1.Surgical strategies of total aortic arch replacement for aortic dissection

Aortic dissection has been remained highly lethal by far,especially for those involving the aortic arch.Many ways have been tried to tackle with aortic arch lesions including open surgery,endovascular therapy and hybrid procedure.Among them,surgical replacement of aortic arch seems to be the most promising on account of its long-term follow-up.However,there is still no uniform surgical procedure for aortic arch replacement.And surgical complications often occur due to its complex anatomical structures.Accordingly,many surgical procedures aiming at simplifying the procedure and lowering the risk of the operation have been raised.This article will introduce these new ways by reviewing related literatures and making brief comments.
2.Investigation of the distribution of Rh blood group in voluntary blood donors in Foshan,Guangdong province,China

Objective:To investigate and analysis the distribution situations of Rh blood group system from voluntary blood donors in Foshan area.Methods:Anti-D,anti-C,anti-c,anti-E and anti-e reagents were used to identify Rh blood group system and the RhD(-) was confirmed.The phenotypes,haplotypes and corresponding genotypes frequencies were calculated and the datas were analyzed based on Hardy-Weinberg law.Results:The characteristics of phenotype frequencies in the voluntary blood donors in Foshan area was CCDee>CcDEe>CcDee>ccDEE>ccDEe>CCDEe>CcDEE>ccdee>ccDee,Ccdee>CCdee,ccdEe>CCDEE,CCdEE,CCdEe,CcdEE,CcdEe and ccdEE.The characteristics of haplotypes frequencies was CDe>cDE>cDe>CDE,de>Cde>cdE>CdE.The characteristics of genes frequencies was D>d,C>c,e>E.The frequency of RhD(-)phenotype was 0.379%.The gene frequency of d was 0.061.The observed value and desired value of the haplotypes and corresponding genotypes were not statistically significant(P>0.05).The data were analyzed well based on Hardy-Weinberg law.Conclusion:The result is reliable according to Hardy-Weinberg law.The distribution of Rh blood group system from voluntary blood donors in Foshan area has geographic and general characteristics.The result is important in guiding the recruitment of voluntary blood donors and banking blood reasonably and enhancing the abilitiy of offering blood for the recipients of rare blood group.
3. Chromatography fingerprint of Schisandra chinensis Chewable Tablets by HPLC-DAD

Objective: To establish the chromatography fingerprint of Schisandra chinensis Chewable Tablets (SCCT) with HPLC-DAD and to evaluate SCCT from 10 different batches. Methods: Luna C18 column (250 mm×4.6 mm, 5 μm) was used, mobile phase was acetonitrile-0.05% phosphate, flow speed was 1.0 mL/min, temperature of column was set at 35℃, detected wavelength was at 220 nm, and injection volume was 20 μL. Results: The chromatography fingerprint of SCCT from 10 different batches was established. In the chromatography fingerprint with HPLC-DAD of SCCT, 39 common peaks were demarcated and the similarities of SCCT were between 0.914-0.999. Schizandrin, schisandrol B, schisantherin A, schisandrin B, schisandrin C, salvianic acid A sodium, salvianolic acid B, cryptotanshinone, tanshinone IIA, 3, 4-dihydroxyhenzaldehyde, luteoloside, and buddleoside were identified. Conclusion: The method is accurate, reliable and with good reproducibility, and can be used as the evidence for the quality evaluation of SCCT.
4.Analysis of the current status of the prevalence and its relative factors of non-alcoholic fatty liver disease in urban community in Ningxia province

Objective To investigate the current status of prevalence and its relative factors of non-alcoholic fatty liver disease (NAFLD) in the elderly in urban community in Ningxia province and to compare the differences in prevalence of NAFLD between Han and Hui ethnicities.Methods 1046 residents aged 55 years and over from five communities were selected.Questionnaire survey and clinical examination were conducted in all subjects.Fasting vein blood samples were collected.The serum total cholesterol,triglyceride,high density lipoprotein cholesterol,low density lipoprotein cholesterol were detected.Results There were 1043 subjects with average age of (66.41 6.65) years finishing the whole examination,including 696 (66.7%) Han and 347 (33.3%) Hui people.The total prevalence of NAFLD was 27.0% (286/1043),and there was a significant difference in the prevalence [23.4% (94 cases) vs.29.9% (192 cases),x2 =5.18,P=0.023]between male and female,but no difference between Han and Hui ethnicities [28.2% (98 cases) vs.27.0% (188 cases) x2 =0.17,P=0.675].Logistic regression showed that age(OR=0.96,95% CI:0.94-0.99),BMI(OR=1.31,95% CI:1.24-1.38),TG(OR=1.71,95% CI:1.47-1.98),central obesity (CO)(OR=5.20,95%CI:2.21-12.23) were the factors correlating with NAFLD.Conclusions The prevalence of NAFLD in Hui elderly people was is similar to that in Han elderly people.The elderly people with overweight,central obesity and high serum level of triglyceride and high level TG have higher risk for NAFLD.
5.Etiopathogenesis of hepatic perfusion disorders: an experimental study in pigs

Objective To explore the causes of hepatic perfusion disorders (HPD), and to analysis the CT features of this phenomenon. Methods Nine experimental pigs were randomly divided into three groups. In Group A, B and C, intrahepatic portal, arterial branches and hepatic vein were selectively embolized, respectively. Then all animals underwent contrast enhanced CT scans immediately and after one week. Results On CT images immediately after intervention, HPD were found in all animals. Multiple transient wedge-shaped hyperattenuation represented during the hepatic arterial phase and isoattenuating areas during the portal venous phase. In group A, the site of HPD was in correspondence with the area of embolization,whereas in group B, the embolized area displayed hypoattenuation and non-embolized area displayed hyerattenuation during arterial phase. In group C, the site of HPD was correspondence with the area of embolization in two pigs, but in one pig, the region of HPD was larger than the region of embolization. After one week, HPD disappeared due to sponges absorption and the vessels reopening in group A and B. In group C which hepatic vein was coagulated by laser, HPD remained partially. Conclusion Intrahepatic vascular occlusions of portal, hepatic arterial and hepatic venous branches are the main factors that cause HPD.
6.The relation between epithelial-mesenchymal transition and bronchiolitis obliterans post lung transplantation in rats

Objective To imitate the pathological process of bronchiolitis obliterans(BO) post lung transplantation and in-vestigate the phenomenon of epithelial-mesenchymal transition ( EMT ) in small airway epithelial cells and its relation to BO . Methods We established the rat model of allogeneic orthotropic left lung transplantation with grafts from F 344 rats transplanted to Lewis rats by the Tri-cuff anastomosis.Then rats were daily injected intraperitoneally with cyclosporine (5 mg· kg-1 · d-1 ) for 10 days, intratracheally applied with lipopolysaccharide(0.5 mg/kg) at the 28th day and killed at the 90th day post-surger-y.We observed the histological structure of the lung grafts by HE and Masson staining , detected E-cadherin and Vimentin ex-pression in lung grafts and normal ones of F344 rats by immunohistochemistry, then analyzed the relation between small airway remodeling and the alterations in EMT markers expression .Enumeration data was analyzed with Fisher exact test and Spearman rank correlation was applied in correlation analysis .Results We found inflammatory cell infiltration , fibroplasia of bronchiole walls and significant lumen stenosis in lung graft mesenchyma.In lung grafts, the positive expression of E-cadherin was 5(total 13), which was significantly lower than that in normal lungs(7/8, P<0.05), and conversely the positive expression of Vim-entin was higher than normal ones(10/13 vs 2/8, P <0.05).The Spearman rank correlation analysis demonstrated that E-cadherin expression bear a negative correlation with Vimentin expression(r=-0.750, P<0.01).Conclusion Our study established a disease model imitating the pathological process of BO .And EMT was observed in small airway epithelial cells of the BO-exhibited lung grafts, indicating that EMT was involved in the process of BO airway remodeling .Thus it may reveal no-vel therapeutic targets for BO post lung transplantation by further investigation into the mechanism and pathological significance of EMT in small airway epithelial cells.
7.CT guided percutaneous radiofrequency thermal ablation of peripheral lung carcinoma

Objective To assess the therapeutic value of CT guided percutaneous radiofrequency thermal ablation(RFA)for peripheral lung carcinoma(PLC).Methods Forty-three patients with PLC were treated with RFA under the CT guidance.The patients included 26 males and 17 females,and the age ranged from 52 to 76 years.The CT guided percutaneous biopsies were performed to obtain the pathology before the procedures.Results The operations in all cases were successful,little amount of pneumothorax was found in 3 cases,liquid-pneumothorax in 1 case,and large amount of pneumothorax was drainaged in 1 case.The volume of the lesion in 40 cases from 43 patients was diminished gradually at 3 and 6 months follow-up after the procedures.No change in the volume of lesion with peripheral enhancement was seen in 3 cases.Conclusion CT guided percutaneous RFA for PLC is a safe,effective,and minimal invasive procedure.
9.Prevalence of High Non-high-density Lipoprotein Cholesterol and Associated Risk Factors in Patients with Diabetes Mellitus in Jilin Province, China: A Cross-sectional Study.

Dyslipidemia is a risk factor for cardiovascular diseases (CVDs) in patients with diabetes, and non-high-density lipoprotein cholesterol (non-HDL-C) is a better predictor of CVDs than low-density lipoprotein cholesterol (LDL-C) in patients with diabetes. Therefore, we aimed to investigate the distribution of non-HDL-C and the prevalence of high non-HDL-C level in Chinese patients with diabetes mellitus and identify the associated risk factors. Non-HDL-C concentration positively correlated with total cholesterol, triglycerides, and LDL-C concentrations. Although both non-HDL-C and LDL-C concentration both related positively with TC concentration, the magnitude of correlation was relatively higher for non-HDL-C. The prevalence of high non-HDL-C (⋝4.14 mmol/L) was higher in two age groups (55-64 years: 46.7%; 65-79 years: 47.3%) than other age groups (18-24 years: 4.2%; 25-34 years: 43.6%; 35-44 years: 38.1%; 45-54 years: 41.0%). It was also higher among overweight (45.1%), generally obese (50.9%), or abdominally obese (47.3%) subjects, compared with normal weight subjects (34.5%). The risk of high non-HDL-C increased with advancing age. Both general obesity [odds ratio (OR)=1.488, 95% confidence interval (CI): 1.003-2.209] and abdominal obesity (OR=1.561, 95% CI: 1.101-2.214) were significantly associated with high non-HDL-C levels.

10.Inhibition of Nogo expression to promote repair after spinal cord injury.

BACKGROUNDOne of the reasons for poor neuroregeneration after central nervous system injury is the presence of inhibitory factors such as Nogo. Here, we tested the inhibition of Nogo by RNA interference both in vitro and in vivo, using recombinant adenovirus-mediated transfection of short hairpin RNAs, to explore a new method of treatment for spinal cord injury.
METHODSWe designed and cloned two Nogo-specific short hairpin RNAs and an unrelated short hairpin RNA, packaged the clones into adenovirus, and amplified the recombinant virus in 293 cells. We then tested the inhibition of Nogo expression both in vitro in adenovirus-transfected oligodendrocytes and in vivo in spinal cord tissue from adenovirus-transfected spinal cord injury model rats. We tested Nogo expression at the mRNA level by reverse-transcription PCR and at the protein level by Western blotting and immunohistochemistry.
RESULTSIn vitro, the two specific Nogo short hairpin RNAs decreased Nogo mRNA expression by 51% and 49%, respectively, compared with Nogo expression in cells transfected with the unrelated control small hairpin RNA (P < 0.005). Similarly, Nogo protein expression decreased by 50% and 48%, respectively (P < 0.005). In vivo, in spinal cord injury model rats, the two specific Nogo short hairpin RNAs decreased Nogo mRNA expression by 45% and 40%, respectively, compared with Nogo expression in spinal cord injury model rats transfected with the unrelated control short hairpin RNA (P < 0.005). The Nogo protein level was similarly decreased.
CONCLUSIONSWe were successful in specifically downregulating Nogo at the mRNA and protein levels by adenovirus-mediated delivery of short hairpin RNAs, both in vitro and in vivo. This confirms the effectiveness of RNA interference for the inhibition of Nogo gene expression and the efficiency of using adenovirus for delivery. Thus gene therapy may be an effective treatment for spinal cord injury.
