Chinese Journal of Radiology 2001;0(01):-
Objective To evaluate the usefulness of MRI in the diagnosis of adenomyosis. Methods Sagittal Turbo SE T 1WI, T 2WI, T 1SPIR and T 2SPIR MRI examination were performed on 30 cases with adenomyosis. Results The lesions in 12 cases with diffuse adenomyosis were demonstrated as diffusely thickened junctional zone (JZ) measured 10~35 mm (mean 18 mm) On T 2WI. 6 of them were of homogenous hypointensity. Another six had hyperintensive foci within the hypointensive lesions on T 2WI, and these hyperintensive foci could also be found on T 1WI in 5 lesions. 23 focal lesions (adenomyomas) in 18 cases were oval, irregular or round masses with hypointensity that were the same as the intensity of JZ on T 2WI. They were from 2.0-7.5 cm in diameter (mean 3.9 cm), and all but one showed ill-defined margins. 15 masses hadhyperintensive foci within the hypointensive lesions on T 2WI and 12 of 15 presented hyperintensity on T 1WI. The diffusely thickened JZ and focal masses correspond to the hyperplastic and hypertrophic musculature around the heterotopic endometrial islands. The hyperintensive foci were correlated with the heterotopic endometrial islands. The hyperintensive foci showed only on T 2WI corresponded to the heterotopic endometrial islands without hemorrhage. The hyperintensive foci shown on both T 2WI and T 1WI were the heterotopic endometrial islands with hemorrhage. Conclusion MRI is the modality of choice for the diagnosis of adenomyosis. T 2WI is the most useful sequence. T 2WI combining with T 1WI, T 1SPIR, and T 2SPIR can improve the accuracy in the diagnosis of adenomyosis.

Objective To discuss the effect of pericardial devascularization plus gastric fundus transaction in advance schis?tosomiasis patients with portal hypertension. Methods Thirty?six advanced schistosomiasis patients with portal hypertension treated with devascularization plus gastric fundus transaction(a portal hypertension group),as well as 10 patients treated with modified Sugiura operation(a modified Sugiura operation group)in the Third People’s Hospital of Yangxin County since 2006 were chosen as the observation objects, and the clinical effects of the two groups were observed and compared. Results The op?eration time, indwelling time of stomach tube, time to taking food after operation, drainage tube removal time of the portal hy?pertension group were all shorten than those of the modified Sugiura operation group(all P<0.05). The hospitalization expenses of the two groups were(25 466.00 ± 2 888.48)Yuan and(34 517.10 ± 4 948.39)Yuan respectively, and the difference was al?so statistically significant(P<0.05). The incidence rates of portal thrombosis of the portal hypertension group and modified Sug?iura operation group were 33.33%(12/36)and 40.00%(4/10),respectively,and the incidence rates of rehaemorrhagia of the two groups 12 months after the operation were 16.67%(6/36)and 10.00%(1/10),respectively,but the differences had no sta?tistically significance(both P>0.05). In addition,1 case with delayed gastric emptying and 1 case with stomal leak of esopha?gus happened in the modified Sugiura operation group, while no corresponding complications happened in the portal hyperten?sion group. Conclusions Pericardial devascularization plus gastric fundus is a relatively easy procedure which has a good short?term clinical effect, and therefore it is suitable for application in primary hospitals. However, its long?term effect still needs fur?ther observation.

Objective To determine the relation between the graduation and type of Modic change and low back pain. Methods Ninety-seven patients with low back pain associated with Modic change were enrolled, and the data of clinical were analyzed. The degree of back pain was evaluated by visual analog score (VAS) and Oswestry disability index (ODI). The relation between Modic change and the degree of low back pain was evaluated. Results In 97 patients, the type of Modic change:28 cases (28.9%) were type Ⅰ, 37 cases (38.1%) were type Ⅱ, 11 cases (11.3%) were Ⅱ/Ⅲ, and 21 cases (21.6%) were type Ⅲ. The graduation of Modic change:39 cases (40.2%) were minimal, 34 cases (35.1%) were moderate, and 24 cases (24.7%) were severe. The VAS of type Ⅰ, type Ⅱ, typeⅡ/Ⅲand typeⅢin patients with Modic change were (3.8 ± 0.4), (2.6 ± 0.2), (1.7 ± 0.5) and (1.0 ± 0.6) scores, and there were statistical differences (P<0.05). The ODI scores of type Ⅰ, type Ⅱ, type Ⅱ/Ⅲ and type Ⅲ in patients with Modic change were (11.8 ± 0.7), (8.4 ± 1.1), (5.7 ± 1.3) and (2.0 ± 1.0) scores, and there were statistical differences (P<0.05). Spearman correlation analysis result showed that the type of Modic change was negatively correlated with the intensity of low back pain (r=-0.342, P<0.01). There were no statistical difference in VAS and ODI score among the patients with minimal, moderate and severe of Modic change (P>0.05). Spearman correlation analysis result showed that the graduation of Modic change had no correlation with the intensity of low back pain (r = 0.351, P>0.05). Conclusions The graduation of Modic change does not directly correlate with the low back pain, but the type of Modic lesion is more important.
