1.Constructing digitized hospitals with Chinese characteristics

A digitized hospital is one with network management as its basic mode, information as the primary driving force of its development, information technology as a prop to enhance its technological strength, information culture as a way to change the ways of peoples education and work and their ideological concepts, and construction of information systems as a new growth point of its advancement. While construction of digitized hospitals is a natural trend of historical development and is presently viable, construction of information systems in domestic hospitals must be based on Chinas realities so as to build digitized hospitals with Chinese characteristics. In light of their hospitals experience in building itself into a digitized hospital, the authors point out that it is necessary to formulate in the first place an overall objective in view of the requirements of digitized hospitals and then proceed step by step according to plan.
2.Master plan of the construction of our hospitals information system and implementation

The authors sum up the experience of their hospital in the construction of the information system, pointing out that the key to success in this aspect lies in the clarification of objectives and the formulation of a master plan. Then they give a further account of ways for doing a good job of the master plan and its implementation from the perspectives of the software, hardware, and cultivation of talents.
3.Comparative study of conscious sedation and intravenous anesthesia used to endoscopic retrograde cholangio pancreatiography in treatment of extrahepatic bile duct stones

Objective To evaluate the safety and effectiveness of conscious sedation and intravenous anesthesia used to endoscopic retrograde cholangio pancreatiography (ERCP) in treatment of extrahepatic bile duct stones.Methods A total of 100 cases of extrahepatic bile duct stones patients in treatment of ERCP were encoded by the group order,50 cases of odd used intravenous anesthesia (intravenous anesthesia group),intravenous injection of propofol; 50 cases of even used conscious sedation (conscious sedation group),muscle injection of diazepam and pethidine.Intraoperative reaction (extubation behavior,own postural changes),changes in vital signs,operating time,the success rate of stone and complication were observed in two groups.Results The incidence of intraoperative extubation behavior and own postural changes in conscious sedation group were significantly higher than those in intravenous anesthesia group [24% (12/50) vs.2% (1/50),18% (9/50) vs.0],and the differences were statistically significant (P < 0.01).The heart rate and mean artery pressure in two groups were decreased at 5 minutes after administration than that before operation,and the difference was statistically significant (P < 0.05),but there was no statistical significance at 10 min after entering the mirror and postoperative awake compared with before operation (P > 0.05).Pulse oxygen saturation between two groups had no statistical significance (P >0.05).The success rate of stone in two groups were 98% (49/50).Operating time in intravenous anesthesia group was obviously shorter than that in conscious sedation group [(38.2 ± 6.3) min vs.(49.1 ± 9.9) min] (P < 0.01).The complications between two groups had no statistical significance (P > 0.05).Conclusion Conscious sedation and intravenous anesthesia can be used to ERCP in treatment of extrahepatic bile duct stones,but intravenous anesthesia can obviously reduce patients discomfort,shorten the operation time.
4.~(99m)Tc-octreotide scintigraphy and planar X-ray mammography diagnosis of breast cancer

Objective To compare the value of (~99m) Tc-octreotide scintigraphy and planar X-ray mammography in the diagnosis of breast cancer. Methods A total of 32 patients with suspected breast cancer were enrolled into this study. All patients underwent (~99m) Tc-octreotide scintigraphy and planar X-ray mammography. The diagnostic efficiency of (~99m) Tc-octreotide scintigraphy and planar X-ray mammography were compared based on the post-surgery pathology. Results The sensitivity of (~99m) Tc-octreotide scintigraphy and planar X-ray mammography in the diagnosis of breast cancer was 91.67% and 66.67%, the specificity was 100% and 37.50%, the accuracy was 93.75% and 59.38%, respectively. Combining (~99m) Tc-octreotide scintigraphy with planar X-ray mammography, the sensitivity, specificity, accuracy, positive predicting values and negative predicting values for breast cancer was 95.83%, 100%, 96.88%, 82.14% and 47.06%, respectively. Conclusion The diagnostic efficiency of (~99m) Tc-octreotide scintigraphy for breast cancer is superior to planar X-ray mammography. (~99m) Tc-octreotide scintigraphy combined with planar X-ray mammography, the diagnostic efficiency for breast cancer will be further improved.
5.Simultaneous determination of 4 phenolic acids in cangerzi by ultra-performance liquid chromatography.

In this study, an analytical method was developed and used to quantify simultaneously protocatechuic acid, neochlorogenic acid, cryptochlorogenic acid and 1, 3-dicaffeoylquinic acid--four bioactive compounds contained in Fructus Xanthii using UPLC. The contents of four phenolic components of 28 batches of samples collected from different product areas and markets were determined and compared by means of this established method. The mobile phase was composed of methanol and water containing 0.1% phosphoric acid. Chromatography was monitored at dual-wavelengths--220 and 327 nm. Flow rate was 0.4 mL x min(-1) and column temperature was 35 degrees C. The correlation coefficient between concentration and chromatographic peak area of protocatechuic acid, neochlorogenic acid, cryptochlorogenic acid and 1, 3-dicaffeoylquinic acid was over 0.9999 in the range of 0.3570-35.70, 2.500-250.0, 1.060-106.1, 1.010-101.0 microg x mL(-1), respectively. The average recoveries of the four compounds were 97.68%, 99.55%, 97.92% and 100.4%, respectively. In conclusion, the established method can rapidly attain an accurate and reproducible result used to control the quality of Fructus Xanthii.
6.Value of 131I SPECT/CT on the re-staging and recurrence risk stratification and its impact on subsequent treatment strategy in post-surgical patients with differentiated thyroid carcinoma

Objective To evaluate the role of 131I SPECT/CT in post-surgical re-staging and recurrence risk stratification in patients with DTC and its impact on subsequent treatment strategy.Methods 131I-WBS and 131I SPECT/CT were performed at the same time 5 to 7 d after 131I treatment in 118 patients (33 males,85 females,average age 45 years) with DTC.Difference in the localization and qualitative diagnosis of 131I uptake lesions between 131I-WBS and 131I SPECT/CT were compared.Value of 131I SPECT/CT in the diagnosis of TNM staging,risk stratification and impact on the treatment strategy was evaluated.Paired χ2 test was used for data analysis.Results A total of 509 foci with 131I uptake were detected.131I-WBS found 449 foci with 131I uptake,354 of which (78.84%) were correctly diagnosed.131I SPECT/CT found 509 foci with 131I uptake,and 504(99.02%) were correctly diagnosed.The difference was statistically significant (χ2=21.51,P<0.01).131I-WBS changed the clinical staging in 13 cases with diagnostic accuracy of 5/13.131I SPECT/CT changed the clinical staging in 19 cases and with diagnostic accuracy of 19/19 (χ2=74.41,P<0.01).131I-WBS changed the risk stratification of 13 patients after operation and the accuracy was 5/13,the corresponding data were 22 and 100%(22/22) for 131I SPECT/CT (χ2=74.41,P<0.01).The treatment strategy was changed in 50 patients with 131I SPECT/CT.Conclusions Compared with 131I-WBS,131I SPECT/CT could provide more accurate positioning and qualitative information for 131I treatment and is more accurate in re-staging and risk stratification.
7.Comparison of chemical components between Baishao and Chishao water extracts and their effects on proliferation of rat thoracic aorta smooth muscle cells.

OBJECTIVETo compare the chemical components in Baishao and Chishao water extracts and investigate their effects on the proliferation of rat thoracic aorta smooth muscle cells in vitro.
METHODSThe contents and chemical structures of monomers separated from the water extracts of Baishao and Chishao were analyzed using high-performance liquid chromatography and mass spectroscopy. Rat thoracic aorta smooth muscle cell line A7r5 and its platelet-derived growth factor-BB (PDGF-BB)-induced proliferation model were exposed to different concentrations of Baishao and Chishao water extracts, and the cell viability was analyzed by mitochondrial-dependent reduction of MTT and real-time cell analyzer.
RESULTSThe growth of A7r5 cells was significantly stimulated by 300 µg/ml Baishao water extract (P<0.01), but Chishao water extract produced no such effect (P>0.05). In PDGF-BB-induced cell proliferation model, the cell growth was significantly suppressed by 100-500 µg/ml Chishao water extract (P<0.01), while Baishao water extract showed no obvious effect on the cell proliferation (P>0.05).
CONCLUSIONBaishao and Chishao water extracts have different chemical components and produce different biological effects.
