1.THE DETECTION OF PROTEINURIA IN DIABETIC NEPHROPATHY AND ITS PATHOGENESIS

Sixty diabetic patients and twenty normal controls were studied. The results showed that: 1) Counter-immuno-electrophoresis is a sensitive and simple test suitable for the detection and monitoring of diabetic microproteinuria; 2) The proteinuria in diabetic nephropathy is glomerular in origin which is supported by the finding of a marked increase in urinary albumin excretion and a normal level of urinary ?2-microglobulin; 3) The serum ?2-microglobulin appears to be a more sensitive index than serum creatinine and creatinine clearance rate for the detection of impairment of renal function in diabetic patients; 4) Diabetic nephropathy is related to glycemic control and duration of diabetes mellitus.
2.Changes of bone mineral density of the proximal femur in patients with revision total hip arthroplasty

[Objective]To understand the changes of periprosthetic bone mineral density(BMD) of the proximal femur in patients with failed hip arthroplasty by measuring with dual energy X-ray absorptiometry(DEXA). [Method]Periprosthetic BMD was determined in 9 patients with revision total hip arthroplasty by dual energy X-ray absorptiometry(DEXA),defining regions of interest according to Gruen(1~7),and comparing the operation side with the nonoperation side.Clinic results of the cases were evaluated according to Harris Hip Score(HHS).[Result]The mean HHS was 61 points in 9 patients at this follow-up examination.There was a significant decrease(8.9%~27.6 %) in BMD in every Gruen zone as compared with the nonoperation side.[Conclusion]The bone loss in the proximal femur in patients with failed hip arthroplasty is significant,and the pattern of bone loss around failed hip arthroplasty may be different from the typical remodeling seen after successful hip arthroplasty.
3.The relationship between microalbuminuria and atherosclerotic vascular disease in type 2 diabetes

ObjectiveTo investigate the associations between microalbuminuria and early atherosclerosis in type 2 diabetes.MethodsWe measured the combined intima-media thickness (IMT) of the common carotid arteries and the inner diameter (ID) of tibia artery with high resolution ultrasound in 30 type 2 diabetic patients,and compared with 13 age-matched non-diabetics as control.The diabetic patients were subgrouped into DM-A subjects whose urinary albumin excretion were normal,and DM-B subjects who were with microalbuminuria,and DM-C subjects who with macroalbuminuria.ResultsThe results showed that the IMT values were greater in diabetic patients than in control subjects(0.65±0.12 mm vs 0.48±0.06mm,P<0.001),and greater in group of DM-B and DM-C than in group of DM-A.The ID of tibia artery were smaller in diabetic subjects than in controls (1.98±0.49mm vs 2.45±0.30mm,P<0.01),and smaller in DM-B and DM-C group than DM-A.Besides,multiple linear regression analysis demonstrated that the risk factors for IMT and ID of tibia artery of type 2 diabetes were diabetic duration,total cholesterol levels,blood pressure and urinary albumin excretion.ConclusionIt can be concluded that microalbuminuria is related to atherosclerosis at early stage of the disease process.
4.Relationship of nocturnal hypoglycemia to fasting and postprandial glycemia in diabetic patients

Observations were carried out in 10 moderately well controlled diabetic patients on their usual therapeutic regimens from 10Pm to 10Am on two days.On the control day,samples were obtained without intervention.On another day,nocturnal hypoglycemia was induced.Nocturnal hypoglycemia did not appear to cause clinically important fasting and postprandial hyperglycemia in diabetic patients.It is sug- gested that the morning hyperglycemia is not caused by Somogyi effect but dawn phenomenom.

6.Comparison of postoperative parameters between Chinese-made Z-type stainless steel stent and Ni-Ti alloy mesh stent in treating 79 patients with advanced esophageal cancer

0.05). However, incidences of VRS paining grade, nausea and vomiting in Z-type stainless steel stent group were significantly higher than Ni-Ti alloy mesh stent group (P 0.05). All patients in both groups did not have general toxicity and hypersensitivity reaction, and they had normal cell component, number and form of hematopoieticsystem. CONCLUSION: Both Z-type stainless steel stent and Ni-Ti alloy mesh stent are suitable for alleviative treatment of advanced esophageal cancer; however, side effects induced by Ni-Ti alloy mesh stent is less than by Z-type stainless steel stent in an early postoperative stage.
8.Effect of wheat-grain moxibustion on the expression of 5-HT and cortisol in the serum, and MR and GR in the hippocampus in rats with hypothyroidism complicated with depression.

OBJECTIVE:
To observe the effect of wheat-grain moxibustion on behavior, 5-hydroxytryptamine (5-HT) and cortisol in the serum, mineralocorticoid receptor (MR) and glucocorticoid receptor (GR) in the hippocampus in rats with hypothyroidism complicated with depression, and to explore the possible mechanism of wheat-grain moxibustion on improving depression in rats with hypothyroidism.
METHODS:
A total of 32 SPF SD rats were randomly divided into a blank group, a model group, a medication group and a wheat-grain moxibustion group, 8 rats in each group. Except for the blank group, the rats in the remaining groups were treated with intragastric administration of 0.1% propylthiouracil (PTU) suspension at 1 mL/100 g, once a day for 4 weeks to establish the rat model of hypothyroidism, and whether the rats were accompanied with depression-like behavior determined through behavioristics evaluation. The rats in the medication group were intervened with euthyrox at 0.9 mL/100 g, once a day, for 4 weeks; the rats in the wheat-grain moxibustion group were treated with wheat-grain moxibustion at "Dazhui" (GV 14), "Mingmen" (GV 4), "Shenshu" (BL 23) and "Pishu" (BL 20), 7 cones each acupoint, once a day, six times a week for 4 weeks. After the intervention, the depression status was observed by behavioristics test; the contents of thyroid stimulating hormone (TSH), total thyroxine (TT4), 5-HT and cortisol in the serum were detected by ELISA; the protein expressions of MR and GR in hippocampus were detected by Western blot; the expressions of MR mRNA and GR mRNA in the hippocampus were detected by real-time PCR.
RESULTS:
Before the intervention, compared with the blank group, the scores of open field test (OFT) were decreased and the immobility time of tail suspension test (TST) was prolonged (P<0.05); the serum TSH contents were increased and TT4 contents were decreased (P<0.01) in the other three groups. After the intervention, compared with the model group, the vertical score of OFT was increased and the immobility time of forced swimming test (FST) was prolonged in the medication group (P<0.05), while the scores of three items of OFT were increased (P<0.05, P<0.01), and the immobility time of FST and TST was shortened in the wheat-grain moxibustion group (P<0.01, P<0.05). Compared with the medication group, the immobility time of TST and FST in the wheat-grain moxibustion group was shorter (P<0.05, P<0.01). Compared with the blank group, in the model group, the contents of serum TSH and cortisol were increased (P<0.01, P<0.001), while the contents of serum TT4 and 5-HT were decreased (P<0.01, P<0.001). Compared with the model group, the contents of serum TT4 and 5-HT were increased, while the contents of serum TSH and cortisol were decreased in the medication group and wheat-grain moxibustion group (P<0.01, P<0.05). Compared with the blank group, the protein and mRNA expression of MR, GR in the hippocampus in the model group was decreased (P<0.01, P<0.05, P<0.001); compared with the model group, the protein and mRNA expression of MR in the hippocampus in the medication group were increased (P<0.05), and the protein expression of MR, GR and mRNA expression of MR in the hippocampus in the wheat-grain moxibustion group were increased (P<0.05, P<0.01). Compared with the medication group, the expression of MR mRNA in the wheat-grain moxibustion group was increased (P<0.05).
CONCLUSION
Wheat-grain moxibustion could significantly improve thyroid function and depression in rats with hypothyroidism. Its mechanism may be related to up-regulating the protein and mRNA expression of MR and GR in the hippocampus, and then affecting the expression of serum cortisol and 5-HT.

9.Effect of acupuncture at "Zhibian" (BL 54) through "Shuidao" (ST 28) on the expression of apoptosis-related factors in rats with premature ovarian insufficiency based on oxidative stress.

OBJECTIVE:
To explore the possible mechanism of acupuncture at "Zhibian" (BL 54) through "Shuidao" (ST 28) on premature ovarian insufficiency (POI) from the perspective of oxidative stress.
METHODS:
Sixty female SD rats were randomly divided into a blank group, a model group, a sham acupuncture group, a medication group, and an acupuncture group, 12 rats in each group. Except the blank group, the rats in the remaining groups were intraperitoneally injected with cyclophosphamide to establish the POI model. After the model was successfully established, the rats in the acupuncture group were treated with acupuncture at "Zhibian" (BL 54) through "Shuidao" (ST 28), with a depth of about 12 mm, and the needle was retained for 30 min; the acupuncture was given once a day, for a total of 4 weeks. The rats in the sham acupuncture group were treated with blunt-head needle to tap the skin surface of "Zhibian" (BL 54), without penetrating the skin, once a day for 4 weeks. The rats in the medication group were treated with estradiol valerate by gastric gavage for 4 weeks. After the intervention, the level of reactive oxygen species (ROS) in the ovarian tissue was detected by fluorescence probe; the expression of c-Jun N-terminal kinase (JNK), forkhead box O1 (FoxO1), tumor suppressor gene protein 53 (p53) and p53 up-regulated modulator of apoptosis (Puma) mRNA and protein in ovarian tissue were detected by real-time fluorescence quantitative PCR and Western blot.
RESULTS:
Compared with the blank group, the level of ROS and the expression of JNK mRNA, p-JNK protein, FoxO1, p53, Puma mRNA and protein in the ovarian tissue in the model group were increased (P<0.01). Compared with the model group, the level of ROS and the expression of p-JNK protein, FoxO1, p53, Puma mRNA and protein in the ovarian tissue in the sham acupuncture group were slightly reduced, but the difference was not statistically significant (P>0.05). The level of ROS and the expression of JNK mRNA, p-JNK protein, FoxO1, p53, Puma mRNA and protein in the ovarian tissue in the acupuncture group and the medication group were reduced (P<0.01).
CONCLUSION
Acupuncture at "Zhibian" (BL 54) through "Shuidao" (ST 28) could improve the level of oxidative stress, down-regulate the expression of apoptosis-related factors JNK, FoxO1, p53 and Puma induced by oxidative stress, and inhibit the premature failure of ovarian reserve function caused by apoptosis of ovarian granulosa cells in POI rats.

10.Analysis on project funding from the National Natural Science Foundation of China in geriatric medicine

Objective:To review the development and progress of geriatric research in China from the perspective of funding from the National Natural Science Foundation of China, and to provide supporting data for further development of the field.Methods:Based on project funding data of the National Natural Science Foundation of China for geriatric medicine from 2008 to 2017, the number and composition of projects, funding amount and funded institutions were statistically analyzed, and the overall trend of change was examined.The research hotspots in recent years were summarized by using word frequency analysis.Results:From 2008 to 2017, a total of 446 projects were approved in the field of geriatric medicine.Both the number of projects and the amount of funding showed an increasing trend.Project types were gradually expanded and the composition was constantly optimized.Since 2014, the fund types each year included 6 or more and breakthroughs in major projects were achieved.East China remained the leading region in the number of projects and amount of funding, with Beijing, Shanghai and Zhejiang receiving the highest numbers of project approvals and accounting for more than 10% of the national total in the number of project approvals and the amount of funding.During this period, 114 institutions received funding, with 74 winning 2 or more projects.In word frequency analysis, senescence(229 times), cell(161 times)and protein(120 times)were the three most frequently used keywords.Conclusions:In the field of geriatric medicine, the project approval level from the National Natural Science Foundation of China has significantly improved in quantity and quality, but it has always been under-represented in the branch of medical sciences.The regional and institutional distribution of projects is unbalanced and tends to skew in favor of institutions with prominent academic disciplines.Basic research is the main type of funded projects.Geriatric research is centered on the mechanisms of aging.
