1.Analysis on working intention in rural areas and its influence factors of medical students in western medical colleges from the perspective of urban and rural comparison

This paper intends to analyze the working intention in rural areas and its influence factors of medical students .A random sampling method was used to select the sample for the study .This investigation was conducted on the medical students of grade 3 to 5 in undergraduate medical schools , junior medical colleges and technical seconda-ry medical schools from 10 western provinces .The methods of exploratory factor analysis , chi-square test , one-way ANOVA and binary logistic regression analysis were used to make the data mining .As per the results of this study , 52 .1%of the sample medical students have the working intention in rural areas .The working intention of the medical students with rural hukou (56.4%) is significantly higher than that of the medical students with urban hukou (42. 7%) .The influencing factors mainly include hukou , basic conditions of working unit , core conditions of working u-nit, gender, type of medical college, family income level, and major.In conclusion, to further expand the recruit-ment number and scope of medical students with rural hukou , and to promote the working intention in rural areas of medical students to be translated into practical working behavior , improving the core conditions of medical institutions in rural areas , and loosening and lower the recruit standards and conditions to some extent when hiring the new medi -cal workers should be viewed as some of the strategies to use .
2.Cause Analysis of Stoma Recurrence after Anterior Resection of Rectal Cancer (Report of 91 Cases)

Objective To investigate the cause of stoma recurrence after anterior resection of rectal cancer and discover the methods of prevention and treatment.Methods A total 91 patients with stoma recurrence after anterior resection of rectal cancer (or Dixon) were analysed retrospectively between 1985 and 1996. Fourty seven patients experienced re radical resection (Miles), 27 cases palliative resection, and 11 cases only exploration. Thirty two cases had been followed up for 5 years and obtained 1,3,5 year survival rate for re radical radical resection (Miles). Diagnosis and treatment of stomal recurrence after Dixon were evaluated. Results One, three and five year survival rate of re radical resection (Miles) was 93%,77%,45% respectively.Conclusion To amplify blindly the adaptation of Dixon is to raise the rate of stoma recurrence. Digital rectal examination and fiberopic colonoscopy (and biopsy) are very essential methods for the diagnosis of stoma recurrence, and we strive to do re radical resection (Miles) for the patients with stoma recurrence after Dixon's operation.
3.Diagnosis and surgical treatment of splen-aneurysm

Objective To investigate the diagnosis and surgical treatment of splen aneurysm.Methods The diagnosis and surgical treatment of 12 patients with splen-aneurysm are analysed retrospec tively between 1987 and 2000.Results The clinical findings of most cases were urgent and critical including 8 patients of severe abdominal pain,2 patients of disruption and bleeding of tumor body,4 abdominal pain and u pset,andabdominal impulsive mass,except that 1 patient had no clinical finding owing to health survey.Ultrasonic examination by color Doppler's method was the most essential means in diagnosis,even if a patient was suspected having disrupt ion and bleeding of tumor body,he should be given up to do ultra-B examination b edside.All patients were operated in emergency or deadline and had no allo-blood supply of spleen and no perioperative death.Conclusions Al most all patients obtained final diagnosis by depending on Ultrasonic examinatio n by color Doppler's method.We suggest that the ligation of splenic artery aneur ysm be selected as far as possible according to several patient's conditions bec ause it has simple?safe and effective speciality.
4.Optimization of hospital outpatient service process:practice and comments

Taking the Second Affiliated Hospital of Zhejiang University School of Medicine as an example,the paper discussed its practices in optimizing outpatient service process.These practices include innovating convenient outpatient service process,establishing a coordination center of hospital's beds to strengthen pre-hospital one-stop management,implementing day surgery to improve inpatient service,implementing professional and humanized hospital clinical care,introducing the concept of fast tract surgery to promote accelerated rehabilitation after surgery,and establishing a follow-up center for inpatients discharged.Such efforts have improved the medical services' efficiency and quality,and improved the patients' satisfaction.
5.Observation of the therapeutic effect of combining hysterosalpingography and Bultraphonic monitor for the follicle on the infertilitas feminis

Objective To investigate the therapeutic effect of combining hysterosalpingography and B ultra-phonic monitor for the follicle on the infertilitas feminis.Methods 146 infertility patients were randomly divided into observation group and control group according to digital table method,73 cases in each group.Patients in control group were treated with comprehensive optimization of treatment,while the observation group received fallopian canal side of progesterone based on the integrated optimization of treatment.All patients underwent uterine oviduct radiography,and the observation group used the fallopian tube with contrast -enhanced ultrasound in monitoring follicle to identify patients with dominant follicle.The pregnancy rate was compared between two groups.Results 146 cases of infertility patients with the uterus and fallopian tube radiography inspection found hydrosalpinx in 39 cases,accounted for 26.71%,29 cases of tubal blockage,accounted for 19.86%,tubal and the poor in 51 cases (34.93%),fallopian tube was through 27 cases (18.49%).There was no significant difference in the contrast between the two sides of the fallopian tube in the observation group and the control group (P >0.05).The intrauterine pregnancy rate of the obser-vation group (41.10%)was significantly higher than the control group (20.55%),while the ectopic pregnancy rate (28.77%)was significantly lower than the control group (50.68%)(χ2 =7.23,7.32,all P <0.05).The ectopic pregnancy in patients with tubal obstruction was significantly higher than that of tubal obstruction,tubal obstruction and tubal tubal type (χ2 =10.57,5.92,13.85,all P <0.05).Conclusion Combining hysterosalpingography and B ultraphonic monitor can accurately predict the date of ovulation,help guide the pregnancy intervention measures, improve the infertility of intrauterine pregnancy rate and prevent the occurrence of ectopic pregnancy.
6.Determination of Phenylhydrazine Residues in Edaravone by HPLC

OBJECTIVE: To establish an HPLC method for the determination of phenylhydrazine residues in edaravone.METHODS: The determination was performed on Diamonsil C18 column with 0.05 mol?L-1 ammonium acetate-acetonitrile(80∶20) as mobile phase at a flow of 1.0 mL?min-1.The wavelength of UV detector was set at 233 nm and the sample size was 20 ?L.RESULTS: The linear range of phenylhydrazine was 22~550 ng?mL-1(r=0.999 9);The average recovery was 98.10%(RSD=0.85%),and the detection limit was 44 pg.CONCLUSION: The analytical method is simple,reliable and sensitive,and suitable for the determination of phenylhydrazine residues in edaravone preparation.

Objective The purpose of this investigation was to observe the effects of the glutamate/aspartate transporter(GLAST) antibody to auditory brainstem response(ABR) and the pathologic morphology of hair cells in the guinea pig's cochlea. Methods 20 guinea pigs were randomly divided into the experimental group and control group. By perfusing the antibody to GLAST into tympanic canal in the cochlea of guinea pigs in experimental group and artificial perilymph into the guinea's cochlea in control group,the results of ABR ,basilar membrane stretched preparation and transmission electron microscope were observed. Results After antibody perfusion,the ABR was not induced from the third day to the ninth day.In control group,the ABR was nearly absent on the third day,but could be evolved,on the sixth day and the ninth day in all guinea pigs with the average threshold of 62.50?5.25 dB SPL and 47.50?6.18 dB SPL,respectively. BACKGROUND: Different derivatives of chitosan with different molecular weights or degrees of deacetylation show different anti-tumor effects.OBJECTIVE: To study the inhibition effect of water-soluble chitosan and its derivatives, such as sulfonated chitosan, carboxymethyl chitosan and chitooligosaccharides for the growth of hepatocellular carcinoma cell line SMMC7721.DESIGN, TIME AND SETTING: Controlled experiments based on observation were carried out in Jiangxi Institute of Digestive Disease (Nanchang, Jiangxi, China) from January 2004 to December 2006.MATERIALS: Hepatoma cell line SMMC7721 was provided by Jiangxi Institute of Digestive Disease (China). 85.5% deacetylated chitooligosaccharides and 85% deacetylated water-soluble chitosan were produced by Jinan Haidebei Ocean Biological Engineering Co., Ltd (China); Carboxymethyl chitosan and 88.5% deacetylated chitosan were the products of Shanghai Qisheng Biological Products Co., Ltd (China).METHODS: Sulfonated chitosan was prepared using 88.5% deacetylated chitosan and chlorosulfonic acid-formamide, and then was detected with infrared spectroscopy in the Detection Analysis and Test Center, East China University of Science and Technology. SMMC7721 cells in the log phase were inoculated into 96-well culture plates, which were then added with water-soluble chitosan, sulfonated chitosan, carboxymethyl chitosan and chitooligosaccharides with the final concentrations of 25, 50, 100, 200, 400 and 800mg/L. This test was repeated for 3 times, while the control group was also set each time. After 72 hours of routine culture, MTT solution was added into each well and inoculated for another 4 hours. After the culture was terminated, dimethyl sulfoxid was added. The absorbance value of each well was measured at 490nm wavelength on a microplate reader. Three tests were measured to obtain the mean value. Also the inhibition rate was calculated.MAIN OUTCOME MEASURES: Growth inhibition effect of chitosan and its derivatives on the hepatoma cell line SMMC7721.RESULTS: Among the chitosan and its derivates at four kinds of concentrations, water-soluble chitosan and sulfonated chitosan could significantly inhibit the growth of SMMC7721 cells (P<0.001), and the effect was the most significant in the case of sulfonated chitosan. Treatment with water-soluble chitosan and sulfonated chitosan at the concentration of 50mg/L could inhibit the growth of SMMC7721 cells in a dose-dependent manner, and reached a peak at the concentration of 400mg/L and 800mg/L, respectively. Carboxymethyl chitosan and chitooligosaccharides showed no growth inhibition effect (P>0.05).CONCLUSION: Water-soluble chitosan and sulfonated chitosan have significant antigrowth effects on hepatoma carcinoma cells, while carboxymethyl chitosan and chitooligosaccharides are ineffective.
9.Regulation of Baicalin for Serum Progesterone Level and Its Effects on Neural Functions after Cerebral Ischemia in Rats

Objective To investigate the effects of baicalin on serum progesterone and related hormones in female normal and cerebral ischemia rats; To explore whether baicalin plays a role in cerebral protection of neurological functions by regulating progesterone levels.Methods With vaginal smear method, the adult estrus female SD rats were selected and divided into normal group, baicalin normal group, and molding groups. The left side of the middle cerebral artery of rats in the molding groups was blocked to establish the permanent middle cerebral artery occlusion (pMCAO). After modeling, the rats were randomly divided into model group, baicalin treatment group, progesterone treatment group and progesterone inhibitor group. The baicalin normal group and baicalin treatment group were given intraperitoneal injection of baicalin solution; the normal group and model group were given normal saline of the same quantity; progesterone treatment group was given intramuscular injection of progesterone; progesterone inhibitor group was given intraperitoneal injection of baicalin solution and intragastric administration of mifepristone solution.The neurological function deficit scores were evaluated and rat forelimb holding power was detected by Grip Strength Meter respectively at different time points. Serum was taken from the rats and the progesterone and related hormones levels in the serum of every group were measured by ELISA. Results Compared with normal group, neurological functions of rats in molding groups were damaged, and neural functional behavior scores of different time points were the most strongly increased (P<0.001), and rat forelimb holding power was the most strongly reduced (P<0.001). 5 days after treating, baicalin showed the trend of improvement of neurological functions (P>0.05) and more significant improvement of the forelimb holding power (P<0.01); 10 days after modeling, baicalin treatment group significantly increased neural functional behavior scorce (P<0.001) and the most significantly improved the forelimb holding power (P<0.001). Compared with baicalin treatment group, the progesterone inhibitor group had a significant inhibitory effect on neural functional recovery (P<0.05) 10 days after modeling, and the group also had a significant inhibitory effect on the recovery of holding power (P<0.05) 5 days and 10 days after treating. At the same time, compared with the model group, progesterone level in baicalin treatment group increased significantly (P<0.05), and FSH and LH decreased (P>0.05).Conclusion After applying mifepristone to block progesterone, baicalin neurologic protection is significantly inhibited. The results demonstrated that baicalin may play a role in cerebral protection via up-regulating serum progesterone level.
