OBJECTIVE:To assess the preventing effect of nicardipine on stress reaction induced by direct laryngoscopy.ME_THODS:30 patients undergoing elective direct laryngoscopy were randomly divided into two groups:the routine group(group C)and nicardipine group(group N),15 patients for each group.In group C,anesthesia was performed with routine anesthetics,in group N,addition to routine anesthetics,nicardipine was administered by a bolus of 10?g/kg,there after 1～2?g/(kg?min)continuously intravenous administration.Blood pressure(BP),heart rate (HR) and blood sugar(Glu)were recorded perioperatively.RESULTS:The BP and HR increased in both groups from intubation to extubation,but when compared to the base value,the increase in group C had statistical significance (P0.05);There were significant differences between two groups at the same time points(P

At first,this paper introduces a kind of sickbed desk,which can be manipulated easily,and patients could live more comfortable with it.Secondly,it introduces universal lamp holder.Different light sources are used in different medical equipments and the connection of their bulbs and lamp holders are different,too.These lamp holders are expensive and apt to be damaged.In order to prolong the using time of the chilly light,and reduce the spoilage and cost,this paper designs an economic and durable universal lamp holder.

Objective To explore occupational stress level and chronic illnesses prevalence of nurses in Xinjiang first-class hospital and provide scientific basis of reducing the nurses' occupational stress and improving health interventions.Methods Using stratified cluster sampling method,1 585 nurses were investigated occupational stress level and the chronic diseases who were from Xinjiang around the five states 3 armour hospital.Results Compared with national norm,the nurses had high professional task scored 177.79±29.85 and tension scored 102.46±22.18,individual coping scores from resources scored 119.84±24.06 were lower.The propotion of low,moderate,high tension nurse respectively was 6.1％ (97/1 585),47.1％ (746/1 585),46.8％ (742/1 585).Night shift frequency,nationality,title and average monthly income difference were statistically significant in terms of occupational stress level (x2=1.28-37.96,all P＜0.05).Joint disease,neck lumbar disease,chronic digestive system disease prevalence of nurse were higher,its morbidity rates were 10.9％(172/1 585),24.9％(394/1 585) and 22.1％(350/1 585) respectively.Presence of chronic diseases of the digestive system in high,medium and low tension was statistically difference (x2=9.558,P ＜ 0.01).Conclusions Occupational stress level of nurse is higher,and chronic diseases occurences are associated with occupational stress level.So the nurses occupational stress need to control so as to reduce chronic diseases.

Objective To analyze the relationship between family factors and learning motivation of students in Xinjiang Medical University so as to provide scientific basis for promoting the medical undergraduates' learning motivation in the future.Method From April to June in 2014,using stratified cluster random sampling,2 000 college students from Xinjiang Medical University were surveyed by family factors and learning motivation questionnaire,and 1954 effective questionnaire were recycled.SPSS 17.0 was used to analyze the data,T test and variance analysis were conducted to the single factor and multiple linear regression analysis was made to multiple factors.Results All dimensions of 1 954 medical students' learning motivation shown were average,and the score of fear of failure was the lowest (2.82 ± 1.01).There were statistically significant differences in the scores of multiple dimensions of learning motivation of the medical students of different gender,nationality and grade (P＜0.05).And the scores in learning motivation in multiple dimensions of the medical students of different types of accounts,different family relationships,and different parental education level and whether they were the only child also had statistically significant difference (P＜0.05).Multi factor analysis showed that learning motivation,family account type,grade,gender,race,whether they were poor students and family relationships were the factors influencing the learning motivation of medical students.Conclusion Family factors have a great influence on the generation and maintenance of medical students' learning motivation,and it is necessary to change the poor education environment of the family,so as to interfere the students' learning motivation.

Objective:To investigate the feasibility of rectus muscle insertion measurement with anterior segment optical coherence tomography (AS-OCT) after corrective surgery of concomitant exotropia.Methods:A cross-sectional study was performed.Twenty-five eyes of 25 patients with concomitant exotropia were included in Shenyang Aier Eye Hospital from March 2018 to February 2019, and all patients received concomitant exotropic corrective surgery.The distance of limbus to rectus muscle insertion was measured before operation, at the beginning and at the end of operation with a caliper and at postoperative 2 weeks, 1 month, 3 months and 6 months with AS-OCT, respectively.Degree of strabismus was examined by the alternate prism cover test at 33 cm.The research followed the Declaration of Helsinki and the protocol was approved by a Medical Ethics Committee of Shenyang Aier Eye Hospital (No.2017KJB005). Written informed consent was obtained from the patients/custodians prior to any medical examination and treatment.Results:A total of 44 rectus muscles of 25 eyes with concomitant exotropia were measured, including 25 lateral rectus muscles and 19 medial rectus muscles.The detection rate of the muscle insertion by AS-OCT was low at 2 weeks and 1 month after surgery.The mean distance of limbus to lateral rectus muscle insertions was (12.30±1.40), (11.72±1.45) and (11.84±1.27)mm at the end of operation and postoperative 3 and 6 months, respectively, showing a significant difference among them (F=4.085, P=0.023), and the distance measured at the end of operation was significantly larger than that at postoperative 3 months (P=0.018). The mean distance of limbus tomedial rectus muscle insertion was (5.12±0.42), (5.56±0.61) and (5.28±0.57)mm at the end of operation, postoperative 3 and 6 months, respectively, and there was no significant difference among the three time points (F=3.912, P=0.055). The degree of strabismus of the patients at various time points after surgery were significantly reduced in comparison with that before surgery (all at P<0.05).Conclusions:AS-OCT can track the position of extraocular muscle insertions after concomitant exotropic corrective surgery.

Objective To evaluate the efficacy of preoperative and postoperative intravesical instillation of pirarubicin for the prevention of postoperative recurrence of non-muscle invasive bladder cancer (NMIBC). Methods From September 2012 to March 2014, 120 patients who were diagnosed as NMIBC and accepted trans-urethral resection of bladder tumor (TUR-Bt) were collected and randomly divided into two groups, including treatment group (60 cases) and control group (60 cases). 30 minutes before the operation, instillation of pirarubicin (40 mg) was performed in the treatment group, then pirarubicin perfusion was given within 24 h after operation, once a week for 8 weeks, and once a month for 10 months after operation. In control group, instillation of pirarubicin (40 mg) was given within 24 h after operation, and other treatments were the same as the treatment group. The differences of lesion detection probability, the 2-year recurrent rate and the adverse reaction rate were compared between the two groups. Results The lesion detection rates were 42.70 % (38/89) and 1.85 % (2/108) in the treatment group and control group, respectively (χ2=50.303, P<0.05). 119 patients were follow-up for 24 months. The 2-year recurrence rates were 8.47%(5/59), 21.67% (13/60) in treatment group and control group respectively (χ2= 4.033, P< 0.05). The adverse reactions mainly included hematuria (3 cases in treatment group, 3 cases in control group), urinary tract irritation (3 cases in treatment group, 2 cases in control group) and urethral stricture (1 case in treatment group, 1 case in control group). The difference of the adverse reaction incidence between the two groups was not significant statistically [11.86 % (7/59) vs. 10.00 (6/60), χ 2 = 0.106, P > 0.05]. Conclusions The preoperational intravesical instillation of pirarubicin can fix and diagnose malignant lesions in the bladder without adverse reactions, which will improve the rate of tumor resection and prevent postoperative recurrence of tumors.

Objective To establish the quality standard of flavone extracts in licorice residue; To provide the methods and basis for the quality control of total flavonoids extracted from licorice residue.Methods TLC was applied to compare the extracts with liquiritin and licorice; UV-spectrophotometry was used to measure the content of total flavones; EDTA-2Na titration was used to determine the content of calciumion.Results In the chromatogram of the test product, the spots of the same color were displayed on the corresponding position of the control medicinal material, and the fluorescent spots of the same color were displayed on the corresponding position of the control product. UV-spectrophotometry showed glycyrrhizin had a good linear relationship in the range of 3.32–9.96 μg/mL (r=0.9997), the absorbance between 0.3 and 0.7. The average recovery rate was 102.41% (RSD=3.22%), and the average content of total flavonoids in three batches of flavonoids extract was 8.82%. The average content of calciumion was 0.107% (RSD=3.61%).Conclusion This method has good repeatability, high sensitivity and accurate results, and can be used for the quality control of flavonoids in licorice residue.

Objective To generate high purity and maturity DC from human peripheral blood monocytes in vitro.Methods PBMC were isolated directly from whole blood by density gradient centrifugation and purified by collecting the attached cell,DC were then generated by induction and culturing PBMC for five days with RPMI1640 medium containing rhGM-CSF and rhIL-4 in vitro,and under the condition of 37 ℃,5% CO2.On the fifth day,rhTNF-α was added into DCs cultures,which were then incubated for three additional days.The morphology was monitored by light microscopy,and the phenotypes were determined by FCM.Results After eight days of culture,the cells developed typical and significant dendritic morphology and plenty of cells expressed CD1a, CD80 and CD83,features of DC.Including(78.07±9.43)%CD1a,(60.11±20.50)% CD80 and(46.82±14.15)% CD83 were expressed.About (3.12±1.30)x106 DC cells were derived from 40ml human peripheral blood.Conclusion The way to generate DCs is simple and easy.The DCs produced by this method acquired high purity and maturity antigenic characteristics of DCs.

Objective To evaluate the efficacy and feasibility of 125I radioactive seed interstitial implantation for the local advanced urinary tract epithelial carcinoma without total resection.Methods The clinical data of 21 patients with local advanced urothelial carcinoma without completely surgical resection who were treated by 125I radioactive particles implantation were retrospectively analyzed.The patients were divided into two groups and received preoperative plan.In group A, 14 patients received implants during operations, including 7 bladder transitional cell carcinoma patients who underwent a transurethral resection surgery or partial bladder resection and another 7 ureteral carcinoma cases who underwent semi urine road resection.After these operations, all of patients had tumor residues.After furthest resection of the tumor, the 125I seeds were implanted at the residual suspicious tumors and their surroundings.In group B, 7 patients were implanted under the guidance of color ultrasound or CT.According to the evaluation criteria of solid tumor in 2009, the tumor remission rate, survival rate, distribution and the mobile information of the particles were observed.Results The operations were successfully completed in all of 21 patients, and serious complications did not appear during the operation.Particle distribution and lesions were basic coincidence.A total of 2 particles displaced, but every patient had no adverse reactions.There were 2 patients with local fever after operation within 4 months, while the rest of patients had no adverse reactions.After median follow-up for 36 months (3-75 months) , the 1-year survival rate was 100.0 ％ (21/21), the 2-year survival rate was 90.5 ％ (19/21), and the 3-year survival rate was 61.9 ％ (13/21).The tumor remission rate of group A was 85.7 ％ (12/14) after 6-9 months, and was 42.9 ％ (6/14) after 12 months.The bladder was preserved in 7 cases with bladder tumors with the 2-year survival rate of 100.0 ％ (7/7) and the 5-year survival rate of 71.4 ％ (5/7).The tumor remission rate of group B after 6-9 months was 71.4 ％ (5/7), and was 42.9 ％ (3/7) after 12 months.Conclusion 125I seed implantation is a good choice for locally advanced urothelial carcinoma with high local tumor control rate, which is expected to be applied in the individual treatment of advanced urothelial cancer.

Objective To develop an HPLC method for determination of paeoniflorin in Fufang Chishao Granula. Methods The separation was performed on a Waters C18 column (250 mm× 4.6 mm, 5μm) with acetonitrile-water (30∶70) as the mobile phase at the flow rate of 1.0 mL/min. The detection wavelength was set at 230 nm. Results Paeoniflorin showed good linearity (r=0.999 9) in the range of 0.266-5.32 μg, and the average recovery was 98.00%, RSD=2.22%. Conclusion The method is convenient, sensitive, accurate and repeatable. It can be applied to determination of paeoniflorin in Fufang Chishao Granula and control the quality of the preparation.
