1.Value of digital tomosynthesis for physical examination of chest

Objective To investigate the significance of digital tomosynthesis (DTS) for suspicions lesions in the physical examination of the chest.Methods Totally 1 000 physical examinees were divided into two groups,with 500 younger ones in one group and the remained 500 ones in the other.The examinees underwent examination with digital X-ray radiography,and then the suspicious cases went through DTS examination to analyze the detection rate of pulmonary positive results.Results There were 110 suspected cases found by digital X-ray radiography,including 8 young ones and 102 old ones.The suspected cases went through DTS examination,and totally 92 nodules were found including 4 ones in the young persons and 88 ones in old persons.CT examination found 89 carcinomatous nodules in the 92 ones.Conclusion DTS examination can detect the lesion of the physical examinee,and lays a foundation for early diagnosis and treatment.
2.The effect of phrenic nerve electrical stimulation combined with respiratory training on lung function and on the trunk stability and balance of stroke survivors

Objective To observe the effect of phrenic nerve electrical stimulation combined with respiratory training on the pulmonary function,trunk stability and balance of stroke survivors.Methods Thirty-six stroke survivors were randomly divided into a control group and a training group,each of 28.Both groups were given routine rehabilitation treatment (including physical agent therapy and combined training of the limbs),while the training group was additionally provided with phrenic nerve electrical stimulation combined with respiratory training once a day for six weeks.Before and after the treatment,the forced vital capacity (FVC),forced expiratory volume in one second (FEV1) and peak expiratory flow (PEF) of both groups were recorded.The Sheikh trunk control scale (TCT) was used to quantify their trunk stability,and balance was quantified using the Berg balance function scale (BBS).Results After six weeks of treatment the average FVC,FEV1 and PEF values of the training group were significantly higher than before the treatment,and significantly better than those of the control group after the treatment.Moreover,significant improvement was observed in the average TCT and BBS scores of both groups after the intervention,with the training group's average significantly higher than that of the contrcl group.Conclusion Phrenic nerve electrical stimulation combined with respiratory training can further improve the pulmonary function and balance function of stroke survivors as a supplement to routine rehabilitation treatment.Therefore,it is worth popularizing in clinical practice.
3.The effect of respiratory muscle training on the emotional state and autonomic functioning of depressed stroke survivors

Objective:To observe the effect of respiratory muscle training on autonomic function and the emotional state of persons with post-stroke depression (PSD).Methods:Fifty-two PSD patients were randomly divided into an observation group and a control group, each of 26. Both groups were given routine drug therapy, physical agents therapy and comprehensive training for hemiplegia, but the observation group additionally received progressive inspiratory muscle resistance training and respiration control training twice a day. Before and after four weeks of treatment, the autonomic functioning, emotional state and the ability in the activities of daily living (ADL) in both groups was evaluated using dynamic electrocardiography, the Hamilton Depression Scale (HAMD) and the modified Barthel index (MBI).Results:After the treatment, the average heart rate variability index in the observation group, the standard deviation of 24-hour R-R intervals(SDNN), the root mean square of successive differences of R-R intervals (RMSSD)and percentage of NN interval deviations greater than 50ms and the high-frequency power (HF) were all significantly higher than before the treatment and significantly higher than the control group′s averages. The average HAMD and MBI scores of the observation group were also significantly better.Conclusion:Breathing training can usefully supplement traditional rehabilitation interventions to alleviate post-stroke depression. It will also improve their autonomic nerve function and ADL ability. Such combined treatment is worthy of promotion and wider clinical application.
4.The effect of respiratory muscle training combined with occupational therapy on the upper limb function of stroke survivors

Objective:To observe the effect of combining respiratory muscle training with occupational therapy in rehabilitating the upper limb function of stroke survivors.Methods:Fifty stroke survivors with upper limb dysfunction were randomly divided into an observation group and a control group, each of 25. Both groups were given routine rehabilitation treatment including proper positioning of the affected limb, physical therapy and motor function training. The observation group also received progressive resistance training of the inspiratory muscles and respiration control training combined with occupational therapy twice daily for 4 weeks. The trunk control test (TCT), Berg balance scale (BBS), Fugl-Meyer Upper Extremity Assessment (FMA-UE), Action Research Arm Test (ARAT), Modified Ashworth Scale (MAS) for the Hemiplegic Upper Limb and the Modified Barthel Index (MBI) were used to assess the core stability, balance, upper limb functioning, upper limb muscle tension and ability in the activities of daily living of all of the subjects.Results:Before the treatment there were no significant differences in any of the indexes between the two groups. Afterward the average TCT, BBS, FMA-UE, ARAT, MAS and MBI scores of both groups had improved significantly, but the improvements were all significantly greater in the observation group.Conclusions:Combining respiratory muscle training with occupational therapy can further improve the function of the upper limbs and daily living ability beyond what is observed with traditional rehabilitation therapy after a stroke.
5.Surveillance on drinking water-borne endemic fluorosis in Shandong Province from 2013 to 2017

Objective To understand the implementation effects of control measures and the dynamic prevalence of drinking water-borne endemic fluorosis in Shandong Province.Methods From 2013 to 2017 in Shandong Province drinking water-borne endemic fluorosis areas,in accordance with the method of simple random sampling,10 monitored counties were selected,10 water fluorine decreasing projects were investigated of its running effect,and water fluoride was detected.In each monitored county,3 villages in the diseased areas were selected as fixed monitored villages.If the monitored village had changed its water,the operation of the project would be investigated.If the monitored village had not changed the water,one water sample would be collected according to 5 directions of east,west,south,north and middle to determine the water fluorine.All children aged from 8 to 12 in the village were also examined for dental fluorosis.X-ray fluorosis and urinary fluorosis in adults aged over 25 were detected in 2013 and 2017.Results From 2013 to 2017,in monitored counties,the normal operation rate of improved-water defluoridation projects was 99.19% (368/371),the qualified rate of water fluoride content was 66.85% (248/371).In 128 improved water projects,the normal operation rate was 100.00% (128/128),the qualified rate of water fluoride content was 67.19% (86/128).The average detection rate of dental fluorosis and community fluorosis index (CFI) in children aged 8-12 were 33.92% (2 608/7 689) and 0.68 in all monitored counties from 2013 to 2017.The detection rate presented a declining trend year by year (x2 =152.47 P < 0.01).The average detection rate and CFI in the villages with normal operation projects and qualified fluoride content were 26.10% (1 311/5 023) and 0.47.The detection rate presented a declining trend year by year (x2 =207.67 P < 0.01).The detection rate and CFI in the villages with water improvement projects under abnormal operation or excessive fluoride were 48.65% (1 297/2 666) and 0.90.The detection rate of dental fluorosis in the villages with normal operation projects and qualified fluoride content was lower than the rate of the villages with water improvement projects under abnormal operation or excessive fluoride (x2 =193.98,P < 0.01).In 2013 and 2017,X-ray detection rates of skeletal fluorosis in adults aged over 25 were 8.04% (119/1 481) and 6.04% (81/1 342),respectively.The adult urinary fluoride geometric mean values (1.24,1.49 mg/L) were lower than the normal limit of 1.6 mg/L both in qualified villages of water ffuoride content and exceeding-standard villages in 2017.Conclusions In Shandong Province,the effective water improvement has acquired some effects in control of drinking water-borne endemic fluorosis.But the water fluoride content of improved water defluoridation projects exceeds the national standard seriously and fluorosis has not been effectively controlled.
6.Drinking water-borne endemic fluorosis in Shandong Province in 2016

Objective To grasp the current situation of drinking water-borne endemic fluorosis in Shandong Province,and provide a scientific basis for formulating preventive strategies.Methods In 2016,according to the "National Surveillance Scheme of Drinking Water-Borne Endemic Fluorosis" and "Shandong Provincial Surveillance Scheme of Drinking Water-Borne Endemic Fluorosis",10 counties (cities,districts) were selected in Shandong Province,3-5 villages were selected in each county (city,district),to investigate the operation of water improvement and fluorine reduction project and the water fluorine,and 1 tap water sample in each village was collected to detect water fluorine.At the same time,dental fluorosis examination was performed on all children aged 8-12 years old and fluorosis X-ray examination was performed on adults aged ≥25 years old.Water fluorine was detected by ion selective electrode method;the water fluorine was determined according to "Standards for Drinking Water Quality" (GB 5749-2006);criteria for diagnosis,grading and epidemic intensity of dental fluorosis were based on "Diagnosis of Dental Fluorosis" (WS/T 208-2011);the diagnostic and grading criteria for skeletal fluorosis were based on the "Diagnostic Criteria for Endemic Skeletal Fluorosis" (WS 192-2008).Results All the 32 villages in 10 counties (cities,districts) changed water,water improvement and fluorine reduction project ran normally,24 villages had qualified water fluorine (≤ 1.20 mg/L),and 8 villages had water fluorine exceeding the standard.The detection rate of dental fluorosis in children was 25.75% (419/1 627),and the difference between 10 counties (cities,districts) was statistically significant (x2 =288.55,P < 0.01);the dental fluorosis index was 0.48,and the defect rate was 2.77% (45/1 627).The detection rate of skeletal fluorosis in adults was 4.72% (72/1 525),and the difference between 10 counties (cities,districts) was statistically significant (x2 =18.83,P < 0.01).Conclusions The problem of water fluorine exceeding the standard in water improvement and fluorine reduction project in Shandong Province is still serious.The condition of fluorosis has not been fully controlled,and the prevention and treatment measures need to be further strengthened.
7.Lenvatinib- and vadimezan-loaded synthetic high-density lipoprotein for combinational immunochemotherapy of metastatic triple-negative breast cancer.

Metastatic triple-negative breast cancer (TNBC) is the most aggressive type of breast cancer. Combination of systemic chemotherapy and immune checkpoint blockade is effective but of limited benefit due to insufficient intratumoral infiltration of cytotoxic T lymphocytes (CTLs) and the accumulation of immunosuppressive cells. Herein, we designed a lenvatinib- and vadimezan-loaded synthetic high-density lipoprotein (LV-sHDL) for combinational immunochemotherapy of metastatic TNBC. The LV-sHDL targeted scavenger receptor class B type 1-overexpressing 4T1 cells in the tumor after intravenous injection. The multitargeted tyrosine kinase inhibitor (TKI) lenvatinib induced immunogenic cell death of the cancer cells, and the stimulator of interferon genes (STING) agonist vadimezan triggered local inflammation to facilitate dendritic cell maturation and antitumor macrophage differentiation, which synergistically improved the intratumoral infiltration of total and active CTLs by 33- and 13-fold, respectively. LV-sHDL inhibited the growth of orthotopic 4T1 tumors, reduced pulmonary metastasis, and prolonged the survival of animals. The efficacy could be further improved when LV-sHDL was used in combination with antibody against programmed cell death ligand 1. This study highlights the combination use of multitargeted TKI and STING agonist a promising treatment for metastatic TNBC.
