1.Factors influencing the quality of life of spouses of spinal cord injury patients

Objective To explore quality of life (QOL) of spouses of spinal cord injury patients,as well as its influencing factors.Methods Sixty-eight spouses of spinal cord injury patients were recruited as an observation group.At the same time,another 70 healthy married people who had no serious chronic diseases in the family but were matched in gender,age,occupation and education were chosen as a control group.The QOL of all the subjects in both groups was investigated by way of a questionnaire survey.The results of both groups were compared and factors that influenced their QOL were explored.Results The QOL of spouses of spinal cord injury patients,including total QOL ( 3.22 ± 0.37 ),physiological function ( 60.57 ± 14.22 ),psychological state ( 60.09 ± 8.06 ),social relations (63.37 ± 8.10 ) and the living environment (66.86 ± 11.26 ) were significantly lower than in the control group (4.62 ± 0.75 ; 82.48 ± 10.92 ; 86.02 ± 8.75 ; 85.94 ± 10.13 and 83.23 ± 9.54,respectively).The QOL of spouses of spinal cord injury patients was closely related to the patient's injury type,injury position,the treatment protocol,as well as their own gender,vocation and education.Conclusion The QOL of spouses of spinal cord injury patients is significantly lower than that of normal people,and closely related with the patients' injury type,injury position and treatment,as well as their own gender,vocation and education.
2.Effect of bushenyangxue prescription on the levels of anti-mullerian hormone in rat with premature ovarian failure

Objective Establish premature ovarian failure ( POF ) model in female Sprague Dawley by tripterygium wilfordii , and investigate the effect of bushenyangxue prescription on the levels of anti -mullerian hormone ( AMH) .Methods After POF model was established , rats were gave by gavage of different dosage of Bushenyangxue prescription for 30 days.The changes of histomorphology on rat ovarian tissue were observed by hematoxylin -eosin staining. Serum AMH concentraction , protein and mRNA expression of AMH were measure with ELISA , immunohistochemical staining and qRT-PCR, respectively .Results The follicle and corpus luteum were atrophied after tripterygium wilfordii challenge, which was improved after treatment with Bushenyangxue prescription .Serum AMH, protein and mRNA expression of AMH were decreased tripterygium wilfordii-treated rats; this decrease was inhibited after treatment with Bushenyangxue prescription .Conclusions Our study indicates that Bushenyangxue prescription could preserve the AMH levels of POF rats.These findings suggest that Bushenyangxue prescription may be a useful strategy to treat POF .
3.The efficacy of motor imagery therapy in treating neurogenic bladder control difficulties after spinal cord injury

Objective To explore the clinical efficacy of motor imagery therapy in treating neurogenic bladder dysfunction after traumatic spinal cord injury (SCI).Methods Seventy patients with neurogenic bladder control problems after SCI were randomly divided into an experimental group and a control group using a random number table.All patients of the two groups were given general bladder function intervention,including intermittent catheterization,inducing voiding by reflex detrusor contraction,Credé's maneuver urination,etc.Additionally,the patients in the experimental group were given supplemental motor imagery therapy.The times of urinary incontinence,average bladder capacity,maximum voided volume and residual urine volume of the two groups were measured before treatment and at 2 months after treatment.The two groups' outcomes were quantified using a quality of life (QOL) score.Results Incidents of urinary incontinence,average bladder capacity,residual urine volume,voided volume and the QOL score showed significant improvements in both groups,but the experimental group showed better improvements than the control group.The differences were statistically significant.Conclusion The combination of general bladder function intervention with motor imagery therapy can improve the voiding function of patients with neurogenic bladder disorders after SCI more significantly and enhance their QOL.
4.INFLUENCE OF ARTESUNATE ON INFECTIVITY OF GAMETOCYTES OF PLASMODIUM FALCIPARUM

A study was carried out in south of Vietnam 15 Patients with Plasmodium falciparum gametocytes and asexual parasites were distributed into groups A, B and C. Artesunate was given orally at a total dose of 600 mg for 5 days in group A, 200 mg for 2 days in group B and intravenously at a total dose of 360 mg for 5 days in group C respectively. Gametocytes count was done before medication and daily after medication. Meanwhile, Anopheles dirus as a vector was employed to study the infectivity of gametocyte, The result showed that the mean gametocyte clearance time in three groups were respectively 15.4?5.0, 20.6?4.8 and 20.3?4.0 days. The mosquitoes were not infected from the blood in 2, 5 and 5 of 5 patients respectively on days 7, 14 and 21 in group A; 1 and 5 of 5 patients on day 14 and 21 in group B; 2 of 5 patients and 3 of 3 patients on days 14 and 21 in group C. It indicates that artesunate has remarkable effect on Plasmodium falciparum gametocytemia and its infectivity to mosquitoes.
5.Influence of Clinical Pharmacy Service on Community Hypertension and Coronary Heart Disease Patients with Chronic Diseases

Objective: To evaluate the effects of clinical pharmacists on the community hypertension and coronary heart disease management.Methods: The clinical pharmacy service management program for community chronic disease patients was designed, and the medication files of the patients with hypertension and coronary heart disease were established, and then the medication safety cognition and chronic disease medication of the patients were investigated.Pharmaceutical interventions were given twice for the irrational drug use and living habits and the views of pharmacy were put forward.The improvement of medication compliance, pharmaceutical opinion acceptance and blood pressure etc was evaluated, and then the drug cost and the cost of health care products were analyzed and evaluated.Results: The medication and hypertension compliance increased, the patients were satisfied with the clinical pharmacy service, and the acceptance rate of pharmaceutical intervention opinions was improved significantly.The total cost of medication was without significant change, and the cost of health care products decreased significantly.Conclusion: It is an effective way to expand the impact of clinical pharmacy performed by clinical pharmacists on chronic disease medication management in communities, which can improve the rationality and safety of drug use in the patients with chronic diseases and has positive effects in promoting the management of community chronic diseases.
6.Effects of Bazhen Decoction combined with Baizhu Fuzi Decoction on glucose metabolism and estrogen levels in rats with polycystic ovary syndrome

Objective To study the regulation effects of Bazhen Decoction combined with Baizhu Fuzi Decoction on glucose metabolism and estrogen level in rats with polycystic ovary syndrome.Methods The rat model of polycystic ovary syndrome was established by subcutaneous injection of human chorionic gonadotropin(HCG)combined with insulin.Rats were randomly divided into the polycystic ovary syndrome group,the Baizhu Fuzi Tang group(6.4 g/kg),the Bazhen Tang group(9.2 g/kg),the Bazhen Tang combined with Baizhu Fuzi Tang group(11.55 g/kg)and the diethylstilbestrol group(0.5 mg/kg).A blank control group(unmodulated rats)was set up with 10 rats in each group.The uterine index of rats was determined and calculated.Fasting blood glucose(FBG),2 h postprandish blood glucose(2 h PBG)and fasting insulin(FINS)were determined and insulin sensitivity index(ISI)and insulin resistance index(IR)were calculated.Serum estradiol(E2),testosterone(T),luteinizing hormone(LH),follicle stimulating hormone(FSH),prolactin(PRL),anti-Mullerian tube hormone(AMH)and insulin-like growth factor 1(IGF-1),C-reactive protein(CRP),interleukin(IL)-6,tumor necrosis factor(TNF)-α,total peroxidase activity(T-AOC),superoxide dismutase(SOD)and malondialdehyde(MDA)levels in ovarian tissue were detected by enzyme-linked immunosorbent assay(ELISA).The pathological changes of ovarian tissue were detected by hematoxylin-eosin(HE)staining.Real-time quantitative polymerase chain reaction(qPCR)was used to determine mRNA levels of ARA70,CBP,SCR1 and HOXA10 in endometrium and mRNA expressions of AMPK,GLUT4 and PPARγ in ovarian tissue.Western blot assay was used to determine expression levels of AMPK,GLUT4 and PPARγ in ovarian tissue.Results Compared with the polycystic ovary syndrome group,uterine index,FINS,FBG,2 h PBG,IR levels,serum T,LH,FSH,PRL,AMH,IGF-1 levels,ovarian tissue CRP,IL-6,TNF-α,T-AOC and MDA level,endometrial ARA70,CBP,SCR1 mRNA level of rats decreased significantly in the Baizhu Fuzi decoction group,the Bazhen decoction group and the Bazhen decoction combined with Baizhu Fuzi decoction group(P<0.05).ISI level,serum E2 level,ovarian tissue SOD level,endometrial HOXA10 mRNA level,ovarian tissue AMPK,GLUT4 and PPARγ mRNA and protein levels were significantly increased(P<0.05).The above indexes were significantly changed in the Bazhen decoction and Bazhu Fuzi decoction group than those of the Bazhu Fuzi decoction group and the Bazhen decoction group(P<0.05).Conclusion Bazhen Decoction combined with Baizhu Fuzi Decoction can regulate glucose metabolism,inhibit ovarian tissue oxidation and inflammatory damage,improve endometrium tolerance,regulate estrogen level,and improve the progression of polycystic ovary syndrome in rats.The mechanism may be related to the regulation of AMPK/GLUT4/PPARγ pathway.
7.Effect of pulmonary function training instrument combined with resistance abdominal breathing training on respiratory function of patients with cervical spinal cord injury

Objective To observe the effect of pulmonary function training instrument combined with resistance abdominal breathing training on respiratory function recovery of patients with cervical spinal cord injury.Methods Sixty cases of patients with cervical spinal cord injury were randomly divided into the research group(n=30)and the control group(n=30).The control group adopted sputum drainage nursing , abdominal breathing training and the comprehensive rehabilitation treatment such as limbs functional rehabilitation training , while the research group used pulmonary function training instrument combined with resistance abdominal breathing training in addition for eight weeks .Forced vital capacity ( FVC), forced expiratory volume in one second(FEV1.0), maximal voluntary ventilation ( MVV) and the detected rate of pulmonary pathogens were observed and compared.Results The FVC, FEV1.0, MVV was respectively (68.6 ±11.3)%, (75.8 ± 9.3)%and ( 67.4 ±11.6 )% in the research group after eight weeks , and ( 60.1 ±14.9 )%, ( 64.3 ± 13.2)%and (57.8 ±13.5)%in the control group, with statistically significant differences (t=2.78,2.52, 2.61, respectively;P<0.05).The rate of pulmonary pathogens was 6.67%(2/30) in the research group and 30.00%(9/30) in the control group, and the difference was statistically significant (χ2 =21.23,P<0.05). Conclusions Pulmonary function training instrument combined with resistance abdominal breathing training can improve respiratory function recovery and reduce infection rate in patients with cervical spinal cord injury .
8.Effects of early enteral nutrition combined with probiotics on intestinal flora and immune function in patients with severe ischemic stroke

Objective To explore the effects of early enteral nutrition (EN) combined with probiotics on intestinal flora and immune function in patients with severe ischemic stroke. Methods Sixty-nine severe ischemic stroke patients were admitted and continuously enrolled in Taizhou First People's Hospital from June 2017 to June 2018, and they were randomly divided into an EN combined with probiotics group (35 cases) and a simple EN group (34 cases). Early EN support was given to both groups and probiotics (Live Combined Bifidobacterium, Lactobacillus and Enterococcus capsules) was added to the EN combined with probiotics group, 0.42 g each time, 3 times a day for 14 days. The changes of serum inflammatory markers [hypersensitive C-reactive protein (hs-CRP), procalcitonin (PCT), tumor necrosis factor-α (TNF-α), interleukin-10 (IL-10)], intestinal flora (Bifidobacterium, Lactobacillus, Clostridium, Enterobacter, Enterococcus, Bacteroides), intestinal mucosal barrier [endotoxin, D-lactic acid, diamine oxidase (DAO), intestinal fatty acid binding protein (I-FABP) ], and immune indexes [immunoglobulins (IgA, IgG, IgM), human leukocyte DR antigen (HLA-DR)] were observed in two groups of patients after treatment. Results With the prolongation of time, Bifidobacteria, Lactobacilli, HLA-DR and IgA, IgG, IgM after EN in both groups all decreased first and then had a tendency of increase, all reaching the lowest value on the EN 3rd day and then gradually elevated arriving at the peak value on the EN 14th day, and the levels in EN combined with probiotics group were significantly higher than those in the simple EN group [Bifidobacterium (×107 cfu/g): 8.31±1.49 vs. 7.49±1.32, Lactobacillus (×107 cfu/g): 8.04±1.45 vs. 7.19 ±1.37, HLA-DR: (67.22±9.11)% vs. (61.21±9.69)%, IgA (mg/L): 170.34±40.13 vs. 149.54±38.76, IgG (g/L):4.88±0.88 vs. 4.31±0.86, IgM (mg/L): 879.47±100.82 vs. 821.52±97.75, all P < 0.05]. With the prolongation of time, the Clostridium, Enterobacter, Enterococcus, Bacteroides, hs-CRP, PCT, TNF-α, endotoxin, D-lactic acid, DAO, I-FABP after En in both groups all increased first and then had a tendency of decrease, reaching the highest level on the EN 3rd day, then gradually decreased arriving at the valley value on the EN 14th day, and the levels in the EN combined with probiotics group were significantly lower than those in the simple EN group [Clostridium (×107 cfu/g): 5.23±0.87 vs. 5.79±0.91, Enterobacter (×107 cfu/g): 7.45±1.21 vs. 8.62±1.32, Enterococcus (×107 cfu/g): 7.32±1.05 vs. 8.12±1.23, Bacteroides (×107 cfu/g): 9.16±1.35 vs. 9.87±1.42, hs-CRP (mg/L): 18.45±12.98 vs. 25.47±15.55, PCT (ng/L): 3.24±1.21 vs. 4.18±1.32, TNF-α (ng/L): 9.43±8.69 vs. 13.59±9.45, IL-10 (μg/L): 39.45±10.72 vs. 48.52±11.42, endotoxin (U/L): 6.74±2.12 vs. 9.21±3.28, D-lactic acid (mg/L): 98.74±20.74 vs. 114.78±19.89, DAO (mg/L): 21.45±8.49 vs. 29.47±9.41, I-FABP (ng/L): 1.4±0.2 vs. 1.5±0.2, all P < 0.05]. Conclusion Early EN combined with probiotics can effectively regulate the intestinal flora and intestinal mucosal barrier function, reduce the level of inflammatory response and enhance the body immunity in patients with severe ischemic stroke.
9.Influencing factors analysis on using active imagery therapy in spinal cord injury patients with neurogenic bladder

Objective To investigate the influencing factors of using active imagery therapy in spinal cord injury patients with neurogenic bladder. Methods Ninety-two spinal cord injury patients were arranged for bladder function training by active imagery therapy. According to the training effect, they were divided into two groups, and a total of eleven factors were observed including age, education level, time of rehabilitation intervention, the type of spinal cord injury and etc. The logistic regression was used to analyze the possible influence factors of the two groups. Results The analysis results indicated that there were significant differences in education level, depression state, the type of neurogenic bladder, rehabilitation time and course of treatment which were affect the bladder training (χ2 =5. 851, 6. 903, 6. 901, 30. 597, 4. 211,respectively;P<0. 05). The results showed that below the junior college level, HAMD≥17, time of rehabilitation intervention≥60 d, treatment course <8 weeks, incontinence of urine were the adverse factors. Conclusions Education level, depressive state, time of rehabilitation intervention, treatment course, type of neurogenic bladder were the prognostic factors in spinal cord injury patients who was arranged for bladder function training by active imagery therapy. Aiming at these factors, medical staff should provide necessary guidance to improve the results of treatment.
10.Application of external diaphragm pacemaker with postural drainage treatment for cervical spinal cord injury patients with tracheotomy

Objective To evaluate the clinical effects of external diaphragm pacemaker with postural drainage treatment for cervical spinal cord injury patients with tracheotomy .Methods Totals of 50 cervical spinal cord injury patients with tracheotomy , treated from 2009 to 2012 , were randomly divided into the research group (n=25) and the control group (n=25).Patients in the control group were given routine nursing care and were assisted sputum excretion , and given anti-infection treatment and functional training .Patients in the research group , in additional to the regular treatment , were given external diaphragm pacemaker with postural drainage treatment .All the patients were treated for four weeks .The pulmonary ventilation function , lung auscultation, ability of expectoration and self-sensation were analyzed before and after treatment within each group.Results The pulmonary ventilation function of SpO2, respiratory rate, PaO2, and PaCO2 were (96.72 ± 1.89)%, (16.48 ±0.62) per min, (85.15 ±0.43)mmHg and (39.65 ±0.41)mmHg, respectively, in the research group, and (93.83 ±2.11)%, (22.05 ±0.59) per min, (74.80 ±0.29) mmHg and (33.11 ± 0.45)mmHg, respectively, in the control group, and the differences were statistically significant ( t =6.57, 3.45, 9.69, 4.67, respectively;P<0.05).The grades of lung auscultation and ability of expectoration in the research group were (2.74 ±0.27) and (2.67 ±0.32), which were significantly higher than those of control group [(2.22 ±0.33) and (2.15 ±0.41)], with significant differences (t=8.78,13.56,respectively; P<0.01).The improvement of the self-sensation in the research group was better than that in the control group (χ2 =8.86, P<0.05).Conclusions The external diaphragm pacemaker with postural drainage treatment has obvious better effect for the cervical spinal cord injury patients with tracheotomy , and it is worthy of generalizing .
