1.Histological study on TMJ of New Zealand rabbit at the age of 7 to 10 months old

Objective: To investigate the characters of the ageing changes in TMJ of New Zealand rabbit from 7 to 10 months old. Metho ds: 12 New Zealand rabbits aged 7,8,9 and 10 months old and 3 in eac h age group, were killed under anesthesia. Bilateral TMJs were histologically examined with HE stainnig. Results: Quite similar histolo gical structures were found in the TMJs in different age groups. Very few change s occurred in the fibre layer of osteo-articular surface. The thickness of the proliferate layer was decreased slightly in anterior and middle part saggitally, resulting in comparable increase of the hypertrophy layer with age. No hypertro phy layer was found in the cartilage over tempora articular surface. The thickne ss of the cartilage-like osteo layer was uneven, the thickest part was in the area towards condyle anterior slope and without age change. The thickness of d isc increased a little bit with age. Conclusion: The thick ness of the cartilage over TMJ articular surface of New Zealand rabbit at the a ge from 7 to 10 months old changes a little. The layer structures keep considera bly constant.
2.Specific clinical and imaging features of osteogenesis imperfecta V

Objective To evaluate specific clinical and imaging features of osteogenesis imperfecta V and to improve diagnostic accuracy of this disease. Methods Data of 15 patients with osteogenesis imperfecta type V were retrospectively analyzed for their clinical and imaging features. There were 10 males and 5 females, aged from 1 year and 30 years old (median age,12.5 years ). All 15 patients had plain X-ray, and 4 of 15 had CT. All data were analyzed by 3 experienced deputy chief doctors in OI according to OI V standard. Results X-ray:calcification of the interosseous membrane between radius-ulna was detected in all patients and calcification of the interosseous membrane between tibia-fibula was detected in 2 of 15 patients. Dislocation of the radial head was seen in 13 of 15 patients,bilateral in 9 and utilateral in 4.All patients showed restriction in the pronation and supination of the forearm and restricton in the flexion and extention of the elbow joint. Patients with dislocation of raidal head were associated with large coronoid process and olecranon of the ulna. Hyperplastic callus of the extremities were detected in 7 of 15 patients (7 at femur , 3 at humerus, 1 at tibia.In early stage, hyperplastic callus showed thin cortice, and clear boundares with the diaphysis showing and low density, irregular, mesh-like lamellation inside. In the later stage, there were dense calcification inside hyperplastic callus, and no difference in density with the diaphysis. Diaphysis surrounded by hyperplastic callus had clear boundaries with the hyperplastic callus. No cortical destruction was detected. CT:there were sparse needle-dot calcification inside hyperplastic callus, with the patterns of granular, ring-and-arch,irregular streaky mineralization. The cross section of proximal femoral shaft showed irregular shape , flat square shape and tiny medullary cavity, with no calcification on the edge of hyperplastic callus. CT value:-91 HU inside hyperplastic callus; 283 HU in femoral shaft. Conclusions Interosseous membrane between radius-ulna or tibia-fibula, hyperplastic callus ,dislocation of the radial head are specific features in osteogenesis imperfecta V. X-ray can make a definitive diagnosis of osteogenesis imperfecta V. CT scan is helpful in the differential diagnosis of osteogenesis imperfecta V from osteosarcoma.
3.Inhibitory effects of homoharringtonine and/or methoxsalen on highly metastatic human mucoepidermoid carcinoma Mc3 cells

Objctive: To study the inhibitory effects of homoharringtonine(HHT) or methoxsalen (8-MOP) or in combinnation on humen highly metastatic mucoepidermoid carcinoma Mc3 cells. Methods: Mc3 cells were exposed to the drugs or their combination at various concentrations. The inhibitory effects were tested with MTT assay. Results: The IC 50 values of HHT and 8-MOP were 79.43 ng/ml and 5 980 ng/ml respectively, HHT at the doses of 1-320 ng/ml combined with 8-MOP at 800, 4 000 and 20 000 ng/ml the CI 50 values were 0.76?0.33 and 0.15 respectively.Conclution: The data indicate that combination of HHT with 8-MOP is synergic in the inhibition of Mc3 cell growth.
4.Early warning of high altitude pulmonary edema

Objective By analyzing the clinical symptoms, heart rate ( HR) , arterial oxygen saturation ( SaO2 ) and the number of white blood cells (WBC), we aimed to explore the implication of the above-mentioned indexes for early warning of high altitude pulmonary edema ( HAPE) .Methods Based on the Lake Louise Self-assessmeat Scoring System ( LLSS) and the scoring of respiratory symptoms, 628 subjects were divided into three groups: group A ( the healthy;score<3),group B(acute mountain sickness, AMS; score>3 and excluding HAPE),and group C (HAPE).Moreover, we analyzed the incidence of some clinical symptoms, HR and SaO2 , as well as the WBC number of some subjects in the three groups.Results The incidence of respiratory symptoms and WBC number were significantly increased in group C compared with group B(P<0.05).In addition,unlike group A and B, the mean HR in group C rose gradually in the first 3 days and was markedly increased at the third day(F=6.37,P<0.05).The mean SaO2in group C was remarkably lower than in group A and B in the first 3 days(F=8.21,F=8.77,F=9.58,P<0.01).Conclusion Those who enter high altitude for the first time with notable respiratory symptoms, WBC increase, HR maladaptation and decrease in SaO2 (>30%) have high risk of HAPE.It is of special importance to detect HAPE earlier at high altitude.
5.Biomechanical analysis of a finite element model of rotator cuff

Objective To establish a finite element model of rotator cuff which can be used to simulate the rotator cuff injury and to evaluate the biomechanical effects of rotator cuff surgery.Methods The Dicom CT images of the right shoulder of a Chinese healthy volunteer were used to establish models of the scapula,humerus and clavicle.The rotator cuff structures were separated and modeled based on the MRI images and anatomical knowledge.After the rotator cuff model was introduced into the finite element analysis software Abaqus 6.12,the anatomical positions were simulated when the shoulder was at 30° internal rotation,30° external rotation,30° abduction,30° adduction,30° flexion and 30° extension.Results When the shoulder was in 30° flexion,the average stress was 52.2 kPa on the supraspinatus,223.0 kPa on the inffaspinatus and the teres minor,and 90.4 kPa on the subscapularis.When the shoulder was in 30° extension,the average stress was 105.0 kPa on the supraspinatus,78.2 kPa on the infraspinatus and the teres minor,and 55.7 kPa on the subscapularis,indicating that the muscle and tendon of the supraspinatus was subjected to greater stress and the humerus and the scapula produced less stress compared with the shoulder in 30° flexion.Conclusion Since our finite element model of the rotator cuff can simulate common activities of the shoulder joint and obtain stress values of the corresponding rotator cuff muscles,it can be used in simulation of rotator cuff injury and its surgery.
6.Comparison of breath-hold and respiratory-triggered proton MR spectroscopy in quantification of liver fat content

Objective To compare the consistency and correlation of multiple breath-hold (BH) with respiratory-triggered (RT) 1H-MRS for quantification of hepatic lipid content. Methods Sixty subjects were underwent RT 1H-MRS of the liver (Couinaud segment VII) and BH 1H-MRS at 1.5 Tesla Magnetic Resonace Imaging (MRI). The peak areas of water and methylene obtained on RT and BH 1H-MRS were recorded respectively and the liver fat fraction was calculated. Pearson correlation coefficient , Bland-Altman 95% limit of agreement, and concordance correlation coefficient were calculated. Results Mean liver fat fraction measured in RT and BH 1H-MRS were (8.6 ± 8.7)% and (9.4 ± 9.3)% respectively. There was a strong correlation between RT and BH 1H-MRS(r = 0.973, P < 0.000 1, concordance correlation coefficient = 0.95). With the Bland-Altman method, 91.7% data points were within the 95% limits of agreement. Conclusion RT and BH 1H-MRS are alternative tools for intrahepatic lipid quantification. These two methods have a strong correlation and perfect consistency.
7.Surgical management for sciatic nerve injury.

OBJECTIVETo summarize surgical treatments and their corresponding curative effects on sciatic nerve injuries.
METHODSSurgical treatments on sciatic nerve injury were performed in 28 patients from January 1990 to July 2000. The treatments included neurolysis, neurolysis plus partial nerve anastomosis, nerve anastomosis and nerve transplantation. The curative effect was evaluated according to Sunderland criteria.
RESULTSOf 28 cases, 22 patients were followed up with a follow-up period of 13 months to 5 years (average 30 months). Of 22 nerves, 7 were excellent, 5 good, 7 fair and 3 poor, with an excellence rate of 54.5%.
CONCLUSIONSThe fair results of sciatic nerve injury are related to its structural character. Surgical exploration should be performed if nerve function does not recover 3 months after primary operation and if Tinel's sign and electromyogram show no signs of nerve regeneration. Electrophysiological monitoring in the operation is useful in electing surgical methods and predicting the results of nerve anastomosis.

Objective To analyze the risk factors of postoperative recurrence and metastasis of of BCLC stage A (BCLC-A) hepatocellular carcinoma (HCC).Methods Data of 210 cases of liver resection for HCC were retrospectively analyzed from Nov 2013 to June 2016.Multivariate logistic regression analysis was used to analyze the risk factors for postoperative recurrence.The cumulative survival rates were calculated using the Kaplan-Meier method and the diffrences of the related factors between various groups were analyzed by Log-rank test.Results Univariate analysis showed that alpha-fetoprotein (AFP),neutrophil-lymphocyte ratio (NLR),hepatitis B virus surface antigen (HBsAg) positive and intraoperative transfusion were associated with early recurrence and metastasis after hepatectomy (P < 0.05).Multivariate analysis showed that AFP > 400 ng/ml is an independent risk factor for early postoperative recurrence and metastasis (P =0.008).HBV DNA positive can affect the disease-free survival rate after HCC resection (P =0.030).Conclusion AFP is an independent risk factor for early postoperative recurrence and metastasis.Preoperative HBV DNA positive significantly decreases the disease-free survival rate in HCC patients.
10.Clinical significance of serum prealbumin-bilirubin score (PALBI) in predicting posthepatectomy liver failure in patients with HBV-related hepatocellular carcinoma

Objective To study the value of serum prealbumin-bilirubin score (PALBI) in predicting posthepatectomy liver failure (PHLF) for patients with HBV related hepatocellular carcinoma (HCC).Methods A retrospective study was conducted on 919 HBV-related HCC patients who underwent hepatectomy from September 2013 to December 2016 at the Affiliated Tumor Hospital of Guangxi Medical University.These patients were divided into a training cohort (n =689) and a validation cohort (n =230) using the 3 ∶ 1 matching principle.The training cohort was divided into the control group (n=546) and the PHLF group (n=143) according to whether PHLF occurred.The multivariate logistic regression model was used to analyze the factors related to PHLF in the training cohort,and then the PALBI score was established.The ability of the PALBI score to predict PHLF was evaluated by the area under the receiver operating characteristic curve (AUC) and compared with the Child-Pugh,model for end-stage liver disease (MELD),and albumin-bilirubin (ALBI) scores.Results Univariate and multivariate logistic regression analyses showed the factors including HBV-DNA≥ 103 IU/ml,total bilirubin,prealbumin,platelet count,AST,prothrombin time,intraoperative blood loss ≥400 ml and major liver resection were closely related to PHLF.The ability of the PALBI score (AUC =0.733) to predict PHLF preoperatively was superior to the ChildPugh score (AUC =0.562),the MELD score (AUC =0.652) and the ALBI score (AUC =0.683) in the entire training cohort.Similar results were obtained in the entire validation cohort (AUC:0.752 vs.0.599 vs.0.641 vs.0.678).To eliminate the effect of a small residual liver volume on PHLF,the ability of each of these scores in the training and validation cohorts to predict PHLF was calculated respectively in these 2 cohorts of patients who underwent only minor liver resection,and similar results were obtained.Conclusion The PALBI score was significantly superior to the Child-Pugh,MELD and ALBI scores in predicting PHLF in patients with HBV-related HCC who underwent liver resection.The PALBI score is a simple,non-invasive and reliable novel model in predicting PHLF.
