1.The Value of Diagnosis and Prognosis of Chest Imaging Feature for Severe Acute Pancreatitis

Objective To study mechanism and relationship of acute lung injury that be complicated with severe acute pancreatitis(SAP).To analyze the chest imaging feature of X-ray and CT of SAP and improve the recognition of clinical value of diagnosis and prognosis.Methods Thirtynine SAP patients(17 male,22 female)age ranged from 26 to 64 of SAP that be diagnosed by clinic and CT were analysed retrospectively.All cases were checked by chest X-ray film and rechecked at least three times,of them chest CT were taken in 11 cases.Results Ascites was found in 34 cases,pleural effusion was found in 19 cases.Pulmonary edema sign of X-ray and CT was found in 16(41%) cases when SAP was diagnosed by pancreas CT.The sign was found in 24(61.5%) cases in 24 hours and in 37(94.9%)cases in 48 hours and all cases in 72 hours.In the 16 early pulmonary edema cases,There were pleural effusion in 11 and pericardiac effusion in one case,10 cases of them developed to acute respiratory distress syndrome(ARDS)and 7 cases died.Conclusion The chest imaging feature of SAP has important significance for early diagnosis of ARDS and prognosis of SAP,and can be served as a clinical indication of severe degree of SAP.
2.Effect of ginsenoside Rg3 on Cx26 gene expression and gap junctional intercellular communication in human breast cancer cell line MCF-7

Objective To study the function of ginsenoside Rg3 on proliferation in human breast cancer cell line MCF-7 and effect of ginsenoside Rg3 on Cx26 gene expression and gap junctional intercellular communication (GJIC) in MCF-7, cultured in vitro. MethodsHuman breast cancer cells MCF-7 was exposed to ginsenoside Rg3 at differential concentrations for 24 h, respectively. The cell proliferation inhibition was measured by 3-[4,5-dimethylthiazo-2-yl]-2,5 diphenyl tetrazolium bromide (MTT) assay. The expression of Cx26 mRNA was measured by RT-PCR in experimental groups and control goup. The GJIC function of MCF-7 cell was examined with scrape-loading dye transfer assay.ResultsHuman breast cancer cell line MCF-7 was exposed to ginsenoside Rg3 at a concentration of 10, 20, 40, 80, 160 μg/ml, respectively.The inhibition ratio was 3.1%, 5.2 %, 16.0 %, 26.3 %, 29.1% respectively after 24 h. Compared with control group, the concentration of 40 μg/ml above could significantly inhibit MCF-7 cell proliferation (P <0.05), so the experimental groups were exposed to ginsenoside Rg3 at a concentration of 40, 80, 160 μg/ml,respectively. The expression of Cx26 mRNA in every experimental group compared with control group was enhanced when MCF-7 cell was exposed to ginsenoside Rg3 at a higher concentration. It was observed that Lucifer yellow fluorescent staining was limited to a single cell in control group through fluorescent microscope,but Lucifer yellow fluorescent transfered through gap junction cells to neighboring cells, then came into being flake fluorescent staining in experiment groups. ConclusionGinsenoside Rg3 can enhance the expression of Cx26 mRNA in MCF-7 cell and restore the gap junctional intercellular communication, which may be one of important mechanisms of ginsenoside Rg3 in antitumor.
3.Lumbopelvic reconstruction for treatment of unstable sacral fractures

Objective To evaluate the clinical outcome of lumbopelvic reconstruction in treatment of unstable sacral fractures.Methods A retrospective study was performed on 17 cases (12 males and 5 females; at 23-55 years of age,mean 35.5 years) of unstable sacral fractures treated from January 2007 to June 2012.There were 11 cases of zone Ⅱ fracture and six zone Ⅲ fracture according to Denis classification and nine cases of type B fracture and eight type C fracture according to Tile classification.Sacral nerve injury assessed by Gibbons criteria was 3 points in seven cases and 4 points in 10 cases.Lumbar-pelvic ring stability of the patients was restored by posterior decompression and lumbar pedicle screw fixation combined with sacral pedicle screw or iliac screw fixation.Fracture reduction and healing were measured by X-ray film or CT scan; functional outcomes by Majeed scale; neurological outcome by Gibbons criteria.Results All the cases were followed up for mean 16 months (range,8-24 months).X-ray and CT follow-up revealed all fractures had bone union at average 6 months in the absence of remnant sacrum malformation,pseudarthrosis and fracture redisplacement.Iliac screw loosening not yet breakage happened to one case.In total,12 cases had full recovery of neurological function; four significant improvement,but experienced different degree of footdrop and hypoesthesia of lower extremities; one poor improvement and experienced not only lower extremity dysfunction but also bladder and bowel dysfunction.According to Majeed scale in the final follow-up,clinical functional outcome was excellent in 12 cases,good in three,fair in one and poor in one,with excellent-good rate of 88%.Gibbons score improved from preoperative (3.29 ±0.47) points to postoperative (1.53±0.94) points (t=12.94,P<0.01).Conclusion Posterior decompression plus lumbar pedicle screw fixation combined with sacral pedicle screw and/or iliac screw fixation is an effective method for treatment of unstable sacral fracture,for it can restore general stability of spine-pelvis,facilitate neurological function recovery and allow early weight-bearing.
4.Analysis on Medication Experiences of Famous Senior Chinese Medicine Expert Huang Chunlin for Treatment of Chronic Nephritis Based on Data Mining Method

This paper was aimed to summarize professor Huang Chunlin’s medication experiences, rules and prescription characteristics in the treatment of chronic nephritis through data mining on patients’ records of chronic nephritis. The records of 92 outpatients which met the criteria were collected. The total treatment number was 276. Text conversion was made on patients’ records. And the archive of patients’ data was established. With the support of Traditional Chinese Medicine Inheritance Support System, the medication experiences and prescription rules of professor Huang were summarized. The results showed that the top 20 herbs with relatively high using frequencies were mostly sweet in flavor and neutral in nature. The meridian distributions were mostly the kidney, spleen and liver meridian. The medication effects were mainly qi-reinforcing, yang-reinforcing, water-clearing and dampness-removing. Results from data mining showed 5 couplet medicines and 8 new prescriptions in chronic nephritis treatment. It was concluded that in Professor Huang’s syndrome differentiation of chronic nephritis, it was related to the spleen and kidney, and also related to dampness and stasis. The characteristics in medication were “high-dose for severe conditions”, “sweet in flavor and neutral in nature with outstanding effects”, “always related to the spleen and kidney”, “always related to dampness and stasis”.
5.Effects of teletherapy and located hyperthermia on VX_2 rabbits'cytokines

Objective To observe teletherapy and local hyperthermia effects on VX2 carcinoma rabbits'cytokines and curative effect. Methods 60 male white health rabbits were selected, and divided them into three group. The group of teletherapy were 20. The group of local hyperthermia were 20. So were the group of comparison. Their tumors' changes, rates of death and changes of IL-2, IL-6, IL-8, TNF, IGF, CA-50 before and after VX2 carcinoma replanted and those treated after one, three, four weeks were observed. Results The diameters of tumors and rates of death from large to small in turn was comparison's group, hyperthermia's group and teletherapy's group. After VX2 carcinoma replanted IL-2, IL-8 were low but IL-6, TNF, IGF, CA-50 were high. After being treated one, three or four weeks, IL-2, IL-8 were getting lower and lower but there was significant difference between teletherapy's group with other group in IL-2 and IL-8(P
6.Effects of fatty acid binding protein-5 silencing on proliferation, apoptosis, invasion of human glioma cells and expressions of CD44+and CD133+

BACKGROUND:It has been demonstrated that there are different expressions of fatty acid binding proteins (FABPs) in most malignant tumors such as breast cancer, prostate cancer, liver cancer, lung carcinoma and bladder carcinoma;therefore, FABPs are closely related to the occurrence, metastasis, invasion and drug resistance of malignant tumors. OBJECTIVE:To investigate the effects of fatty acid binding protein-5 (FABP-5) silencing on the proliferation, invasion and apoptosis of U251 cel s (human glioma cel s). METHODS:siRNA molecules targeting the mRNA of FABP-5 was designed and chenical y synthesized, which was transiently transfected into U251 cel s. U251 cel s were divided into three groups:Lv-shRNA-FABP-5 was added into FABP-5-shRNA group, LV-shRNA-NC added into negative control group, and blank control group underwent normal culture. mRNA and protein expressions of FABP-5 were detected by RT-PCR and western blot assay, respectively. The cel proliferation in vitro was determined by cel counting kit-8 assay, the cel cycle, apoptosis and expressions of CD44+and CD133+were detected by flow cytometry, and the apoptosis was observed using TUNEL staining. RESULTS AND COUNCLUSION:mRNA and protein expressions of FABP-5 in the FABP-5-shRNA group were significantly lower than those in the negative control and blank control groups. Compared with the negative control and blank control groups, the cel growth rate was significantly decreased, the cel cycle arrested in the G0/G1 phase, and the cel number in the S phase was decreased, and moreover, the proportion of CD44+/CD133+expression was significantly decreased in the FABP-5-shRNA group (P<0.05). Besides, compared with the negative control and blank control groups, the apoptosis rate was significantly increased, and the cel proliferation and invasiveness were significantly decreased in the FABP-5-shRNA group (P<0.05). In conclusion, it is possible that FABP-5 directly or indirectly regulates the cel cycle and apoptosis of glioma cel s, and its expression changes share a close relationship with the invasiveness of tumor cel s.

Objective To investigate the effect of megestrolacetate(MA)in improving the quality of life in patients with advanced malignant neoplasms. Methods Seventy-two patients with advanced malignant neoplasms were randomized into two groups,treatment group(37 patients) were treated with MA plus supportive care,control group(35 patients) were treated with supportive care only.The changes of appetite,weight,pain,KPS and overall survival were observed in each group.Results In the treatment group,29 cases(80.6%) got appetite improving,19 cases(52.8%) gained weight,15 cases((41.7)%)got pain relief,21 cases(58.3%)got KPS score increasing;In the control group,8 cases((22.9)%) got appetite improving,4 cases(11.4%) gained weight,3 cases(8.6%)got pain relief,5 cases(14.3%)got KPS score increasing.The differences in two groups were significant(P
8.The Relationship Between KRAS Status and Response of Cetuximab in Combination with Oxaliplatin,Leucovorin and Fluorouracil/Xeloda in the First Line Treatment of Advanced Colorectal Cancer

Objective This study assessed whether the best overall response rate(ORR) of cetuximab combined with oxaliplatin,leucovorin and fluorouracil/Xeloda was superior to that of this method alone as first-line treatment for metastatic colorectal cancer.The influence of KRAS mutation status was investigated.Methods Patients received cetuximab(400mg/m2 initial dose followed by 250mg/m2,wk thereafter)less than 2 times plus chemotherapy(oxaliplatin 130mg/m2 on day 1,plus leucovorin 200mg/m2 and fluorouracil as a 400mg/m2 bolus followed by a 600mg/m2 infusion during 22 hours on days 1 and 2) or chemotherapy alone.Treatment was continued until disease progression or unacceptable toxicity.KRAS mutation status was assessed in the subset of patients with assessable tumor samples.Results The confirmed ORR for cetuximab plus oxaliplatin,leucovorin and fluorouracil/Xeloda was higher than that with alone(42.86% vs 21.74%).A statistically significant increase in the odds for a response with the addition of cetuximab to oxaliplatin,leucovorin and fluorouracil/Xeloda could be established.In patients with KRAS wild-type tumors,the addition of cetuximab to oxaliplatin,leucovorin and fluorouracil/Xeloda was associated with a clinically significant increased chance of response(ORR 54.55% vs 21.74%) and a lower risk as compared with chemothrapy alone.Cetuximab plus oxaliplatin,leucovorin and fluorouracil/Xeloda was generally well tolerated.Conclusion The clinical effcacy of chemothrapy(oxaliplatin,leucovorin and fluorouracil/Xeloda) plus cetuximab is better than only chemothrapy.KRAS mutational status was shown to be a highly predictive selection criterion in the treatment decision regarding the addition of cetuximab to oxaliplatin,leucovorin and fluorouracil/Xeloda for previously untreated patients with metastatic colorectal cancer.
9.Ischemic preconditioning improves hepatic regeneration with reduced injury following reduced-size rat liver transplantation

BACKGROUND: Recently,liver transplantation technique has been developed rapidly,and prevention of ischemia/reperfusion injury and protection of liver regeneration have become a research focus.Ischemic preconditioning(IPC)is an effective method for protecting liver ischemic injury.However,the mechanism remains controversial.OBJECTIVE: To investigate the mechanism of IPC on hepatic injury and regeneration after reduced-size rat liver transplantation.METHODS: Animals were randomly divided into 3 groups.Rat reduced-size liver transplantation model was established in liver transplantation group.IPC+liver transplantation group underwent first porta hepatis blocking for 10 minutes before liver graft reperfusion,followed by reperfusion for 15 minutes.The ligament around the liver was dissociated in the sham-surgery group.The samples were collected 0.5,2,6 and 24 hours post-operation.The hepatic injury was examined by the serum alanine aminotransferase(ALT)and hepatic tissue histopathology analysis of grafts.Semi-quantitative immunohistochemistry and westernblotting were used to examine the redox factor-1(Ref-1)protein expression.The hepatic regeneration of the grafts was examined by the expression of proliferating cell nuclear antigen(PCNA)in hepatic cells.RESULTS AND CONCLUSION: Compared with liver transplantation group,the ALT values at 6 and 24 hours after operation in IPC group decreased significantly(P < 0.05; P < 0.01).Pathological analysis indicated that there were lots of inflammation cells around the portal veins,the serious sinus hepaticus dilation and damage of hepatic tissue in liver transplantation group.However,the tissue injury observed in IPC group was comparatively slight.Semi-quantitative immunohistochemistry revealed that Ref-1 protein was more abundant in IPC grafts tissue compared to liver transplantation group.These observations were supported by westernblotting studies where Ref-1 protein was shown to be over-expressed in IPC specimens at 24 hours after reduced-size liver transplantation(P < 0.05).In addition,the number of PCNA-positive cells in IPC group was more than liver transplantation group at 2,6 and 24 hours after operation(P < 0.05).IPC improves hepatic regeneration and relieves grafts injury in earlier period after reduced-size rat liver transplantation,which is associated with the over-expression of Ref-1protein.
10.Treatment choices for atlantoaxial complex fractures

Objective To discuss the clinical characteristics,injury mechanisms,and treatment options of atlantoaxial complex fractures.Methods Twenty-three patients with atlantoaxial complex fractures were enrolled in the study,including 13 men and 10 women with age ranging from 23 to 63 years (average 42.6 years).Treatment options were determined on the basis of injury patterns of the patients,including nonoperative treatments for 11 patients and operative treatments for 12 patients.Complications and prognosis were evaluated for all patients in follow-up.Results All patients were followed up for average 32 months (range,6-58 months),which showed the recovery of atlantoaxial stability and the bony union or fusion.At postoperative six months,Japanese Orthopedics Association (JOA) score of all patients averaged 14.1 points (range,11.5-15.8 points) with the improvement rate of 82.7%.No complications like spinal cord injury,vertebral artery injury or cerebrospinal fluid leakage occurred in operative treatment group.Conclusions Mechanisms of atlantoaxial complex fractures are largely overextension/compression and lateral bending/compression.Furthermore,treatment plans should be determined in the light of fracture types.
