1.Application of narrow-band imaging endoscopy in diagnosis of nasopharyngeal carcinoma

OBJECTIVE To study the application of narrow-band imaging in diagnosis of the nasopharyngeal malignant lesions. METHODS A total of 52 cases of nasopharyngeal malignant lesions were examined and diagnosed by white and NBI endoscopy between June 2014 and August 2015, and their diagnosis efficacies were evaluated based on pathological diagnosis as a gold standard. RESULTS Of 52 cases, 46 cases were malignant lesion(88.46%) and 6 were non malignant lesion(11.54%). The diagnostic accuracy by NBI for the nasopharyngeal malignant lesions was 86.54%(45/52), which was significantly higher than that of white light 69.23%(36/52, χ2=4.522, P<0.05). The sensitivity of NBI and white light to detect nasopharyngeal carcinoma was 97.83%(45/46) and 78.26%(36/46) respectively(χ2=8.364, P<0.05). CONCLUSION NBI has high application value in diagnosis of nasopharyngeal carcinoma, and is expected to become one of the important means to diagnosis of the nasopharyngeal carcinoma.
2.The value of target biopsy using narrow band imaging endoscopy for diagnosis of laryngeal carcinoma.

OBJECTIVE:
To estimate the clinical significance of target biopsy in clinical diagnosis, and accurate pathologic specimen acquisition of laryngeal lesion by using narrow band imaging (NBI) endoscopy.
METHOD:
A total of 156 cases of patients with laryngeal lesions were collected from the department of Otolaryngology Head and Neck Surgery of Tianjin Union Medical Center from Aug 2012 to Dec 2014. All patients were randomly divided into regular biopsy group and NBI target biopsy group. Image data were recorded and the patients' samples were biopsied. The pathologic diagnosis was used to evaluate the accuracy of regular biopsy and target biopsy.
RESULT:
Based on the pathologic diagnosis, the correct accurate pathologic specimen acquisition rates in NBI target pathology group at stage 0 (Tis), stage I and stage II laryngeal cancer diagnosis compliance rate were higher than those in regular biopsy group, stage 0 (Tis) (χ² = 22.00, P < 0.05), stage I (χ² = 8.33, P < 0.05) and stage II (χ² = 4.55, P < 0.05).
CONCLUSION
NBI endoscopy plays an important role in the clinical diagnosis and accurate pathologic specimen acquisition of laryngeal carcinoma and can be a regular means to detect laryngeal lesions in clinical.

3.The Groningen Valve Voice Prosthesis

Objective To evaluate the results of the Groningen valve voice prosthesis in China.Methods The Groningen valve was used to date in 10 patients with a mean age at insertion of 60.2 years from January 2003 to January 2004. Seven patients underwent primary valve insertion at the time of surgery and in three patients the prosthesis was inserted at a later date. The speech quality of this group was assessed.Results The median score was 4.6(range 3.3~6.9)which compared favourably with a similar analysis of other valve.Conclusion The Groningen valve provides a highly acceptable means of obtaining vocalization after total laryngectomy in patients.
4.Clinical Application of Videokymography

Objective To study the results of clinical application of videokymography. Methods 50 patients with voice pathology and 5 normal voice persons were studied with videokymography. Results With videokymography, left - right asymme- tries, close or open quotient differences, abnormal periocity and mucosal waves were visualized in 50 patients. The vibration of vo- cal folds was slight asymmetry in 1 normal voice person and normal in other 4 normal voice persons. Conclusion Videokymography provides a simple way to study irregular vibrations of the vocal folds. It is helpful for a more accurate diagnosis of the vocal folds. Information obtained allows a first time quantification of vibrations registered.
5.The role of the posterior elements of the spine in the classification of th oraco-lumbar burst fractures and spinal cord injuries

Objective To observe the role of the posterior e lements of the spine in the spinal cord injuries and to provide the basic evidence for proper evaluation of the se verity of spinal cord injuries and th e possibilities of functional recove ry.Methods Experimental study:The experiment al model of spinal burst fracture was established in the thoraco-lumbar column section which subjected high velocity vertical loadi ng in static pattern.The pressure changes in the vertebral canal and the severity of posterior elements of the spin e were observed,and then their relationship was studied.Clinical Study:The data were obtained from 127consecutive cases of thoraco-lumbar burst fracture associated with paraplegia.The relationship between t he severity of spinal cord injury,functional recovery and the posterior elements injuries was studied retrosp ec-tively.Results The experimental study showed that g reat pressure changes appeared in th e vertebral spec-imens without posterior elements in juries.On the contrary,the peak value of pressure was low in cases with posterior elements injuries.Clini cal results:Of 38cases(69.09%)with spinal cord injuries in 55patie nts without posterior elements injurie s,functional recovery was found in 24cases(63.16%).In another group,of 52cases(72.22%)with spinal cord injuries in 72patie nts with posterior elements injurie s,functional recovery was found in 20cases(38.46%).The statistic difference between t he two groups indicated that the severity of spinal cord injuries in t he patients without posterior eleme nts injuries was more severe than that with posterior elements injuries.Conclusion The spinal posterior elements play a n important role in spinal cord injuries.The severity of posterior elements injuries can be used as a sca le in the evaluation of the severity o f spinal cord injuries,functional re covery and improvement of Denis classification.[
6.Rejection reactions after acupoint embedding with chromic catgut: case report and discussion.

Four cases of rejection reactions after acupoint embedding with chromic catgut as well as the diagnosis and treatment course were reported to reveal the importance of rejection reaction and its generality. Besides, the cause and mechanism were discussed. By combining the treatment experience, it was recommended to replace the catgut with third-generation absorbable medical suture, which could prompt execution of standardized manipulation of acupoint embedding.

7.Experimental study of fracture loading test of cracked teeth with CEREC CAD/CAM ceramic inlays/onlays

Objective To test the fracture loading of extracted cracked teeth with CEREC CAD/CAM ceramic inlays/onlays. Methods All the extracted teeth were divided into two groups (A: control group; B: test group; n=15 teeth in each group). All the teeth were prepared into those imitating cracked teeth. Those teeth in group B were strengthened with CEREC CAD/CAM ceramic inlays/onlays. All teeth were thermocycled and mounted for testing and then were loaded until fracture. Results The force of anti-fracture of extracted teeth imitating cracked teeth with CEREC CAD/CAM ceramic inlays/onlays (group B) was obviously higher than that of the teeth without CEREC CAD/CAM ceramic inlays/onlays (group A). Conclusion CEREC CAD/CAM ceramic inlays/onlays can effectively prevent the crack of teeth in clinical practice.
8.Anti-fracture strength of teeth with wedge-shaped defects repaired with CEREC CAD/CAM ceramic inlay

Objective To study the effect of CEREC CAD/CAM ceramic inlay on repair of wedge-shaped defects of teeth and test its anti-fracture properties. Methods Extracted teeth were divided into control group ( n = 16) and experimental group ( n = 16) ,and prepared into imitating teeth with wedge-shaped defects. Teeth in experimental and control groups were repaired with CEREC CAD/CAM ceramic inlay and light-cured composite resin,respectively. Anti-fracture strength of teeth with wedge-shaped defects repaired with CEREC CAD/CAM was assayed on a universal testing device with its maximal loading recorded. The anti-fracture strength was compared between the 2 groups. 9.Complications of the CO_2 laser assisted laryngeal microsurgery

OBJECTIVE To analyze the complications of the CO2 laser-assisted laryngeal microsurgery for laryngeal tumors. METHODS The clinical data of 520 patients who underwent CO2 laser assisted laryngeal microsurgery for laryngeal tumors were retrospectively analyzed. The complications were recorded. RESULTS Complications caused by CO2 laser were found in 61 case(11.73 %) and those caused by suspended laryngoscope in 71 case(13.65 %). CONCLUSION The complications of the CO2 laser-assisted laryngeal microsurgery for laryngeal tumors may be caused by CO2 laser or suspended laryngoscope.
10.Protection of Ulinastatin on gut barrier in septic rats

Objective To investigate the protective role of Ulinastatin UTI on gut barrier of septic rats. Methods Twenty-two SD rats were divided into three groups: sham laparatomy(S), cecal ligation and punture(CLP), and CLP plus UTI. Septic rat model was estabilished through CLP method. Fluorescence spectrometry was used to measure FITC-dextran concentration from bowel lumen to portal vein. Ultrastructure of intestinal mucosa was observed under transmission eletron microscope (TEM).
