This paper discussed the concept and connotation of doctor -patient contradiction , and the ethics of doctor-patient conflicts reason carried on the thorough analysis .On this basis , from the macro and micro probes into the ethical path of resolving doctor -patient contradiction , specific include:the macro level , to accelerate the process of basic medical and health law ,public welfarereturn path , to protect the justice of medical treatment;Attaches great importance to the medical and health investment , increase the medical popular science knowledge propaganda , to improve medical limitations of cognitive science .Micro level, accelerate the reform of public hospi-tal modernization management , improve the medical staff occupational ethics quality , safeguard human dignity;at-taches great importance to the doctor -patient contradiction early warning mechanism and the patients interests ex-pression path construction , guarantee justified and lawful rights and interests of patients .
2.Empirical research of the impacts of the essential medicine system based on the double-difference model upon the first contact orientation of the residents

Objective To analyze the policy effect of first contact care options of residents under the essential medicine system.Methods The samples were divided into a treatment group and a control group depending whether the policy is implemented.Difference-in-differences model was used to estimate the changes in the First Contact Care choice of residents before and after the implementation.Results The net effect of policy is 6.15%,in which the resident category of the household head and per capita income were statistically significant.Conclusion Though the system promotes the first contact of residents for primary institutions,top-level design of the system calls for optimization,efficiency and equity of the health resource allocation of urban and rural health need to be improved,and the division of responsibilities of the two-level medical institutions needs to be clarified in first contact care system.
3.Application of the prospect theory in human resource development of township hospitals

Introduced are the concept and connotation of the prospect theory in behavioral economics,and the theory is used to explore the psychological characteristics and irrational decision behaviors of both supply and demand sides of talents in human resource development of township hospitals.The study found that the limited rational decision of people influenced the policy effectiveness of human resource policy for such hospitals,and the certainty effect strengthened the demand for restoration of government accountability and the breaking of staffing quota limits.The change and difference of reference point resulted in the lack of incentives and turnover of posts in such hospitals.The presence and change of the mental account caused unscientific human resource development.Based on such findings,the prospect theory can provide good explanatory power and theoretical enlightenment for such development.
4.Study on health policy instrument options for sinking high-quality medical resources from cities to primary care

Objective To study and identify factors and evaluation criteria for optional health policy instruments for policy making reference.Methods A field survey was made to the10,000 primary care doctors to rural households campaign in Jining city,covering 453 doctors sent to villages,294 village doctors,and 572 rural residents,with the data obtained subject to descriptive analysis.Results In terms of equity of the campaign,97.8% of the rural residents have entered into agreements,and 1.715 million of free visits have been provided by family doctors; in terms of economics,most of the superior doctors,rural doctors and rural residents believed that the policy could encourage high quality resources to sink to rural areas,and enhance the competence of public health services by rural doctors.In terms of the adaptability,60.4 % of the superior doctors and 92.7% of the rural doctors hold that they are competent for health care role,but 51.8% of the superior doctors were found not used to the current working mode.In terms of the enforceability,main complaints of superior doctors and rural doctors were excessive workload,overlong working hours,insufficient medical equipments,and reduced institutional revenue In terms of acceptability,90.4% of the rural residents are supportive Conclusion The promotion of such a policy calls for a combination of compulsory,voluntary and mixed policy instruments in sinking the resources,with appropriate selection and optimization of such instruments,to ensure outcomes of the policy.
5.Effects of carbachol on gastric emptying and gastric blood flow in oral fluid resuscitation of burn shock in rats

Objective To observe the effects of carbachol(a cholinergic agent)on gastric emptying and gastric blood flow in resuscitation of early shock with oral administration of glucose-electrolyte solution(GES)in scalded rats.Methods Seventy-eight male Wistar rats were randomly assigned into five groups:shame scald(n=6),scald without fluid resuscitation(n=18),scald resuscitated with oral GES alone(n=18),scald resuscitated with oral carbachol alone(CAL,n=18),and scald resuscitated with oral carbachol plus GES(GES/CAL,n=18).About 35% TBSA scalded injury on back of rats was produced with boiling water(100℃,15s)in the latter four groups.GES(2ml/kg,which is equivalent to half volume of fluid infusion in first 24 hour according to Parkland' formula)was given twice in two-hour intervals by gavage beginning from 30min after the injury in GES and GES/CAL group.Carbachol(60?g/kg)was gavaged 30min after the injury in CAL and GES/CAL group.The gastric emptying rate was determined by phenol red emptying method and gastric blood flow was measured with laser Doppler at 2h,4h and 6h after the scald injury.Results The gastric emptying was obviously delayed and gastric blood flow was significantly reduced in scald animals compared with those in sham group(P
6.Effects of oral fluid resuscitation on circulatory oxygen dynamic parameters in dogs with burn shock

Objective To investigate the effect of oral fluid resuscitation on circulatory oxygen dynamic parameters in dogs with burn shock.Method Eighteen male Beagle dogs were surgically prepared for the cannulation of carotid cartery and jugular vein,subjected to flame injury 50%total body surface area(TBSA)with fullthick ness 24 hours later,and they then randomly divided into 3 equal groups.The oral fluid resuscitation group (OR group)was intragastrieally injected with infusion of glucose-electrolyte solution(GES)according to parkland formula 0.5h after burn with a dose of 4mL·kg-1·% TBSA-1.Intravenous (IV) GES resuscitation group (VR group)was intravenously injected with infusion of GES as the same dose as OR group,and no-fluid resuscitation (NR)group did not receive any treatment during the first 24 hotrs.In the second 24 hours,all dogs received Ⅳ fluid resuscitation.At the end of 72 hours of injury.the mortality in each group was recorded.The mean arterial arterial pressure(MAP),hematocrit(HCT)and blood lactic acid(LAC)were determined,and blood gas analysis was evaluated for oxygen delivery(DO2),oxygen consumption(VO2)and oxygen uptake(O2ext)before injury and 2,4,8,24,48 and 72 hours after injury.Results Burn injury resulted in a 77.1%decrease in MAP,and a 48.5% increase in HCT and 533.7%increase in LAC in NR group,followed by pngressively lowering of DO2,VO2 and Oext till all animals died with in 24 hours after burn.MAP and HCT levels oftwo resuscitation groups gradually returned to the pre-injury levels within 72 hours after burn,but the LAC levels sill remained significantly higher than the pte-injury levels(P<0.01).The MAPs of OR group were higher at corresponding intervals within 24 hours post burn than those of NR group(P<0.01),but they were lower than those of VR group(P<0.01).The serum LAC in OR group was markedly lowered than that in NR group,but it was higher than that in VR group.Twenty-four hours after burn injury,the DO2 level in OR group showed no significant differences compared with that of the VR group,but the levels of the VO2 and Oext were still much lower than those of VR group (P<0.01).At the end of 72 hours,3 dogs of NR group died and none of IV group died.Caadusions Oral fluid resuscitation improves oxygen dynamic,alleviates hyperlactacidemia and reduces the mortality of animals with severe burn shock.
7.Safety and effectiveness of ozone versus hyaluronic acid for treating knee osteoarthritis:a Meta analysis

Objective To compare the safety and effectiveness between ozone (O3) and hyaluronic acid (HA) in treating knee osteoarthritis(KOA) by using the meta analysis method.Methods The relevant randomized controlled trials(RCTs) in PubMed,Cochrane Library (issue 1,2016),Embase,CNKI,CBM,VIP,and Wan-Fang databases were retrieved from their establishment to January 23,2016.Two reviewers independently screened the literatures,extracted the data and evaluated the quality of the included RCTs.The results were performed the statistical analysis by using the RevMan5.3 and Stata13.0 software.Results Twenty RCTs involving 2 136 KOA patients were included.Compared with the HA treatment of KOA,the O3 treatment had higher treatment effective rate[odds ratio(OR) =2.78,P<0.01],and better pain relief effect[at 1,3,6 month after treatment:mean difference(MD) =-0.25,-0.71,-1.70,P<0.01].There were no statistically significant differences in complications between the two treatment methods[OR=0.84,P=0.56].Conclusion Current evidences indicate that the short-term therapeutic effect of O3 for KOA is superior to HA,and the safety is similar.
8.The effect of hemodynamics on 1.8 % hypertonic electrolyte glucose solution in enteral resuscitation of burn shock

Objective To study the resuscitative effect of hypertonic electrolyte glucose solution (HEGS) on the haemodynamics pa-rameters in enteral resuscitation of burn shock. Methods Eighteen beagle dogs with 35% TBSA third degree burned were used in this stud-y. They were random divided into no-resuscitation group (NR group), enteral resuscitation with HEGS group (EH group) and intravenous resuscitation with isotonic electrolyte glucose solution (lEGS) group (Ⅱ group). The fluid resuscitation was given from half an hour after TBSA). The haemodynamics parameters (cardiac index, mean arterial pressure, intrathoracic blood volume index, and systemic vascular re-sistance index) were continuously assessed by PICCO. Result The cardiac output index reduced markedly after bum in the three groups, and then returned after 2h in two resuscitation groups, which were higher than that in the NR group( P<0.05). The mean arterial pressure reduced in the three groups, which was higher in the two resuscitation groups than that in NR group(P < 0.05). The intrathoracic blood volume index was rapidly reduced in the three groups. It returned in EH group from 2 hours after burned, which was still higher than that in the other two groups (P < 0.05). Meanwhile the systemic vascular resistance index was increased quickly. It was reduced in EH group from 2 hours after burned, which was lower than that in the other two groups (P < 0.05). Conclusion The results show that it was feasible for 35% TBSAⅢ° burn-injury dogs to be resuscitated with 1.8% hypertonic electrolyte-glucose solution by enteral, which can markedly reduce liquid quantity needed.
9.Study on the correlation factors of residents in preferentially using essential medicines based on Andersen behavior model

Objective To analyze the correlation factors that promote or impede the residents to preferentially use essential medicines.Methods Adopting stratified random sampling method,1 700 households selected from 5 cities of Shandong province were investigated with a questionnaire.The framework of Andersen behavior model of health service utilization was used as the framework,with such methods as descriptive analysis and univariate logistic regression models for the analysis and evaluation of relevant information.Results The residents′ tendency factor,ability factor and environmental factor influence their preference to use essential medicines,while the requirement factor plays a minimal role.There was a significant difference for the preference of combined medication,first visit preference and self-medication experience,the efficacy and policy response of essential medicine from logistic regression analysis.Conclusion At present,the government should focus on the construction of the formation mechanism of the residents′drug use behavior and the policy response mechanism of essential medicine system.
10.Research on the source of endothelial cells in tumor vessels by A 549 tumor model with GFP nude mouse

Objective To explore the source of endothelial cells in tumor vessels by A 549 tumor model with GFP nude mouse.Methods To establish the A 549 lung cancer models with GFP nude mice, expression of CD 31 was determined by immunofluorescence to label tumor vessels; to observe and take a picture of the tumor frozen section by confocal microscopy and invert microscope;expression of GFP in tumor vessels was determined by immunohistochemistry. Result The results of immunofluorescence showed:Tumor interstitial vascular endothelial cells or endothelial cells clusters and micro-vascular lumen size and shape are clearly visible by immunofluorescence, and part of vessels with no obvious lumen or irregular lumen.We can see green fluorescent in tumor cells of tumor tissue and endothelial cells which form of tumor vessels.The results of immunohistochemistry showed: expression of GFP was determined in cytoplasm of tumor stromal cells and endothelial cells in tumor vessels.Conclusion The endothelial cells which formed tumor neovessels that derived from GFP nude mice partly and the other part derived from tumor cells.
