1.Effects of CRRT at early stage in patients with hyperlipoidemia severe acute pancreatitis

Objective To investigate the effects of early continuous renal replacement thempy(CRRT)for hyperlipoidemia severe acute pancreatitis (HLSAP)patients.Methods 32 HLSAP patients were randomly divided into control group and CRRT treatment group.The levels of serum triglyceride(TG),serum and urine amylase,the APACHEⅡscore,the incidence of mechanical ventilation were compared between the two groups.Results The levels of serum TG was significantly lower in CRRT group than that in the control group in day 3(t =2.885,P =0.003). The APACHE II score,incidence rate of mechanical ventilation,the fatality rate were significantly lower in CRRT group than those in the control group in day 3(χ2 =4.572,t =4.912,χ2 =4.043;P =0.032,0.027,0.044).There were no significant differences between late group than that early group in age,gender,APACHE II score(all P >0.05).There were no significant differences between the two groups in the levels of amylase in serum and urine in day 3(all P >0.05).Conclusion Early CRRT therapy can eliminate the level of TG in HLSAP patients,may improve the prognosis of HLSAP patients.
2.Expression and significance of Slug in papillary thyroid carcinomas

Objective To explore the clinical significance of Slug expression in papillary thyroid carci-noma(PTC). Methods Employed Ventana immunohistochemistry assay to determine the expression of Slug in 107 cases of PTC and para-tumorous normal tissue. The relationship with Slug expression in PTC and clinico-pathology data were also analyzed. Results Expression of Slug in PTC (65.4%, 70/107)and para-tumorous nor-mal tissue (14.0%,15/107)were statistically different (P < 0.001). Overexpression of Slug in PTC was signifi-cantly associated with capsular invasion and lymph node metastasis (P < 0.05). Conclusion Overexpression of Slug in PTC is associated with capsular invasion and lymph node metastasis , these may suggest some clinical significance of Slug expression in PTC in predicting lymph node metastasis and prognosis.
3.Observation of the effect of ganglion glycosides in the treatment of acute cerebral infarction with cognitive impairment

Objective To study the clinical effect of the treatment of cognitive dysfunction of acute cerebral infarction with ganglion glycosides.Methods 83 acute cerebral infarction patients with cognitive dysfunction were selected.83 patients were randomly divided into two groups:the conventional group (41 cases) and the intervention group (42 cases).The conventional group was given conventional control infection,anti platelet aggregation and control hemorrhage of digestive tract and routine rehabilitation training for the main therapy.The intervention group was given conventional treatment plus ganglioside 100mg + 0.9% sodium chloride 250mL,1 time a day,treatment for 21 days.Observation index:(1) total effective rate;before and after treatment,the difference of MMSE score and Barthel index.Results The total effective rate of the intervention group was higher than that of the conventional group,there was statistically significant difference (95.24% vs.75.61%,x2 =8.245,P < 0.01);Before treatment,the MMSE score,Barthel index in the two groups had no significant differences (t =0.372,0.313;P =0.711,0.756).After treatment,the MMSE score,Barthel index of the intervention group improved more significantly compared with the conventional group,the differences were statistically significant(t =7.997,25.530,all P < 0.01).Conclusion The clinical effect of ganglion glycosides in the treatment of acute cerebral infarction with cognitive impairment is accurate,it can effectively reduce the patients'cognitive dysfunction,improve the ability of daily life,it is worthy of promoting.
4.Antidepressant Effects of Salvia Through Neuroinflammatory Signaling Related to Neurogenesis

Major depressive disorder (MDD) is a common affective disorder.With its unique characteristics and advantages,traditional Chinese medicine (TCM) has played an important role in the treatment of MDD.Recent researches have revealed that the neuroinflammation-induced neurogenesis reduction were the key pathology of MDD.This analysis indicated that salvia was frequently used for treatment of MDD from the prescriptions reported in literatures.Salvianolic acid (SalB),one of main active compounds of salvia,had showed anti-inflammation and neuroprotection effects in previous studies.In this review,we summarized the antidepressant effects of SalB through regulating microglial activation,inhibiting neuroinflammation and promoting neurogenesis.According to the theory of removing blood stasis for promoting tissue regeneration,it was suggested that the novel antidepressant strategies for antiinflammatory and neuroprotective activities were through microglia-modulating pathway with TCM.
5.Influence of dexmedetomidine to inflammatory cytokines in mechanical ventilation for patients with acute exacerbation of COPD

Objective To analyze the effects of dexmedetomidine(Dex) on levels of inflammatory cytokines in patients with acute exacerbation of chronic obstructive pulmonary disease (COPD). Methods From April 2015 to April 2017,94 patients with acute exacerbations of COPD who received mechanical ventilation in Wenling Hospital Affiliated to Wenzhou Medical College were randomly divided into two groups according to the digital table,with 47 cases in each group. Group A was given Dex,and group B was given midazolam. The levels of TNF - α,IL - 1, hs - CRP between the two groups were compared. Results There were no statistically significant differences in TNF - α,IL - 1,hs - CRP between the two group before treatment (all P > 0. 05). 6.Mechanical stress regulates apoptosis in vascular smooth muscle cells

BACKGROUND:With the development of biomechanics,its research into cardiovascular diseases has become more and more extensive.By studying the mechanical properties of blood vessels,the pathogenesis of cardiovascular diseases such as atherosclerosis and restenosis can be effectively revealed and new ideas and methods can be developed for the treatment of cardiovascular diseases. OBJECTIVE:To review the research status of apoptosis of vascular smooth muscle cells induced by mechanical stress and search for potential target molecules and signaling pathways for clinical treatment,thereby improving the clinical treatment effect on cardiovascular diseases such as atherosclerosis and restenosis. METHODS:We searched the literature in CNKI,PubMed and ScienceDirect databases from January 1992 to May 2023.The search terms were"vascular smooth muscle cell,mechanical stress,shear stress,stretch stress,apoptosis"in Chinese and English.Finally,63 articles were included for review and analysis. RESULTS AND CONCLUSION:Physiological and pathological apoptosis of vascular smooth muscle cells is an adaptive remodeling in response to the changes in vascular mechanics.Vascular smooth muscle cells in different parts have different mechanical stimuli and their pathogenesis is also different.Low shear stress,physiological shear stress and high shear stress directly interact with surface molecules,receptors and proteins of vascular smooth muscle cells to regulate apoptosis-related signaling molecules and inhibit cell proliferation,thus regulating the apoptosis of vascular smooth muscle cells.In this part,the research on promoting proliferation is not summarized.Low stretch stress,physiological stretch stress and high stretch stress can all cause apoptosis of vascular smooth muscle cells,but it is still controversial.There are many mechanoreceptors(such as integrins and receptor tyrosine kinases)on the surface of vascular smooth muscle cells,which can transform mechanical signals into intracellular chemical signals(such as the Hippo pathway),activate the apoptosis signals of vascular smooth muscle cells and regulate the apoptosis of vascular smooth muscle cells.In short,different mechanical stimuli start a variety of signal pathways and regulate the apoptosis of vascular smooth muscle cells through various signal molecules.For example,shear stress affects Fas/FasL and Akt pathways mainly by stimulating prostaglandin secretion and transforming growth factors.Strech stress mainly regulates the YAP pathway and Notch pathway through Yes-related proteins.At different times or intensities,these molecules may play opposite two-way roles.For example,when mouse vascular smooth muscle cells are stretched at 10%physiological tension for 1 hour,cell proliferation increases.However,the proliferation of human vascular smooth muscle cells can decrease after 12 hours of stretching.Clinically,the key molecules of mechanical transduction can be disturbed by searching for key molecules that interfere with mechanical transduction at their critical time points of action.
