1.Establishment and Benefit Analysis of PACS in Small & Medium-Sized Hospitals

Objective To improve work efficiency and lower management cost. Methods Based on experiences from the 95th hospital in six years, PACS application benefits were analysed from such seven aspects as digital film management,economic benefit, efficiency and quality of diagnosis, evidence support for solving medical dispute,simplified treatment procedures, social effect, and remote disgnosis. Results Better economic benefits and social effect were obtained. Conclusion It is proved by practice that construction of PACS in small & midium-sized hospitals is necessary and feasible.
2.Research and Development of Whole Flow Management Information System for Hospital Material Barcode

Objective To develop a set of hospital material management information system with the mature technique of barcode so as to realize the whole flow management of material.Methods By analyzing the main problems in present hospital material management,we put forward ways and procedures of material management,and gave an overall introduction to the composition of the system from three aspects: warehouse management on material and drugs,the second level storehouse management,such as material management in department,inspect reagent consumptive material management,no stock for high-cost medical consumables management,etc and the cost accounting and collecting system.The key technologies and implementation methods adopted in the designing process of this system were also introduced.Results The achievement of whole flow tracking management from material purchase to material consumption not only ensures a real whole flow management of materials,but effectively reduces the consumption of hospital materials.Conclusion With the mature and advanced technique of barcode and wireless network,the management of material can be better achieved.
3.Effects of Chinese Medicine Compound ZDS on Serum SOD and MDA of Burn Rats

Objective To investigate the protective effects of Chinese medicine compound ZDS against digestive tact damage caused by oxygen free radical after the burns. Methods 20 SD rats were randomly divided into ZDS (with ZDS treatment,n=10) and Con (control,n=10) groups. The rats were inflicted with 30%TBSA full thickness burn on the back and fed with Chinese medicine compound ZDS or warm water (2 mL/time,2 times a day) . On the 3rd and 9th postburn day, the serum samples were harvested from portal vein and the serum superoxide dismutase (SOD) activity,the levels of malondiadehycle (MDA) were detected. Meanwhile, the bacteria translocation of parenteral organs and the histology of intestine were also observed. Results The SOD activity in ZDS group on day 9 post burn increased significantly than that in Con group( <0.05);but at the same timepoint, compared with Con group, the MDA level in ZDS group was decreased obviously ( <0.05) . Conclusion Chinese medicine compound ZDS can partly protect the digestive tact against damage caused by oxygen free radical after the burns.
4.Value of 131I SPECT/CT on the re-staging and recurrence risk stratification and its impact on subsequent treatment strategy in post-surgical patients with differentiated thyroid carcinoma

Objective To evaluate the role of 131I SPECT/CT in post-surgical re-staging and recurrence risk stratification in patients with DTC and its impact on subsequent treatment strategy.Methods 131I-WBS and 131I SPECT/CT were performed at the same time 5 to 7 d after 131I treatment in 118 patients (33 males,85 females,average age 45 years) with DTC.Difference in the localization and qualitative diagnosis of 131I uptake lesions between 131I-WBS and 131I SPECT/CT were compared.Value of 131I SPECT/CT in the diagnosis of TNM staging,risk stratification and impact on the treatment strategy was evaluated.Paired χ2 test was used for data analysis.Results A total of 509 foci with 131I uptake were detected.131I-WBS found 449 foci with 131I uptake,354 of which (78.84%) were correctly diagnosed.131I SPECT/CT found 509 foci with 131I uptake,and 504(99.02%) were correctly diagnosed.The difference was statistically significant (χ2=21.51,P<0.01).131I-WBS changed the clinical staging in 13 cases with diagnostic accuracy of 5/13.131I SPECT/CT changed the clinical staging in 19 cases and with diagnostic accuracy of 19/19 (χ2=74.41,P<0.01).131I-WBS changed the risk stratification of 13 patients after operation and the accuracy was 5/13,the corresponding data were 22 and 100%(22/22) for 131I SPECT/CT (χ2=74.41,P<0.01).The treatment strategy was changed in 50 patients with 131I SPECT/CT.Conclusions Compared with 131I-WBS,131I SPECT/CT could provide more accurate positioning and qualitative information for 131I treatment and is more accurate in re-staging and risk stratification.
5.Simultaneous determination of 4 phenolic acids in cangerzi by ultra-performance liquid chromatography.

In this study, an analytical method was developed and used to quantify simultaneously protocatechuic acid, neochlorogenic acid, cryptochlorogenic acid and 1, 3-dicaffeoylquinic acid--four bioactive compounds contained in Fructus Xanthii using UPLC. The contents of four phenolic components of 28 batches of samples collected from different product areas and markets were determined and compared by means of this established method. The mobile phase was composed of methanol and water containing 0.1% phosphoric acid. Chromatography was monitored at dual-wavelengths--220 and 327 nm. Flow rate was 0.4 mL x min(-1) and column temperature was 35 degrees C. The correlation coefficient between concentration and chromatographic peak area of protocatechuic acid, neochlorogenic acid, cryptochlorogenic acid and 1, 3-dicaffeoylquinic acid was over 0.9999 in the range of 0.3570-35.70, 2.500-250.0, 1.060-106.1, 1.010-101.0 microg x mL(-1), respectively. The average recoveries of the four compounds were 97.68%, 99.55%, 97.92% and 100.4%, respectively. In conclusion, the established method can rapidly attain an accurate and reproducible result used to control the quality of Fructus Xanthii.
6.Impacts of sevoflurane combined with lung protective ventilation strategy on pulmonary ventilation function and lung compliance in obese patients undergoing laparoscopic weight loss surgery

Objective To investigate the impacts of sevoflurane combined with lung protective ventilation strategy on pulmonary ventilation function and lung compliance in obese patients undergoing laparoscopic weight loss surgery.Methods 60 obese patients underwent laparoscopic weight loss surgery were randomly divided into two groups.The control group was given lung protective ventilation intervention alone during anesthesia,and the study group was given sevoflurane inhalation anesthesia combined with lung protective ventilation intervention.Arterial blood was collected before tracheal intubation(T0),5 min after tracheal intubation(T1),40 min after tracheal intubation(T2)and 5 min after tracheal extubation(T3)for blood gas analysis.The pulmonary ventilation function and lung compliance of patients in the two groups were compared.Results Peak airway pressure(Ppeak)and plateau airway pressure(Pplat)at T2 were lower in the study group than those in the control group,and the differences were statistically significant(P<0.05);At T2 and T3 time points,the dynamic lung compliance(Cldyn)of the study group was higher than that of the control group,and the differences were statistically significant(P<0.05);7 days after surgery,the forced vital capacity(FVC)and forced expiratory volume in one second(FEV1)in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05);At time points T1,T2 and T3,the levels of serum transforming growth factor-β1(TGF-β1),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)in the study group were lower than those in the control group(P<0.05);After surgery,the awakening time,spontaneous breathing recovery time,and extubation time in the study group were shorter than those in the control group,the number of adverse events during the recovery period was less than that in the control group,after awakening,the Ramsay score was lower than that in the control group(P<0.05).Conclusion The combination of sevoflurane and lung protective ventilation strategy can reduce inflammatory response,improve pulmonary ventilation function,and improve lung compliance in obese patients undergoing laparoscopic weight loss surgery,with good safety and fast postoperative recovery.
7.Effect of pressure controlled ventilation-volume guaranteed mode on respiratory mechanics and gas exchange function in pediatric patients undergoing laparoscopic herniorrhaphy

Objective To investigate the effects of pressure controlled ventilation-volume guaranteed(PCV-VG)mode on respiratory mechanics and gas exchange index in pediatric patients underwent laparoscopic herniorrhaphy.Methods 90 patients,scheduled for elective laparoscopic herniorrhaphy under general anesthesia of tracheal intubation,were randomly divided into 3 groups(n=30 each)using a random number table method:PCV-VG group,pressure controlled ventilation(PCV)group and volume controlled ventilation(VCV)group.At 5 min before pneumoperitoneum(T1),10 min after pneumoperitoneum(T2)and 5 min after release of pneumoperitoneum pressure(T3),respiratory mechanical indexes[inspiratory tidal volume(VTinsp),peak airway pressure(Ppeak),mean airway pressure(Pmean),dynamic lung compliance(Cldyn)]were recorded and gas exchange index[alveolar-artery oxygen partial pressure gradient(PA-aO2),respiratory index(RI)and oxygenation index(OI)]were recorded in three groups.The occurrence of pulmonary complications were recorded within 7 d after operation in three groups.Results Compared with the VCV group,Ppeak was significantly decreased and Cldyn was significantly increased at T2 and T3 time points in PCV-VG group and PCV group,the differences were statistically significant(P<0.05);Compared with PCV group,Ppeak was decreased at T2 and T3 time points in PCV-VG group,the difference was statistically significant(P<0.05).Compared with T1 time points,Ppeak and Pmean were increased and Cldyn was decreased at T2 and T3 time points in VCV group,Ppeak and Pmean were increased and Cldyn was decreased at T2 in PCV-VG group and PCV group,the differences were statistically significant(P<0.05).There were no significant differences in PA-aO2,RI and OI among three groups at T1,T2 and T3 time points(P>0.05).Compared with T1 time point,PA-aO2 and RI were increased and OI was decreased at T2 and T3 time points in three groups,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of postoperative complications among the three groups(P>0.05).Conclusion PCV-VG mode can effectively reduce Ppeak and improve lung compliance,which is suitable for laparoscopic herniorrhaphy in pediatric patients.
