1.Repair and functional reconstruction of common peroneal nerve injuries: A retrospective study of 18 cases

BACKGROUND: The repair and functional reconstruction of peripheral traumatic nerves depend, on posttraumatic operation time and operative techniques which have certain referential value.OBJECTIVE: To explore the effects of related factors on the repair and functional reconstruction of common peroneal nerve (CPN) after injuries.DESIGN: A case analysis.SETTING: Department of Orthopaedics, Fourth Hospital Affiliated to China Medical University.PARTICIPANTS: Eighteen patients diagnosed with simple CPN injury received treatment in the Department of Orthopedic Surgery of the Fourth Affiliated Hospital, Chinese Medical University, between January 1991 and January 2001. There were 15 males and 3 females aged 12-61 years old with the mean age of 26.5 years; there were 9 cases of complete transection, 3 cases of incomplete transection, 1 case of compression, 4 cases of contusion, and 1 case of irreversible injury. Posttraumatic treatment at hospital was obtained 0.5 hour to 6 months after injury.METHODS: All patients were subjected to CPN explorative operation;neuroanastomosis was performed on 12 cases, neurolysis on 2 cases, tendon graft on 1 case and conservative treatment on 3 cases. Antibiotics were administered after operation together with blood activating medicine and neurotrophic medicine. Moreover, the patients were required to do passive functional exercise in order to avoid arkylosis and myoatrophy.MAIN OUTCOME MEASURES: Functional recovery of CPN was assessed at postoperative 6 months based on the myodynamia of the same side anterior tibia muscles (excellent: above grade 4; good: grade 3; passable: grade 2; poor: grade 1 or grade 0).RESULTS: All the 18 patients entered the results analysis; their outcomes were assessed as excellent in 10 cases, good in 6 cases, and passable in 2cases, with the excellent rate being 89% (16/18).CONCLUSION: Patients diagnosed with CPN injury should be treated with primary repair of traumatic nerves as early as possible, which will benefit the functional recovery of limbs.
2.The Application of Spiral CT Three-Dimensional Reconstruction Imaging in Congenital Dislocation of Hip in Children

Objective To explore the value of the spiral CT three-dimensional reconstruction imaging in congenital dislocationof hip(CDH) in children.Methods A total of 45 patients(59 hips) with CDH were checked-up with spiral CT and three-dimensionalreconstruction imaging before operation.The femoral neck anteversion(FNA) and the acetabular index(AI) were measured and alsocompared with the seeing in operation.Results The patients' FNA were all greater than normal,with the greatest was 90?and the smaller was 40?,the average was 42.72??12.06?.Their AI were also greater,with the average of 35.4??9.5?,the acetabulum were hypogenesis with different degreee.Conclusion The 3D CT of hip joint provides precise information of the imaging compare with the X-ray.It is a very useful technique in the FNA and AI measuring and it is also useful for the diagnosis and treatment planning of CDH.

4.Effects of Fuzheng Huayu Capsule on Expressions of Rock1 and Rock2 in Rats with Myocardial ;Infarction

Objective To observe the effects of Fuzheng Huayu Capsule on the expressions of the main roles, Rock1 and Rock2, in RhoA/Rock signal transduction pathway in rats with myocardial infarction (MI);To explore the possible mechanism of Fuzheng Huayu Capsule to improve ventricular remodeling in myocardial fibrosis. Methods The MI model was induced by ligation of the left coronary artery. Rats with MI were randomly divided into the model group, Fuzheng Huayu group and Fasudil hydrochloride group. A sham-operation group threading without ligation was setting as a control group, with eight rats in each group. The rats were treated with corresponding medicine for 4 weeks from the second day after modeling. The expression of Rock1, Rock2 and its mRNA were detected by immunohistochemical and real-time PCR method. Results The protein expression of Rock1 and Rock2 in model group were significantly higher than those in the sham-operation group (P<0.05). The protein expression of Rock1 and Rock2 in the Fuzheng Huayu group and Fasudil hydrochloride group were lower than those in the model group. The mRNA expression of Rock2 was significantly higher in the model group than that in the sham-operation group (P<0.05). The mRNA expression of Rock1 in the Fasudil hydrochloride group was lower than that in the model group (P<0.05). The mRNA expression of Rock2 in the Fuzheng Huayu group and Fasudil hydrochloride group was lower than that in the model group (P<0.05). Conclusion Fuzheng Huayu Capsule can decrease the expression of Rock1, Rock2 and Rock2 in the marginal zone of myocardial infarction in rats with MI. The anti ventricular remodeling mechanism of Fuzheng Huayu Capsule maybe related with this.
5.Discussion on Ethical Censorship of New Medical Technology

Since the Beijing health and family planning commission promulgated the new medical technology management system,we have accepted and reviewed 14 key medical technologies in Beijing,such as resection of the skull base tumor (intracranial tumor outside communication),resection of intracranial important functional areas and large vascular malformation,renal vascular reconstruction technique,the technique of artificial joint replacement,coronary interventional diagnosis and treatment technology,etc.Through the existing laws and regulations requirements from medical new technology,the ethical review way,submit materials requirements and ethical review focus is analyzed,and combining with the experiences of our hospital in the medical technical review,aims to explore suitable for medical technology ethical review specification.
6. The three-dimensional digitalized muscle anatomy research of one stillbirth with cleft palate based on Micro-CT

Objective:
The cadaveric specimens stained with iodine-potassium iodide can show the fine structure of muscles by Micro-CT scanning. A specimen with cleft palate was scanned using this technique to explore the relationship between the muscle fibers of pharyngeal muscles in order to guide the muscle reconstruction in cleft palate repair, so as to understand the lingual pathology secondary to cleft palate.
Methods:
One unclaimed 28-week aborted stillbirth, diagnosed as Ⅱ-degree cleft palate, was dissected.The anatomic structure between orbital floor plane and mouth bottom plane was saved, and soaked in 4% for maldehyde for 8 hours. Thereafter, the subject was soaked in 3.75% iodine-potassium iodide for 7 days for staining. The stained specimens were scanned with Micro-CT. The muscle fibers were drawn in the exported DICOM images, for three-dimensional muscle reconstruction.
Results:
The main muscles of velopharynx were located in the tomographic image and the three-dimensional reconstructed model, including: the levator muscle of palatine velum, the tensor velipalatini, the palatopharyngeus muscle, the palatoglossus muscle, and the upper part of the superior pharyngeal constrictor. Some local anatomical features of the muscles were also found: (1)The tensor velipalatini has a branch ended at the lateral pharyngeal wall.(2)The superior pharyngeal constrictor has multiple connections with the levator muscle of palatine velum, the tensor velipalatini, the palatopharyngeus muscle.(3)There was no obvious muscle fiber crossing between the internal muscles of the cleft palate. The muscle of uvula was not found either.
Conclusions
With the application of high-resolution Micro-CT technology, new branches of pharyngeal muscles and crossings of muscles can be recognized. In the cleft palate specimen, the main functional muscles of pharyngeal are mainly associated with superior pharyngeal constrictor. The muscles in the palate and the pharynx are interrelated and form a three-dimensional " velopharyngeal muscle complex" .
