1.The influence of statins on the blood inflammatory factors and fatty acid of erythrocyte membrane inhypercholesterolemic patients.

3.64 mmol/L)received 8-week double blind comparisons treatment.After treatment we randomly selected 15 patients for each group.The fasting serum lipid was measured at the end of 4-week and 8-week,and the serum hs-CRP and IL-6 were measured too.Meanwhile seven hypercholesterolemic patients from rosuvastatin group and seven ortholiposis out-patients were included.These patients were treated with statins for 4 weeks.The erythrocytes before and after treatment were kept to be measured the FA.Results (1)Compared with the pretreatment,the level of LDL-C,TC,hs-CRP remarkably reduced in two treatment groups at the end of 4-week therapy.These inflammatory parameters did not further reduced,but hs-CRP reduced further at the end of 8-week therapy.(2)The percent age of SFA was obviously higher in hypercholesterolemic group than ortholiposis group.On the other hand,PUFA and ?-3 FA were obviously lower.After 4-week treatment the percent age of SFA reduced,while PUFA and ?-3 FA increased significantly in hypercholesterolemic group.Conclusion (1)Statins reducing inflammatory factors is independent of lipid-modifying ability.(2)Rosuvastatin treatment can effectively reduce the level of LDL-C and TC,and no significant difference in the effect is observed between two treatment groups.(3)The erythrocyte membrane FA proportion was obviously different in hypercholesterolemic group from ortholiposis group.Statins treatment could make the proportion of FA in erythrocyte membrane restore normal.
2.Nutritional support for human immunodeficiency virus-infected patients during perioperative period

Objective To investigate the efficacy of nutritional support for human immunodeficiency virus (HIV)-infected and malnutritional patients during perioperative period.Methods The clinical data of 283 HIV-infected patients who were admitted to the Shanghai Public Health Clinical Center from October 2008 to February 2012 were retrospectively analyzed.All patients were divided into malnutritional group and non-malnutritional group according to nutritional condition before operation.Twenty-eight patients with malnutrition (7 patients did not receive perioperative nutritional support due to emergency operation,and they were excluded from the study) and 248 patients in normal nutritional status were enrolled in the study.Nutritional support was given for 5-7 days before operation for patients with malnutrition,and all patients received nutritional support after operation.Differences in the immune function,nutrional status,the incidences of surgical incisional infection and sepsis,and perioperative mortality between the 2 groups were compared.Measurement data were analyzed using the paired t test,and the enumeration data were analyzed using the chi-square test.Results The levels of CD4,CD8 and hemoglobin in patients with malnutrition were (205 ± 24)× 106/L,(559 ± 55)× 106/L and (103 ± 24)g/L after perioperative nutritional support,which were significantly higher than (150 ± 33) × 106/L,(491 ± 45) × 106/L and (97 ± 19)g/L before nutritional support (t =-2.561,-1.302,-1.349,P < 0.05).The levels of CD4,CD8 and hemoglobin were (197 ± 43) × 106/L,(547 ± 52) × 106/L and (103 ± 22) g/L at postoperative day 14,which were decreased compared with those after nutritional support,while no significant difference was detected (t =-1.108,0.600,-0.148,P > 0.05).The incidence of incisional infection in patients with malnutrition was 42.9% (12/28),which was lower than 45.2% (112/248) of patients in normal nutritional status,with no significant difference (P >0.05).The incidence of postoperative sepsis and perioperative mortality were 75.0% (21/28)and 3.6% (1/28) in patients with malnutrition after operation,which were significantly higher than 32.7% (81/248)and 0.8% (2/248) in patients in normal nutritional status (P < 0.05).Conclusion The mobidity of HIV-infected patients is high,and nutritional support for HIV-infected patients during perioperative period can improve their immunity.
3.Effect of rosuvastatin on serum hsCRP, IL-18 levels in patients with acute myocardial infarction

Objective To observe the effect of rosuvastatin on serum hs-CRP,IL-18 levels in patients with acute myocardial infarction.Methods By randomized,double-blind,controlled study,102 patients with acute myocardial infarction were randomly divided into the treatment group(rosuvastatin 10mg/d,continuous medication 14d) and the control group(not used rosuvastatin,other treatment and care were same with the treatment group).Before treatment,24h after treatment,after 2 months of follow-up,the serum hs-CRP and IL-18 levels were detected and compared.Results After treatment,the serum hsCRP level increased and then decreased,24h after treatment,the serum hsCRP level of the treatment group increased to (15.54 ±2.51) mg/L,which was significantly lower than the control group (19.26 ±.2.92) mg/L (t =4.65,all P < 0.05).2 months after treatment,the serum hs-CRP levels of the two groups were decreased to (3.21 ± 1.39) mg/L and (7.67 ± 2.07) mg/L,the difference was statistically significant (t =5.54,4.63,all P < 0.05).After treatment,the serum IL-18 level decreased,24h after treatment,the serum IL-18 level ofthe treatment group decreased to (29.13 ±6.34)pg/L,which was significandy lower than the control group (33.01 ± 7.34) pg/L(t =3.59,P < 0.05).2 months after treatment,serum IL-18 levels of the two groups were decreased to (27.52 ± 5.33) pg/L and (32.01 ± 6.24) pg/L,the difference was statistically significant (t =3.87,3.28,P <0.05).Conclusion Rosuvastatin can significantly reduce the serum hsCRP and IL-18 levels in patients with acute coronary syndrome,it has better anti-inflammatory effect and can be used as a new therapeutic target for acute myocardial infarction.
4.Preliminary study on the expression and clinicopathological significance of VEGF-C mRNA in prostate cancer

Objective Prostate cancer is one of the most common cancer in males .The article was to investigate the expres-sion and clinicopathological significance of VEGF-C mRNA in prostate cancer , finding the molecular marker for early diagnosis and prognosis assessment of prostate cancer . Methods TaqMan real-time polymerase chain reaction ( PCR) analysis was used to detect the expression of VEGF-C mRNA in 3 prostate cancer cell lines(PC-3, DU145 and LNCap), 32 cases of prostate cancer (Pca) sam-ples and 15 cases of benign prostatic hyperplasia (BPH).Analysis was also made on the correlations of VEGF-C mRNA expression, clinicopathological features and prognosis . Results High levels of VEGF-C mRNA were detected in PC-3 ( 153 .31 ±26 .24 ) and DU145(194.62 ±41.36)compared to LNcap(1.00 ±0.00).The expression of VEGF-C mRNA in prostate cancer tissues was 3.43 folds higher than that in the benign prostatic hyperplasia tissues ([13.67 ±1.95] vs [11.89 ±1.63], P=0.004).The high expres-sion of VEGF-C mRNA in prostate cancer was associated with high Gleason score ( P =0.004 ) and lymph node metastasis ( P =0.015).In patients with high expression and low expression of VEGF-C mRNA, the 3-year survival rate was 12.5%and 40.0%re-spectively(P=0.033). Conclusion The VEGF-C mRNA expression may be related to the biological behavior of prostate cancer .It is suggested that VEGF-C mRNA can be used as a prognostic marker for prostate cancer .

6.Content Determination of Oleanolic Acid and Ursolic Acid from Different Medicinal Parts in Tibetan Medi-cine Pterocephalus hookeri by UPLC-PDA

OBJECTIVE:To establish the method for the determination of oleanolic acid and ursolic acid in different medicinal parts of Tibetan medicine Pteocephalus hookeri,and compare the differences among the different parts. METHODS:The contents of oleanolic acid and ursolic acid from different medicinal parts(whole plant,aerial part,underground part)of P. hookeri were de-termined by UPLC-PDA. The separation was performed on Acquity UPLC HSS T3 column(150 mm×2.1 mm,1.8 μm)with mobile phase consisted of methanol-0.1 mol/L ammonium acetate(88:12,V/V)at the flow rate of 0.2 mL/min. The detection wavelength was set at 210 nm,and column temperature was 30 ℃. The sample size was 5 μL. RESULTS:The linear ranges of oleanolic acid and ursolic acid were 10.65-1065 μg/mL (r=0.9996) and 18.8-1880 μg/mL (r=0.9994),separately. The recoveries were 96.95%(RSD=1.24%,n=9) and 98.12%(RSD=2.13%,n=9),separately. RSDs of precision,stability and reproducibility tests were all less than 3%. The contents of oleanolic acid and ursolic acid from different medicinal parts in P. hookeri were in de-scending order of aerial part>whole plant>underground part;the average total content of oleanolic acid and ursolic acid from whole plants was 0.35%,the aerial part reached 0.56% and underground part was 0.09%. CONCLUSIONS:The method is rapid, accurate and reproducible,and it is suitable for the content determination of oleanolic acid and ursolic acid in different medicinal parts of Tibetan medicine P. hookeri. The contents of oleanolic acid and ursolic acid from aerial part of P. hookeri are higher than whole plant and underground part. It is suggested to use aerial parts of medicine.
7.The evaluation and treatment of cyclodialysis clefts

The cyclodialysis cleft is a rare disease characterized by ocular hypotony and vision loss.Gonioscopy,ultrasound biomicroscopy and anterior segment optical coherence tomography are used to evaluate the cleft before treatment.The evaluation of cyclodialysis clefts provide a basis for the selection of treatment and the evaluation of effect.Current popular therapeutic approaches for cyclodialysis clefts include medical management,laser photocoagulation,transscleral diathermy,cryotherapy,cyclopexy and so on.Medical treatment is effective in small-size cyclodialysis cleft.But the indications and course of medical treatment remains controversial.Although laser photocoagulation,transscleral diathermy and cryotherapy are effective to treat cyclodialysis clefts in some cases,there are some risks,such as ocular damage,uncertainty about the effect and so on.Cyclopexy is the principal method for cyclodialysis clefts that failed to respond to conservative management,including extraocular cyclopexy,intraocular cyclopexy and cyclopexy under endoscopic.The modified cyclopexy is effective which reduces the surgical risk.For keeping the lens,intraocular cyclopexy and cyclopexy under endoscopic are not suitable.Vitrectomy combined with other method for cyclodialysis is effective for cyclodialysis clefts which combined with serious posterior segment damage.
8.The prognostic value of gray-white matter ratio on brain computed tomography in comatose adult survivors from cardiac arrest

Objective To evaluate the correlation between the gray-white matter ratio (GWR) and the outcomes of comatose adult survivors from cardiac arrest (CA) in Chinese.Methods Sixty-one CA patients checked with CT scans within 72 hours of resuscitation from January 2011 to January 2016 were included in this single-center retrospective study.Gray and white matter density (Hounsfield units) were measured,and the GWRs were calculated according to previous studies.The prognostic values of the GWRs in predicting poor outcomes (Cerebral Performance Category 3-5) were analyzed.Results The density values of gray matter were significantly higher in the good outcome group than those in the poor one.All GWRs were significantly higher in the good outcome group (P < 0.05).A GWR (basal ganglia) < 1.18 predicted poor outcomes with a sensitivity and specificity of 50.0% and 88.2%,respectively (P =0.012).Conclusions Low GWRs,determined from brain CT scans in comatose CA patients after resuscitation,were associated with poor neurological outcomes.GWR determination from brain CT can be a useful indicator for outcome prediction aiding in an optimal clinical decision process in comatose survivors from CA.

Objective To investigate the pathogen spectrum and analyze the associated risk factors of pulmonary moniliasis in the patients with liver failure.Methods A retrospective analysis of pulmonary moniliasis complicating liver failure(n=150) was performed by case-control study on the data of patients admitted to PLA 302 Hospital from Jan.1986 to Nov.2007.A cohort of 154 patients with liver failure but without fungi infection(n=154) served as the control group.By analyzing the epidemiological and clinical data,the spectrum of fungi was identified,and the risk factors in pulmonary moniliasis pneumonia inpatients with liver failure were identified.Results The number of patients with acute on chronic liver failure(69/150,46.0%) were greater than patients with chronic liver failure(78/150,52.0%),while the number of patients suffering from moniliasis pneumonia was smaller than that of control group(20/154,13.0% and 129/154,83.8%,respectively,P

Aim To investigate the contribution of cardiac L-and L/T-type Ca~2+ channels in the calpain mediated myocardial damage following myocardial infarction(MI).Methods Rat MI model was established by permanent ligation of the left coronary artery, infarcted rats were orally treated with placebo, amlodipine(L-channel blockade, 4 mg?kg~-1 ?d~-1 ) or mibefradil(L/T-Channel blockade, 10 mg?kg~-1 ?d~-1 ) beginning 7 d before induction of myocardial infarction. Protein levels of u-calpain and m-calpain were measured 1,3,7 and 14 d post coronary occlusion in the noninfarcted and infarcted myocardium.Infarcted size,left ventricular dilation were determined in picrosirius red stained hearts.Results Myocardial infarction induced an up regulation of u-calpain protein and activity in the noninfarcted myocardium(maximum day 14 days post infarction), whereas protein and activity of m-calpain were increased in the infarcted myocardium 3 d post infarction. Amlodipine inhibited protein up-regulation of u-calpain and decreased left ventricular dilation and interventricular septal thickness. Mibefradil attenuated protein up regulation of m-calpain 14 days post infarction, reduced infarct size more obviously.Conclusions Infarction-induced cardiac hypertrophy was accompanied by an up-regulation of u-calpain, whereas m-calpain was up-regulated in the infarcted myocardium in the processing of cardiac infarcted pathogensisi. Cardiac L and L/T-type Ca~2+ channel blockade differentially reduced post infarction remodeling associated with selective inhibition of cardiac u-calpain and m-calpain, respectively.
