1.The change level of insulin and thyroxin has associate with the operation wound

Objective To evaluate the effect of operation wound on insulin and thyroxin in serum. Methods Left hemiihepatectomy,splenectomy,orchiectomy or pseudo-operation were carried out separately on 65 healthy rabbits, which were randomly divided into experimental and control groups. The insulin and thyroxin in serum were examined by peripheral venous blood samples drawn by one day prior,24 hours, 48 hours,one week and one month after the operation respectively. Results The post-operative insulin in serum increased obviously in experimental group.One week later,it decreased gradually and reached to pre-operative level after one month of the operation.The level of TT4 and TT3 significantly decreased at 24 hour postoperatively.and gradually rose at 48 hour and up to normal levels in the first week postoperatively the raised degree of TT3 was higher than TT4, both of them approached the same level of preoperative measurement in the first month. The change insulin and thyroxin level of insulin on hemihepatectomy were higher than those on splenectomy.orchiectomy effect smaller. As a comparison,pseudo-operation had little effect on the serum insulin. Conclusions The reaction of higher insulin after operation wound is probably caused by insulin resistance. The syndrome of normal thyroid disease after opereation wound,is probably acused by stress,cytolcine,serum 5'-deiodinase or it's reduce. The change levels of insulin and thyroxin have correlation with the operation wound.
2.The Value of Diagnosis and Prognosis of Chest Imaging Feature for Severe Acute Pancreatitis

Objective To study mechanism and relationship of acute lung injury that be complicated with severe acute pancreatitis(SAP).To analyze the chest imaging feature of X-ray and CT of SAP and improve the recognition of clinical value of diagnosis and prognosis.Methods Thirtynine SAP patients(17 male,22 female)age ranged from 26 to 64 of SAP that be diagnosed by clinic and CT were analysed retrospectively.All cases were checked by chest X-ray film and rechecked at least three times,of them chest CT were taken in 11 cases.Results Ascites was found in 34 cases,pleural effusion was found in 19 cases.Pulmonary edema sign of X-ray and CT was found in 16(41%) cases when SAP was diagnosed by pancreas CT.The sign was found in 24(61.5%) cases in 24 hours and in 37(94.9%)cases in 48 hours and all cases in 72 hours.In the 16 early pulmonary edema cases,There were pleural effusion in 11 and pericardiac effusion in one case,10 cases of them developed to acute respiratory distress syndrome(ARDS)and 7 cases died.Conclusion The chest imaging feature of SAP has important significance for early diagnosis of ARDS and prognosis of SAP,and can be served as a clinical indication of severe degree of SAP.
3.Effect of hyperthermia on VX_2 carcinoma bearing rabbits

Objective To study the effect of hype rt hermia on cytokines of VX_2 carcinoma bearing rabbits. Method Forty healthy male rabbits were randoml y divided into heat therapy group and control group. Rabbits in heat therapy gro up underwent hyperthermia 11 days after inoculated with VX_2 cells. IL-2, tumor necrosis factor (TNF) and insulinlike growth factor (IGF) were measured b efore and after heat therapy. Results In heat therapy group survival was prolonged for 15 days. IL-2 d ecreased gradually from (134?6)ng/ml before heat therapy, (81?7)ng/ml one week after, (61.8?2.0)ng/ml at 3 weeks, and (54.8?2.6)ng/ml at 4 weeks along with an increase of TNF and IGF (4.5?0.9)ng/ml, and ( 96?28)ng/ml; (7.6?1.8)ng/ml and (678?183)ng/ml; (15.1?3 .0)ng/ml and (991?108)ng/ml; (17.2?2.1)ng/ml and (1136?89 )ng/ml respectively at the corresponding time period. Conclusion Heat therapy is effective for the tre atment of superficial tumors probably through a mechanism of antigen-antibody r eaction.
4.Treatment of non-small cell lung carcinoma by hyperthermia therapy combined with chemotherapy

Objective To evaluate therapeutic effects of hyperthermia therapy in combination with chemotherapy for the treatment of nonsmall cell lung carcinoma. Methods Fifty-two patients with non-small cell lung carcinoma underwent hyperthermia therapy in combination with chemotherapy(NP regimen) in this hospital between September 2003 and November 2006.The therapeutic effects were evaluated after 3 cycles of the combination therapy,and were compared with those from 16 patients receiving only chemotherapy between January 2006 and November 2006.Results The combination therapy offered significantly better curative short-term effects than the simple chemotherapy.The total response rate was 65.4%(34/52) for the combination therapy and 37.5%(6/16) for the simple chemotherapy(Z=-2.419,P=0.016).Significant pain relief was achieved in patients after the combination therapy,while no analgesic effect was noted after the simple chemotherapy,the difference between the two groups of patients being statistically significant(Z=6.486,P=0.000).There was no significant difference in the incidence of complications between the two groups,which mainly included nausea,granulocytopenia,thrombocytopenia,and abnormal liver functions.Conclusions Hyperthermia therapy in combination with chemotherapy compares favorably to simple chemotherapy in the treatment of non-small cell lung carcinoma.
5.Influence of intravesical chemotherapy combined with hyperthmia on TNF 、IGF in carcinoma of bladder after TURBt

Objective To explore the influence of intravesical chemotherapy combined with hyperthmia on levels of insulin growth factor( IGF) and tumor necrosis factor( TNF) in bladder cancer after operation. Methods 64 cases of transitional cell carcinoma of bladder accepted TURBt were randomly divided into two groups: intravesical chemotherapy combined with hyperthmia group and intravesical chemotherapy group. Levels of ICF and TNF in two weeks before therapy 、two weeks after therapy, four weeks after therapy were tested. Results The IGF levels were (65.97±5.25)μg/L,(49.93±1.98)μg/L,(36.82±1.89)μg/L in intravesical chemotherapy combined with hyperthmia group and(61.87 ±5.41)μg/L, (57.86 ±2. 18) μg/L, (48. 68 ±2. 26) μg/L in intravesical chemotherapy group. The TNF levels were(2.32 ±0. 36)μg/L, (3. 84 ±0. 31) μg/L, (8. 79 ±0. 46)μg/L in intravesical chemotherapy combined with hyperthmia group and(2. 21 ±0. 19)μg/L,(2. 89 ±0.47) μg/L, (3. 87 ±0.61)μg/L in intravesical chemotherapy group. The influence of intravesical chemotherapy with hyperthmia on levels of IGF and TNF were significant than intravesical chemotherap, and the influence degree showed in time-dependent manner (all P < 0. 05 ). Conclusion Intravesical chemotherapy combined with hyperthmia had curative effect in inhibiting the recurrence of tumor after operation.
6.Discussion of surgery treatment of serious dislocation of acromio-clavicular joint

[Objective] To explore the surgery protocol of the serious dislocation of acromio-clavieular joint,and compare the outcome of ORIF with AO/ASIF clavicle hook plate only to ORIF with eoraco-clavieular ligament reconstruction.[Method]Fity-two cases of the dislocation of acromio-clavicular joint were reviewed,Group A was ORIF only including 29 cases(male 20,female 9,mean age 45),according to the system of Rockwood,as TypeⅢ in 12 patients,TypeⅣ in 15 patients and TypeⅤ in 2 patients,the other 23 cases ORIF with eoraeo-clavieular l ligament reconstruction was Group B(male 15,female 8,mean age 50.6),according to the system of Rockwood,as TypeⅢ in 9 patients,TypeⅣ in 13 patients and TypeⅤ in 1 patients,AO/ASIF claviadar hook plates were used for all of the ORIF,and modified Weaver's method was used to reconstruct eoraco-clavieular ligament.Outcome was evaluated by Lazzcano scale after 8~12 months,and data was analyzed by SPSS 10.0.[Result]Good outcome was achieved in all of the patients,there was no significanty statistical difference between the two groups.[Conclusion]Clavicle hook plate is a kind of perfect internal fixation.Reconstruction of eoraco-clavleular ligament is not necessary if the AO/ASIF plate is used,unless serious avulsion of deltoid and trapezius muscle involved.
7.Transcatheter thrombolysis via the small saphenous vein for acute lower limb deep venous thrombosis

Objective To evaluate transcatheter thrombolysis via the small saphenous vein in the treatment for lower limb acute mix-deep venous thrombosis. Method From Jan 2005 to Mar 2007.37 patients with lower limb acute mix-DVT underwent catheter-directed thrombolysis via the small saphenous vein with urokinase(149 ±71)×104 IU continuous infusion.The venous patency score and the rate of patency improvement were observed by venograms before and after therapy.Twenty-two patients were followed up for(12 ±4)months. Results Venous patency score were significantly improved(Z=-5.330,P<0.01).The mean rate of venous patency was 50.17%±15%,and there was no complication.Venogram on 6~12 month follow up showed a venous patency of 58%±13%(Z=-3.545,P<0.01),and that on 13~18 months was 68%±20%(Z=-2.201,P<0.05). Conclusion This preliminary experience suggests that catheter-directed thrombolysis via the small saphenous vein with urokinase for acute lower limb mix-DVT iS safe and effective.
8.Clinical analysis of central venous stenosis In 5 hemodialysis patients without a previous history of catheterization

Objective To evaluate imaging findings and treatment experience in central venous stenosis without a history of previous catheterization in hemodialysis patients. Methods Clinical data of 5 haemodialysis cases of central vein stenosis without a previous catheterization history in our hospital from July 2006 to July 2008 were analyzed retrospectively. Results Patients were three women and two men aged 43 to 65 years with mean age(53±8)years and all had arm swelling as the main complaint.The vascular accesses were located at the wrist in all the patients.The mean duration of the vascular accesses from the time of creation was(33.6±35.4)months.Venography showed occlusion in 2 cases and stenosis in 3 cages of central vein including 1 case of stenosis in brachiocephalic vein.1 case of stenosis beth in branchiocephalic vein and subclavian vein,1 case of stenosis in two segments of subclavian vein.The stenosis of branchiocephalic vein was fixed anterior to the tracheal and CT showed the compression of the vein by the aorta.Symptoms were resolved by the treatment of PTA.subclavian vein-contralateral subclavian vein bypass and ligation of the access. Conclusions Central venous stenosis in haemodialysis patients without a history of catheterization may be due to the intimal hyperplasia of the compression site or valve which is accelerated by the high flow of vascular access.Venography is the first choice for the diagnosis and the current management of central venous stenosis is far from being effective for the long term.
9.Development and evaluation of portable pulse vacuum pressure steam sterilizer

Objective Due to poor effects of sterilization,low efficiency and inconvenience of field emergency transportation for portable exhaust pressure steam sterilizer,we aimed to design and develop a portable pulse vacuum pressure steam sterilizer in order to effectively improve quality and efficiency of sterilization for sterile medical equipment in disaster and emergency medical treatment.Methods From the beginning of February 2013,we focused on the features of small size,light weight,reliable sterilizing effect and fast speed of sterilization,and conducted series of technology researches,and finally developed portable pulse vacuum pressure steam sterilizer which was suitable for individual carrying or air delivery.Results We performed outdoor sterilization tests for portable vacuum pressure steam sterilizer and portable exhaust pressure steam sterilizer under the same condition.Portable pulse vacuum pressure steam sterilizer achieved a 100% qualification rate,average sterilization cycle was 28.6min per time,sterilization by weight was 48L/h,average water consumption was 0.4L/boiler,average power consumption was 0.7kw/h per time,and sterilization operation index,maneuverability and transportability were significantly better than those of portable exhaust pressure steam sterilizer.Conclusion The developed 4 portable pulse vacuum pressure steam sterilizer have higher cost performance,and have been successfully applied for medical service training and non-war medical treatment activities in our hospital.
