Intermittent hypoxia (IH) is a direct consequence of obstructive sleep apnea syndrome (OSAS), which results in left ventricular dysfunction , remodeling and myocardial cell injury .Understanding the mechanisms of OSAS is important for us to prevent and treat the cardiovascular complications in the clinical practices .We summarize the effects of IH on left ventricular function , and analyze the mechanisms at the cellular and molecular levels , including oxidative stress, ion exchangers and inflammation .In addition, the effects of endothelin-1 and hypercapnia on IH-induced cardiac injury are discussed .

4.A Clinical Study on the Level of Lymphocyte Sub-population and NK Cell in Patients with Primary Lung Malignant Tumor and COPD Patients

Objective To explore the clinical value of lymphocyte sub-population and NK levels in patients with primary lung malignant tumor and patients with chronic obstructive pulmonary disease (COPD). Methods The percentage of CD3 +,CD4 +, CD8 +, CD19 + and NK cells were measured in 60 COPD patients and 50 primary lung malignant tumor patients and 40 normal control subjects using alkaline phosphatase antialkaline phosphatase (APAAP) assay, and CD4 +/CD8 + ratio was calculated. Results The levels of CD3 +,CD4 +,CD4 +/CD8 + and CD19 + cells were significantly lower in the patients with primary lung malignant tumor or COPD than those in the normal control subjects, which CD8 level was significantly higher (P0.05). Objective To investigate the therapeutic effects of citalopram on alleviating painful symptoms in type 2 diabetes mellitus(DM) patients with complication of painful peripheral neuropathy.Methods Thirty-eight type 2 DM patients with painful peripheral neuropathy were randomly divided into control group(eighteen patients) and treatment group(twenty patients).The diabetes control(including fasting blood glucose) and general health state(including body mass index,liver function,renal function) of patients were recorded.Patients in control group were given conventional drugs(including hypoglycemic drugs,anodyne,neurotrophy drugs,antiplatelet drugs),and those in treatment group received conventional drugs plus antidepressant drugs(citalopram,20mg,q.i.d.for 14 days).Hamilton Depression Rating Scale for Depression(HAMD-17) was used to assess the depressive symptoms,and numeric pain intensity scale(NPIS) was used to assess the pain intensity on baseline and the 1st,7th and 14th day.At the same time,the levels of analgesic agents were recorded.Results On the 7th and 14th day,the HAMD-17 scores in treatment group(22.14?8.92,18.69?9.4) were significantly lowered compared with that in control group(26.81?9.79,25.71?9.92),P

0.05).The stimulative index(SI)tested by MTT in MLR at stimulator-to-reactor cell ratio 1∶1,1∶10,1∶20,1∶40 and 1∶80 was 2.7,4.1,3.1,2.5 and 1.6,respectively.The proliferation of T cells stimulated by DCs could be detected at all stimulator-to-reactor ratio.Conclusion DCs could be successfully induced from peripheral blood monocyte in the patients with colorectal cancer by AIM-V serum-free media or by traditional FBS-contained media.AIM-V is more suitable for clinical immunotherapy because it can avoid the risk of allergic reaction for clinical purpose.AIM-V might take the place of serum-contained media to culture DCs.According to the results of MLR,DCs could stimulate allogenetic T cells proliferation at all stimulator-to-effector ratio,furthmore the optimal ratio is 1∶10 .

An adult patient with hypophosphatasia caused by compound heterozygous mutations in alkaline phosphatase,liver /bone /kidney(ALPL)gene was investigated through comprehensively reviewing the medical history and clinical records of the proband and her family members in order to better understand the disease.The proband and her older sister had mild decreased serum alkaline phosphatase level accompanied with frequently nontraumatic fractures at limbs and all the teeth fell off at the age of 20 and 7, respectively.Both of them carried a missense mutation c.407G>A(p.Arg136His)in exon 5 and a deletion mutation c.1318_1320delAAC(p.Asn440del)in exon 12 simultaneously.Other four family members were p.Arg136His mutation carriers and two members were p.Asn440del mutation carriers.We found that p.Asn440del mutation was associated with the oral disorders.In this family, compound heterozygous manifested more serious symptoms, while heterozygous showed relatively mild symptoms.In addition, it is necessary to differentiate it from primary osteoporosis and other diseases of disturbed bone mineralization.
8.Dominance analysis for relation between sleep quality and anxiety and depression emotion in patients with coronary heart disease

Objective To explore the relative importance of factors of sleep quality in predicting the anxiety and depression emotion. Methods Sleep quality and emotional condition of 112 patients with coronary heart disease were investigated with Pittsburgh sleep quality index(PSQI), self-rating anxiety scale (SAS)and self-rating depression scale (SDS). The relation between sleep quality and emotional condition was analyzed deeply by dominance regression analysis. Results After controlling the demographic vari-able of the sex,factors of the sleep quality such as sleep efficiency and function in the daytime had predict-ing effect on depression emotion;factors such as function in the daytime, sleep quality, sleep efficiency and sleep time possessed predicting effect on anxiety emotion. Conclusions According to different contribu-tion of various factors of sleep quality to anxiety and depression emotion, we should implement selective medical and nursing intervention during clinical work.
9.Test of simple effect of convalescent patients with schizophrenia supportive intervention program

Objective In view of the schizophrenia risk factors during the recovery,to explore a supportive intervention program suitable for the rehabilitative schizophrenic patients.Methods Totally 64 rehabilitative schizophrenic patients were divided into the experimental group and the control group with 32 cases in each group.All the patients received the same treatment with antipsychotic drugs.The patients in the control group received the routine health education.In addition,the patients in the experimental group received supportive intervention for 4 months.The patients were assessed by IBS,APGAR,GSES and SDSS before and after intervention.Results Main effect of group factor and time factor of GSES,SDSS,APGAR and IBS had statistical significances.There were significant interaction in the scores of SDSS,APGAR and IBS.Simple effect analysis demonstrated the scores of GSES,SDSS,APGAR and IBS in the experimental group were higher than those of the control group after the intervention.Except the score of GSES,there was no differences in other scales before and after the intervention of the control group.Conclusions Supportive intervention program for the rehabilitative schizophrenic patients have good practice guidance.It can improve the patients' self-efficacy,family function and social function,also can correct irrational beliefs and to help patients better adapt to society.
10.The clinical effect of skin flaP transPlantation to rePair traumatic tissue defects

Objective To investiGate the clinical effect of skin flap transplantation to repair traumatic tissue defects. Methods One hundred and twenty-eiGht cases with skin and soft tissure defects of foot injury were treated from January 2007 to December 20l2. AccordinG to the situation of foot soft tissue defects,there were dorsal metatarsal artery island flap in l7 cases,the medial plantar flap in 8 cases,the flap of lateral foot in 9 cases,the fibula free flap in l4 cases,the sural neurovascular fascia cutaneous island flap in l7 cases,the anterolateral femoral flap in 45cases,and the chest-umbilicus flap in l8 cases. Results One hudred and twenty-eiGht cases of the flap survived completely,and follow-up visit time lasted from 2 month to l year and the satisfactory results of appearance and functions were achieved in 98 cases. Twenty-eiGht cases of flap were somewhat swellinG. Conclusion DependinG on the soft tissue defect in the situation to choose the appropriate skin flap to repair the foot soft tissue defect after trauma have many advantaGes,such as low infection rate,shorter lenGth of stay and the function and pedal contour were reserved maximally .
