1.Expression of pancreatic thioredoxin-1 in rats with acute necrotizing pancreatitis and effect of melatonin on thioredoxin-1

Objective To investigate the expression of pancreatic thioredoxin-1 (TRX-1) in rats with acute necrotizing pancreatitis (ANP) and the effect of pretreatment of melatonin on its expression. Methods Male Spraque-Dawley rats (n = 12) were randomly divided to ANP group, melatonin group, control group with 24 rats in each group. The rats in ANP group received three intraperitoneal injections of 25 ml/kg body weight 6% L-arginine at an interval of 1 h to induce ANP. The rats in melatonin group received intraperitoneal injections of 25 ml/kg body weight 6% melatonin 30 min before ANP induction; rats in ANP group and control group received intraperitoneal injections of same amount of saline. Rats were sacrificed at 6 h, 12 h and 24 h after ANP induction. The serum level of amylase was measured and the pathological evaluation of pancreatic tissues was performed. The concentrations of malondialdehyde (MDA) and myeloperoxidase (MPO) in pancreatic tissues were measured. The expressions of TRX-1 protein were detected by immunohistochemistry and the expressions of TRX-1 mRNA in pancreatic tissues were determined by RT-PCR.Results In ANP group, serum level of amylase, MDA, MPO, TRX-1 mRNA and TRX-1 protein in pancreatic tissues were (3 012 ±1 425) U/L, (4.13 ± 1. 85)nmol/mg prot,(7.45 ± 1.26)nmol/mg prot, 0.68 ±0. 18, 66.8 ±8. 1, while they were (1 835±499)U/L, (3.03 ±2.12) nmol/mg prot, (5. 32 ± 1.06) nmol/mg prot, 0.50±0.09, 80. 29 ±8. 14, respectively in melatonin group, the values in melatonin group were significantly lower thanthose in ANP group (P < 0.05). The peak value of TRX-1 mRNA and TRX-1 protwein expressions shifted from 12 h after ANP induction in ANP group to 6 h after ANP induction in melatonin group. Conclusions The expression of pancreatic TRX-1 protein and TRX-1 mRNA in rats with ANP was significantly increased. Melatonin pretreatment could promote pancreatic tissues to express TRX-1 protein and TRX-1 mRNA, and may be protective for pancreatic tissues damages.
2.Observations on the Therapeutic Effect of Gentle Moxibustion on Old Interphalangeal Collateral Ligament Injury

ObjectiveTo investigate the clinical efficacy of gentle moxibustion in treating old interphalangeal collateral ligament injury.MethodSixty patients were randomly allocated to two groups. The treatment group of 30 patients received gentle moxibustion 20-30 min once daily for two consecutive courses of treatment. The control group of 30 patients received TDP irradiation 20-30 min once daily for two consecutive courses of treatment.ResultThe excellent rate and the total excellent rate were 56.7% and 83.3%, respectively, in the treatment group and 36.7% and 76.7%, respectively, in the control group; there were statistically significant differences between the two groups (P<0.01).ConclusionGentle moxibustion has a marked therapeutic effect on old interphalangeal collateral ligament injury.
3.Application of self-made venous compressor in hemostasis after withdrawing femoral vein catheter in patients with liver failure

Objective To explore the effect of self-made venous compressor on hemostasis after withdrawing femoral vein catheter in patients with liver failure.Methods 200 patients with liver failure undergoing femoral venous intubation were divided randomly into observation group and control group in equal number.Hemostasis was performed after withdrawing the catheter using the self-made venous compressor and bandage combined with pressure dressing in the former and latter group respectively.The two groups were compared in terms of the complication rate,time for hemostasis manipulation and time limb immobilization.Results The rate of complications in the observation group was significantly lower than that of the control group.The time for hemostasis manipulation and time for limb immobilization in the observation group were both significantly shorter than those of the control group(P<0.01). Conclusion Performance of hemostasis using self-made venous compressor may lower the rate of complication from withdrawing femoral vein catheter and shorten the time for hemostasis manipulation and the time for limb immobilization.
4.Effect of Passy-Muir speaking valve on swallowing biomechanics of dysphagia in stroke patientst after tracheoto-my

Objective To explore the effect of Passy-Muir speaking valve(PMV)on swallowing biomechanics of dysphagia by us-ing high resolution manomety(HRM)in stroke patients after tracheotomy,and to evaluate the application value of pressure parameters of pharynx and upper esophageal sphincter(UES)in PMV therapy. Methods From January,2020 to January,2024,100 patients in People's Hospital of Xinjiang Uygur Autonomous Region were randomly divided into control group(n = 50)and PMV group(n = 50).The control group accepted routine swallowing therapy,and PMV group accepted PMV additionally,for two weeks.Before and after treatment,the clinical swallowing function was evaluated with modified Mann Assessment of Swallowing Ability(MMASA);and then they were observed with HRM,and the velopharynx maximal pressure,velopharynx time,UES residual pressure and duration of UES relaxation were collected.The correlation of the scores of MMASA and pressure parameters of pharynx and UES was analyzed. Results Four cases dropped down in each group.After treatment,UES residual pressure significantly decreased(t>47.403,P<0.001),and the score of MMASA,the velopharynx maximal pressure,the velopharynx time and the duration of UES relaxation significantly increased(t>19.621,P<0.001);and all the indexes were better in PMV group than in the control group(t>2.050,P<0.05).The scores of MMASA were significantly positively correlated with velopharynx maximal pressure(r = 0.924,P<0.001),velopharynx time(r = 0.948,P<0.001)and duration of UES relaxation(r = 0.954,P<0.001),negatively correlated with UES residual pressure(r =-0.939,P<0.001). Conclusion PMV could improve the swallow function and biomechanics of pharynx and UES in stroke patients with tra-cheotomy.The pressure parameters of pharynx and UES may predict the effect of swallow function by using PMV.
