2.Risk and Management in Clinical Drug Trial

OBJECTIVE: To strengthen the risk management in clinical drug trial so as to minimize the risk.METHODS: Based on modern risk management theory as well as the practical condition of the current clinical drug trial,an aggregate analysis was done and some opinions and suggestions were put forward.RESULTS: Under current model,it is objective and inevitable for the risks involved in clinical trial,but which could be reduced or avoided by taking some effective measures such as risk predication,standardization of clinical trial,ADR monitoring etc.CONCLUSION: There is risk in the drug clinical trial and it is necessary to conduct risk management.
3.Pharmacoeconomics Evaluation of Insulin Degludec in the Treatment of Type 1 and Type 2 Diabetes Mellitus

OBJECTIVE:To systematically review the cost-effectiveness of insulin degludec(IDeg)in the treatment of type 1 diabetes mellitus(T1DM)and type 2 diabetes mellitus(T2DM). METHODS:A systematic review of literatures was conducted via PubMed,EMBase,The Cochrane Library,CNKI,Wanfang Data,VIP,IDF,ISPOR,ADA and EASD from the inception to Janu-ary 2016,to identify pharmacoeconomics evaluation literatures and non-economic studies related to IDeg in the treatment of T1DM and T2DM,compared with other basic insulin [insulin glargine(IGlar),insulin detemir(IDet),neutral protamine hagedom insulin (NPH)]. RESULTS:A total of 8 studies were included and all of them were carried out in European countries. In the short-term (one year),IDeg was more economical when compared with other basic insulin in patients with T1DM. The studies related to the long-term treatment of T1DM demonstrated different conclusions,but most of studies came to a conclusion that IDeg had good cost-effectiveness. For patients with T2DM,all of the studies demonstrated that IDeg was cost-effectiveness compared with IGlar. CONCLUSIONS:Compared with other basic insulin,IDeg can improve therapeutic efficacy and the quality of life,as well as re-duce the cost of ADR as hypoglycaemia. IDeg is a dominant or cost-effective treatment opinion.
4.Inhibitory effect and mechanism of Dsmosdumotin-C B ring derivative on MCF-7 cell proliferation

Objective To study the inhibitory effects of TPP,a desmosdumotin-C B ring para-fluoro modified derivative,on human breast cancer MCF-7 cell proliferation,and investigate the possible mechanisms.Methods MTT assay was used to measure the proliferation suppression of MCF-7 cells after treated with 1.0,2.5,5.0,10.0,and 20.0 μg/mL TPP for 48 h,and then the cell apoptosis rate and expression rate of NF-κB P65 positive cells were tested by flow cytometry after 20.0 μg/mL TPP treatment for 0,24,and 48 h.Results MTT assay showed that,after treatment for 48 h,1.0,2.5,5.0,10.0,and 20.0 μg/mL TPP all exhibited the inhibitory effects and showed a dose-dependent relationship.Flow cytometry results showed that 20.0 μg/mL TPP induced cell apoptosis after treatment for 24 and 48 h.TPP (20.0 μg/mL) significantly reduced the rate ofNF-κB P65-positive cells in MCF-7 cells after treatment for 48 h.Conclusion TPP could inhibit the proliferation of MCF-7 cells,which may be induced by cell apoptosis.Down-regulation of NF-κB is possible to be related with apoptosis.
5.Using conidia of Pyricularia oryzae screening for bioactive portions from marine organisms

Objective:To screen for the bioactive marine organisms and to study the active constituents from them.Methods:Bioactivities of marine organisms extracts were screened by Pyricularia oryzae bioassay method and the constituents were isolated and identified by a combination of multi chromatography and spectral analysis.Results:Twenty ethanolic extracts and 5 aqueous extracts of the tested marine organisms showed activities causing morphological abnormality of P.oryzae mycelia.Fifty six compounds were obtained and identified from Sargassum carpophyllum,Ishige okamurai and Natarchus leachii freeri ,among which 28 compounds showed activities against P.oryzae and exhibited cytotoxicities on various cultured tumor cell lines,eight compounds showed inhibition effects on Fonsecaea pedrosoi in vitro .Conclusion:The screening data and bioactivity evaluation suggest the P.oryzae bioassay is suitable for preliminary screening of bioactive agents from marine organisms.

Research progress of flavanone compounds synthesis was reviewed ,various types of synthesis methods of fla‐vanone compounds were summarized ,and methodological support for flavanone compounds in new drug research and develop‐ment was provided .Research progress in synthesis of flavanone compounds includes chalcone cyclization ,Friedel‐Crafts reac‐tion ,Knoevenagel condensation ,Hoesch single acetoxylation ,and asymmetric synthesis of flavanone compounds .Solvent‐free synthesis of flavanone compounds and other green chemistry methods were also introduced .
7.Investigation of Risk Factors of Stroke in Community Residents Aged 40 years and Above

To understand and master the distribution of risk factors of stroke in community residents aged 40 and above in Xiaogan city, to provide reference basis for the targeted development of stroke prevention and control. Methods Adopting the method of cluster sampling, 1780 community residents aged 40 or higher, using the unified " stroke risk groups and intervention project risk assessment chart" issued by the national health commission, a questionnaire survey was conducted to meet eligibility criteria, physical testing and laboratory examination. Results Of the 1 274 people who were assessed as high risk of stroke, 595 men, 679 women and 8 risk factors were among the top three of the patients with high blood pressure, obesity and blood lipids, 81. 5 percent, 61. 1 percent and 50. 2 percent respectively. The risk factors for ages 50~59 and 60~69 were more significant than those of other age groups. Conclusion The risk factors of high-risk groups were at a high level, and the distribution of risk factors was statistically significant (P<0. 05). We should aim to make healthy education for high-risk groups in the community, actively control risk factors and reduce the incidence of stroke.

Autophagy is a conservative lysosome-dependent material catabolic pathway, and exists in all eukaryotic cells. Autophagy controls cell quality and survival by eliminating intracellular dysfunction substances, and plays an important role in various pathophysiology processes. Erectile dysfunction (ED) is a common male disease. It is resulted from a variety of causes and pathologies, such as diabetes, hypertension, hyperlipidemia, aging, spinal cord injury, or cavernous nerve injury caused by radical prostatectomy, and others. In the past decade, autophagy has begun to be investigated in ED. Subsequently, an increasing number of studies have revealed the regulation of autophagy contributes to the recovery of ED, and which is mainly involved in improving endothelial function, smooth muscle cell apoptosis, penile fibrosis, and corpus cavernosum nerve injury. Therefore, in this review, we aim to summarize the possible role of autophagy in ED from a cellular perspective, and we look forward to providing a new idea for the pathogenesis investigation and clinical treatment of ED in the future.
