Objective To assess the association of subclinical thyroid dysfunction with fractures. Methods Medline, Embase, Pubmed, Cochrane Library, CBM, CNKI, Wan Fang, and VIP databases were systematically searched from January 1990 to August 2015 to identify prospective cohort studies which have studied the risk of fracture in patients with subclinical thyroid dysfunction. The relative risks ( RR) of cohort studies were pooled respectively, depending on the result of heterogeneity test among the individual studies search. The Stata (version 13. 0) software was used for meta-analysis. Results Nine prospective cohort studies including 292460 participants were identified as eligible for the meta-analysis. RR of subclinical hyperthyroidism for fracture was 1. 39(95%CI 1. 24-1. 55);for hip fracture, RR was 1. 24(95%CI 1. 10-1. 40);for nonspine fracture, RR was 1. 32 (95%CI 1. 09-1. 60). Different gender for subclinical hyperthyroid was associated with higher fracture rates:for females, RR was 1. 15(95%CI 1. 04-1. 27); for males, RR was 1. 31 (95% CI 1. 08-1. 59). The incidence of fracture in patients with subclinical hyperthyroidism was higher during the follow-up. For subclinical hypothyroidism, the RR was 1. 21(95% CI 1. 03-1. 42). Subgroup analysis indicated that there were significant differences between endogenous/exogenous subclinical hyperthyroidism and euthyroid, but no differences between endogenous/exogenous subclinical hypothyroidism and euthyroid were found. Conclusion Subclinical hyperthyroidism is associated with an increased risk of fracture in the population, especially hip fracture and nonspine fracture. During the course of subclinical hyperthyroidism, the incidences of fracture should be noticed both in females and males. However, there is no evidence which could prove a definite association between subclinical hypothyroidism and the risk of fracture.

Objective To discuss the training method to promote nursing error reporting initiative and investigate its effects on nursing error and accident incidence.Methods Totally 70 nursing units in our hospital were divided into the test group and the control group with 35 units in each group.One nurse was selected from each unit in the test group to composite the training group and received tripartite training,which included the relevant theoretical knowledge lectures,group discussions and analysis,training scenarios under the guidance of professional teachers.The control group received no special training.Before and after the training,the nulpses'attitude of the erroneous theory of cultural knowledge sharing.nursing error reporting initiative.and its effects on nursing error and accident rate were investigated and compared in the two groups by paired t test.Results After the training,the nurses'sharing the erroneous theory of cultural knowledge and attitude and willingness to report nursing error were statistically different(P＜0.01).Conclusions The training method Can improve initiative and willingness of nursing error reporting,decrease nursing error and accident incidence.

Objective To study the characteristics of vestibular autorotation test (VAT) in patients with the posterior circulation transient ischemic attacks(PCTIA) ,further reveal the pathogenesis vestibular function damage because of PCTIA .Methods VAT was performed on 41 patients with posterior circulation transient ischemic at‐tacks(PCTIA group) and 30 healthy controls(control group) .Parameters analyzed included gains ,phases and a‐symmetry of VAT .The positive result of the test could be defined if any of the paramaeters was abnormal .Results In the PCTIA group and control group ,gains were enhanced without being reduced as showed in 25 (62 .5% ,25/40) cases and 5(16 .7% ) ,respectively (χ2 =7 .09 ,P<0 .01) ,gains were enhanced with reduced as showed in 10(25% ) cases and 0(0% ) ,respectively (χ2 =8 .78 ,P<0 .01) .Conclusion The results of VAT in patients with the PCTIA can show gains enhanced with reduced in different frequency .VAT can provide valuable reference on positioning as‐sessment of vestibular system damage in patients with PCTIA .

Objective To develop an intelligent cabinet with constant temperature and adjustable height,which is safer,more scientific and comfortable for heat preservation after surgical operation.Methods According to the demands of microscopic surgery we analyzed the disadvantages of existing equipment in order to seek new materials to solve the current problems.Results Using nanometer electric heating film to control temperature at 30～35 ℃ and adjust temperature in this range.Moreover debugging angle of stainless steel adjustable upholder at 0～45°.Conclusions Nanometer electric heating film is a kind of high safety,reliability and strong stability heating material and can be used for the microscopic surgery insulation.Adjustable lifting upholder makes limbs more comfortable with freely height.It shows that setting temperature at 30～31℃,angel value at 20～25°would be relatively suitable after our experiment.

Objective: To explore the relationship among somatostatin (SS), neuron-specific enolase (NSE) and early vascular dementia.Methods: A total of 40 patients with early vascular dementia treated in our hospital were selected as vascular dementia group, another 40 inpatients with cerebral infarction (CI) treated during the same period were enrolled as CI group.Plasma NSE and SS levels were compared between two groups during different periods.Results: Compared with CI group at onset, one month and three months after CI, there was significant rise in plasma NSE level[(22.08±7.05) ng/ml vs.(26.39±6.80) ng/ml, (23.92±4.25) ng/ml vs.(28.12±4.06) ng/ml, (25.55±4.72) ng/ml vs.(30.10±4.33) ng/ml], and significant reduction in plasma SS level[(1084.50±133.00) ng/ml vs.(748.30±129.10) ng/ml, (836.40±160.20) ng/ml vs.(624.25±140.50) ng/ml, (690.25±146.30) ng/ml vs.(432.70±151.00) ng/ml]in vascular dementia group, P<0.05 or <0.01.Plasma NSE level gradually rose and SS level gradually reduced along with the time went by(P<0.05 or <0.01).Conclusion: Early dynamic detection of somatostatin and neuron-specific enolase levels in patients with cerebral infarction may help to early diagnosing and treatment of vascular dementia.

OBJECTIVETo optimize extraction and purification methods of acidic polysaccharide from Moerella iridescens (MIAP).

METHODSWith alkali extraction process and orthogonal experiment,the time consumption,temperature,pH value of the solution and alcohol concentration during the extraction were optimized. The crude products were deprived of protein,pigment and ion,then were purified with DEAE-cellulose ion-exchange chromatography and verified with Sephadex G-100 and cellulose acetate membrane electrophoresis,and examined with infrared spectrum.

RESULTSThe optimized extraction conditions were as follows: extraction time 6 h,extraction temperature 70 degree,the solution pH 8.0 and the concentration of alcohol precipitation 70%. Intuitive features showed that the MIAP was pure white crystalline granular with slight dark brown color. The purification results demonstrated that the target MIAP was eluted and identified as a homogeneous components by DEAE-cellulose ion exchange column,Sephadex G-100 and cellulose acetate membrane electrophoresis. Infrared spectral scanning suggested that MIAP was α-D-type terminated glucopyranose. Intuitive features showed that MIAP was soft and cottony white.

CONCLUSIONThe extraction process with orthogonal test has been optimized and the acidic polysaccharide from Moerella iridescens is successfully isolated.

OBJECTIVE: To study the feature and treatment method of patients with empty sella merger cerebro-spinal fluid leakage of nasal. METHOD: There were 8 cases with empty sella merger cerebrospinal fluid leakage of nasal, 2 cases were accepted the repairing surgery of cerebrospinal fluid leakage one time, 4 cases were accepted the repairing surgery of cerebrospinal fluid leakage used endoscope 2 times, 1 case was accepted repairing surgery of cerebrospinal fluid leakage used endoscope merge craniotomy and ventricle celiac bypass, 1 case recurrences after repairing surgery of cerebrospinal fluid was recurred after conservative treatment. Some postoperative were stayed in bed for three weeks and lumbar drainage for 1 week. RESULT: One case of cerebral hemorrhage after surgery was cured with craniotomy, followed for 2 years without recurrence. One case was recurred after conservative treatment. Two cases recurrences after surgery 3 years ago were accepted surgery again followed by one year without recurrence. One case who recurrence 1 year later was accepted repairing surgery of cerebrospinal fluid leakage used endoscope merge craniotomy and ventricle celiac bypass followed six months without recurrence. One cash after once surgery was followed half a year without recurrence. One case with recurrence 5 years later was accepted repairing surgery again. CONCLUSION The patient with empty sella combined cerebrospinal fluid leakage of nasal was rare, the main method was endoscopic sinus surgery treatment, but it recurred usually. The patients with repeatedly recurrence can be considered to accepted the surgery of ventricle celiac bypass. It required long-term postoperative follow-up and review.
