1.Protective effect of procyanidin single active ingredient B2 on human en-dothelial progenitor cells under high glucose stimulation

AIM:To study the protective effect of procyanidin single active ingredient B2 (PC-B2) on human endothelial progenitor cells ( EPCs) stimulated with high glucose.METHODS: The human EPCs were isolated from pe-ripheral blood of healthy people and identified.The EPCs were divided into control group (PBS treatment), hypertonic control group (25 mmol/L mannitol treatment), high glucose (30 mmol/L) group, and different concentrations (2, 10 and 50 mg/L) of PC-B2+30 mmol/L glucose groups.The viability of EPCs was detected by CCK-8 assay.The levels of LDH, MDA, SOD and GSH in the EPCs were detected.The changes of NO, ET-1, ICAM-1 and VCAM-1 in the EPCs cultured medium were measured by ELISA.The cell apoptotic rate and reactive oxygen species ( ROS) in the EPCs were analyzed by flow cytometry.The expression of VEGF and VEGFR-2 in the EPCs were determined by Western blot.RE-SULTS:Compared with control group, the viability of human EPCs was decreased significantly in 30 mmol/L glucose group (P<0.05).The LDH leakage, MDA content and the releases of ET-1, ICAM-1 and VCAM-1 were induced signifi-
cantly (P<0.05), but SOD and GSH activity and NO production were decreased significantly (P<0.05).The ROS and cell apoptotic rate were increased significantly (P<0.05).The expression of VEGF and VEGFR-2 in the EPCs were de-creased (P<0.05).When human EPCs were treated with different concentrations of PC-B2 and 30 mmol/L glucose, the viability was obviously rebounded (P<0.05), the LDH leakage, MDA content and the releases of ET-1, ICAM-1 and VCAM-1 were decreased gradually (P<0.05), the SOD, GSH activity and NO production were increased significantly (P<0.05), the ROS and cell apoptotic rate were decreased significantly (P<0.05), and the expression of VEGF and VEGFR-2 in the EPCs was increased gradually ( P <0.05 ) .CONCLUSION: PC-B2 enhances the viability of human EPCs under high glucose condition, reduces high glucose-induced oxidative damage, restores the EPCs normal function, and reduces the releases of inflammatory cytokines and apoptosis, thus playing a protective effect on human EPCs through inducing the expression of VEGF and VEGFR-2.
2.Mapping analysis of application of nursing interventions classification in pediatric general surgery nursing documentation

Objective:To analyze the clinical applicability of the sixth edition of nursing measures classification in children's general surgery, to provide a basis for standardizing clinical nursing terms and to promote the clinical use of standardized terminology of nursing measures.Methods:The research method of this study was mapping method. The electronic nursing records of the general surgery of Shanghai Children′s Medical Center Affiliated to Shanghai Jiaotong University School of Medicine from October 1st to December 31st, 2018, including the records of care records for free entry and the records of nursing measures related to medical orders were selected. The degree of matching between electronic care records and classification of nursing measures (sixth edition of nursing measures classification) was compared with conceptual consistency. Difficulties and time-consuming analysis of the characteristics of the standardized care measures obtained by matching.Results:A total of 4 560 independent electronic care records mapped 86 treatments in seven areas. Among them, 98.84% (4 507/4 560) of the nursing records were completely matched with the nursing measures, 0.83% (38/4 560) of the nursing records were partially matched with the nursing measures, and 0.33% (15/4 560) of the nursing records did not match the nursing measures. Difficulty and time-consuming were expressed as standard deviation, which were 2.20±0.45 and 2.03±1.25 respectively.Conclusions:The classification of nursing measures can be used to describe the daily behavioral activities of the children's general surgery nurses, and can also solve the standardization problem of the nursing records in the presentation, and is suitable as a standardized nursing language for the nursing records of children's general surgery.
3.Hippocampal neuronal damage and its possible mechanism in status epilepticus

Objective To investigate hippocampal neuronal damage and dynamic change of α-amino-3-hydroxy-5-methyl-4-isoxazole propionic acid (AMPA) receptor subunit GluR2 in status epilepticus, to find out whether GluR2/glyceral dehyde-3-phosphate dehydrogenase (GAPDH) interaction has any change.Methods Male Wistar rats (62 cases) were induced to status epilepticus by using LiC1-pilocarpine.The 62 rats were divided into 1 h (6 cases),6 h (12 cases), 24 h (12 cases),72 h (12 cases)and 7 d (20 cases)after status epilepticus.At the same time, the healthy control group (12 cases) was established.Morphologic changes of hippocampus and the amount of apoptotic cells in healthy control and SE model groups at different time points (6 h, 24 h, 72 h, 7 d) (6 cases each group) after status epilepticus were quantified by adopting Nissl staining and terminal deoxynucleotidyl transferase-mediated dUTP-biotin nick end-labeling staining respectively.Expressions of GluR2 in healthy control and SE model groups at 1 h,6 h,24 h,72 h and 7 d (6 cases each group) after status epilepticus were detected by using Western blot.Co-precipitation and Western blot techniques were used to investigate whether the GluR2/GAPDH interaction in the hippocampus was increased.Results Compared with the healthy control group, the number of nerve cells in the hippocampal CA1 and CA3 regions was significantly reduced at all studied time points(F =30.866,24.043, all P <0.05).Apoptotic cells in hippocampal CA1 and CA3 regions were significantly increased at 24 h,72 h and 7 d after status epilepticus (F =84.762,52.574, all P < 0.01).GluR2 at 1 h,6 h after status epilepticus was equal to that of the control group (P > 0.05), but it was shown to be significantly down-regulated at other studied time points (F =76.506,P < 0.01);when compared with the healthy control group,the GluR2/GAPDH interaction was significantly up-regulated in the hippocampus at 72 h after status epilepticus (t =7.029, P < 0.05).Conclusions Status epilepticus can lead to neuronal damage in the hippocampus.Down-regulation of GluR2 and increase of the GluR2/GAPDH complex formation might be one of the mechanisms involved in hippocampal neuronal damage.
4.Applicability of nursing interventions classification for nursing records of Pediatric Respiratory Medicine Department

Objective:To analyze the applicability of the 6th edition of nursing interventions classification in Pediatric Respiratory Medicine Department, so as to provide a basis for standardizing clinical nursing terms and promoting the clinical use of standardized nursing terminology.Methods:The research method of this research was mapping method. The electronic nursing records of Department of Respiratory Medicine in a ClassⅢGrade A children's specialty hospital from September 1 to November 30, 2018 were collected, including the nursing interventions recorded by free text and records of nursing interventions related to medical orders. The matching degree between electronic nursing records and standardized nursing interventions (the 6th edition of nursing interventions classification) was compared by concept consistency. Characteristics of standardized nursing interventions matched were analyzed by analyzing difficulty and time-consuming.Results:Totally, there were 84 standardized nursing interventions mapped from 3 258 independent electronic nursing statements, which involved 7 domains. Of the 3 258 statements, 3 183 (97.70%) were labelled as "perfect match", 55 (1.69%) as "partial match", and 20 (0.61%) as "not match at all". The average difficulty and time spent for the standardized nursing interventions were 1.76 and 2.02.Conclusions:Nursing interventions classification can be used to describe the daily behavior and activities of nurses in Pediatric Respiratory Medicine Department, and it can also solve the problem of standardization shown in nursing records. The results of this study can be used in the performance analysis of nursing staff with combination of difficulty and time-consuming empowerment in the future.
5.Genetic diagnosis and follow-up study in pediatric neurofibromatosis 1 patients

Objective:Based on the genetic diagnosis and follow-up study on pediatric neurofibromatosis 1 (NF1) patients, interrogating the genotype-phenotype correlations of patients with NF1 mutations.Methods:32 Patients from age of 2 months to 5 years old (17 male and 15 female) suspected for neurofibromatosis 1 were recruited during September 2016 to January 2018 in Shanghai Children′s Medical Center retrospectively. Genetic diagnosis was applied to detect pathogenic variants. Long-term follow-up study were conducted to reveal progress of the disease and genotype-phenotype correlations.Results:27 patients were detected with pathogenic NF1 variants, among them three were not reported. 3 patients inherited pathogenic variants from their NF1 diagnosed parents, all the other variants were de novo. Progressive development of phenotypes wasn′t observed in most patients during the follow-up (14/27). Some patients were diagnosed with short stature, pulmonary artery stenosis and developmental delay during the follow-up(7/27). Short stature and pulmonary artery stenosis may be associated with missense mutation and severe truncation mutation of NF1 gene, respectively. Conclusions:Genetic diagnosis is required in young patients of NF1.Follow-up plan of pediatric patients should be adjusted based on genetic findings. Early follow-up of cardiovascular abnormalities should be noted in patients with missense mutation. Height development in patients with severe truncating variants are needed.

7.Safety and efficacy of the day surgery model for the treatment of unilateral primary aldosteronism

Objective:To explore the safety and feasibility of adrenal tumor resection under day surgery mode for the treatment of unilateral primary aldosteronism (UPA).Methods:The clinical data of 83 patients who underwent adrenalectomy for the treatment of UPA from January 2020 to January 2023 were retrospectively analyzed. There were 42 males (50.6%)and 41 females (49.4%), age (49.0±12.3)years old. Body mass index (25.4±3.6)kg/m 2. There were 17 patients with diabetes, 58 patients with preoperative hypertension, and the duration of hypertension was 8.2(1, 15)years. Systolic blood pressure was 151.8(137.0, 160.0)mmHg (1 mmHg=0.133 kPa), and diastolic blood pressure was 97.4(87.0, 107.0)mmHg. 20 cases had severe preoperative hypokalemia, with preoperative blood potassium levels of (2.9±0.6)mmol/L. The ratio of preoperative plasma aldosterone to renin activity was 54.1(13.0, 77.2). Tumors were located on the left side in 47 cases (56.6%)and on the right side in 36 cases (43.4%). The maximum diameter of the tumor is 1.5(1.0, 1.7)cm. Patients diagnosed with unilateral primary aldosteronism were included, and patients diagnosed with adrenal pheochromocytoma, adrenal malignancy, or severe cardiovascular disease that cannot tolerate surgery were excluded. The patient was admitted to the hospital on the same day, underwent surgery on the same day, and was evaluated after extubation within one day after surgery. The patient had no fever or lower back pain after surgery, was able to freely get out of bed, and had no obvious abnormalities in blood routine, electrolyte and other test indicators. The patient was discharged from the hospital and received continuous care outside the hospital. The patient was guided to undergo regular outpatient follow-up. Evaluate the efficacy using the Primary Aldosterone Hyperaldosteronism Surgical Outcome (PASO)criteria. Results:The patients had a mean length of hospital stay of 43.0(40.3, 44.6)hours, an operation time of 89.6(70.0, 103.0)minutes, and an operation cost of 23 820.2 (21 150.8, 24 948. 3)yuan. The postoperative free position time was 29.5(20.8, 39.7)hours, the postoperative time for fluid placement of the diet was 25.2(20.1, 27.8) hours, and 42.2(37.9, 41.9)hours of postoperative drain removal. The overall postoperative clinical remission rate was 85.5%. There were four postoperative complications, accounting for 4.8% of cases. These included two cases of interosseous vein thrombosis, one case of delayed healing, and one case of peritoneal rupture. No case of readmission was reported within 30 days.Conclusions:UPA adrenal tumour resection in ambulatory mode is safe and effective when strict inclusion and exclusion criteria are followed, adequate preoperative preparation is carried out, and expert physician assessment is provided.
