1.Analysis of Reliability and Validity for Clinical Experts Questionnaire about Coronary Heart Disease

Objective To study the reliability and validity of clinical experts questionnaire of syndrome factors, characteristics and evolution of coronary heart disease. Methods The clinical expert questionnaire was sent to 305 clinical doctors of cardiovascular disease. The questionnaire of coronary heart disease was described through four phases data, and reliability and validity of each module and the total scale were tested. Results Cronbach's ? coefficient of the modules varies between 0.959 3~0.972 5 and higher than 0.8, the value of the scale was 0.990 3. Split-half reliability of the modules varies between 0.891 8~0.922 5, Cronbach's ? of split-half reliability of the total scale was 0.920 7. Factor analysis showed that each module was abstracted four factors and the total contribution rate was more than 50%, the result after varimax rotation was consistent with the theory. Conclusion The questionnaire can fully reflect symptoms and signs of patients with coronary heart disease, and can be used for syndrome studies of coronary heart disease.
2.Changes of circulating Tc17 cells in progression of colorectal cancer

Objective:To explore the changes and possible mechanisms of circulating Tc 17 cells in patients with colorectal cancer ( CRC) following disease progression .Methods:The peripheral blood were collected from 54 patients with colorectal cancer . the proportions of Tc17 cells in peripheral blood mononuclear cells (PBMCs) and cultured PBMCs treated by IL-1β, IL-6 and TGF-βin different concentrations were determined by flow cytometry;the levels of IL-1β, IL-17A, IL-23 and IL-6 in sera were measured by ELISA.Results:The proportions of Tc17 cells and the levels of IL-1β, IL-17A, IL-23 and IL-6 were significantly higher than those in healthy controls (P<0.05).Compared with early CRC, the proportions of Tc17 cells and the levels of IL-1β, IL-17A in advanced CRC were significantly decreased, but the level of IL-6 was significantly increased (P<0.05).The difference of IL-23 level between early and late groups was not significant (P>0.05).In vitro experiments confirmed that IL-1βor high concentrations of IL-6 and TGF-βcould significantly increase the number of Tc 17 cells in PBMCs.Conclusion: The changes of circulating Tc 17 cells in the progression of colorectal cancer are possibly modulated by IL-1β, IL-23, IL-6 and TGF-β.
3.The Changes of Circulating Th17 and Tc17 Cells in Patients with Colorectal Polyp,Adenoma and Cancer

Objective To compare ratio of circulating Th17 over Tc17 cells in patients with colorectal polyp (CRP), colorectal adenoma (CRA) and colorectal cancer (CRC),and analyse their relationship. Methods The ratio of circulating Th17 over Tc17 cells in 93 patients(17 CRP, 34 CRA and 42 CRC)and 17 healthy donors were examined by flow cytometry;Serum levels of IL-17A and IL-23 were tested by ELISA. Results The ratio of circulating Th17 over Tc17 cells and the se-rum levels of IL-17A and IL-23 were the lowest in healthy control groups, medium in CRP and CRA groups and the highest in CRC groups (P<0.05). The proportions of Tc17 cells in CRA groups were significantly higher compared to those in CRP groups,but no statistical significance of proportion of circulating Th17 cells as well as serum levels of IL-17A and IL-23 be-tween CRA and CRP groups was observed (P>0.05). Further analysis showed that there was a positive correlation between proportions of circulating Th17 and Tc17 cells (rs=0.594, P<0.001). Conclusion The proportions of circulating Th17 and Tc17 cells were positively related and the ratio of Th17 over Tc17 was markedly increased from CRP group or CRA group to CRC group.
4.Pharmacodynamics of Tongyushu Capsules in the Treatment of Dysmenorrhea

Objective:To observe the therapeutic effects of Tongyushu capsules on dysmenorrhea. Methods:With the intravenous injection of oxytocin in rats, the influence of Tongyushu capsules on rat uterine smooth muscle was observed in vivo. With the intraper-itoneal injection of oxytocin to replicate the mouse model of dysmenorrhea, the effect of Tongyushu capsules on dysmenorrhea was ob-served. Using acetic acid writhing method, the effect of relieving pain Tongyushu capsules in mice was observed. By the method of ear swelling induced by xylene in mice, the anti-inflammatory effect of Tongyushu capsules was observed. By the method of ice water bath and subcutaneous injection of adrenaline, the influence of Tongyushu capsules on the acute blood stasis model in rats was observed. Results:Compared with those of the model group, the contraction of uterine smooth muscle of the groups treated with Tongyushu cap-sules at various dosage was decreased obviously (P<0. 05), the number of body torsion reaction induced by oxytocin and acetic acid in mice was obviously decreased in 15 min (P<0. 05), the degree of ear swelling in mice treated with Tongyushu capsules was signifi-cantly reduced (P<0. 05), and the whole blood viscosity, plasma viscosity, hematokrit, erythrocyte sedimentation rate and erythro-cyte electrophoresis time were significantly decreased (P<0. 05). Conclusion:Tongyushu capsules have obvious effects of the inhibi-tion of excessive contraction of uterine smooth muscle, dysmenorrhea alleviation, anti-inflammation and relieving pain, which show good therapeutic effect on dysmenorrhea.
6.The changes of circulating Tc17 and Treg cells in the patients with colorectal adenoma and the pa-tients with colorectal cancer

Objective To observe the changes of circulating Tc 17 and Treg cells in the patients with colorectal adenoma(CRA)and the patients with colorectal cancer (CRC)and to investigate the correla-tions between them.Methods Peripheral bloods were collected from 63 patients (30 CRA and 33 CRC) and 20 healthy subjects .The proportions of circulating Tc 17 and Treg cells in peripheral blood mononuclear cells ( PBMCs) were detected by flow cytometry .The serum levels of IL-17A and IL-23 were measured by ELISA.Results Compared with healthy controls , the proportions of circulating Tc 17 and Treg cells as well as the serum levels of IL-17 A and IL-23 in CRA and CRC groups were significantly up-regulated ( P<0.05).The proportions of circulating Tc17 cells and the serum levels of IL-17A and IL-23 in CRC group were significantly higher than those in CRA group (P<0.05), but there was no significant difference in the expressions of Treg cells between the two groups .As the disease progressed , the proportions of circulating Tc17 cells and the serum levels of IL-17A and IL-23 in CRAⅠgroup and the early CRC group were respec-tively higher than those in CRAⅡgroup and the advanced CRC group (P<0.05), while the expressions of Treg cells was just the opposite , and no correlation between the expressions of Tc 17 and Treg cells was found (r=-0.227, P=0.073).Conclusion The changes in the percentage of Tc17 cells in peripheral blood are contrary to Treg cells in patients with CRA or CRC as the disease progresses , but there is no correlation be-tween them .
7.Assessment of the accuracy of real-time continuous glucose monitoring system and its correlated factors

Objective To assess the factors that influence the accuracy of real-time continuous glucose monitoring system (RT-CGM).Methods A total of 79 diabetic patients wore RT-CGM for three days continuously while calibrating by interphalangeal glucose values 4-8 times a day.We counted matching rate of interphalangeal glucose values and RT-CGM probe value,and analyzed correlation of the matching rate with MAGE,SDBG,MBG,AUC10,AUC3.9,and NGE by Pearson correlation analysis and multiple linear regression analysis.Then we grouped corresponding match rate by MAGE and MBG,compared the difference among groups.Furthermore,we analyzed correlation of RT-CGM probe values of all-day,fasting-postprandial,and nighttime and the corresponding interphalangeal glucose values by Pearson correlation analysis.Results Pearson correlation analysis showed that the matching rate was negatively correlated with the MAGE,SDBG,MBG,and AUC10.MAGE,MBG,and NGE were in the regression equation.MAGE <3.9 mmol/L,matching rate was 81%,3.9 ≤ MAGE<7 mmol/L,matching rate was 67%,MAGE≥7 mmol/L,the matching rate was 52%,the difference among 3 groups was statistically significant (P<0.01).MBG<7.8 mmol/L,matching rate was 78%,7.8 ≤ MBG< 10 mmol/L,matching rate was 68%,MBG ≥10 mmol/L,the matching rate was 57%,the difference among 3 groups was statistically significant (P < 0.01).When Youden index was 0.409 and MAGE was less than 3.745 mmol/L,the match rate was more than 75%.When Youden index was 0.369 and MBG was less than 8.38 mmol/L,the matching rate was more than 75%.The correlation coefficients of all day,fasting,postprandial and nighttime probe values and the corresponding interphalangeal glucose values were 0.958,0.963,0.944,and 0.965 (all P<0.01).Conclusions The probe values show good consistency with the interphalangeal glucose values,are able to be used for blood glucose assessment.Blood glucose fluctuations influences accuracy of RT-CGM,the smaller the blood glucose fluctuations,the smaller the mean glucose concentration,the smaller the effective blood glucose fluctuations in frequency,the higher would be the accuracy.It seems to be more effective to do calibration during fasting and nighttime.
8.Variety methods study on hemolytic effect of colored injection doxorubicin hydrochloride in vitro

Objective The visual inspection method were not appropriate to perform a hemolysis evaluation for colored injection like doxorubicin hydrochloride,this article adopted three methods to evaluate the hemolysis test of doxorubicin hydrochloride in vitro and provide reference for clinical drug safety.Methods Using rabbit erythrocytes as experimental object,the durg concentration 4.0 and 2.0 mg/mL was chosen which range of clinical concentration and preclinical safety evaluation concentration,to evaluate the hemolysis test of doxorubicin hydrochloride injection with blood analyzer test,direct colorimetric assay,and indirect colorimetric assay.Results The evaluation results of three different methods were very consistent.The tube's hemolysis rate of 4.0 mg/mL dose was far greater than 5%,which means serious hemolysis;Only 0.1 mL tube of 2.0 mg/mL dose (according to the drug concentration equal to 0.5 mL tube of 0.4 mg/mL drug concentration) without hemolysis occurring,the other tubes' hemolysis rates were far greater than 5%,which means serious hemolysis.Conclusion The hemolysis phenomenon may occur when 2.0 mg/mL dose of doxorubicin hydrochloride solution for iv injection is used in clinic and dilution (final concentration not more than 0.4 mg/mL) is recommended.
9.Caspase recruitment domain-containing protein 9 gene deficiency associated invasive candidiasis: a case report and literature review

Objective:To summarize the clinical characteristics, diagnosis and treatment of caspase recruitment domain-containing protein 9 (CARD9) gene deficiency associated invasive candidiasis, and report a novel mutation in CARD9 gene.Methods:The clinical characteristics, laboratory tests, treatment and the outcome of follow-up in a boy with invasive candidiasis were described. The boy′s main clinical manifestations were central nervous system infection and retroperitoneal mass. Whole-exome sequencing was performed and Sanger sequencing was verified to identify the CARD9 gene mutations in the patient and his parents. A literature search for “CARD9”and “invasive candidiasis”was conducted in PubMed, Wanfang and CNKI databases from their establishment to May 2020.Results:A 10-year-old boy suffered onset symptom of chronic diarrhea, which lasted for two months. The symptom was followed by progressive neurological symptoms such as headache, vomiting, seizures and disorder of consciousness. His unusual medical history was absent. Candida albicans were cultured several times in cerebrospinal fluid and blood, and yeast-like fungi were found in the stool high power field of vision. Cerebral magnetic resonance imaging indicated obstructive hydrocephalus and abdominal CT scan showed retroperitoneal mass and thickening of the intestinal wall. The whole-exome sequencing analyses of blood samples from the boy and his parents were performed. The results showed that there was a homozygous mutation of c.952-12_956delinsAG in the CARD9 gene, which was an unreported pathogenic mutation. This was confirmed by Sanger sequencing. There was no significant relief from intravenous combined antifungal medications. After lateral ventricular drainage surgery and injection of amphotericin B into the lateral ventricle, improvement of clinical symptoms and cerebral spinal fluid abnormalities was observed after nine weeks, and the retroperitoneal mass shrank. At follow-up after four-month oral combined antifungal medications, the child had no complaint except fatigue. However, cerebral spinal fluid analysis showed increased protein level and decreased glucose. Persistent hydrocephalus and periventricular white matter abnormal signals were revealed on the brain magnetic resonance imaging and the smaller retroperitoneal mass than before on the abdominal CT scan. In addition to this case, totally 21 cases with CARD9 gene deficiency associated invasive candidiasis have been reported worldwide, most of which featured central nervous system infections.Conclusions:CARD9 gene deficiency is an autosomal recessive primary immunodeficiency that confers human susceptibility to fungal disease. The associated invasive candidiasis often affects the central nervous system and makes the patient severely ill. Adequate systemic antifungal therapies should be given, and patients with hydrocephalus need surgical treatment. A novel mutation is reported that expands the variant diversity of CARD9 gene. For patients with unexplained invasive candidiasis, including those without a history of previous recurrent infection, genetic testing is recommended for primary immunodeficiency including CARD9 gene deficiency.
