Results The adult giant young cell anemia majority in elderly patients of(34/48) 70.8% ,its cause a majority in gastrointestinal diseases (41/48) for 85.4% , among them 9 cases of chronic gastritis(18. 8% ) Gastrointestinal diverticula 8 cases, the digestive tract of release,duodenal ulcer and gastric 16.7% each 4 cases total resection for 8.3, hiatal hernia and gastric ulcer each 3 cases of gastric polyps, oral 6.3% lesions, jejunum resection 2 cases 4.1 percent of all the rest a few Crohn,Crohn's disease,intestinal lymphoma,chronic colitis each 1 case of 2.0%.The gastrointestinal lesions accounted for only 14.6% cases. Gelastic epilepsy is a special refractory epilepsy accompany with ictal laughing and/or crying which can be divided into cortical induced gelastic epilepsy and hypothalamic hamartomas induced gelastic epilepsy according to their epileptogenic focus.Currently, domestic scholars had lack of systematic understanding of the mechanism and clinical treatment involved in gelastic epilepsy.Recently, foreign scholars had achieved great progress in the research of gelastic epilepsy which included the ictal laughing mechanism in gelastic epilepsy, the primary molecular mechanism in hypothalamic hamartomas induced gelastic epilepsy, the secondary epileptogenesis mechanism in hypothalamic hamartomas induced gelastic epilepsy and the clinical treatment of gelastic epilepsy.Now, the study progress of pathogenesis and clinical treatment of gelastic epilepsy were reviewed.

Objective To establishe the reference range of low hemoglobin density (LHD ％) of health population in Humen District of Guangdong,and discuss the clinical application value.Methods Randomly selected from January 2015 to September 2016 in Humen Hospital of Dongguan City physical examination of 1 650 cases of healthy people as control group,and selected the same period by the clinical diagnosis of iron deficiency anemia (iron-deficiency anemia,IDA) group,67 cases of patients with IDA Sysmex XT-1800i fully automatic blood analyzer were used respectively to detect blood routine,and then based on the average haemoglobin concentration (mean corpuscular hemoglobin concentration,MCHC),calculate the LHD％,and the calculation results were analyzed.Results The control healthy crowd LHD％ value was 2.41％ ± 0.85 ％,95％ reference range was 0.74 ％ ～4.08％,male was higher than the female,the difference was statistically significant (t =3.209,P＜0.05).IDA patients before treatment LHD％ was 30.97％ ± 18.65％,significantly higher than the control group,the difference was statistically significant (t=19.536,P＜ 0.01),and with the level of hemoglobin (Hb) showed a negative correlation (r=-0.4313).LHD％ values after treatment in patients with IDA group were significantly lower,but the recovery speed was slower than the Hb and MCHC.By ROC curve analysis,when the cut-off of LHD％ value was 4.10％,the LHD％ for IDA early diagnostic sensitivity of 96.78％,specificity of 100％.Conclusion Established the reference range of low hemoglobin density (LHD ％) of healthy population in Humen district of Guangdong,and LHD％ is IDA good indicator of early diagnosis and curative effect of dynamic monitoring.

Objective To discuss the clinical features of primary epididymal rhabdomyosarcoma and review the diagnostic and therapeutic advances. Methods One 16-yesr-old male presented with right scro-turn swelling mass for 4 months. Physical examination revealed an ovoid and solid mass of 4.5×3.5×3.0 cn in right scrotum. The mass was hard and its surface was smooth without tenderness. Transillumination test was negative. There was no palpable inguinal lymph node. Pre-operative diagnosis was inflammatory nodule of right epididymis. Results Right epididymectomy was performed under caudal anesthesia une-ventfully. The pathological study showed that the neoplasm was composed of small round shape ceils with small and anachromasis nucleus. The immunohistochemical study showed that these cells were positive for myoglobin, aetin and desmin, but negative for smooth muscle actin. Electron microscopy showed that paral-leling thin filaments were found with no asrcomere-like structure in the intracytoplasm. The pathologic diag-nosis was right epydidymal rhabdomyosarcoma. The patient refused further therapy. A mass was found with rapid growth accompanied with pain in the right scrotum 2 months after the first operation and was clinically diagnosed as tumor recurrence. This patient had then undergone right radical orchiectomy. Frozen section analysis revealed negative surgical margin. The pathologic diagnosis was embryonal rhabdomyosarcoma re-currence. Patient underwent subsequent chemotherapy with ifosfamide, etoposide and vincristine. After one year follow-up, he was generally well without evidence of recurrence. Conculsions Primary embryonal rhabdomyosarcoma is a rare tumor with aggressive growth patterns. The diagnosis depends on the pathologi-cal examination because the clinical presentation is not specific. Combination therapy with radical orchiecto-my, chemotherapy and radiotherapy provides patients an excellent long-term prognosis.

BACKGROUND:Molars are characterized by multi-root, multi-root canal, multi-directional, different geometric shape and distribution. Single-root canal teeth post-core theory was used to guide molar repair in the clinic. It is easy to cause root canal perforation or vertical fracture due to excessive post preparation. Therefore, it is necessary to make further study and investigation in the design of fiber post-resin core for repairing molars. OBJECTIVE: To evaluate the clinical therapeutic effects of fiber post-resin core with different numbers of posts in the repair of molar residual roots and crowns. METHODS: A total of 54 human molar residual roots and crowns with sound root canal filing in 48 patients were selected and restored with fiber post of different numbers and resin core as wel as complete coronal restoration. There were 17 cases (20 samples) restored with single fiber post core, 16 cases (18 samples) restored with double fiber post cores, and 15 cases (16 samples) restored with three fiber post cores. They were folowed up for 24 months and the repair results were compared. RESULTS AND CONCLUSION:After 24 months of folow-up, the success rates were 85%, 94% and 94% in the single fiber post, double fiber post and three fiber post groups, respectively, and no significant difference was detected among the three groups. Five failures were observed among 54 teeth: three cases of fiber post shedding in the single fiber post group, one case of fiber post shedding in the double fiber post group, and one case of fiberpost shedding in the three fiber post group, and no root fracture occurred. Three kinds of fiber post-resin cores for repairing molar residual roots and crowns can get a better short-term clinical result. The repair effects were not different because of the different numbers of fiber posts.

Objective To study the relationship of serum leptin and inflammation with nutrition status in continuous ambulatory peritoneal dialysis(CAPD) patients.Methods 30 CAPD patients and 30 healthy controls were enrolled in the study.Leptin was measured by ELISA,inflammatory factors C reactive protein(CRP),IL-6,TNF-?.These patients were classified to three nutritional categories based on SGA normalnutrition,mild-to-moderate malnutrition,and severe malnutrition.Results CAPD group had significantly higher serum leptin,peritoneal dialysis fluid leptin,insulin,CRP,IL-6 and TNF-?,lower serum Alb than the control group.IL-6 was significantly positive correlation with TNF-?.Serum leptin was no correlation with Alb,CRP,IL-6 and TNF-?.Conclusion Serum leptin and inflammation are closely related to the occurrence of malnutrition in CAPD.

Objective To investigate the roles of oxidative stress and inflammatory reaction in pathogenesis of endothelial dysfunction and atherosclerosis in patients with chronic renal failure(CRF).Methods 63 CRF patients were divided into non-dialysis group and hemodialysis group and 27 healthy volunteers were involved in the study.The levels of plasma malondialdehyde(MDA) were determined by thiobarbituric acid reaction,the activity of glutathione peroxidase(GSHPx) was mearsured by spectrophotometry,plasma nitric oxide(NO),high-sensitive C-reactive protein(hs-CRP),endothelin-1(ET-1) and tumor necrosis factor(TNF)-? were determined by sensitive enzyme-linked imunosorbent assay(ELISA).Results Levels of plasma MDA,hs-CRP,IL-6,TNF-?and ET-1 in CRF patients were higher than those in healthy controls,while NO,GSHPx were lower than those in healthy controls and hemodialysis aggravated these changes.Multivariate stepwise regressive analysis showed that ET-1,vWF and NO were related to MDA,TNF-?,hs-CRP,Cr(R2=0.502,0.631,0.476,respectively,P

[Objective] To search a reasonable approach to the treatment of nasopharyngeal carcinoma (NC) with traditional Chinese medicine integrated with western medicine. [ Methods ] Seventy - six cases of NC were randomized into Group A and Group B. Group A ( n = 39) was treated with herbs for activating blood and removing blood - stasis, nourishing yin and clearing heat during radiotherapy and herbs for replenishing qi and nourishing yin, generating body fluid, moistening dryness and clearing heat after radiotherapy. Group B ( n = 37) was treated with radiotherapy alone. Short - term therapeutic effect, long - term survival rate, the relief of main symptoms, the reduction of cervical lymph node metastasis and side effect of radiotherapy in the two groups were observed. Alemtuzumab (Campath) was successfully injected in 21 kidney transplant patients,7 islet transplant patients and 1 simultaneous kidney and islet transplant patient for either prevention or treatment of graft rejection.Prophylactic administration was successfully completed in all patients without discontinuation.Adverse events were not observed in 11 patients (38%),but hypertension in 18 patients (62%),shivering in 3 patients (10.3%),high fever in 3 patients (10.3%),and bronchospasm in 1 patient (3%),respectively.All complications alleviated after proper therapy.During the prophylactic administration of alemtuzumab,strict,timely and proper ward-management was needed.Care for lung,perineum,skin,diet and psychological nursing were necessary.Neither graft acute rejection nor graft chronic rejection episode occurred in all patients during 6 months to 2 years follow-up.Therefore,long term effects of Alamtuzumab and consequences of lymphocytopenia need further observation.
