV58E6E7-GST fusion protein as an antigen was expressed and purified.Before or after immunized with the vaccine,the C57BL/6 mice were challenged by B16-HPV58E6E7 cells.Anti-tumor transplantation and tumor growth inhibition experiment were performed to observe prevention and treatment effects on the vaccine.Specific humoral and cellular immune responses in the immunized mice were detected by ELISA,enzyme linked immunospot assay (ELISPOT) and cytotoxic T lymphocyte (CTL) method.Results In the anti-tumor transplantation experiment,tumor formation rate was only 9/15 in the mice which were immunized by PVAX1-HPV58mE6E7FcGB vaccine,time before tumor formation was the longest [(13.6 ± 1.7) days] and tumor growth was the slowest in the vaccine group.In tumor growth inhibition experiment,inhibition rate reached 81.4％ in the vaccine group.Except tumor formation rate,all data in the vaccine group was superior to the pure antigen PVAX1-HPV58mE6E7Fc group (P ＜ 0.05).Humoral immune effect showed that both the vaccine and the pure antigen could induce specific antibody in the immunized mice,and the highest titer were 1 ∶ 25600 and 1 ∶ 12800,respectively.Although there was not significant difference of antibody titer between the vaccine and the pure antigen group (P ＞ 0.05),the number of activated T cells in the vaccine group was almost four times as that in the pure antigen group [(219 ±34)/4 × 105 spleen lymphocytes versus (55 ±25)/4 × 105 spleen lymphocytes,P ＜ 0.05],and the highest specific CTL that vaccine induced was significantly higher than that of pure antigen (43.3％ versus 31.3％,P ＜ 0.05).Conclusions Humoral and cellular immune response could be effectively stimulated by PVAX1-HPV58mE6E7FcGB composite DNA vaccine.Growth of B16-HPV58E6E7 cells was significantly inhibited in the immunized mice.The cellular immune effect on the vaccine was superior to the pure antigen.Therefore,PVAX1-HPV58mE6E7FcGB could be used as a candidate vaccine for immune therapy to the HPV58 positive tumors and precancerous lesions.

To study the effect of urokinase on platelet activation and aggregation in the patients with glomerulonephritis. Platelet counts(PC),mean platelet volume (MPV), and platelet distribution width (PDW) were determined with automatic blood cell analytic apparatus, the expressive levels of platelet granular membrane protein (GMP 140) and fibrinogen receptor(GPⅡb Ⅲa) on the surfaces of platelets were assayed with flow cytometry, and the levels of fibinogen and fibrin degradation products (FDP) in blood were measured with ELISA in 102 children with glomerulonephritis. The results showed that urokinase led to PC decreas,MPV increase and PDW exceeding normal value in 102 children with glomerulonephritis, including micro change (MC),glomerulosclerosis (GS),membranous nephropathy (MN), and proliferative glomerulonephritis (PGN).The expressive levels of GMP 140 after the treatment with urokinase were significantly higher than that before the treatment and controls( P 0 01). The results suggested that urokinase used in the patients with glomerulonephritis increased platelet activation function and decreased platelet aggregation function.

Objective To investigate the effect of probiotics on the blood lipid metabolism of elderly patients with coronary artery disease (CHD).Methods The clinical data of 100 elderly CHD patients who were admitted in our hospital from May 2012 ～November 2015 were collected.The enrolled patients were randomly divided into observation group and control group,50 cases in each group.All patients were routinely given medication for secondary prevention of CHD,patients in the observation group were additionally given oral probiotics.Then we observed the changes in the lipid levels of patients before and after 12 weeks treatment.Results The serum total cholesterol (TC),triglyceride (TG),low density lipoprotein cholesterol (LDL-C) of patients in the observation group were improved more significantly than the control group,the difference was significant (P＜0.05).The serum total High-density lipoprotein cholesterol (HDL-C) was improved more significantly than the control group.Conclusion Probiotics adjuvant therapy can significantly improve the blood lipid indexes in elderly CHD patients.

In order to improve the diagnosis and treatment of severe hemolytic uremic syndrome (SHUS),we summarized the rescuing experiences in 3 children with SHUS.Therefore,close observation of gastro intestinal symptoms,collaborated with repeated examinations of the blood,renal function,urinary output,blood pressure,and manifestations of the central nervous system could help predict to certain extent the occurrence of further hemolysis and renal failure in these patients.Energetic treatment of malignant hypertension and relieving the symptoms of center nervous system could successfully improve renal functions.If urinary output decreased quickly,creatinine (Cr) and blood urea nitrogen (BUN) increased,or severe infection occurred in SHUS patients,hemodialysis should be undertaken as early as possible to prevent caute renal failure and multiple organ failure syndrome (MOFS).Therefore,the treatment of SHUS in children has its peculiarity and complexity.

Objective To evaluate the clinical application of thoracoscope mediastinal tumor resection with the laryngeal mask anesthesia. Methods 40 cases of mediastinal tumors were randomly divided into two groups from 2012 to 2015, 20 cases in each group. Laryngeal mask group performed thoracoscope mediastinal tumor resection with laryngeal mask anesthesia and the Intubation group performed thoracoscope mediastinal tumor resection dou-ble-lume nendo tracheal intubation anesthesia, then compare the overall effect. Results All the patients were suc-cessfully completed with the thoracoscope surgery, no transfer to open chest cases. The preoperative anesthesia time of laryngeal mask group was shorter, blood pressure and pulse range of wave was smaller in the process of anesthesia and surgery, move restlessly was less, postoperative recovery time was shorter, and lower incidence of postoperative pharynx ministry unwell and sore throat, so the laryngeal mask group shows statistically significance comparing with the intubation group (P < 0.05); Operative time, operative field exposure satisfaction, intraoperative blood loss, post-operative extubation time and hospitalization days have no statistical significance (P >0.05). Conclusion The laryn-geal mask airway intravenous anesthesia in thoracoscope mediastinal tumor resection in practical good, in the preop-erative anesthesia and postoperative recovery time and postoperative sore throat was better than that of intubation group, so should be extended in clinical treatment.

Objective To observe the ultrasonic manifestation of metatarsal fatigue fractures and probe into the clinical values of ultrasonic diagnosis and follow-up to the disease.Methods The clinical data and sonographic features of 30 cases with metatarsal fatigue fracture confirmed by X-ray and CT were analyzed retrospectively.Results All the subjects in this study were metatarsals insufficiency fractures located in the second or third diaphysis.2D-ultrasound found microfracture in the cortex and the periosteal elevation by increased vascularity in the early stage.During the 3-month follow-up,thickened cortex and callus shown as the shape of mushroom or cauliflower and a great amount of neovascularisation were visualised with color Doppler flow imaging.Then,the cortex surface became gradually changing from rough to smooth and obvious posterior shadows were observed with the illness progression.Conclusions Ultrasonography could be used to detect the cortical continuity,callus characteristics and blood flow perfusion of fatigue fracture in the different damage stages.

Objective To study the role of the expression levels of 5 type Ⅰ interferon (IFN)-inducible genes (LY6E, OAS1, OASL, MX1, and ISG15) in the diagnosis and disease activity evaluation of systemic lupus erythematosus (SLE). Methods Peripheral blood was obtained from 68 SLE patients, 50 patients with other connective tissue diseases and 26 normal controls, and total RNA was extracted and reverse transcribed into complementary DNA. Real-time PCR technique was used to determine gene expressions at transcription level. An IFN score for each individual was calculated according to the expression of 5 1FN genes. Comparisons of gene expression and IFN score were made among groups. The genes expression levels in patients with SLE were analyzed using receiver operative characteristic curve. The association between IFN scores and disease activity, as assessed by the SLEDAI scores and 24 h proteinuria, was analyzed using Spearman correlation analyses. Results ① The expression levels of MX1, OASL, OAS1, ISG15 and LY6E mRNA in SLE patients were significantly increased as compared with normal controls and disease controls (P all＜0.01 ).② IFN scores in SLE patients (17.9±29.1) were significantly increased as compared with normal controls (0±3.3)and disease controls (3.0±8.1) (P all＜0.01 ). ③ IFN scores area under the ROC curve (AUCROC) was 0.846. When The IFN scores reached 2.56, its sensitivity and specificity for the diagnosis of SLE were 93.1%and 78.3%, respectively. ④ Levels of IFN score was positively correlated with SLEDAI scores (r=0.256,P＜0.05) and 24 h proteinuria (r=0.337, P＜0.05). Conclusion The 5 IFN-inducible genes are highly expressed in SLE patients. IFN score level is valuable for the diagnosis of SLE and a high IFN score is usually associated with an elevated disease activity.

OBJECTIVE:To observe clinical efficacy and toxic reaction of bronchial artery infusion(BAI)chemotherapy com-bined with Cinobufacini capsule in the treatment of advanced non-small cell lung cancer(NSCLC). METHODS:A total of 126 cas-es of advanced NSCLC diagnosed in stage Ⅲb-Ⅳ were randomly divided into observation and control group,63 cases in each group. Both of them were treated by BAI with taxotere/cisplatin(TP regimen),once every three weeks for a cycle,a total of 5 cy-cles;observation group was additionally given Cinobufacini capsule 500 mg/time,three times a day,on the basis of BAI chemo-therapy,for 15 weeks. Clinical efficacy,KPS,survival rate and toxic reaction of 2 groups were observed. RESULTS:The total ef-fective rate(82.54%)of observation group was better than that(63.49%)of control group,with statistical significance(P<0.05). KPS score of observation group was significantly better than that of control group,with statistical significance (P<0.05). 1-year survival rate of observation group and control group were 65.08% and 30.15%,2-year survival rate of them were 19.05% and 4.76%,with statistical significance(P<0.05). Adverse reactions of two groups was mainly marrow suppression and gastrointesti-nal reaction,marrow suppression degree and the incidence of nausea and vomiting in observation group was lighter than control group,with statistical significance (P<0.05). CONCLUSIONS:BAI combined with Cinobufacini capsule in the treatment of ad-vanced NSCLC can improve short-term curative effect and long-term survival rate,and can improve the survival quality with little toxic effect.
