1.Value of elastography and fine flow imaging in differential diagnosis of cervical lymph nodes

Objective To explore the diagnostic value of ultrasonic elastography and fine flow imaging in differential diagnosis of cervical lymph nodes.Methods Totally 87 lymph nodes were examined with ultrasonic elastography,fine flow and conventional ultrasonography.The results were confirmed with postoperative pathology or fine-needle biopsy as well as follow-up.Using consistency check of Kappa and ROC curve to analyse the accuracy of three types of ultrasonic examination.Results Taking the results of postoperative pathology or fine-needle biopsy as well as follow-up as golden standards,the Kappa value of conventional ultrasonography was poor (Kappa =0.153,P =0.078),fine flow was moderate (Kappa =0.526,P <0.01) and ultrasonic elastography was better (Kappa =0.741,P <0.01).The area under ROC of ultrasonic elastography,fine flow and conventional ultrasonography was 0.899 (P <0.01),0.787 (P <0.01) and 0.568 (P =0.241),respectively.Conclusions Ultrasonic elastography is more valuable to differentiate malignant from benign lymph nodes.
2.The value of differential diagnosis of the configuration of QRS complex in lead aVR in patients with inferior wall myocardial infarction

Objective To investigate the value of differential diagnosis of the configuration of QRS complex in lead aVR in patients with inferior wall myocardial infarction. Methods The configuration of QRS in 52 patients with pathological Q-wave both in lead Ⅲ and aVF were analyzed and the result of selective coronary arteriography was compared. Results 13 patients with the configuration of QRS in lead aVR appeared rS ( s), while 10 patients appeared QS(qs) and 29 Q(q)r,correlated with 12,4 and 0 patients with coronary arteriography showed stenosis or occlusion lesion in fight coronary artery or left circumflex artery (χ2 = 35.56, P = 0.000). Conclusions The con-figuration of QRS in lead aVR is helpful to differential diagnosis of the patients with pathological Q-wave both in lead Ⅲ and aVF. Patients with the configuration of QRS in lead aVR appear rS(s) could be diagnosed as old myocardial infarction,but excluded from old myocardial infarction while appearing Q(q)r.
3.Effect of Tongxinluo capsule on the plasma C-reactive protein and endothelin-1 in acute coronary syndrome patients

Objective To study the effect of Tongxinluo on the plasma C-reactive protein(CRP)and endothelin-1(ET-1)in acute coronary syndrome(ACS) patients.Methods 100 patients with ACS were randomly divided into conventional therapy group and treatment group(conventional therapy+Tongxinluo gelatin capsule).The changes of CRP and ET-1 in the first day,7th and 14th day were observed.Results In the treatment group,CRP and ET-1 were significantly decreased in the 7th and 14th day(P<0.05,P<0.01),and there was significant decrease only in the 14th day(P<0.05)in the conventional therapy group.CRP and ET-1 levels in the treatment group were significantly different as compared with conventional thereapy group(P<0.01).Conclusion Tongxinluo capsule may protect blood vessel endothelium through inhibiting CRP and ET-1 to decrease the inflammatory response of endangium.
4.Experimental study of Gexia-Zhuyu decoction combined with cyclophosphamide on tumor interstitial in rats of EAC liver cancer

ObjectiveTo study the anti-cancer effects and mechanism of Gexia-Zhuyu decoction combined with cyclophosphamide on EAC liver cancer mice.MethodsChoose EAC liver cancer mice cell lines and production of animal models of tumor.The mice were completely randomized into blank group,model group,the Chinese medicine group,CTX group,Chinese medicine and CTX group.The blank group and model group were given normal saline 0.4 ml/d; traditional Chinese medicine group was administrated with Gexia-Zhuyu decoction (11 g/kg body weight ) 0.4 ml/d gavage; CTX group was given intraperitoneal injection of 2 mg/ml of cyclophosphamide 0.4 ml/d; and traditional Chinese medicine and CTX group was given Gexia Zhuyu decoction 0.4 ml/d orally and CTX intraperitoneal injection of 0.4 ml/d.Ten days after the administration,the body weight of mice and the general condition were tested,and morphology was observed under the light microscope electron microscope mesenchymal tumor angiogenesis situation.Results Gexia-Zhuyu decoction combined with cyclophosphamide in mice could improve appetite,gain weight,and greatly reduce the tumor stroma in the number of capillaries under the electron microscope together with significant pathological morphological changes.Conclusion Gexia-Zhuyu decoction combined with cyclophosphamide had anti-cancer effects which can improve the quality of life and inhibit the growth of tumor in interstitial capillaries.
5.Adipogenic capacity of CD54+/CD54- adipose-derived stem cells

BACKGROUND: Studies have shown that adipose-derived stem cells have pluripotent differentiation potential, but only 30%-40% of cells can differentiate into mature adipocytes with low adipogenic differentiation potential. Therefore, how to improve the adipogenic differentiation ability of adipose-derived stem cells is a key problem to be solved in the process of soft tissue regeneration. OBJECTIVE: To observe the relationship between the surface marker CD54 of rabbit adipose-derived stem cells and their adipogenic capacity, and to explore the adipogenic differentiation of CD54+/CD54- adipose-derived stem cells underthe same induction. METHODS: We successfully isolated and cultured the adipose-derived stem cells from inguinal subcutaneous fat pads (3 ml) of New Zealand white rabbits, aged 8-12 weeks, which were induced into multi-differentiation and used to detectsurface markers. We sorted the passage 3 adipose-derived stem cells by immunomagnetic beads and divided into two categories including CD54+ and CD54- adipose-derived stem cells. After 14 days of adipogenic induction, the cells in the two groups were subjected to oil red O staining and were compared by detecting the density of mature adipocytes and lipid droplet contenT.RESULTS AND CONCLUSION: The cultured adipose-derived stem cells possessed the characteristics of mesenchymal stem cells that could differentiate into mature adipocytes, osteoblasts and chondrocytes, with CD29, CD44, CD49d, CD54, CD73, CD90 and CD105 positive expression while CD31, CD34 and CD45 negative expression. Fourteen days after adipogenic induction, the density of mature adipocytes and the intracellular lipid droplet content in the CD54+ group were significantly higher than those in the CD54- group (P < 0.05). We also found that the mRNA expressions of PPARγ,ADD1, C/EBPα related to adipogenic differentiation in the CD54+ group were significantly higher than those in the CD54- group (P < 0.05). Taken together, CD54+ adipose-derived stem cells have excellent adipogenic differentiation capacity.
6.Echo-tracking technology for evaluating femoral artery endothelial function in patients with Grave's disease.

OBJECTIVETo assess the value of echo-tracking technology in evaluating endothelial function of the femoral artery in patients with Grave's disease.
METHODSThirty-four patients with Grave's disease patients and 30 normal adults as controls were recruited in this study. The intima-media thickness (IMT), arterial stiffness (β), pressure strain elastic modulus (Ep), arterial compliance (AC), pulse wave conducting velocity (PWVβ) and augmentation index (AI) parameters were examined using echo-tracking technology for evaluating the right femoral arterial elasticity.
RESULTSCompared with the control subjects, the patients with Grave's disease showed significantly increased β, Ep, and PWVβ and significantly decreased AC (P<0.05), but the argumentation index were similar between the two groups (P>0.05). In patients with Grave's disease, β and Ep were positively correlated with FT3, FT4, TT3, TT4, and PWVβ was positively correlated with FT3 and FT4.
CONCLUSIONSEcho-tracking technology can provide more accurate quantitative evidences for early diagnosis of femoral artery endothelial dysfunction in patients with Grave's disease, but the influence of procedural factors on the measurement accuracy should be considered in the evaluation.

8.Rapid chemome profiling of Artemisia capillaris Thunb.using direct infusion-mass spectrometry

Background:As one of the most popular traditional Chinese medicines (TCMs) for the treatment of various liver diseases,virgate wormwood herb (Artemisia capillaris Thunb.) has a long application history in TCM practices.It has been well established that the chemical composition is responsible for the pronounced therapeutic spectrum of A.capillaris.Although they are comprehensive,the time-intensive liquid chromatography coupled to tandem mass spectrometry (LC-MS/MS) assays cannot fully satisfy the analytical measurement workload from many test samples.Direct infusion-MS/MS (DI-MS/MS) may be the optimal choice to achieve high-throughput analysis if the mass spectrometer can universally record MS2 spectra.Methods:According to the application of gas phase ion fractionation concept,the MS/MSALL program enables to gain MS2 spectrum for each nominal m/z value with a data-independent acquisition algorithm via segmenting the entire MS1 ion cohort into sequential ion pieces with 1 Da width,when sufficient measurement time is allowed by DI approach.Here,rapid clarification of the chemical composition was attempted for A.capillaris using DI-MS/MSALL.A.capillaris extract was imported directly into the elec-trospray ionization interface to obtain the MS/MSALL measurement.After the MS1-MS2 dataset was well organized,we focused on structural characterization through retrieving information from the available databases and literature.Results:Twenty-six compounds were found,including 12 caffeoyl quinic acid derivatives,7 flavonoids,and 7 compounds belonging to other chemical families.Among them,24 ones were structurally iden-tified.Compared with the LC-MS/MS technique,DI-MS/MSALL has the advantages of low-costing,solvent-saving,and time-saving.Conclusions:Chemical profiling of A.capillaris extract was accomplished within 5 min by DI-MS/MSALL,and this technique can be an alternative choice for chemical profile characterization of TCMs due to its extraordinary high-throughput advantage.
9.Effectiveness and annual cost-effectiveness analysis of extracorporeal cardiopulmonary resuscitation in 54 adults

Objective:To analyze the effectiveness and annual cost-effectiveness of extracorporeal cardiopulmonary resuscitation, ECPR) in adults.Methods:Totally 60 patients received ECPR from April 2015 to March 2020 in Emergency Medicine Department of the First Affiliated Hospital of Nanjing Medical University were retrospectively analyzed. The patients were grouped by discharge survival/hospital death and shockable/unshockable initial rhythm. Age, gender, initial rhythm, survival rate, ECMO treatment time, time-to-death, length of stay and hospitalization costs were analyzed. All discharged survivors were followed up for 1 year, then cost-effectiveness analysis was performed using total cost of ECPR as the cost and 1-year survival rate as the effect.Results:Fifty-four adult patients with ECPR were enrolled, and 17 (31.5%) patients survived and discharged, of whom 15 (88.2%) patients had good neurological outcomes and survived at 1-year follow-up. The median ECMO time was 5 ( IQR 1-8) d, time-to-death was 4 ( IQR 1-9) d, length of stay was 10 ( IQR 3-18) d, total hospitalization cost was 209 122 ( IQR 121 431-303 822) RMB, and the daily cost was 23 587 ( IQR 13 439-38 217) RMB. The rate of shockable initial rhythm was significantly higher in the discharge survival group than the hospital death group. The survival rate of ECPR patients with shockable initial rhythm was significantly higher than that of patients with unshockable initial rhythm, and there was no difference in cost. Conclusions:ECPR is a resource-intensive treatment with a total cost of about 200 000 RMB. Moreover, the effectiveness and annual cost-effectiveness are superior for patients with shockable initial rhythm.
10.Early lactate is a novel prognostic indicator of prognosis in patients with extracorporeal cardiopulmonary resuscitation

Objective:To investigate the prognostic value of early lactate in patients with extracorporeal cardiopulmonary resuscitation (ECPR).Methods:A retrospective analysis was performed on the clinical data of patients with ECPR in the Emergency Medicine Department of The First Affiliated Hospital of Nanjing Medical University from March 2015 to August 2021. The age, sex, etiology, initial rhythm, prognosis, blood lactate and pH of patients with ECPR were collected, and their difference between the deceased and survived patients was compared.Results:Totally 95 patients were enrolled, with an average age of 47 years; male accounted for 69.5%, and the survival rate was 29.5%. There was no significant difference in age and sex ratio between the deceased and survived patients. However, the deceased patients had a significant lower rate of shockable rhythms (31.3% vs. 60.8%), a higher level of lactate [16.4 (11.2, 19.1) vs. 9.2 (3.2, 15.0), mmol/L], and a lower pH [7.01 (6.88, 7.23) vs. 7.37 (7.10, 7.43)] than the survived patients. Multivariate binary logistic regression analysis showed that shockable rhythm [odds ratio ( OR) = 0.295, 95% confidence interval ( CI): 0.118-0.739), lactate ( OR=1.159, 95% CI: 1.068-1.258) and pH ( OR= 0.017, 95% CI: 0.002-0.157) were independent risk factors for poor prognosis. Furthermore, a lactate level >24 mmol/L was the best threshold to predict mortality with a specificity of 100%. Combined application, the cutoff point was lactate level>16 mmol/L and pH <6.828. Conclusions:Shockable rhythm, higher early lactate and lower pH value are independent risk factors for prognosis in patients with ECPR. Early lactate > 24 mmol/L or lactate > 16 mmol/L companied with pH < 6.828 are novel indicators of the termination of ECPR.
