OBJECTIVE To sum up the cases of hospital infection,in order to further improve the experience of the work of hospital infection.METHODS The investigation of hospital infection situation among inpatients 2005-2007 was reviewed.RESULTS The hospital infection rates in 2005-2007 were 1.81%,1.65% and 1.52%,respectively,mainly in the lower respiratory tract infection and surgical incision.CONCLUSIONS Hospital infection is closely related with the age,the underlying diseases, the antibiotic misuse and the environment.

Objective To investigate certain risk factors for and their impact on abnormal liver function in middle-aged and elderly adults. Methods A case-control study was constructed based on the SAGE cohort of 8642 registered residents aged 50 years or over in Shanghai.Of them,137 individuals with abnormal liver function, defined as aspartate transaminase (AST)> 40 U/L or alanine aminotransferase (ALT)> 40 U/L,were randomly selected as the observation group, while 411 healthy controls were 3 ∶ 1 matched with the cases in the observation group by gender and age (1 year).Face-to-face administered questionnaires and physical examinations were conducted and serum samples were tested for ALT,AST,glucose (GLU),total cholesterol (TC),triglycreide (TG),hepatitis B surface antigen (HbsAg) and anti-hepatitis C virus antibody (anti-HCV Ab).Chi square test and rank sum test were used for single factor analysis, and logistic regression analysis was used for multiple factors. Results The prevalence of HBsAg positive patients was 12.4 % (68/548) Univariate analysis showed that hepatitis virus infection and body mass index (BMI) were associated with abnormal liver function (both P<0.000).Multivariate logistic regression analysis showed that hepatitis virus infection (OR=1.85,95% CI:1.04 3.29,P-0.036) and obesity (OR=3.60,95%CI:1.92-6.73,P<0.001) increased the risk of abnormal liver function, whereas chronic medication (OR=0.51,95% CI:0.32-0.80,P =0.004) decreased the risk of abnormal liver function. Conclusions Among the study population, hepatitis virus infection and obesity are risk factors for abnormal liver function in middle-aged and elderly people. After adjustment for potential confounders, chronic medication is negatively correlated with abnormal liver function and may be a protective factor for liver function.

Objective? To analyze the current situation of clinical research on cardiac rehabilitation in China from 2008 to 2018, so as to provide references for clinical research and practice in this field. Methods? CNKI and Wanfang database were used as retrieval tools to retrieve published clinical research literature on cardiac rehabilitation. Bibliometric methods were used to analyze the annual distribution, regional distribution, authors, journals and fund support of the published literature. Results? A total of 603 articles were included in this study and the numbers of articles and funds showed a rapid growth trend. Both of them were widely distributed but not balanced in different regions. Among the 582 first authors, 563 authors were with one article, accounting for 96.74%. 40 articles published by 19 core authors who have published over 2 articles were accounting for 6.63%. The co-authorship rate was 65.51% and the degree of cooperation was 2.50. There were 218 journals involved and only 5 of the 15 core journals belonged to the core journals of science and technology of China in 2018. Conclusions? From 2008 to 2018, the clinical research of cardiac rehabilitation in China was in a rapid development stage. Researchers in this field were highly collaborative but dispersive and lack of sustainability. The quality of core journals was unsatisfactory. It is suggested that more attention should be paid, more technology and talents should be input and balanced, fund supports should be provided to produce high quality researches and high level results.

Objective:To investigate the clinical efficacy of femoral component revision surgery in addressing polyethylene sleeve wear in rotating hinge knee joints. Methods:A retrospective analysis was conducted on 21 patients who underwent treatment for polyethylene sleeve wear in rotating hinge knee joints at the Department of Orthopaedics, the Fourth Medical Center, PLA General Hospital, spanning from August 2008 to April 2023. There were 8 males and 13 females with a median age of 63(38, 76) years. Among them, 11 cases involved the left side, and 10 cases involved the right side. The average body mass index (BMI) was 25.0(22.5, 31.0) kg/m 2, and all cases underwent unilateral knee joint revision. The time interval from receiving the hinge knee joint prosthesis to revision was 10.5(6.0, 17.0) years, with 18 cases undergoing primary revision and 3 cases undergoing revision for the second time. Revision surgery of the femoral component sleeve was performed in all patients due to polyethylene sleeve wear. Visual analogue scale (VAS), range of motion (ROM) of the knee joint, changes in hip-knee-ankle angle (HKA), and Knee Society score (KSS) were collected and recorded to assess the surgical outcomes. Results:All patients were followed up for a median duration of 12.5(11.7, 13.4) months. Preoperatively, the VAS score was 5(3, 7) points, which significantly decreased to 0(0, 1) points at the last follow-up ( Z=5.721, P<0.001). At the last follow-up, the KSS scores decreased from 41.19±13.65 points preoperatively to 79.81±7.80 points, indicating a statistically significant difference ( t=12.560, P<0.001). HKA increased from 174.65°±5.20° preoperatively to 178.71°±2.79°, with a statistically significant difference ( t=5.533, P<0.001). Knee joint ROM increased from 69.05°±34.37° preoperatively to 110.00°±10.12°, also demonstrating a significant improvement ( t=5.960, P<0.001). Notably, patients exhibited knee joint stability with no passive flexion or extension laxity. Conclusion:Femoral component sleeve revision surgery effectively addresses polyethylene sleeve wear in rotating hinge knee joint prostheses, thereby mitigating the substantial loss of bone and soft tissue associated with overall revision procedures. The early postoperative clinical outcomes are promising and indicative of the efficacy of this surgical intervention.
