Objective To evaluate the clinical application, dosimetry and moving penumbra of traversing beam therapy with moving table technique for total spinal cord irradiation (TSCI). Methods Five patients received 60 Co TSCI traversing beam therapy with moving table which was developed by the Zhejiang Cancer Hospital and Zhejiang University. The maximum moving distance of the table was 60 cm. The dose was measured by ionization chamber. The radiation field size was 5 cm?10 cm and the table moved forward and back. Results Traversing beam therapy was able to increase the percentage depth dose with the dose distribution homogeneous and much better than that of multiple field irradiation. None of these 5 patients developed acute nervous system reactions. Conclusion The 60 Co traversing beam therapy is advantageous in dose distribution, more effective and safer than the traditional irradiation in TSCI .
2.Comparison of therapeutic effects among three intramedullary fixation system for treatment of tibial fracture in 79 cases

A total of 79 tibial fracture patients (47 males and 32 females; 16 to 73 years old) were selected from Department of Orthopedics, Huangbu Hospital of Huidong City, including 19 cases of open fractures and 60 of closed fractures. Forty-eight fractures in 79 patients were fixed with interlocking nails, 19 by revolving self-locking nail, and 12 by auto compressive locking nail. Operations were all successful. The average operation time was (102 ?10) minutes in the interlocking group, (65?10) minutes in the revolving self-locking nail group and (100?10) minutes in the auto compressive locking nail group. The patients were followed up for 6-10 months. In the interlocking group, one breakage of the screws was found, with one superficial infection and the other 3 achieved delayed bone fracture. The healing time of interlocking group was (21.0?3.2) weeks and the excellent rate of function recovery was (85.42?5.20)% by Johner-Wruh. In the revolving self-locking nail group, one developed delayed bone facture and the other one achieved dislocation; the healing time was (20.0?3.6) weeks and the excellent rate of function recovery was (84.21?4.10)%. In the auto compressive locking nail group, one had loosening screw and the other one achieved superficial infection. The healing time was (17.0?2.8) weeks and the excellent rate of function recovery was (91.67?3.70)%. 3.Reduction and internal fixation for atlantoaxial dislocation with posterior screw-rod fixation system

Objective To evaluate the outcome of posterior screw-rod fixation system in reduction and internal fixation of atlantoaxial dislocation. Methods A retrospective study was done on 27 patients with atlantoaxial instability including 18 male and 9 female (at age range of 13-51 years, mean 31 years) from January 2007 to May 2009. There were 11 patients with chronic odontoid fractures, five with isolated bone odontoid, seven with transverse ligament rupture of atlas and four with rheumatoid arthritis. Skeletal reduction was performed in all the patients. The anterior atlantodens interval (ADI)ranged from 8 mm to 15 mm, average 11 mm. All the patients underwent an intraoperative reduction by posterior C1 lateral mass and C2 pedicle screws with rod fixation. According to American Spine Injury Association (ASIA) impairment scale, there were eight patients at grade B, 15 at grade C and four at grade D. Results All patients were followed up for 6-24 months (average 13 months), which showed that the neck symptoms were improved, with bony union. The ADI was reduced to 2-4 mm (average 2.8 mm)postoperatively. Postoperative ASIA scale was grade C in four patients, grade D in 12 and grade E in 11.There were no neurologic or vascular complications occurred, or no failure of the internal fixtors, pseudarthrosis or instability. Conclusion Posterior screw-rod fixation system has advantages of simple procedures, few complications and good results and can be used for intraoperative reduction of atlantoaxial dislocation.
4.Association of multiple anthropometric indices with in 944,760 elderly Chinese people

OBJECTIVES:
The aims of this study were to update the latest data on the prevalence of hypertension (HTN) in the elderly Chinese population and to assess relationships between new anthropometric indices and HTN.
METHODS:
Data were obtained from the Basic Public Health Service (BPHS) survey for Jiangsu Province, China. A total of 944,760 people aged 65 years and older were included in this study. Blood pressure was measured by trained investigators. Body weight, body mass index (BMI), waist circumference (WC), waist-to-height ratio (WtHR), conicity index (COI), body roundness index (BRI), and a body shape index (ABSI) were included in the analysis as anthropometric indices. Logistic regression analysis and restricted cubic splines were used to evaluate the association of anthropometric indices with HTN.
RESULTS:
The prevalence of HTN among elderly residents of Jiangsu Province was 64.7% (95% confidence interval, 64.6 to 64.8). After adjusting for multiple covariates, all anthropometric indices except ABSI showed significant non-linear positive dose-response associations with HTN across sex (pnonlinear<0.001). Among participants with BMI <28 kg/m2, abnormal weight, WC, WtHR, BRI, COI, and ABSI were positively associated with HTN.
CONCLUSIONS
The prevalence of HTN in the elderly in Jiangsu Province is gradually increasing. It is necessary to consider the combination of ABSI and COI with BMI for screening elderly individuals for HTN in follow-up prospective studies.
5.Epidemiological characteristics of HIV/AIDS patients newly received highly active antiretroviral therapy during 2005-2013 in Jiangsu province.

OBJECTIVETo analyze the epidemiological characteristics of the HIV/AIDS patients newly received highly active antiretroviral therapy (HAART) during 2005-2013 in Jiangsu province.
METHODSAccording to the baseline data of HIV/AIDS patients newly received HAART during 2005-2013, an Excel database was established and statistical analysis was performed using SPSS 16.0 software.
RESULTSThere were 5 788 HIV/AIDS patients newly received HAART during 2005-2013 and the number increased annually. 79.7% of these patients were from the local province, with the ratio of male to female as 4.69 : 1, mean age as 39.9 years old, 55.8% of them married, 52.0% of them were treated at CDC. 91.9% of them were infected through sexual contacts. Mean baseline CD4(+)T cell count of cases was 187 cells/µl. Median of the interval between the time of HIV confirmation and HAART initiation was 4.0 months. Proportion of the HIV/AIDS patients that were being male, elderly, unmarried, not local resident, treated in hospitals and with higher baseline CD4(+)T count etc. showed a trend of increase.
CONCLUSIONHIV/AIDS patients had the opportunity to start the HAART at early stage of the disease. However, many of them started the treafment late as the results of being diagnosed late. Programs related to both earlier detection and diagnosis on HIV cases should be strengthened.
