1.The detection of serum creatinine, urine output, and urinary chalone C in children with acute kidney injury and its clinical significance

Objective To detect the expression of serum creatinine (SCr),urine output,and urinary chalone C (CysC) in children with acute kidney injury (AKI) and to explore its clinical significance.Methods A total of 34 children infected by H1N1 or EV71 virus were enrolled in this study.Thirty-four cases of children were divided into AKI group(12 cases) and non-AKI group(22 cases).The level of SCr,urine output and CysC in two groups was compared on admission and 1,3 d after admission.Results The incidence rate of AKI in virus infected children was 35.3% (12/34).There was no significant difference in SCr and urine output between two groups on admission (P > 0.05).But the CysC in AKI group was higher than that in non-AKI group [(1.64 ± 0.23) mg/L vs.(0.85 ± 0.16) mg/L] (P < 0.05).There was no significant difference of SCr,urine output and CysC in non-AKI group (P > 0.05),but the level of SCr and CysC in AKI group after admission was significantly higher than that on admission [(38.25 ±7.18),(40.54 ± 7.62) μ mol/L vs.(25.26 ± 5.42) μ mol/L; (2.04 ± 0.33),(2.56 ± 0.41) mg/L vs.(1.64 ± 0.23) mg/L] (P < 0.05 or < 0.01),and urine output was significantly lower[(0.37 ± 0.12),(0.41 ± 0.14) ml/(kg·h) vs.(0.54 ±0.20) ml/ (kg·h)](P< 0.05).There was no significant difference in AKI group in SCr and urine between 1 day and 3 day after admission,while the CysC was statistically significant (P < 0.01).Correlation analysis showed that the expression of serum CysC and SCr had positively significant correlation (r =0.412,P <0.05),and the urine volume had significantly negative correlation (r =-0.364,P < 0.05).Conclusions High incidence of AKI occurs in children of virus infection.CysC value in early diagnosis of AKI is significantly superior to SCr and urine volume,and is expected to be an index in early prevention and treatment of children with AKI.
2.The effect of valproate acid on the activity of neutrophils in children with epilepsy and its clinical significance

Objective To investigate the effect ofvalproate acid(VAP) on the activity ofneutrophils in children with epilepsy and its clinical significance.Methods A total of 34 cases of children with epilepsy(epilepsy group) were enrolled in this study.Another 30 healthy children were as control group.The activation rate of neutrophils was detected by flow cytometry.The levels of serum high sensitivity-C reactive protein(hs-CRP) were measured by latex enhanced immunoturbidimetric analysis.The levels of serum tumor necrosis factor-alpha (TNF-α) and interleukin-6 (IL-6) were measured by enzyme linked immunosorbent assay analysis.Results The activation rate of neutrophils had no significant difference between epilepsy group before treatment and control group (P > 0.05).The activation rate of neutrophils after treatment of 6,12 months in epilepsy group was higher than that in control group [(12.36 ± 4.72)%,(15.87 ± 5.68)% vs.(5.32 ± 1.41)%,P < 0.01].The activation rate of neutrophils after treatment of 12 months was higher than that after treatment of 6 months in epilepsy group (P < 0.05).There was no significant difference in the levels of serum hs-CRP,TNF-α and IL-6 between epilepsy group before treatment and control group (P > 0.05).The levels of serum hs-CRP,TNF-α and IL-6 after treatment of 6,12 months in epilepsy group were higher than those in control group [(3.64 ± 1.22),(6.96 ± 2.64) mg/L vs.(1.46 ± 0.27) mg/L,(74.72 ± 22.58),(96.67 ± 30.25) ng/L vs.(31.72 ± 12.16) ng/L,(32.59 ± 8.45),(46.74 ± 12.16) ng/L vs.(15.36 ± 4.45)ng/L,P < 0.01],and those after treatment of 12 months were higher than those after treatment of 6 months (P < 0.05).The activation rate of neutrophils was positively related with the serum levels of hs-CRP,TNF-αand IL-6 (r =0.328,0.402,0.344,P < 0.05).Conclusions The high activation rate of neutrophils is found in children with epilepsy.The abnormal activation of neutrophils makes the body undergo high oxidative stress status.

4.Metagenome and detection of animal microbial pathogens

Metagenomics ( metagenome) obtained all the microorganisms genome in specific biological environment from DNA samples directly from soil, water and skin, body cavity of human and animals.The metagenomic library was constructed by cloning them into alternative host cells using the vector, which is a discipline of screening new active substances and new genes by high-throughput sequencing.Therefore,using metagenomics not only can effectively detect the composition of microbial community of specific environment,but also greatly expanded the utilize the space of the microbial resources, developed of the next sequencing, enriched the content of bioinformatics. Based on the advantages of metagenomics,we summarized the major advances of metagenomics in human and animal pathogenic microorganisms in recent years,which might be used for the investigation and analysis of microbial pathogens of laboratory animal, animal epidemic monitoring and diagnosis of biological safety.
5.Effect of drinking water at different pH on intestinal microbial diversity in SPF mice evaluated by high throughput sequencing

Objective To study the effect of drinking water at different pH on intestinal microbial diversity in SPF mice.Methods Ninety 21-day-old SPF mice were randomly divided into three groups: drinking acidified water (pH 3.0), neutral water (pH 7.0) and alkaline water (pH 9.0), respectively, for 3 months, 30 mice in each group.10 mice were sacrificed at the end of 4, 8 and 12 weeks in each group.Bacteria, virus and parasites in serum were detected according to the national standard for SPF.Then, primer sequences were designed according to the conservativeness of bacteria 16S rRNA, and libraries were constructed.Subsequently, the ileocecal contents were subjected to high-throughput sequencing (Illumina).10 G data were obtained and analyzed.Results The sequencing results revealed that the intestinal microbial abundance of the mice receiving pH 3.0 drinking water was moderate with a high stability.Conclusions Under the conditions of large-scale breeding, the mice receiving pH 3.0 drinking water can maintain the intestinal microbial diversity for a long time, and reach a stable platform.This study provides theoretical and technical support for the stable long-term maintenance of animal quality production.It is also a specific application of gastrointestinal macrogenomics in animal quality control.
6.Short rib-thoracic dysplasia syndrome (type Ⅲ) caused by compound heterozygous mutation in DYNC2H1: a case report

We reported one fetus who was identified with significantly short humeri and femora,bulging abdomen and narrowed chest at 22+2 weeks' gestation,which was consistent with clinical findings at birth.Genetic analysis revealed that this was a case of short-rib thoracic dysplasia syndrome (type Ⅲ) caused by compound heterozygous mutation in DYNC2H1.We summarized the features of prenatal ultrasound imaging and results of postpartum genetic analysis of this case to provide information for prenatal ultrasound diagnosis and postpartum consultation.
7.Prenatal diagnosis of two Turner syndrome fetuses with 46,X,i(X)(q10)

Objective To summarize the prenatal diagnosis and genetic counseling of Turner syndrome fetuses with 46,X,i(X)(q10).Methods Two gravidas admitted to the Obstetrics and Gynecology Hospital of Dalian were enrolled in this study.One gravida,who was admitted in October 2016,was classified as high risk of Down syndrome based on prenatal serologic screening and systematic ultrasonography,which found remarkably shorter humeri and femora than fetus of the same gestations.The other was suggested to be monosomy X after non-invasive prenatal testing and admitted in November 2017.Fluorescence in situ hybridization (FISH) and karyotyping were performed for prenatal diagnosis.Peripheral blood karyotyping was also offered to the two women and their partners.Results FISH test for amniotic fluid did not find numerical abnormality in 13,18,21,and sex chromosomes in these two fetuses.Karyotype analysis showed that the two fetuses were both 46,X,i(X) (q10),while their parents were normal.Both cases were terminated after genetic counseling.Conclusions Prenatal serological screening,systematic ultrasonography and non-invasive prenatal testing may help to identify Turner syndrome fetus of 46,X,i(X) (q10).Timely and accurate prenatal diagnosis may prevent the affected fetus from being born.
8.Epidemiological characteristics of geriatric hip fracture in Beijing: a multicenter analysis of 2,071 cases

Objective:To explore the epidemiological characteristics of geriatric hip fractures in Beijing so as to provide evidence for effective prevention and control measures.Methods:This multicenter study was conducted in 3 urban (Beijing Jishuitan Hospital, Beijing Hospital and Beijing Anzhen Hospital) and 3 suburban hospitals (Beijing Shunyi District Hospital, Beijing Changping District Hospital and Beijing Liangxiang Hospital) in Beijing from November 2018 to November 2019. Eligible patients were those aged ≥ 65 years with hip fracture confirmed by X-ray and being admitted to hospital within 21 days of injury. To explore the epidemiological characteristics of geriatric hip fractures in Beijing, such data were collected as patients' age, gender, comorbidities, as well as type, site, time and cause of the fracture.Results:① A total of 2,071 patients were included in this suevey. They were 653 males and 1,418 females (M∶F=1∶2.17). Their age ranged from 65 to 102 years (average, 79.8 years). The patients aged from 75 to 84 years were the most common, accounting for 44.81% (928/2,071). ② Femoral neck fractures accounted for 43.41% (899/2,071), and intertrochanteric fractures accounted for 56.59% (1,172/2,071). The age of the patients with femoral neck fracture was (78.6±7.7) years, which was significantly younger than that of those with intertrochanteric fracture [(80.7±7.4) years] ( P<0.05). ③ 94.69% of the hip fractures (1,961/2,071) were caused by falling, and 71.27% fractures (1,476/2,071) happened at home. ④ Approximately 83.00% of the patients (1,719/2,071) had one or more comorbid conditions. Hypertension was the most prevalent disease (57.89%, 1,199/2,071), followed by diabetes (27.09%, 561/2,071), and coronary atherosclerotic heart disease (22.02%, 456/2,071). Conclusions:In Beijing, significantly more geriatric females sustain a hip fracture than males, and the proportion of those aged from 75 to 84 year is the largest. The proportion of intertrochanteric fractures increases with age. Falls are the leading cause for geriatric hip fractures. Most of the patients have one or more chronic comorbid conditions. Corresponding prevention and intervention measures should be formulated according to the distribution characteristics of elderly hip fractures in Beijing.
